[An immunological study on the effect of simultaneous intrahepatic injection of an immunomodulator (OK-432) and adriamycin--an experiment in rats].

An immunological study of the effect of a simultaneous intrahepatic injection of adriamycin (ADM) and OK-432 (PIC) in seven-week-old S.D. rats was carried out. Three experimental groups included an ADM-PIC-0.5 group which was administered 0.5 mg/kg ADM and 0.5 KE/kg PIC, an ADM-PIC-1.0 group which was administered 1.0 mg/kg ADM and 1.0 KE/kg PIC and an ADM-PIC-2.0 group which was administered 2.0 mg/kg ADM and 2.0 KE/kg PIC. A control group was injected with normal saline. The numbers of T-cells, B-cells, helper T-cells and suppressor T-cells were counted before, 1 day after and 5 days after intrahepatic injection. The suppressor T-cell count decreased (p less than 0.05) in the ADM-PIC-0.5 group and the T-cell count (p less than 0.05), B-cell count (p less than 0.05), helper T-cell count (p less than 0.01), suppressor T-cell (p less than 0.05) and the ratio of suppressor T-cells among lymphocytes were decreased in the ADM-PIC-1.0 group. Decreases in the T-cell count (p less than 0.01), B-cell count (p less than 0.05), helper T-cell count (p less than 0.01), suppressor T-cell (p less than 0.01) and the ratio of suppressor T-cells among lymphocytes (p less than 0.01) were recognized in the ADM-PIC-2.0 group one day after injection. Five days after injection, the ratio of suppressor T-cells among lymphocytes (p less than 0.01) was decreased in the ADM-PIC-0.5 group, and suppressor T-cells (p less than 0.05) and the ratio of suppressor T-cells among lymphocytes were decreased in the ADM-PIC-1.0 group. In the ADM-PIC-2.0 group, suppressor T-cell count (p less than 0.01) and the ratio of suppressor T-cells among lymphocytes were decreased. The result suggested that a simultaneous intrahepatic injection with PIC could improve the immunosuppression produced by an intrahepatic injection of ADM.

[Immunological study on localized intrahepatic injection of Adriamycin--experimental study in rats].

Using Charles River-SD 7-week-old rats, a study was carried out on the immunological changes resulting from localized intrahepatic injection of Adriamycin (ADM). The subjects were categorized into groups injected with either saline solution, ADM-0.5mg/kg, ADM-1.0mg/kg or ADM-2.0mg/kg. Measurements were undertaken prior to injection, on the first day and on the fifth day after injection through cardiac blood sampling to determine the absolute counts of T-cells, B-cells, helper T-cells, and suppressor T-cells. On comparing these values in the saline group to the ADM groups prior to the ADM groups prior to the first day. By the fifth day, however, significant differences were evident between changes which had occurred in the two groups. When changes in the saline group and the ADM-0.5 group were studied prior to localized intrahepatic injection on the fifth day, there no significant differences. The ADM-1.0 group and the ADM-2.0 group showed a decline in T-cells (P less than 0.05) with no changes in B-cells, Th-cells or Ts-cells. As shown above, when the liver was injected with saline, no immunological changes were recognized. On the other hand, reduction in T-cells was ascertained when ADM was injected.

[Clinical evaluation of cases of remission induction failure in acute myelogenous leukemia].

Among 73 patients who were admitted to our hospital between 1972 and 1983, 9 died before starting chemotherapy, 29 achieved complete remission (CR) with an initial regimen of induction chemotherapy, while the remaining 35 patients failed to achieve CR with this initial regimen. Patients with high initial WBC counts (greater than 100,000/cmm), old age, temperature (greater than 38 degrees C) and antecedent hematological disorders, were more prone to failure of remission induction therapy. Comparing CR patients and non-CR patients, there seemed to be no difference in sex distribution, initial platelet counts, initial LDH, percentage of blasts in the blood and bone marrow, the incidence of DIC and the regimen used for remission induction. According to Preisler's criteria, remission induction failure cases were further subdivided into various types and the numbers of each type were as follows: Type I (absolute drug resistance) 1, Type II (relative drug resistance) 16, Type III (regeneration failure) 0, Type IV (hypoplastic death) 6 and Type V (early death) 11. Among 18 M1 cases, 8 were Type II or Type V. Four out of 7 M2 cases belonged to Type II and all 3 M3 cases were Type V. Six out of the 35 remission induction failure patients entered CR with the 2nd chemotherapy regimen and 19 patients died during induction treatment with the initial induction regimen. Causes of death were infection (5 cases), bleeding (6 cases), infection and bleeding (3 cases) and organ failure (5 cases).

