Mucoepidermoid carcinoma of the thyroid gland.

An extremely rare case of mucoepidermoid carcinoma of the thyroid in a 56-year-old woman is presented. The patient clinically having Hashimoto's thyroiditis was noted a nodule in her neck. The tumor was sited in the midportion of the left lobe of the thyroid, and histologically it showed both squamous features and mucin production. The squamous cells were arranged in solid sheets with horny pearls and the mucous cells tended to line dilated duct-like elements. Ultrastructurally, the epidermoid cells had aggregates of tonofilaments and well-developed desmosomal attachments, and the mucous cells contained numerous mucin granules in their cytoplasm. Immunohistochemical studies revealed that cytokeratin antibodies showed positivity for both the lining cells and squamous cells, whereas carcinoembryonic antigen positivity was found in the lining cells and intraluminal material. The tumor cells were negative for thyroglobulin, calcitonin, vimentin, chromogranin, and neuron-specific enolase. These unusual histologic and immunohistochemical features are suggestive of a tumor related to the so-called "solid cell nest" of the thyroid.

Somatotopic organization and columnar structure of vibrissae representation in the rat ventrobasal complex.

The region of vibrissae representation in the ventrobasal complex (VB) of the rat was systematically mapped, based on receptive fields of many single neurons. Results showed that the ventralmost row of vibrissae projected to the rostral part of VB, that the dorsal-most row projected to the caudal part, and that the caudalmost vibrissae of each row projected to the most dorsolateral part of VB and more rostral vibrissae to the more ventromedial part. Further, it was revealed that the clusters of neurons receiving projections from any individual vibrissae formed corresponding columns extending from the anterodorsomedial to the posteroventrolateral direction, and that these columns piled up dorsoventrally and anteroposteriorly, with ventral ones shifted progressively medially. When cross sections of these columns were viewed on an oblique horizontal section of VB, a group of columns corresponding to each row lined up from the dorsolateral to the ventromedial direction with a rostral convexity, which means that the third or fourth vibrissa in each row projected most rostrally in that row. These results confirmed previous physiological mapping studies of vibrissal representation and are in good agreement with anatomical studies on barreloid structure in VB.

Observations of the sensing and the tectorial membrane in bullfrog amphibian papilla: their possible functional roles.

Three dimensional reconstructions of the amphibian papilla were performed with light microscopic observations, mainly for the sensing membrane (SM). In horizontal sections of the papilla, the anteromedial end of the SM, which makes contact with the massive anterior portion of the tectorial membrane (TM), is several times thicker than the posterolateral end close to the column of the innervating nerves. This gradient of thickness is observed in all the sections from the dorsal portion attached to the TM to the ventral floor of the papilla. The SM connects to the TM in a topological manner; the anteromedial portion of the TM relates to the anterior end of the SM and the anterolateral and the middle portions of the TM correspond to the sites shifting posteriorly on the SM. The morphology of the SM and its manner of connection to the TM suggest that the SM plays important roles in the occurrence of frequency selectivity and of tonotopic organization of the amphibian papilla.

[Pharmacological properties of (+/-)-4-[2-hydroxy-3-(3-(2-methoxyphenoxy)-2-propylamino)propoxy]-1(2H) -isoquinolinone (N-1518), a new combined alpha- and beta-adrenoceptor blocking drug].

The alpha, beta-adrenergic blocking, antihypertensive and vasodilating properties of N-1518 were compared with those of labetalol. N-1518 blocked alpha- and beta-adrenoceptors competitively as indicated by parallel rightward displacement of the dose-response curve of each agonist in isolated organs and in anesthetized dogs. As judged by pA2 values and DR10 values, N-1518 was as potent as labetalol in blocking alpha- and beta-adrenoceptors. The beta 1/alpha 1 ratio of N-1518 was 8.3 for pA2 values in isolated organs and 13.6 for DR10 values in anesthetized dogs, respectively. N-1518 inhibited dose-dependently the pressor response to intravenous administration of noradrenaline, but labetalol did not depress the response to noradrenaline in anesthetized dogs. N-1518 is composed of four optical isomers. The SR-isomer was the most potent in blocking beta-receptors, and the RR-isomer was the most potent in blocking alpha-receptors. N-1518 has no intrinsic sympathomimetic activity in reserpinized rats and has no local anesthetic activity in guinea pigs. Single oral administration of N-1518 produced a fall in blood pressure in conscious SHR and renal hypertensive dogs without causing tachycardia. Intra-arterially administered N-1518 in the dog hindlimb resulted in vasodilation as indicated by the increase in blood flow. The magnitude of the responses was approximately 3 times more potent than that of labetalol.

