Quantitative determination of the fatty acid composition of human serum lipids by high-performance liquid chromatography.

A high-performance liquid chromatographic method for the analysis of the fatty acid composition of human serum lipids with fluorescence detection was examined. Both free and total fatty acids extracted from serum were derivatized with 9-anthryldiazomethane and were analysed using methanol-water (94.7:5.3) as mobile phase. Twelve kinds of fatty acid were detected, both in the free and total fatty acids, and were well separated. Concentrations of individual fatty acids of serum lipids were estimated from an internal standard, heptadecanoic acid. The results correlated well with those from two other quantitative analyses. These results indicate that the high-performance liquid chromatographic analysis of fatty acids is a reliable method for determining individual fatty acids of human serum lipids. The compositions of free fatty acids and total fatty acids of serum lipids were analysed and compared in 27 normal subjects, 27 diabetics, and 20 angina pectoris patients by this method.

Neutrophils biosynthesize leukotoxin, 9, 10-epoxy-12-octadecenoate.

An epoxy derivative of linoleate, 9,10-epoxy-12-octadecenoate, was demonstrated to be biosynthesized by neutrophils from various sources such as canine and human blood, and guinea-pig peritonea. It was nominated as leukotoxin from its 'toxic' activity onto mitochondrial respiration. From the reaction mixture of leukocytes with linoleate, an isomer of leukotoxin, 12,13-epoxy-9-octadecenoate, and a 'non-toxic' hydroxy derivative of linoleate, 9-hydroxy-12-octadecenoate, were detected. Such a cascade reaction of linoleate by leukocytes was discussed. Biosynthesis of leukotoxin by neutrophils was substantially enhanced by the presence of calcium ion and calcium-ionophore, A23187. Neutrophils contained leukotoxin, ca. 7 f moles/cell, which was extractable by 60% ethanol, but little of the isomer.

Biosynthesis of leukotoxin, 9,10-epoxy-12 octadecenoate, by leukocytes in lung lavages of rat after exposure to hyperoxia.

In lung lavages of rat after pure oxygen breathing, a toxic linoleate peroxide, 9,10-epoxy-12-octadecenoate, and its isomer, 12,13-epoxy-9-octadecenoate were detected by HPLC analyses. The epoxide(s) was demonstrated to be biosynthesized by incubating linoleate with leukocytes collected from lung lavages, thus nominated to be leukotoxin. The chemical structures of leukotoxin and its isomer were determined by gas-chromatography/mass spectrometry and nuclear magnetic resonance measurements. Leukotoxin showed a potent uncoupling activity to rat liver mitochondrial respiration and a dose-dependent relaxation of rat stomach smooth muscle. These findings were discussed with 'oxygen toxicity' on the lung.

[Two cases of Kallmann's syndrome].

Herein we report two cases of hypogonadism with anosmia or hyposmia (Kallmann's syndrome), a 23-year-old single man (case 1) and a 34-year-old single man. (case 2). Micropenis and small testes were seen in both cases. Central anosima and central hyposmia were found in cases 1 and 2, respectively. Their karyotypes were normal (46, XY). The plasma levels of LH and FSH were low and the reaction to LH-RH test was poor in both cases. After repetitious LH-RH administrations, a good response of plasma LH was recognized in both cases except for a poor response of plasma FSH in case 1, while plasma testosterone remained low in both cases. Their testicular biopsies revealed immature testes without any developed Sertoli and Leydig cells.

[Clinical experience with T-2588 in complicated urinary tract infections].

The clinical effectiveness and safety of T-2588 were evaluated in 21 patients with complicated urinary tract infections. Six hundred mg of T-2588 per day was administered orally in three divided doses for 14 days. The results were excellent in 7 cases (33.3%), moderate in 8 cases (38.1%) and poor in 6 cases (28.6%), and the effectiveness rate was 71.4%. The effectiveness rates of the single infection group and mixed infection group were 91.7% and 44.4%, respectively. The overall bacteriological eradication rate obtained was 77.1%, and those of gram-positive cocci and gram-negative rods were 62.5% and 81.5%, respectively. None of the 21 cases, had any significant side effects and abnormal laboratory findings. From the above results, T-2588 is considered to be a useful antibiotic in the treatment of complicated urinary tract infections.

Mechanism responsible for "down regulation" during successive beta-agonist administration in rat hearts.

The role of phospholipase (PLase) on the mechanism of the so called "down regulation", which decreases the number of beta-adrenergic receptors (beta-AR) of rat cardiac membranes during successive beta-agonist administration, was investigated. In vivo study: The rats were divided into 3 groups: (1) the control group, untreated; (2) the isoproterenol (ISP) one day group, ISP (10 mg/kg) was subcutaneously injected once; and (3) the ISP 6 days group, ISP (10 mg/kg) was injected once a day for 6 successive days. Binding assay using [3H] dihydroalprenolol and analysis of the content and the composition of fatty acids in phospholipids in cardiac membranes were conducted. The endogenous PLase activity in heart homogenate and tissue ATP levels were also determined. In the 6 days group, the equilibrium dissociation constant of beta-AR was not affected; however, the number of beta-AR decreased significantly when compared with the control group. Phospholipid content also decreased in parallel with the decrease in the number of beta-AR and the composition of fatty acids was altered. However, in the one day group, there were no significant changes in either the number of beta-AR or the phospholipid content. The PLase activity of heart homogenate increased significantly, and the tissue ATP levels decreased in both the one day and the 6 days group. In vitro study: Cardiac membranes prepared from intact rats were incubated with 0.03 and 0.1 unit of PLase A2. [Therapeutic effect of Robaveron tablet (KN-7) on urinary disturbance by benign prostatic hypertrophy].

