Superior mesenteric artery syndrome:--report of four cases.

Four patients of SMA syndrome presented in the past 3 years. One case was treated surgically and three others were treated conservatively. Although it is rare, strong suspicion is advocated in evaluating patients with long-standing vague abdominal complaints. Diagnosis is made by upper gastrointestinal barium study and SMA angiogram. However, mild distention of the duodenum may be overlooked in barium meal G.I. series. The air inflation test has been of value in recognizing this syndrome. It has been recognized that there are acute and chronic forms. Peptic ulcer is often associated with SMA syndrome, especially in the chronic form. Conservative treatment must be stressed and performed initially. Surgical intervention is indicated if medical treatment fails. Division of the ligament of Treiz to free the bound position of the duodenum and make the duodenojejunal flexure downward is advocated first during surgery. If the result is not satisfactory the duodenojejunostomy is necessary.

[Immunological study on localized liver injection of immunopotentiator OK-432-- experimental studies in rats].

Using Charles RIver-SD 7-week-old rats, a study was carried out on the immunological effect of localized liver injections of OK-432 (Picibanil, Chugaipharma ceutical Co. Led., Tokyo, Japan). The subjects were categorized into groups injected with saline solution, PIC-0.5 KE/kg, PIC-1.0 KE/kg, and PIC-2.0 KE/kg. Measurements were undertaken prior to injection, on the first day and on the fifth day of injection through cardiac blood sampling to determine the absolute counts of T-cells, B-cells, helper T-cells and suppressor T-cells. On comparing these values in the saline group to the OK-432 groups prior to injection, no significant changes were observed on the first day. By the fifth day, however significant differences were evident between the changes which had occurred in the two groups. When changes in the saline group were studied prior to localized liver injection on the fifth day, there were no significant differences in the absolute counts of T-cells, B-cells, Th-cells and Ts-cells/lymphocytes. In the PIC-0.5 group the T-cells, B-cells, Th-cells and Ts-cells remained basically unchanged, but, a reduction in Ts-cells/lymphocytes (p less than 0.05) was observed. Although there were changes in the absolute counts of T-cells, B-cells and Th-cells in the PIC-1.0 group, reductions in the levels of Ts-cells (p less than 0.05) and Ts-cells lymphocytes (p less than 0.01) were observed. The PIC-2.0 group showed a decline in the Ts-cells (p less than 0.05) and Ts-cell/lymphocyte counts (p less than 0.01) with no changes in the T-cells, B-cells and Th-cells. As shown above, when the liver was injected with saline no immunological changes were recognized, while on the other hand, reductions in the Ts-cells and Ts/lymphocyte counts were ascertained when OK-432 was injected.

Early ultrastructural changes in choroidal arterioles of spontaneously hypertensive rats.

Ultrastructural changes in the vascular smooth muscle cells of spontaneously hypertensive rats (SHR) aged 7-9 months were studied in the long posterior ciliary artery (LPCA) and they were compared with the findings in the same aged control Wistar Kyoto rats (WKY). The systolic blood pressure was higher than 200 mmHg in the SHR at the postnatal fourth month. The muscle cells in the media of the LPCA in the SHR could be divided into three types according to the electron density of their sarcoplasm, i.e., dark, intermediate or light cells. The electron density depended on the content of actin filaments; the number of the filaments per one micron 2 was determined in the specimens where cross sections of the filaments prevailed, and it averaged 48.7 in the dark cells, 34.1 in the light cells. In WKY, all the muscle cells showed the intermediate density and the number of actin filaments averaged 37.9 per micron 2. It was also observed that the muscle cells migrated into the intima and they were called the intimal cells. These cells in SHR appeared as an aggregated mass underneath the endothelium, but in WKY the individual intimal cells were scattered. These findings were thought to be early ultrastructural changes in response to a rapid elevation of the arterial pressure.

Adenocarcinoma arising at a colostomy site. Report of a case.

Primary adenocarcinoma arising at a colostomy or ileostomy site is very rare, and only three cases have been reported following surgical treatment of ulcerative colitis. A 38-year-old man who developed an adenocarcinoma originating at the site of a colonic stoma and producing a large fungating tumor mass in the surrounding abdominal wall is described. This occurred 19 years after Miles' operation for a carcinoma of the rectum.

[Simultaneous determination of multiple elements in airborne particulate samples by X-ray fluorescence spectrometry].

