Granulocytapheresis as a possible cancer treatment.

We assessed the effect of granulocyte apheresis in patients exhibiting increased granulocyte-to-lymphocyte ratio in order to overcome granulocytosis occurring in the terminal stages of malignancies. 17 patients with post-operative recurrent metastatic tumors including 6 gastric, 3 colonic, 2 rectal, 1 esophageal and 5 breast cancers were selected. The granulocytapheresis was performed by extracorporeal vein-to-vein circulation equipped with an apheresis column filled with cellulose acetate beads. Each week the patients underwent one or two sessions of treatment that lasted 30 to 50 minutes per session at a flow rate of 30 to 50 ml/min. 15 sessions formed 1 therapeutic cycle. The effect of granulocytapheresis resulted in partial response (PR) in 4 cases, no change (NC) in 7 cases and partial disease (PD) in 6 cases. The performance status showed 30% remission. None of the patients exhibited significant side effects. Since the treatment demonstrated anti-tumor effects, granulocytapheresis may be applied during combined cancer treatments.

[Alteration in ventricular gradient at rest in patients with ischemic heart disease].

The possibility of using alteration in the ventricular gradient (VG) to discern the presence of ischemic heart disease was studied in 30 patients with effort angina pectoris(AP), 21 with vasospastic angina (VSA), 21 with chest pain syndrome (CPS), and 20 healthy volunteers (control). The VG of each consecutive heart beat over a 22-sec interval was calculated by microcomputer from resting Frank-lead X, Y, Z scalar electrocardiograms. The mean values and standard deviations (SD) for the azimuth, elevation, and magnitude of the VG in each group were calculated. The SD and SD/mean ratios for each parameter were used as indices of VG alteration in the groups, and the indices were compared. The SD and SD/mean for the magnitude and elevation of VG were significantly greater in the AP group than in the CPS and control groups. The SD for the azimuth of VG was significantly greater in the AP group than in the CPS and control groups (p < 0.01). The SD and SD/mean for the magnitude of VG were greater in the AP group than in the VSA group (p < 0.01). The SD/mean for the elevation and magnitude of the VG were greater in the VSA group than in the control group (p < 0.01). The SD/mean of the magnitude of the VG was found to be the best index, as it was higher than the upper limit of the control group. The sensitivity and specificity were 80 and 91% (AP vs CPS, p < 0.001), and 43 and 91% (VSA vs CPS, not significant), respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Pharmacokinetic study of p-chloronitrobenzene in humans suffering from acute poisoning.

The pharmacokinetics of p-chloronitrobenzene (p-CNB) in human subjects suffering from acute poisoning was studied from the urinary excretion of p-CNB metabolites. The time course for excretion of five metabolites--2-chloro-5-nitrophenol,N-acetyl-S-(4-nitrophenyl)- L-cysteine, 2,4-dichloroaniline, p-chloroaniline, and 2-amino-5-chlorophenol-were determined by reversed-phase HPLC analysis of urine collected from six subjects during their hospitalization after accidental exposure to p-CNB. The cumulative excretion amount at infinite time (X infinity) and mean residence time for each metabolite were estimated by moment analysis of excretion rate vs. the time curve. The rate constants for p-CNB metabolism were calculated by nonlinear least squares fitting of the time course data using a one-compartment model. The results indicated that the average values of the ratio of excreted amount of each metabolite to the total amount of the five metabolites (X infinity metabolite/X infinity Total) and mean residence time in the six subjects were, respectively: 12.2% and 6.7 days for 2-chloro-5-nitrophenol, 48.0% and 7.0 days for N-acetyl-S-(4-nitrophenyl)-L-cysteine, 1.2% and 3.7 days for 2,4-dichloroaniline, 29.9% and 10.0 days for p-chloroaniline, and 8.7% and 6.0 days for 2-amino-5-chlorophenol. The average values of rate constants in the metabolism of p-CNB to 2-chloro-5-nitrophenol, N-acetyl-S-(4-nitrophenyl)-L-cysteine, and p-chloroaniline were 0.021, 0.082, and 0.067 day-1, respectively. Similarly those in further metabolism of p-chloroaniline for C-chlorination, C-hydroxylation, and N-conjugation were 0.205, 1.206, and 3.526 day-1, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Transcatheter arterial chemoembolization through collateral arteries for hepatocellular carcinoma after arterial occlusion.

PURPOSE: The purpose of this study was to evaluate the efficacy of transcatheter arterial chemoembolization (TACE) through collaterals after hepatic artery occlusion or severe stenosis in patients with hepatocellular carcinoma (HCC). MATERIALS AND METHODS: During the past three and one-half years, 19 of 167 patients with HCC showed hepatic artery occlusion or severe stenosis after repeated TACE, and 17 patients underwent TACE through collateral arteries. Types of collaterals, results, and effects of TACE through collaterals were evaluated. RESULTS: Common collaterals were periportal collateral, intrahepatic anastomosis, and right inferior phrenic artery. TACE was successfully carried out through all detected collaterals in 40 of 49 TACE trials (82%). In eight of 49 TACE trials (16%), TACE through collaterals was partially carried out. The cumulative survival rates at the end of the first, second, third, forth and fifth year were 100%, 79%, 79%, 59% and 59%, respectively. CONCLUSION: TACE through collateral arteries was able to be performed in most cases and will be an effective treatment to control HCC.
