[Psychological study of questionnaire relating to dentition and orthodontic treatment].

Concern of patients for dentition and receptivity of orthodontic treatment was studied by a general population questionnaire. Subjects (general population) consisted of elder elementary school boys and girls (157), junior high school (274) and high school students (90) and adults (45) in Tokushima, Kagawa and Kouchi prefectures. Twenty-three questions, previously selected from forty-seven using the Quantification III for orthodontic patients. Furthermore, ten questions were selected from these twenty-three using the same method and categorized into two groups. Each category was given a meaning. Five answers in each category were given marks: a positive answer was 'point one' and a negative one was 'point zero'. Mean points in each category were compared with eight groups according to age and sex. AIC (Akaike Information Criterion) was used for statistical analysis. The following results were obtained. 1. Two categories were independently identified. One had meaning in relation to concern for dentition and other had meaning in relation to receptivity of orthodontic treatment. 2. Orthodontic patients showed significantly higher points than general population for concern and receptivity. 3. Females in each group showed higher points for concern of dentition in general population, but males showed higher points for receptivity of orthodontic treatment. 4. Junior high school and high school students showed significantly lower points in two categories, in addition, males in this group showed lower points in these categories. 5. There were no significant differences between subjects in three prefectures. It is possible for general population to evaluate instructions or education methods for promoting interest in dentition by this study. Orthodontic patients will also be encouraged by the present study to receive orthodontic treatment.

Corticonuclear and corticovestibular projections from the uvula in the albino rat: differential projections from sublobuli of the uvula.

The organization of the corticonuclear and corticovestibular projections from the uvula was investigated in the albino rat by an autoradiographic method. The corticonuclear fibers from sublobule a of the uvula terminated in the caudoventral part of the medial cerebellar nucleus, and the caudomedial part of the posterior interpositus nucleus with mediolateral topography. The medial and lateral portions of the sublobule projected to the medial cerebellar and posterior interpositus nuclei, respectively. The corticovestibular fibers from sublobule a terminated in the dorsal and rostral parts of the superior vestibular nucleus, the dorsal part of the lateral vestibular nucleus, and the caudomedial part of the spinal vestibular nucleus. However, the corticonuclear fibers from sublobuli b and c of the uvula terminated additionally in the ventromedial part of the lateral cerebellar nucleus, while the corticovestibular fibers from these sublobuli terminated additionally in the subnucleus y of the vestibular complex, with probable termination in the medial vestibular nucleus. The cortical region which sent efferent projections to the ventromedial part of the lateral cerebellar nucleus and the subnucleus y was located laterally in sublobuli b and c of the uvula. These differential projection patterns from the dorsal and ventral sublobuli suggest the difference of the functional correlates between the sublobuli in the uvula.

A new HPLC fluorimetric method to monitor urinary delta-aminolevulinic acid (ALA-U) levels in workers exposed to lead.

A new sensitive HPLC method for the determination of urinary delta-aminolevulinic acid (ALA-U) was used to evaluate the relationship between blood-lead (Pb-B) and ALA-U levels in male workers exposed to lead. The differences between the ALA-U levels determined by this method (ALA-U-HP) and by a colorimetric method (ALA-U-CL) are discussed. The HPLC method gave values similar to the ALA-U-CL values at high ALA-U level. However, at low blood-lead levels (58 +/- 22 micrograms/l, n = 23), the mean ALA-U-HP level corrected by urinary creatinine level was one-third of the corrected ALA-U-CL level (0.83 +/- 0.14 and 2.4 +/- 0.5 mg/g creatinine, respectively). A significant increase of the mean corrected ALA-U-HP level was observed at 162 +/- 22 micrograms/l Pb-B (P less than 0.05, n = 26), while that of ALA-U-CL was observed at 245 +/- 30 micrograms/l Pb-B (P less than 0.01, n = 37). The regression equation based on the logistic model fitted well to the relationship data between the Pb-B level and the percentage of the subjects with corrected ALA-U-HP above the cut-off point (1.12 mg/g creatinine) and the expected Pb-B level for 50% response was 270 micrograms/l Pb-B, while it did not fit well to the relationship data between Pb-B level and the percentage of the subjects with corrected ALA-U-CL above the cut-off point (3.5 mg/g creatinine).(ABSTRACT TRUNCATED AT 250 WORDS)

Interregional income differentials and migration: their interrelationships.