An energy dispersive X-ray fluorescence spectrometer (EDX) has been applied to determine multielements in the workplace air. The standards for X-ray fluorescence analysis were prepared by the chelate precipitation method on polyvinyl chloride (PVC) membrane filter. And, the specimens were prepared to deposit various metal compounds of different chemical forms by the suspension method on PVC membrane filter, and they were determined with EDX and atomic absorption spectrometer (AAS). The results obtained were as follows. Though there is a difference by each element, an amount less than 3 microgram/cm2 per unit area makes it possible to undergo multielement analysis, that is, is has no influence on fine particle effect (particle size; under 5 microns). Then, effects of the X-ray intensity by different chemical forms are negligible. At the presence the neighboring element and other elements this technique showed greater precision by carrying out on corrective treatment, etc. The coefficient of variation of this technique was in the range of 2.5-6.5% at DDTC-Cu of 0.5-5.0 micrograms/cm2, with the limit of detection for As : 0.002 microgram/cm2, Zn : 0.003 microgram/cm2, Pb : 0.003 microgram/cm2, Cu : 0.004 microgram/cm2, Ni : 0.003 microgram/cm2, Fe : 0.005 microgram/cm2, Mn : 0.008 microgram/cm2, Cr : 0.013 microgram/cm2, respectively. Aerosols collected at the workplace were analyzed with EDX and AAS, and the obtained results showed good agreement with such regression line as y = 1.04 chi + 0.04, the coefficient of correlation being r = 0.995. From these results, this technique was found to be a very excellent method for monitoring of multielements in the workplace air.

[Case of hepatocellular carcinoma with a marked reduction in the tumor size induced by PSK administration alone].

A 56-year-old man was admitted to our hospital because of right abdominal pain. The edge of the liver was felt 4.5 fingers breadth below the xiphoid process. AFP was 20100 ng/ml and CEA was 5.6 ng/ml. The chest X-ray indicated existence of lymphangitis and some nodular density suggesting lung metastasis in the both lower-lung fields. 99mTc-phytate liver scan showed a large defect along the antero-inferior margin of the right hepatic lobe, which revealed an abnormal uptake of 67Ga-citrate. Ultrasonograms demonstrated a solid mass, 8 X 9 cm, in the right lobe of the liver. A CT-scan of the abdomen also showed a large, rounded, low attenuation mass with central necrosis in the right hepatic lobe: the pancreas and the remaining retroperitoneal structures appeared normal. Following the administration of PSK alone, 3 g daily, for three months, a remarkable regression of both hepatomegaly and lung metastasis was observed. Liver scan, ultrasonograms and CT-scan showed a striking resolution of the intrahepatic mass except central necrosis. AFP decreased to 33.7 ng/ml and CEA was 8.2 ng/ml. After about one year, however, ultrasonogramms showed a newly growing solid mass, 3.5 X 3.5 cm, in the left lobe of the liver. A needle biopsy specimen was taken from the intrahepatic mass, and it was interpreted as hepatoma. He is now healthy.

[Phase II study with methyl-6[[[2-chloroethyl) nitrosoamino] carbonyl] amino]-6-deoxy-alpha-D-glucopyranoside (MCNU) in hematological malignancies].

A total of 117 cases with hematological malignancies were treated with MCNU at doses of 70-100 mg/m2. Following are the results obtained. 1. MCNU showed a marked depression of cells in the cases with CML, polycythemia vera and thrombocythemia. The low level of cells was maintained for 2 to 7 months. 2. A good response was observed in several cases with blastic crises of CML. 3. No response was observed in two cases with acute leukemia. 4. Although a fair response was observed in several cases with malignant lymphoma or multiple myeloma, moderate bone marrow suppression was observed in a majority of the cases.

Nephropathy induced in rats by repeated intravenous injections of rabbit anti-FX1A antibody.

Repeated intravenous injections of small amounts of rabbit anti-rat FXIA antibodies (RAFA) induced a renal damage with the morphological features of epimembranous glomerulonephritis with significant and persistent proteinuria. These findings indicated that repeatedly injected RAFA accumulated in glomeruli and that there seems to be a threshold in quantity of RAFA localized in glomeruli to induce renal damage with proteinuria.

Studies on the designing of chemotherapy for gastric cancer in man, based on the tumor tissue concentration of anticancer agents.

5-fluorouracil analogs investigated in this study include a combination of uracil and Ftorafur (UFT), Ethyl (+/-)-t-butoxy-5-fluoro-hexahydro-2, 4-dioxopyrimidine r-5-carboxylate (TAC-278), and 5'-deoxy-5-fluorouridine (5'-DFUR). In a total of 45 patients with gastric cancer, tumor tissue level of 5-fluorouracil (5-FU) was determined at 2, 4, 6, 8, and 12 hours following the oral administration of drug, using a resected stomach specimen as material. As a result, it was demonstrated that oral administration of 200 mg/m2 of UFT maintained above 0.05 microgram/g (minimum inhibitory concentration: MIC) of 5-FU in tumor tissues over 12 hours in 11 of 13 patients. On the contrary, 133 mg/m2 of TAC-278, and 200 mg/m2 or 300 mg/m2 of 5'-DFUR (which is activated by thymidine phosphorylase in man) did not produce an effective 5-FU concentration in tumor tissues. Serum 5-FU level was high in order of TAC-278, UFT, and 5'-DFUR. Clinical response rates obtained with UFT (200 mg/m2 twice a day), or TAC-278 (133-200 mg/m2, 3 times daily) were 27.5% (49 of 178 cases), and 8.3% (3 of 36 cases, by Koyama et al.), respectively. Fisher's direct probability test revealed that there was a significant difference (p less than 0.05) between them in the response rate. It was considered that the measurement of concentration of anticancer drugs in tumor tissues might provide us useful information for the designing of chemotherapy of gastric cancer.
