Argyrophil reactive cells in the normal uterus and differentiated endometrial adenocarcinoma.

Argyrophil cells were seen in 2 of 134 cervical tissue blocks obtained from 18 uteri, but none in the endometrium. In 10 of 30 cases of differentiated endometrial adenocarcinoma cells contained argyrophil granules stained with Grimelius method. These argyrophil reactive glandular cells gave positive d-PAS and Alcian blue (pH 2.5, PH 1.0) reactions, but gave no argyrophil reaction with PAS-Grimelius double staining method. It was also demonstrated that these glandular cells contained sulphomucins, because High iron diamine (HID) method gave positive reaction with HID-Alcian blue pH 2.5 double staining. Dense granules, which were submicroscopically seen in cells of well differentiated endometrial adenocarcinomas, were diagnosed as exocrine secretory granules, because of their localization in the cytoplasm and distinct morphological transformation from condensed vacuoles into mature secretory granules. From these findings, argyrophil granules which were seen in cells of the differentiated endometrial adenocarcinomas were thought not to correspond to endocrine granules but mucosubstances.

[HLA and cervical carcinoma (author's transl)].

For the purpose of studying immunogenetic factors related to disease susceptibility to cervical carcinoma (squamous cell carcinoma), HLA-A,B and DR typings were performed by the standard microlymphocytotoxicity test using about 240 typing antisera in 66 Japanese patients with cervical carcinoma and 206 healthy individuals. The results obtained were as follows: 1) The patients had a significant association with HLA-Bw46 (gene frequency = 6.3%, relative risk = 3.9, p less than 0.025), compared with the controls (g.f. = 1.7%). 2) The patients had a significant association with HLA-DRw8 (g.f. = 17.3%, r.r. = 2.3, p less than 0.05), compared with the controls (g.f. = 8.9%). 3) When the patients with cervical carcinoma were divided into two groups, i.e. 53 patients with invasive carcinoma of the cervix and 13 patients with carcinoma in situ of the cervix, the former had a more significant association with both HLA-Bw46 (g.f. = 6.8%, r.r. = 4.3) and HLA-DRW8 (g.f. = 19.2%, r.r. = 2.6) than the total patients, compared with the controls. However, the latter showed only a negligible association when compared with the controls. 4) The significant linkage disequilibrium between HLA-Bw46 and HLA-DRw8 was found in the patients with cervical carcinoma (haplotype frequency = 0.059, delta = 0.047, p less than 0.002), but not in the controls (h.f. = 0.0097, delta = 0.0075). 5) On the other hand, the significant linkage disequilibrium between HLA-A2 and HLA-Bw46 was found in both the patients and the controls.

Epidemiological analysis of the distribution of antibody to adult T-cell leukemia-virus-associated antigen: possible horizontal transmission of adult T-cell leukemia virus.

About 20% of healthy inhabitants in the endemic area of adult T-cell leukemia/lymphoma (ATL) in Nagasaki Prefecture showed a positive antibody to adult T-cell leukemia virus (ATLV)-associated antigen (ATLA), while none of the inhabitants in a non-endemic area in Aichi Prefecture showed a positive anti-ATLA. The age-specific positive rate of anti-ATLA in the endemic area increased with age, especially in females. The positive rate of anti-ATLA among inhabitants older than 40 years was 21% in males and 36% in females. From epidemiological analyses of the mode of anti-ATLA reactivity among married couples and some family members and relatives with positive and negative anti-ATLA, two routes of transmission of ATLV were suggested. One route is from parents to children, either vertically or horizontally, and the other route is horizontal transmission between spouses, especially from husband to wife. The presence of some vectors of ATLV in the endemic area of ATL is discussed.

Histological study of immune parameters of regional lymph nodes of gastric cancer patients.

A total of 6,394 regional lymph nodes (RLNs) from 319 gastric cancer patients was evaluated histologically for sinus histiocytosis (SH), paracortical lymphocytes (PCL), overall lymphoid cell content (OL), germinal centers (GC), and medullary cord plasma cells (P1). High grade SH, PCL, and OL reactivity was found in cancers confined to the mucosa and submucosa of the stomach wall, and GC and P1 reactivity in cancers that had invaded the subserosa or the serosa of the stomach wall. No stable relationship was found between the RLNs reactivity of patients with and without metastasis to the RLNs. Differences among RLNs reactivities according to location (n1, n2, n3+4) as defined by the Japanese Research Society for Gastric Cancer were evaluated, and n1, the most proximal nodes to the primary cancer, usually had the highest reactivity. As regards the prognostic value of these factors, OL and GC, but not SH, had a significant favorable relation to the 5-year survival rate. In addition, sarcoid-like reaction was evaluated.

Ultrastructure of adenocarcinoma of Bartholin's gland.

