Cytopathic effects of antigens in patients with IgA nephropathy.

The cytopathic effects of extracts of pharyngeal cells from patients with IgA nephropathy on fibroblasts were determined. Autoradiographical analysis of antigens in the fibroblasts was also described. Freeze and thawed extracts of pharyngeal cells obtained from patients with IgA nephropathy, other glomerular diseases and healthy adults were cultured with fibroblasts such as Vero or Hel cells at 37 degrees C for 1 or 2 weeks. The cytopathic effects of fibroblasts were examined with a light microscope. In addition, the eluate obtained from renal tissues of IgA nephropathy was labeled with iodine-125 by the chloramine-T method. These cultured fibroblasts were incubated with iodine-125-labeled eluate from the same or other patients with IgA nephropathy. The degree of cytopathic effects of extracts of pharyngeal cells from patients with IgA nephropathy on Vero or Hel cells was significantly increased compared with that from patients with other glomerular diseases or healthy adults without glomerular diseases. The cytopathic effects of such fibroblasts were not inhibited by filtration of extracts of pharyngeal cells obtained from patients with IgA nephropathy. It was shown that the antibodies eluted from renal tissues of patients with IgA nephropathy specifically bound with the nuclear regions of such fibroblasts. It was suggested that some antigenic substances might exist in the epithelial cells of the upper respiratory tracts of some patients with IgA nephropathy, and that such antigens were able to be transferred to cultured fibroblasts.

Hypolipidemic effect and enhancement of peroxisomal beta-oxidation in the liver of rats by sodium-(E)-3-(4-(3-pyridylmethyl)phenyl)-2-methyl propenoate (OKY-1581), a potent inhibitor of TxA2 synthetase.

The effects of sodium-(E)-3-(4-(3-pyridylmethyl)phenyl)-2-methyl propenoate (OKY-1581) and (E)-3-(4-(imidazolylmethyl)phenyl)-2-propenoic acid (OKY-046), potent inhibitors to thromboxane A2 synthetase, on peroxisomal beta-oxidation and on lipid levels of liver and serum in the rat were studied. When the animals were administered with OKY-1581 at the dose levels of 100 and 500 mg/kg body weight for 2 weeks, the activity of peroxisomal beta-oxidation increased 2.2- and 6.3-fold respectively. Catalase activity increased 1.3-fold, whereas D-amino acid oxidase (DAAO) and urate oxidase activities did not change. Carnitine acetyltransferase and carnitine palmitoyltransferase activities also increased 2.2- - 4.1-fold and 2.7- - 4.2-fold respectively. These changes of the enzymes related to lipid metabolism were also confirmed by the results of a cell fractionation study. Moreover, the induction of peroxisome proliferation-associated polypeptide having a molecular weight of 80000, which is a bifunctional enzyme in the peroxisomal beta-oxidation system was also observed electrophoretically in the light mitochondrial fraction of the liver of OKY-1581-treated rat. The contents of triglyceride and cholesterol in the serum decreased. These results indicated that the action of OKY-1581 in enhancing hepatic peroxisomal-oxidation is similar to that of a potent hypolipidemic peroxisome proliferator such as clofibrate. On the other hand, differing from OKY-1581, OKY-046 at the dose level of 500 mg/kg for 2 weeks showed no effect on serum and liver lipid levels and on the activities of the peroxisomal enzymes, including a cyanide-insensitive fatty acyl-CoA oxidizing system and carnitine acetyl transferase.

[Combination of missile-induced hypertensive intra-aortic infusion chemotherapy in a patient with hepatocellular carcinoma].

A case of hepatocellular carcinoma was treated with anti-alpha-fetoprotein (AFP) antibody-adriamycin via the hepatic artery with induced hypertension by angiotensin II. A minor response was achieved (from 13.5 X 9.0 cm to 10.5 X 7.5 cm) and serum AFP decreased from 310,000 ng/ml to 26,000 ng/ml. The patient has been doing well during a follow-up period of one year. This result suggests that targeting chemotherapy used for each drug delivery system would be more effective if combined with two or more similar modalities.