Therapeutic effect of Robaveron tablet (KN-7) was studied on 101 patients with urinary disturbance accompanied by benign prostatic hypertrophy. Robaveron tablet, which contains 20 mg of a swine prostatic extract per tablet, was administered 6 tablets daily for 3 weeks in principle. Evaluation of drug efficacy was based on residual urine, cystometric findings, urethral pressure profile, uroflowmetry and subjective symptoms. Remarkable decrease of residual urine was observed at all stages of Guyon classification, in parallel with increases of cystometric pressure amplitude and average flow rate. Rate of residual urine reduction rose in proportion to the higher stage or bulkier volume of residual urine. Improvements of subjective symptoms were also obtained. The overall effectiveness, rated slightly improved or better was 76.8%. Side effects were seen at a low rate of 3.9%, and there were no abnormal changes directly due to the drug in clinical laboratory tests. These results indicate that Robaveron tablets act on the detrusor muscle and contribute to improve the depressed voiding efficiency and incidental symptoms of the subjects.

[Transfer of ceftizoxime into serum and prostatic tissue].

Serum and prostatic tissue levels of Ceftizoxime (CZX) were measured in 37 patients with benign prostatic hypertrophy after intravenous infusion of 0.5 g or 1 g or 2 g of CZX. The prostatic tissue level of CZX was sufficient for the treatment of prostatic infection. Serum and prostatic tissue levels of CZX were maintained satisfactorily by the dose of CZX. Neither the weight of extracted prostatic tissue nor the renal function of the patient was correlated to the tissue level of CZX.

[Treatment of organic impotence. Self-administered intracavernous injection of papaverin HCI (a preliminary report)].

Intracavernous injection of 40 mg papaverin HCI caused erection sufficient for intromission in all six cases of organic impotence. The patients were taught to self inject one corpus cavernosum just prior to coitus and reported that they were able to have coitus at home. No complications were encountered. This method should be tried first as an alternative to penile implant for patients with organic erectile failure.

[The use of dimethyl sulfoxide in the treatment of intractable urinary frequency].

Intravesical instillation of dimethyl sulfoxide (DMSO) was used in the treatment of patients with intractable urinary frequency due to chronic prostatitis, chronic cystitis, tuberculous contracted bladder and interstitial cystitis. Before the application of this therapy, all 4 patients were examined carefully to rule out cases of acute infectious diseases of the urinary tract, active urinary tuberculosis, neurogenic bladder and carcinoma in situ of the bladder. Three of the four patients achieved an excellent response both subjectively and objectively. In the United States, intravesical instillation of DMSO had already been established as the specific method in the treatment of interstitial cystitis and no side effects have been reported so far. Therefore, we recommend the use of intravesical instillation of DMSO more commonly in various forms of intractable urinary frequency.

[Synchronous tumors in right renal pelvis and left ureter].

Bilateral synchronous urothelial tumors in the upper urinary tract seem to be rare. To our knowledge, only 34 cases have been reported previously. A 77-year-old-man with the chief complaint of macrohematuria was admitted. Clinical investigations showed left ureteral and right renal pelvic tumors. Resection of the wall of the right renal pelvis and partial ureterectomy and its reanastomosis in the left ureter were done. Pathologic examination revealed that the right renal pelvic tumor was transitional cell carcinoma (G.2, pTlb), and the left ureteral tumor was invasive metaplastic epidermoid carcinoma. The patient is alive with no recurrence or metastasis 1 year after the operation. We reviewed all the reports of bilateral synchronous tumors we could find and discussed the histological findings and treatments reported.

The effect of coenzyme Q10 on the action of phospholipase.

Using dimyristoyl L-alpha-phosphatidylcholine as a substrate, the effect of coenzyme Q10 on phospholipid digestion by phospholipase A2 and phospholipase C was investigated. Free myristic acids released by phospholipase A2, and myristic acids of dimyristoyl glyceride released by phospholipase C were methylated and determined quantitatively by gas-chromatography. Phospholipase A2 or phospholipase C released myristic acids dose-dependently from the substrate. Coenzyme Q10 prevented dose-dependently the hydrolysis of the substrate caused by phospholipase. These results suggest that pharmacological action of coenzyme Q10 could be attributed to its protection of membrane phospholipids against the attack of phospholipases.

[A case of hydrometrocolpos with left renal agenesis].

Herein we report a case of hydrometrocolpos with left renal agenesis of a four-month-old girl. The hydrometrocolpos due to a transverse septum of the vagina and left renal agenesis were confirmed by laparotomy. The transverse septum was incised and cured successfully by the abdominoperineal approach. We have collected 21 cases of hydrometrocolpos with or without urinary tract malformations including our case in the Japanese literature and discussed its etiology and treatments.

[Persistent Müllerian duct syndrome with seminoma: report of a case].

Persistent Müllerian duct syndrome is a male hermaphroditism in which remnants of Müllerian ducts occur from abnormality of Müllerian inhibitory factor and often cause transverse testicular ectopia or hernia uteri inguinalis. A 22-year-old-man was admitted with the chief complaint of painless swelling of the right scrotal content. Preoperative diagnosis was right testicular tumor and left undescended testis. High orchiectomy was done. At operation left transverse testicular ectopia and a remnant Müllerian duct was found incidentally. The right testicular tumor was seminoma pT1. Sixty-seven cases of transverse testicular ectopia have been reported in Japan, 37 of which are reports of a remnant Müllerian duct. Ten of these cases were accompanied by tumors. The pathogenesis and treatment of persistent Müllerian duct syndrome were discussed with review of the literature.
