Leiomyosarcoma of the breast: a case of long survival and late hepatic metastasis.

A case of leiomyosarcoma of the breast that metastasized to the liver 15 years after mastectomy is reported. Leiomyosarcoma appears to have less aggressive biologic behavior than other types of breast sarcomas. Analysis of the reported cases of smooth muscle tumors of the breast revealed some guidelines for the histologic diagnosis of these tumors. Tumors with three or more mitoses per 10 HPF are leiomyosarcomas, and those with no mitotic activity, necrosis, and significant cellular atypia are leiomyomas. The borderline cases are best considered smooth muscle tumors with uncertain malignant potential.

Osteopetrosis: a scanning electron microscopic study.

Scanning electron microscopic investigation was conducted with bone in a case of osteopetrosis by comparing it with normal cortical bone. The fractured surface of the osteopetrotic bone was devoid of the orderly constructed lamellar haversian system seen in normal bone. It was occupied by a massive smooth cartilaginous matrix containing scattered rough areas of abnormal ossification. Tortuous channels were randomly distributed and corresponded to the abortive marrow spaces. The presence of many irregular fracture lines reflected the fragility of the bone. The findings support the concept of defective remodeling in the pathogenesis of osteopetrosis and provide a further explanation for the abnormal fragility of bone in osteopetrosis.

Epidermolysis bullosa letalis associated with congenital pyloric atresia.

A 32-week-old (gestational age) female infant with epidermolysis bullosa letalis (EBL) (confirmed by light and electron microscopy) had a gastric-outlet obstruction on routine roentgenographic examination. An autopsy showed a fibrous cord connecting the stomach and first part of the duodenum in the area of the pylorus. A review of the literature indicated 12 additional cases of epidermolysis bullosa (EBL where type was confirmed) associated with pyloric atresia. The possibility of coexistent pyloric atresia should be considered in a newborn who has suspected EBL.

Functional asplenia in Sezary syndrome.

A case of functional asplenia revealed by radiocolloid scanning is reported in a patient suffering from Sezary syndrome, a lymphoma of cutaneous origin. Heavy, diffuse sinusoidal infiltration of the spleen by the Sezary cells appears to be responsible for the functional asplenia in this case.

Multiple granular cell tumor.

Eleven cases of granular cell tumor were reviewed. In two of the cases multiple sites of involvement were seen. The tumor occurred in the oral cavity in both of these cases and each was initially wrongly diagnosed as squamous cell carcinoma. The most common site was the subcutaneous tissue (nine patients) and the tongue was involved in three cases. In one patient the parotid gland was involved. Eight of the patients were females and three were males; seven were black and four were white. The importance of differentiating between squamous cell carcinoma and granular cell tumor is stressed, as is the need for a simple wide surgical excision.

Characterization of deoxycytidylate methyltransferase in Xanthomonas oryzae infected with bacteriophage Xp12.

Three methods, chromatographic, spectrophotometric and tritium-release assay, were used and compared for the assay of deoxycytidylate methyltransferase. All three methods can be used for assay of this enzyme but the tritium-release assay appears to be the most simple and convenient. With the help of this assay the deoxycytidylate methyltransferase has been isolated and purified from sonically disrupted cells of Xp12-infected Xanthomonas oryzae. Using a procedure that involves fractionation with streptomycin sulfate and ammonium sulfate, filtration through Sephadex G-100 and chromatography on DEAE-cellulose, a 214-fold increase in specific activity was obtained. The enzyme displays a narrow pH optimum at 6.0 Among the buffers tested, 6-morpholinoethane sulfonate with the addition of Mg2 is the best. The enzyme can utilize dCMP as a substrate. The enzyme can also convert tetrahydrofolic acid into dihydrofolic acid. The Km value for dCMP is 31.3 micrometer and the Km value for tetrahydrofolic acid is 71.4 micrometer. There is no absolute requirement of ions for the activity of the enzyme; however, the presence of ions causes stimulating or inhibiting effects on enzyme activity that are dependent on the variety and concentration of ions used.

Observation of nervous tissue in a Wilms' tumor: its histogenetic significance.