[Clinicopathological study of minute stomach cancer].

During the last 10 years, we have experienced 41 cases of minute cancer, which is equivalent to 13.9% of all resected early stomach cancers. Out of 44 lesions in 41 cases, 17 lesions were diagnosed preoperatively, while 27 were unexpectedly detected by serial sections of the resected stomach. Most lesions were located along the lesser curvature of the body and antrum. There was no nodal involvement or vessel permeation. Histologically differentiated adenocarcinomas were encountered in 86.4%, associated with intensive intestinal mucosa in many cases. We have stressed that dye endoscopy using indigo carmine and methylene blue is very useful for the diagnosis of minute cancer.

Successful use of continuous ambulatory peritoneal dialysis (CAPD) in the treatment of patients with renal and cardiac failure.

This report described successful uses of continuous ambulatory peritoneal dialysis (CAPD) in the treatment of two patients with renal and cardiac failure. Case 1 was a 46-year-old woman with renal failure and mitral regurgitation. There were some predictable disadvantages to make a blood access and to increase the cardiovascular stress for her condition. Therefore, we treated her using CAPD and succeeded in improving her condition. Case 2. was a 62-year-old man with renal failure and aortic insufficiency. Beside having renal failure, he showed intractable cardiac failure. He was initially treated using in full ECUM (extracorporeal ultrafiltration method) and CAPD. Two week after admission he was successfully treated only by CAPD. It is suggested that CAPD will be the first choice of treatment in patients with renal and cardiac failure.

Clinical effect of dipyridamole in patients with IgA nephropathy.

Clinical effects of dipyridamole and carbazochrome sodium sulfonate in patients with IgA nephropathy are described. Oral administration of 300 mg of dipyridamole and 180 mg of carbazochrome sodium sulfonate per day was employed in the present study. Urinalysis and renal function tests, i.e. serum creatinine (s-Cr), blood urea nitrogen (BUN), glomerular filtration rate (GFR) and phenolsulfonphtalein (PSP) tests, were performed before and after the administration of dipyridamole. It was demonstrated that the administration of dipyridamole was effective in reducing the level of proteinuria in the patients. The administration of carbazochrome sodium sulfonate was not effective in reducing the proteinuria level. It was concluded that the administration of dipyridamole may be useful for treatment of patients with IgA nephropathy.

Detection of alpha 2-pregnancy associated-glycoprotein (alpha 2-PAG) in cultured human peripheral blood mononuclear cells.

A study of the detection of alpha 2-pregnancy associated-glycoprotein (alpha 2-PAG) by double immunofluorescence in cultured human peripheral blood mononuclear cells (MNC) was performed. MNC was obtained from four healthy adults and isolated on a Ficoll-Hypaque density gradient. MNC was incubated with RPMI-1640 with or without pokeweed mitogen in 5% CO2/air at 37 degrees C for six days. MNC was initially stained with rhodamine-conjugated antihuman IgA, IgM, and IgG antisera, and then stained with FITC-conjugated antihuman alpha 2-PAG antisera. It was concluded that alpha 2-PAG was not produced by IgA, or by IgM and IgG producing cells in healthy adults.

Siblings with IgA nephropathy and diffuse proliferative glomerulonephritis (PGN) associated with identical HL-A antigens.

Siblings with IgA nephropathy and diffuse proliferative glomerulonephritis without mesangial IgA deposits (PGN) who had identical HL-A antigens are described. A 21-year-old woman suffered from IgA nephropathy and her 27-year-old sister showed diffuse proliferative glomerulonephritis (PGN) distinct from IgA nephropathy. These siblings had identical HL-A alloantigens which were A2, Aw24; Bw16, Bw35; Cw4, Cw7; DR4. Microhematuria and increased serum IgA levels were shown in their father and other sister but renal biopsies were not performed. The serotypes for the HL-A antigens of their father and other sister were A2, Aw24; Bw27, Bw35; Cw4; DR4 and A2; Bw16; Cw7; Dr2, respectively. It is demonstrated that IgA nephropathy and PGN share a common HL-A phenotype including DR4. It is postulated that the presence of DR4 antigens is not directly related to the occurrence of IgA nephropathy.