[Experimental development of a chitosan-bonded hydroxyapatite bone filling paste].

Chitosan, a naturally occurring high molecule or weight polymer which is stable in vivo like collagen, has proved a useful biomaterial in applications such as suture thread and artificial skin. A hydroxyapatite bone-filling material was developed with chitosan sol as a binder. Measurements were made of the solubility of chitosan, and of the setting time, compressive strength, and pH value of the material. X-ray microanalysis and diffraction analysis of the material were also conducted. Chitosan is freely soluble in malic acid and succinic acid, but insoluble in citric acid and oxalic acid. The setting time of the bone-filling material tended to decrease with increase in the amounts of its CaO and ZnO components. The setting time increased with increasing ratio of sol to other (powdered) ingredients. The pH value decreased with increasing ratios of chitosan sol to the powdered ingredients. High pH values resulted from increases in both CaO and ZnO. The time between the preparation of the chitosan sol and the onset of kneading significantly affected the compressive strength of the material when set. The strength measured roughly 50% more for times between 90 and 150 minutes than for times shorter or longer than that range. Greater compressive strength was generally observed for increases in both CaO and ZnO. The results of the X-ray microanalysis of the hardened paste showed calcium crystals other than hydroxyapatite.

[A case of cystitis glandularis. Its relationship to bladder carcinoma].

A 41-year-old male was admitted to the hospital for frequent urination and severe retardation before voiding. Roentgenography revealed exophytic lesions which were located mainly in the trigone of the bladder, and he underwent transurethral resection. The pathological diagnosis was cystitis glandularis. There was no literature which reported cystitis glandularis causing urinary disturbance. This case, moreover, had been diagnosed as cystitis glandularis 13 years before by endoscopical biopsy. The origin of cystitis glandularis and its potentiality as a pre-malignant lesion were discussed from the clinical course for over 10 years.

[Cellular metabolic consideration in the pathophysiology and the treatment of shock].

Shock is defined as the summation of cellular dysfunction and resultant impairment of the function in various organs. The typical such condition can be observed in shock-induced multiple organ failure (MOF). Therefore, the most effective and fundamental management against shock should be the improvement of such cellular dysfunction. The present study was undertaken to investigate the cellular metabolic abnormalities and their treatment in shock-induced MOF patients. The severity of cellular damage was evaluated with cellular injury score (CIS), which can be calculated by scoring the values of osmolality gap, arterial ketone body ratio and blood lactate. The CIS correlated significantly with the outcome and the number of failing organs in the MOF patients. Since tissue hypoxia and humoral mediator have been considered to play a key role in the development of such cellular dysfunction, protease inhibitor was administered to such patients and oxygen metabolism was improved with catecholamine. The CIS improved among survived MOF patients following those treatments. These results suggest that the shock-induced MOF is the summation of the cellular dysfunction which can be assessed with the CIS, and that such cellular dysfunction could be improved through the therapeutic approach to the humoral mediators and/or tissue oxygen metabolism.

[Perioperative water and electrolyte metabolism and its abnormalities in aged patients].