The case of a 41-year-old woman with well-differentiated papillary adenocarcinoma of the Bartholin's gland was studied submicroscopically. The tumor consisted of three types of cells with different types of exocrine secretory granules: (1) Principal cells, which were the most frequently observed component of the tumor, had numerous small, secretory granules of round shape with very high electron density. These granules resembled membrane-bound dense bodies observed in normal Bartholin's duct cells. (2) Goblet-like cells had larger secretory granules with low electron density, resembling secretory granules of normal Bartholin's gland cells. (3) Eccrine sweat gland-like cells had large, irregular-shaped secretory granules with moderate electron density. This has been observed in neither normal Bartholin's gland nor duct cells, and resembles secretory granules of eccrine sweat gland cells. Furthermore, fibrillar bodies, frequently described in normal Bartholin's gland cells, were seen in a few tumor cells. These submicroscopic findings suggest that this tumor derived from Bartholin's duct cell, differentiating to gland cell and/or eccrine sweat gland cell.

A controlled study of maintenance chemoimmunotherapy vs immunotherapy alone immediately following palliative gastrectomy and induction chemoimmunotherapy for advanced gastric cancer. Tokai cooperative study group for adjuvant chemoimmunotherapy of stomach cancer.

A randomized trial of surgical adjuvant chemoimmunotherapy was conducted in patients who had undergone palliative gastrectomy for previously untreated advanced stomach cancer. First, all patients received the same induction chemoimmunotherapy with MFC (mitomycin C, 5-fluorouracil, and cytosine arabinoside) plus OK-432 for 6 weeks after surgery. The patients were then randomized to receive either chemoimmunotherapy with MFC plus OK-432 (group A) or immunotherapy with OK-432 alone (group B) for maintenance. The survival rate of patients was significantly higher in group B (44 cases) than in group A (39 cases) during the first 9 months after the start of induction therapy (P less than 0.05). A further division of patients in terms of carcinoma histology revealed a difference in survival rate only in patients with an undifferentiated histology (poorly differentiated adenocarcinoma and signet-ring cell carcinoma), and not in those with a differentiated histology (papillary, tubular, and mucinous adenocarcinomas). These results indicate that simple immunotherapy with Ok-432 is better for maintenance than chemoimmunotherapy involving MFC, particularly in patients with undifferentiated gastric carcinomas.

Granular cell myoblastoma of the bronchus in a child: a case report.

Granular cell myoblastoma of of the right main bronchus in a 5-year-old girl is reported. The tumor extended to the lower trachea from the right main bronchus which was occluded completely by the tumor. The diagnosis was established histologically by a obtaining bronchoscopic biopsy specimen. Right pneumonectomy was carried out to control pulmonary suppuration caused by the tumor obstruction of the bronchus. External radiotherapy and interstitial brachytherapy were successful to control the growth of the residual tumor. She has been free from symptoms for about 12 years after the treatment.

A hypothesis on the etiology of adult T-cell leukemia/lymphoma.

The results of the nationwide survey performed by the T- and B-cell Malignancy Study Group revealed that adult T-cell leukemia/lymphoma (ATLL) was prevalent in limited zones in Kyushu, mainly in coastal areas, warm in winter and humid in summer. It was found that the geographical distribution of filariasis was very similar to that of ATLL. It is known that filariasis affects lymphatic vessels and results in several lymphoreticular ailments, and microfilaria is transmitted by mosquitoes infected with some kinds of not yet fully identified viruses. From analyses of the time trends of the average weight and height of school children by prefecture, it is suggested that the nutritional condition of inhabitants in Kyushu might have been poorer than that of inhabitants of other areas in Japan, especially in the past. In order to elucidate the etiology of ATLL, relevant geographic-pathological information and other related information was accumulated and an etiological hypothesis was formulated. It was hypothesised that repeated exposure to filarial antigen and some viruses might have played an important role in the etiology of ATLL and that undernutrition had also contributed to the progression of ATLL.

Intracytoplasmic type A virus-like particles in angioimmunoblastic lymphadenopathy.

Virus-like particles which seemed to be oncogenic have not been observed clinically in human neoplasms. However, we found some virus-like particles in the cytoplasm of a few proliferating cells in the lymph nodes of one patient with angioimmunoblastic lymphadenopathy. These were very similar to intracytoplasmic type A particles seen in the Mazon-Pfizer monkey virus and a mammary tumor virus in a mouse.

Argyrophil cell carcinoma of the uterine cervix with ectopic production of ACTH, beta-MSH, serotonin, histamine, and amylase.

The case of a 38-year-old female with primary argyrophil cell carcinoma of the uterine cervix is reported. Two years after operation the patient developed widespread metastases with typical Cushing's syndrome. Microscopically, the tumor consisted of solid anaplastic cells, adenocarcinoma, and squamous cells. The plasma levels of ACTH and cortisol were elevated. Many cells of both the primary and metastatic tumors showed argyrophilia. Almost all the cells of the metastases contained numerous round secretory granules measuring about 117 micrometers in diameter. Small rod-shaped or larger round secretory granules, measuring 250 and 430 micrometers respectively, were also found in a few of these cells. The tumors in the right lung, pancreas, and kidney contained high levels of ACTH, beta-MSH, serotonin, histamine, and amylase. This is the first report of ectopic production of these five substances from argyrophil cell carcinoma of the uterine cervix.