Characterization of an acidic nuclear protein recognized by autoantibodies in sera from patients with IgA nephropathy.

A nuclear antigen is recognized by autoantibodies in the sera of some patients with IgA nephropathy. Using these autoantibodies as a reagent, this antigen was purified 77.2-fold by ammonium sulfate fractionation, DEAE chromatography and Sepharose 6BCL gel filtration. The antigenicity of this antigen was sensitive to trypsin but resistant to RNase and DNase, suggesting that the antigenic determinant resided in protein and not nucleic acids. This antigen was inactivated at 56 degrees C for 3 h. Isoelectrophoretic focussing showed that the pI was below 4. The immunoblotting (Western transfer) assay showed a single polypeptide (69,000 Daltons) which proved to be a reactive antigen.

Significant correlation between the immunofluorescence of alpha 2-plasmin inhibitor in glomeruli and the effects of urokinase therapy in patients with IgA nephropathy.

Correlation between the deposition of alpha 2-plasmin inhibitor (alpha 2-PI), which is one of the inhibitory factors of fibrinolytic activities, in the glomeruli and the effects of urokinase therapy in patients with IgA nephropathy is described. Urokinase (UK) is a plasminogen activator derived from fresh human urine. Urinalysis and measurements of renal function tests, i.e., serum creatinine, blood urea nitrogen, glomerular filtration rate and phenolsulfonphtalein, were performed before and at 8 and 48 weeks after the administration of urokinase. There was marked improvement of proteinuria after UK therapy in patients without deposition of alpha 2-PI in the glomeruli. In contrast, the improvement of proteinuria after UK therapy was not observed in patients with positive deposition of alpha 2-PI in the glomeruli. It was concluded that the administration of UK may be useful for treatment of proteinuria in patients with IgA nephropathy.

An adult case of Fanconi's syndrome associated with membranous nephropathy.

An adult case of Fanconi's syndrome associated with membranous nephropathy is described. Renal biopsy specimens revealed typical features of membranous nephropathy associated with severe tubulo-interstitial changes in light microscopy, electron microscopy and immunofluorescent microscopy. The nephrotic syndrome due to membranous nephropathy was observed at the time of definite diagnosis of Fanconi's syndrome in this patient. It is suggested that renal tubular injuries in some patients with membranous nephropathy might be an important factor in the pathogenesis and/or development of Fanconi's syndrome.

[A chronological study on gastric cancer originating from the fundus gland mucosa].

A chronological analysis of gastric cancer originating from fundic gland mucosa was made by measuring the tumor size with reference to the serosal invasion and peritoneal dissemination. As for serosal invasion by cancer cells there were significant differences in the group with tumors exceeding 4 cm in diameter, and peritoneal dissemination was liable to be seen in the group with those exceeding 5 cm. This suggests that gastric cancer originating from fundic gastric mucosa must be detected before it reaches 4 cm in diameter and that the intraperitoneal administration chemotherapeutic agents is recommendable, considering residual microcancer dissemination.

[A new superselective balloon catheter--all-silicone catheter with a floppy tip].

We have introduced concept of "chemical" embolization and have tried to develop a new agent which would enable us to embolize the lesion with one-shot injection. Such an agent must be able to occlude diffusely the lesion distal to the catheter. This has made it mandatory to develop a new catheter which can be introduced into the vessel as close to the lesion as possible with fewer risks of clot formation and/or vessel damage. A new superselective balloon catheter for angiography and infusion of liquid embolizing materials has been developed. This catheter consists of a proximal relatively stiff silicone catheter, a short distal thin-walled flexible silicone catheter and silicone balloon. These three silicone components are connected by silicone adhesives. The distal catheter allows us to catheterize fine arteries such as lenticulostriates, while the proximal catheter assures easy manipulation. This balloon catheter can be used for superselective angiography and infusion of liquid embolizing materials. It has been used on nine patients; one with a dural arterio-venous malformation (AVM), four with meningiomas, and four with brain and spinal cord AVMs. In the case of dural AVM and meningiomas, it was possible to easily introduce into the middle meningeal artery distal to the foramen spinosum. In addition, in one of the cases of meningioma, we were able to catheterize one of the main feeding pedicles beyond the pterion. Chemical embolization was carried out in 5 cases with good results. In the case of brain and spinal cord AVM, useful information was obtained from the superselective angiography.(ABSTRACT TRUNCATED AT 250 WORDS)