The number of aged surgical patients is increasing due to the increase of aged people in the general population. Aged surgical patients above 65 years of age constituted about 30% of all ICU patients in our institution. Perioperative care for aged patients has now become one of the most important clinical activities in the ICU. The perioperative management for those aged patients should be performed with great care because these patients have some abnormalities in water and electrolyte metabolism. Aged patients showed decreased blood levels of atrial natriuretic peptide (ANP) and decreased creatinine clearance even in the preoperative period. The ANP level and the creatinine clearance showed significant negative correlation, indicating that one of the reasons for impaired renal function among aged patients could be the decreased ANP level in blood. The intraoperative insults to the aged surgical patients undergoing radical operation for esophageal cancer tended to be smaller compared to that in the younger patients. Intraoperative infusion volume and urinary output were also smaller in the aged group compared to those in the younger group. The postoperative infusion therapy in the ICU mainly consisted of intravenous hyperalimentation with decreased Na content and the transfusion of fresh frozen plasma. This regimen should be more suitable for the aged patients. Even though the aged and the younger group received the same infusion therapy during their postoperative ICU stay up to the 7th day, the aged patients showed the tendency of Na and Cl retention and increase in anion gap. However, these abnormalities in water and electrolyte metabolism were not so severe as to cause clinical symptoms.(ABSTRACT TRUNCATED AT 250 WORDS)

Prognostic value of serum osmolality gap in patients with multiple organ failure treated with hemopurification.

Serum osmolality gap (OG), the difference between measured and predicted serum osmolality, has been shown to be an excellent parameter to express the amount of conventionally unmeasurable middle-molecular-weight substances. OG was determined on 29 patients with multiple organ failure (MOF) treated with or without hemopurification. OG significantly increased in proportion to the increase in the number of failed organs and correlated well with APACHE II score. Nonsurvivors showed persistently high OG. OG decreased with plasma exchange and hemoadsorption, but not with hemodialysis, indicating that pathogenic factors can be removed effectively with plasma exchange and hemoadsorption. The patients with OG greater than 20 mOsm/kg-H2O had very little possibility of survival. These results indicate that OG is an easily determinable, effective parameter to evaluate the severity of the patients' condition, efficacy of hemopurification in removing pathogenic middle-molecular-weight substances, and the prognosis of the patient in the treatment of MOF.

[Metabolic abnormalities and nutritional management in postoperative multiple organ failure].

Despite its importance, very little has been known on metabolic abnormalities in the patients with postoperative multiple organ failure (POMOF). The present study was undertaken to investigate the metabolic abnormalities and to establish effective nutritional management on these patients. Systemic energy metabolism and hepatocellular mitochondrial function were studied with indirect calorimetry and arterial ketone body ratio measurements on 30 POMOF patients treated between 1984 and 1985. The POMOF patients were hypermetabolic and their hepatic mitochondrial function was impaired. Their caloric intake was depleted for the limitation in administrable water volume due to renal failure. Some of them could not utilize exogenous glucose given by total parenteral nutrition (TPN). To overcome these problems, on 26 POMOF patients treated between 1986 and 1987 continuous hemofiltration (CHF) was applied to remove excess water and ATP-Mg was administered to improve hepatic mitochondrial function during TPN. The sufficiency rate of energy intake (caloric intake/energy expenditure) was improved from 73.3 +/- 14.7% to 92.0 +/- 8.1% with simultaneous CHF and the hepatic mitochondrial function was also improved with ATP-Mg resulting in better utilization of exogenous glucose. These results indicate that CHF and ATP-Mg administration are effective adjunctive treatments in the nutritional management for POMOF.

Hepatic glycolytic intermediates in fed and fasted rats after severe hemorrhage.

The responses of key liver carbohydrate intermediates to severe hemorrhage were investigated in fed and fasted young adult male rats. Forty per cent of intravascular blood was withdrawn and liver was sampled by freeze-clamp at 0, 0.25, 1.0, 3.0, and 4.0-5.0 hours. Fed rats with abundant glycogen showed a threefold increase in glucose-6-phosphate (G6P) concentration, and fasted rats showed a 75% decline in G6P immediately after hemorrhage. This significant difference in response traces to the fact that G6P is one of the first catabolites in fed liver formed by glycogenolysis but is the last intermediate of the gluconeogenic pathway in fasted animals. Phosphoenolpyruvate (PEP), the high-energy intermediate, was markedly depleted in both fed and fasted rats at zero time. In the fasted animal, however, the PEP was rapidly restored, and by 1.0 hour was threefold above normal. The ability of fasted rats to rapidly synthesize glucose from accumulated lactate is attributed to increased amount of gluconeogenic enzymes induced by fasting. In prolonged shock states, this synthetic capacity plays a protective role. Contrariwise, in brief shock states such as hemorrhage, the immediate availability of glucose from stored glycogen appears to be a more important determinant of survival. In the present experiments, fed rats were more resistant to the hemorrhage protocol.