[Clinical study on concentration of FT-207 and 5-FU in serum, lymph nodes and tissues after rectal administration of FT-207 suppositories for malignant diseases of the liver, biliary tract and pancreas].

In order to study the antitumor effect of FT-207 in a solid tumor, it is necessary to determine the concentration of 5-FU and FT-207 in a tissue. This has only been done so far for gastric cancer and colon cancer, but these has been practically no research carried out regarding cancers of the liver, biliary tract and pancreas. A study was therefore made of lymph nodes and tissues after rectal administration of FT-207 suppositories to 12 patients with cancers of the liver, biliary tract and pancreas. These included 7 cases of pancreatic cancer, 2 cases of gall bladder cancer with infiltration to the liver, and 3 cases of hepatoma. In serum, the concentration of 5-FU reached 0.018 +/- 0.006 micrograms/ml at one hour after administration, 0.019 +/- 0.004 micrograms/ml at three hours after administration, and 0.023 +/- 0.008 micrograms/ml at six hours after administration. These concentrations would be expected to maintain a clinically sufficient dose. The concentration of 5-FU in metastatic lymph nodes was high compared with normal lymph nodes (p less than 0.05), its concentration in liver tumors was high while compared with normal liver tissues (p less than 0.05).

The effect of blood pressure on inner ear blood flow.

We investigated the effects of blood pressure on the blood flow in the inner ear as compared with other organs in normotensive rats (NR), spontaneously hypertensive rats (SHR) and renal hypertensive rats (RHR). The cardiac output and the regional blood flow were measured by a radioactive microsphere technique in conscious rats. The haemodynamics of the inner ear and other organs and the effects of a converting-enzyme inhibitor (SA-446) on the regional blood flow were compared among NR, SHR, and RHR, the blood pressure being 107, 152, and 185 mm Hg, respectively. With the exception of the heart, the organ blood flow was lower in SHR than in NR in all organs, including the inner ear. It was also lower in RHR than in SHR. The blood pressure dropped after i.v. injection of SA-446, while the regional blood flow increased. The effect was strongest in RHR, followed by SHR and finally NR.

Renal excretion of an angiotensin I converting enzyme inhibitor (SA-446) in dogs.

The renal excretory mechanism of an orally active inhibitor of angiotensin converting enzyme (SA-446) was examined in anesthetized dogs. Parenteral administration of this compound resulted in production of constant levels of about 2 mg/l in the plasma (PSA) and the urine concentration was 726 +/- 200 mg/l, a level significantly higher than that in the plasma. Renal clearance of SA-446 (CSA) was 2.24 +/- 0.34 ml/g X min and was significantly higher than GFR. The clearance ratio (CSA/GFR) of over 1.0 was indicative of a net tubular secretion. Administration of probenecid resulted in a significant rise in PSA and in a significant decrease in urinary excretion but with no change in the plasma protein binding ratio. CSA decreased significantly from 2.24 +/- 0.34 to 0.71 +/- 0.14 ml/g X min. The inhibitory action of SA-446 (0.02 mg/kg, i.v.) on the pressor response to angiotensin I disappeared at about 50 min, this action being maintained for about 2 h in the probenecid pretreated dog. Since probenecid is a competitive inhibitor of organic anion secretory transport, our results show the net tubular secretion of SA-446, via organic anion transport systems. Prolongation of the action of SA-446, as induced by probenecid may be due to the increase of plasma concentration, by the inhibition of tubular secretion.