"The major objective of this paper is to demonstrate that interregional income differential is exogenous to interregional net migration, but not conversely. Utilizing Japanese prefectural-based data for 1954-82, we conduct the following three kinds of analyses to confirm this statement. Firstly, a simple comparison of the crests of interregional time-series plots was made to see which occurred first. Secondly, Sims', 1972, test of causality was done to investigate causation between urban/rural income differential and net migration. Finally, per capita income and population share were examined by two-dimensional phase diagram for nine regions." (SUMMARY IN FRE AND GER)

Topographic organization of the corticonuclear and corticovestibular projections from the pyramis and copula pyramidis in the albino rat. An autoradiographic orthograde tracing study.

The topographic organization of the corticonuclear and corticovestibular fibers from the pyramis and copula pyramidis in the albino rat was investigated by the autoradiographic orthograde tracing technique. Cortical efferent fibers from the medial part of the pyramis project to the caudoventral part of the caudomedial and middle subdivisions of the medial cerebellar nucleus, whereas those arising from the lateral part of the pyramis project to the caudomedial part of the posterior interpositus nucleus and also to the dorsal part of the lateral vestibular nucleus via the juxtarestiform body. Fibers from the medial part of the copula pyramidis project to the medial part of the anterior interpositus nucleus. On the other hand, fibers from the medial portion of the lateral part of the copula pyramidis project to the posterior interpositus nucleus, while those from the lateral portion, including the lateral limit of the copula pyramidis, project to the rostroventral part of the lateral cerebellar nucleus. These results indicate that the cortical efferent projections from the pyramis and the copula pyramidis are clearly oriented mediolaterally in the albino rat.

[Assessment of occupational exposures to industrial hazardous substances. II. Interday fluctuations of the daily exposure averages among workers exposed to lead].

An assessment of the employee's exposures to industrial hazardous substances using the proposed method described earlier was conducted on 49 workers exposed to lead. As it is assumed that the daily exposure averages are lognormally distributed, geometric standard deviation (sigma g) representing true interday fluctuations of the daily exposures was estimated by personal exposure measurements of every worker for two consecutive days. The estimates (Sg) ranged from 1.00 to 5.35 with a median of 1.4 and a 90% upper limit of 2.4. According to a classification scheme in the proposed method, exposure levels (I to III) were calculated using sigma g of 1.4 and 2.4. An exposure class based on a single day measurement was evaluated and compared to that based on measurements for two consecutive days. As a result, the decision of the exposure levels and classes from only one day monitoring could be made by using both sigma g of 1.4 and 2.4, representing ordinary and high interday fluctuations, respectively. More accurate estimate of geometric standard deviation of interday fluctuations by exposure monitorings would provide a more reliable assessment of the worker's long-term exposure situation.

A new orthodontic force system of magnetic brackets.

Tooth movement in orthodontic treatment is achieved by a combination of mechanical and physiologic forces acting on the teeth and their supporting structures to effect the biologic changes that cause teeth to move through bone. Traditional orthodontic appliances using wires, springs, and elastic forces inevitably cause some degree of discomfort and pain. In the present study, a new magnetized edgewise bracket was designed. The magnet works to move teeth mesiodistally and is not merely a mechanical force. It is well adapted to the biophysical system of tooth and bone interrelationships. Therefore, it works effectively, gives less discomfort, and produces less stress for the patient. The new magnetic appliance provides a force system that approaches the ideal orthodontic requirements.

[An interview survey on health care service in small-sized manufacturers in a city of Osaka Prefecture].