Homotypic pair formation during conjugation in Tetrahymena thermophila.

In the ciliate Tetrahymena thermophila, conjugation has been believed to occur only between cells of different mating types. We found the formation of homotypic pairs during normal conjugation by using micronuclear morphological markers. Homotypic pairs formed preferentially during the first 10 min following the first pair formation and comprised about half of the pairs. These results suggest the involvement of mating-type non-specific adhesion of cells in the initial step of conjugation. Homotypic pairs apparently persist for at least 30 min and then separate into single cells. Homotypic pairs are also formed when conjugant pairs re-form after mechanical separation of heterotypic pairs. Five kinds of glycosidases, three kinds of proteases and phospholipase C showed no effect on either the formation of homotypic pairs of their separation. The relation between the mating-type substances and the molecules responsible for mating-type non-specific adhesion of cells is discussed.

Endotoxin lethality is intensified by inhibited gluconeogenesis.

There are two major etiologies regarding the lethal element in the pathophysiology of endotoxemia and severe gram-negative sepsis: 1) metabolic lesions culminating in terminal hypoglycemia and 2) circulatory deficits resulting in early peripheral and late vital organ perfusion failure. Although not mutually exclusive, a direct test of the relative importance of either hypothesis is needed. The impact of inhibited gluconeogenesis on endotoxin lethality in young adult male rats (180-220 g) was investigated. Fasted rats received 20 mg/kg intravenous E. coli endotoxin (LD10) simultaneously with 500 mg/kg intraperitoneal L-tryptophan. This amino acid rapidly forms quinolinic acid, which blocks liver glucose synthesis. Endotoxin together with tryptophan caused hypoglycemic convulsions, killing 22 of 24 rats, 75% within 6 hours. In parallel studies, liver intermediates were assayed in freeze-clamped samples obtained at 5 hours from ether anesthetized rats. The high-energy intermediate phosphoenolpyruvate was 222 +/- 79 nmole/gm +/- 1 S.D. wet liver in the moderately endotoxic rats (N = 8). In the endotoxin-plus tryptophan group (N = 7), the PEP intermediate had fallen to 58 +/- 24 nmole/gm liver (P = 0.005). Liver lactate was increased 2.8-fold over the value in the endotoxin-only group, to 4390 nmole/gm wet tissue, showing the failure to utilize gluconeogenic precursors. Tryptophan given alone was not lethal. It is concluded that inhibited gluconeogenesis greatly intensifies the hepatic metabolic derangement of endotoxemia.

[An autopsy case of sudden death by obstruction of the pulmonary artery in cardiac rhabdomyosarcoma].

The patient was a 50-year-old man. Chest X-ray films showed a tumor shadow near the heart, which was diagnosed as carcinoma of the left lung. The patient's heart suddenly and unexpectedly stopped and he consequently died when his body was set in the right lateral position for operation on the left pulmonary tumor. A tumor measuring 6 X 3 X 3 cm was found in the region from the right ventricle to the pulmonary artery at autopsy. It was postulated that the tumor might have obstructed the pulmonary artery when the position of the patient's body was changed for surgery. A metastatic nodule was found in the left lung. The tumor was a rhabdomyosarcoma, the pleomorphic type.

Allergic contact urticaria caused by raw prawns and shrimps: three cases.

Contact urticaria from raw prawns and shrimps was suspected in three housewives with hand eczema who visited our clinic. A practice skin test in which patients stripped the shells from raw prawns and shrimps was positive, but an intradermal skin test with an allergen extracted from a mixture of prawns and shrimps was negative. Results of skin tests with fresh extracts of raw prawns and shrimps were positive. The RAST with a paper disc method with use of a raw prawn and shrimp as antigens was also positive in these three patients. It is evident from the results obtained that some common substances causing a local urticaria exist in raw prawns and shrimps.