A case of Wilms' tumor occurring in a horseshoe kidney is reported in which bona fide nervous tissue containing ganglion cells was observed. This finding cannot be reconciled with the currently held "metanephrogenic blastema" theory of the histogenesis of this tumor. Masson's concept of "neuroepithelial origin" of the Wilms' tumor should be re-evaluated.

Testicular relapse in childhood acute lymphocytic leukemia during bone marrow remission.

Seven children with acute lymphocytic leukemia developed testicular swelling during bone marrow remission. Needle biopsies and one orchiectomy specimen revealed leukemic infiltration. Another patient encountered concurrent medullary and testicular relapses. The leukemic nature of the testicular infiltration can be readily confirmed on needle biopsies. Orchiectomy should be discouraged. The leukemic infiltration mainly took place in the interstitial spaces. The seminiferous tubules were atrophic and lined by Sertoli cells. Occasional spermatogenic cells could be found in some cases. The leukemic cells were also observed to invade and accumulate beneath the Sertoli cell layer. Destruction of the tubules by the infiltrate was seen in advanced cases. Postradiation biopsies were characterized by fibrosis. The sigeveloped subsequent marrow relapse despite the effective control of the testicular relapse by irradiation. Another important observation is the high incidence of testicular relapse (16.2% so far).

Multiple sebaceous adenomas and internal malignant disease: a case report with chromosomal analysis.

A case of multiple sebaceous adenomas associated with internal malignant disease is further documented. Thirteen similar cases reported in the literature are also reviewed. We support the thesis that this association represents a unique syndrome. It can be seen in both sexes and usually is manifested during the fifth and sixth decades. A family history of malignant disease was revealed among seven of the nine cases mentioned. Of the other skin lesions, keratoacanthoma is also frequently seen. The internal malignant tumors most commonly afflict the gastrointestinal tract, especially the large intestine. With proper treatment the internal tumors appear to be of a low degree of malignancy. However, these patients have a propensity to develop muliple visceral tumors and require periodic follow-up. Some patients develop skin tumors prior to the discovery of internal malignant disease, an event that offers some prognostic value. Chromosomal study of the cultured blood lymphocytes by the Giemsa banding technique failed to reveal any abnormality.

Xf phage invading the host cells with their protein coats.

The Xf phage coat protein associated with infected cells could not be removed by washing with antiserum and tris-EDTA buffer. Although the infected cells were consecutively washed 6 times with tris-EDTA buffer, the ratios of parental phage 6H-DNA to 14C-protein were not changed. A considerable amount of the parental 14C-protein and 3H-DNA in the original ratio were detected in the membrane and the soluble cytoplasmic fractions of infected cells. The studies of the change in Xf 14C-protein and 3H-DNA incorporation into the host cells and their release showed that DNA and protein penetrate together into the host cells during the first 60 min after infection (p.i.). While virtually all parental DNA was conserved, re-utilized and released from the infected cell 60 min p.i., no apparent release of parental protein was observed. Approx. 40% of the parental protein became degraded and could be washed from the infected cell after 90 min; the rest of the parental protein remained and probably was re-utilized by the host.

Mapping of rfa Genes in Salmonella typhimurium by ES18 and P22 Transduction and by Conjugation.

Loci termed rfa, determining biosynthesis of somatic lipopolysaccharide core, have been mapped in Salmonella typhimurium LT2. The smooth-specific phage P22 co-transduced two leaky rfa alleles with cysE and with pyrE; one of the leaky alleles is perhaps rfaG, and the other is an unidentified gene concerned with synthesis of the heptose-containing part of the core. The lipopolysaccharide-indifferent phage ES18 (or its variant ES18.h1) co-transduced rfaF, rfaG, rfaL, rfa(R-res-1), and rfa(R-res-2) alleles with cysE and with pyrE, at rates indicating the order cysE-rfaF-(rfa[R-res-1], rfa[r-res-2], rfaL)-rfaG-pyrE. One proven (and two suspected) rfaE alleles and five proven rfaH alleles were not co-transduced with cysE or pyrE. Hfr crosses indicated that the proven rfaE allele mapped between serA and strA.