An interview survey on the health care service for workers employed in small-sized enterprises was conducted in a city of Osaka Prefecture. Items surveyed were: 1) number of full-time and part-time employees by age (less than 39, 40-60, and more than 60 yr) and sex, 2) materials and production process, 3) periodic examinations for general health status and occupational diseases, and others. Of the 526 enterprises interviewed, 459 manufacturers with less than 50 employees were analyzed. The results can be summarized as follows: 1. Of the 459 manufacturers, 72.4% were those with less than 10 workers (the mean number of employees: 8.4). The major type of manufacture was electric parts processing industry. 2. The period health examination has been carried out in 372 enterprises (81.0%). The smaller the size of the company, the lower was the rate of the periodic health examination. 3. In 77.4% of the 372 enterprises, only chest X-ray was an item for the periodic health examination, which was usually carried out at the public health center of the community. 4. The number of enterprises conducting the periodic health examination designated by the Industrial Safety and Health Law (1972) was only 49 (10.7%). 5. Fourteen enterprises (3.1%) have carried out periodic examination for occupational diseases. 6. The periodic examination for degenerative diseases such as hypertension, diabetes mellitus, and other cardiovascular diseases was carried out in 57 enterprises (12.4%). The larger the size of the company, the higher was the rate in which such examination was conducted.(ABSTRACT TRUNCATED AT 250 WORDS)

Randomized controlled study of postoperative adjuvant immunochemotherapy with Nocardia rubra cell wall skeleton (N-CWS) and Tegafur for gastric carcinoma.

We performed a randomized controlled study of postoperative adjuvant immunochemotherapy with Nocardia rubra cell wall skeleton (N-CWS) and Tegafur for gastric carcinoma between September 1979 and March 1983. A total of 309 patients were entered into this trial. Of the 309 patients, there were 98 evaluable patients in the chemotherapy group and 115 evaluable patients in the immunochemotherapy group. In both groups, Tegafur was given as chemotherapy at a daily dose of 400 to 800 mg, starting at 24-29 days after gastrectomy. In the immunochemotherapy group, 400 micrograms of N-CWS was injected i.d. within the 2nd postoperative week. It was given weekly during the first month and subsequently monthly for as long as practicable. The patients were surveyed for length of survival in March 1985. The postoperative survival rate was analyzed for all cases, and for patients with various histopathological stages of carcinoma for comparison between the two treatment groups. No statistical difference was detected between the two groups in terms of age, sex, surgical curability, or stage of carcinoma. The overall survival rate for all patients was significantly higher in the immunochemotherapy group than in the chemotherapy group (p less than 0.05). With stage III plus IV disease, 53 patients from the chemotherapy group and 61 patients from the immunochemotherapy group were included for the analysis. As a consequence, a highly significant survival rate was observed in patients with stage III plus IV carcinoma in the immunochemotherapy group (p less than 0.005) as compared to the chemotherapy group. The overall 5-year (1800 days) survival rate after surgical treatment was 60.2% for the chemotherapy group and 73.2% for the immunochemotherapy group. In patients with stage III plus IV disease, the 5-year survival rates of the two treatment groups were 28.8% and 52.4%, respectively. Accordingly, the 50% survival period of patients with stage III plus IV cancer was 1800 days or more in the immunochemotherapy group, whereas it was only 722 days in the chemotherapy group. These results emphasize the effectiveness of N-CWS as an adjuvant immunotherapeutic agent in postoperative gastric cancer patients. The main side effects of N-CWS were skin lesions in the injected sites and fever, but these were temporary and not serious.

Cerebellar corticonuclear and corticovestibular fibers from the posterior lobe of the albino rat, with comments on zones.

The topographic organization of cerebellar cortical efferent fibers from the posterior lobe of the albino rat was studied by silver impregnation methods. The corticonuclear fibers from the vermis of the posterior lobe project to the caudomedial and middle parts of the medial cerebellar nucleus (MN) with medio-lateral localization; fibers from the medial portion of the vermis project to the caudomedial part of the MN (MNcm) and those from the lateral portion project to the middle part of the MN (MNm). Corticovestibular fibers originate in the caudal vermis lateral to the area projecting to the MNcm and MNm, and terminate in the dorsal part of the lateral vestibular nucleus (LVN). The origins of corticonuclear fibers to the dorsolateral protuberance of the MN (MNdlp), the posterior interpositus nucleus (PIN) and the anterior interpositus nucleus (AIN) are located latero-medially in the intermediate cortex. Fibers from the area adjacent to the vermis and rostral to the prepyramidal fissure project to the MNdlp, while those from the area lateral to the origin of the corticovestibular fibers and caudal to the fissure project to the medial AIN. Together, these areas comprise a medial portion of the intermediate cortex. Corticonuclear fibers to the PIN and lateral AIN originate from the lateral portion of the intermediate cortex. The corticonuclear fibers to the dorsolateral hump and lateral nucleus originate from the medial and lateral portions of the lateral cortex, respectively.