Enhancement of peroxisomal beta-oxidation in the liver of rats and mice treated with valproic acid.

The effects of valproic acid on peroxisomal beta-oxidation and on lipid levels of liver and serum in the rat and mouse were studied. When the animals were fed diet containing 1% valproic acid for 2 weeks, the activity of peroxisomal beta-oxidation increased 4-fold in the rat liver and 2-fold in the mouse liver. Other peroxisomal enzymes such as catalase and urate oxidase also increased by the treatment though to a lesser extent than beta-oxidation. The contents of triglyceride and cholesterol in the serum decreased significantly in the rat but not in the mouse. The time course curves of the activities of cyanide-insensitive palmitoyl-CoA oxidation and carnitine-dependent palmitoyltransferase indicated that peroxisomal beta-oxidation was enhanced more rapidly than that of mitochondrial. The distributions of these enzymes were not changed by the treatment with valproic acid, though increases in liver weight and protein content were observed. These results indicate that the action of valproic acid in enhancing hepatic beta-oxidation is similar to that of clofibrate and other hypolipidemic drugs.

Developmental changes in the activities of peroxisomal and mitochondrial beta-oxidation in chicken liver.

The activities of peroxisomal and mitochondrial beta-oxidation and carnitine acyltransferases changed during the process of development from embryo to adult chicken, and the highest activities of peroxisomal beta-oxidation, palmitoyl-CoA oxidase, and carnitine acetyltransferase were found at the hatching stage of the embryo. The profiles of these alterations were in agreement with those of the contents of triglycerides and free fatty acids in the liver. The highest activities of mitochondrial beta-oxidation and palmitoyl-CoA dehydrogenase were observed at the earlier stages of the embryo; then the activities decreased gradually from embryo to adult chicken. The ratio of activities of carnitine acetyltransferase in peroxisomes and mitochondria (peroxisomes/mitochondria) increased from 0.54 to 0.82 during the development from embryo to adult chicken. The ratio of activities of carnitine palmitoyltransferase decreased from 0.82 to 0.25 during the development. The affinity of fatty acyl-CoA dehydrogenase toward the medium-chain acyl-CoAs (C6 and C8) was high in the embryo and decreased with development, whereas the substrate specificity of fatty acyl-CoA oxidase did not change. The substrate specificity of mitochondrial carnitine acyltransferases did not change with development. The affinity of peroxisomal carnitine acyltransferases toward the long-chain acyl-CoAs (C10 to C16) was high in the embryo, but low in adult chicken.

Effect of clofibrate on peroxisomal and mitochondrial beta-oxidation in chicken liver.

The effect of a 0.25% clofibrate diet for 2 weeks on peroxisomal and mitochondrial beta-oxidation in chicken liver was studied. The activities of antimycin antimycin A-insensitive palmitoyl-CoA oxidation (peroxisomal beta-oxidation) and carnitine acetyltransferase increased about two-fold. The activities of palmitoyl-CoA-dependent O2 consumption (mitochondrial beta-oxidation) and carnitine palmitoyltransferase were also slightly activated by the administration of clofibrate, but not significant. Thus, clofibrate may be a typical drug which activates the peroxisomal beta-oxidation more than the mitochondrial one in various species. The effect of clofibrate on peroxisomal carnitine acetyltransferase was the same as that on the mitochondrial one in chicken liver. Serum lipids were not lowered, but hepatomegaly was observed in the present experiment with chicken.