[Facial nerve schwannomas: report of two cases].

Although schwannoma may arise from any true cranial nerve, it is well known that the acoustic and trigeminal nerves are frequent origins. Schwannomas of the facial nerve are rare. In this communication, two cases of facial nerve schwannoma arose from the horizontal portion are reported. Case 1. A 44 year-old male was admitted to our clinic complaining of left hearing loss and facial asymmetry. About three years prior to the admission, he first noticed left hearing disturbance which was gradually deteriorated and was transiently accompanied by left tinnitus during the progression. Then muscle spasm developed on the left eyelid which resolved spontaneously in a few weeks, then asymmetry of the face developed. Neurological examination on admission revealed left hearing loss and left peripheral facial palsy. As a result of neuro-otological examinations, left hearing impairment and left facial palsy were thought to be due to retrocochlear and suprageniculate lesions respectively. Plain skull radiograms and tomograms revealed marked destruction of left pyramis and enlarged internal auditory canal. Computed tomography of the brain showed low-density mass in the left middle fossa and defect of the tip of the left pyramis. After bolus injection of contrast material, peripheral portion of the middle fossa mass was enhanced non-homogeneously and enhanced mass extended to the posterior fossa. Left middle and posterior fossas were explored by carrying out a osteoplastic temporal flap and suboccipital craniectomy. A large extradural mass was noted to have filled the middle fossa which extended to the posterior fossa through destroyed pyramis and enlarged internal auditory canal. [Facial palsy following head injury: topognosis, prognosis and indications for surgical decompression].

It is a common experience that the facial nerve is affected in closed head injury. 781 patients with head injury were treated at Kobe Central Municipal Hospital over a period of 5 years from January 1977 to April 1982, and facial palsy occurred in 25 cases (3.2%). The male to female ratio was 22: 3 and age ranged from 9 to 78 (average 36). The patients were studied, using radiological, topognostic and electrodiagnostic methods. Facial nerve decompression was done in 5 cases and 20 cases were treated conservatively. Results were as follows. The overall rate of complete recovery was 64% (16 of 25 cases). In the conservative treatment group, all patients with incomplete paralysis and normal maximal stimulation test (MST) recovered completely. With complete paralysis, if the initial nerve excitability (MST) was normal, recovery could be expected by conservative treatment. Even if the patients had abnormal nerve excitability (MST) at first, complete recovery could be observed, when signs of recovery could be elicited and normalization of MST occurred within 3 weeks from the onset of the palsy. In severe cases, even with abnormal MST from the onset, complete or partial recovery can be expected after surgery. Therefore, microsurgical decompression of the facial nerve is indicated in the following cases: patients with complete paralysis and no response to MST from the onset. patients with complete paralysis and deteriorating response to MST 1 month after onset. Maximal stimulation test is a good guide for determining the necessity for surgical intervention.

Subcellular localization of the methylcitric-acid-cycle enzymes in propionate metabolism of Yarrowia lipolytica.

The subcellular localization of the four characteristic enzymes of the methylcitric acid cycle was studied with glucose-grown as well as n-alkane-grown cells of Yarrowia lipolytica. Microsomes and peroxisomes showed no cycle enzyme activities. The four cycle enzymes were constitutively localized in mitochondria, with the exception of the dual localization of the fourth enzyme, 2-methylisocitrate lyase, in mitochondria and cytoplasm, where the lyase may function to supply pyruvate (the end-product of the catabolism of the propionate residue) to various reactions.
