Relationship between estrogen receptors and risk factors of breast cancer in Japanese pre- and postmenopausal patients.

The relation between estrogen receptors (ER) in breast cancer and risk factors was studied in 456 Japanese patients. ER was shown to be positive in 55.3% (252/456) of patients. There was no difference in ER positivity between pre- and postmenopausal patients. In premenopausal patients, only the age at menarche and obesity showed some influence on the ER positivity of breast cancer. Among postmenopausal patients, on the contrary, ER(+) cancers were predominant in the patient groups that have been known to be higher in the risk of incidence of breast cancer. The factors included were the age at marriage, number of pregnancies, number of live children, and body weight. Of these, the body weight of patients was the strongest influence on the ER positivity in the postmenopausal patients. After excluding the effects of body weight, some of the reproductive factors such as number of pregnancies and number of live children were shown to be related to the ER status. These results may suggest the combination of lower incidence of breast cancer and lower percentage of ER(+) cancers in Japanese postmenopausal women as compared with the Western countries.

Histopathological features and their association with steroid receptors in women with breast cancer in Fukuoka, Japan.

In Fukuoka, Japan, infiltrating duct carcinoma predominated among 213 breast cancer cases, age 45 to 74. Lobular and medullary carcinomas were infrequent. About thirty three percent of the women had marked productive fibrosis, based on the UICC code type. Both rough margin and productive fibrosis of the cancer increased with age of the patients. On the contrary, most of accompanying hyperplastic lesions decreased with age. The difference was significant for cystic formation between 10-year age groups of 45-54 and 55-64 (chi 2 = 6.61, p less than 0.02). Hormone receptors of 115 of the cases were independently measured and characterized according to histologic patterns manifesting host reaction (stromal cell infiltration and lymph node histiocytosis). Positive estrogen receptors were more frequent among ductal carcinomas with slight or no cellular reaction than those with extensive cellular reaction. The same pattern was also present for progesterone and androgen receptors. The extent of sinus histiocytosis in lymph nodes without metastasis was also inversely correlated with the presence of these hormone receptors. The correlation was significant for androgen receptor (chi 2 = 5.31, p less than 0.05). Positive estrogen receptor was more common among ductal carcinomas with prominent fibrosis than among those with minimal fibrosis.

[Quantitative study of spinal bone atrophy by computed tomography].

A method of quantitative measurement of vertebral bone mineral content using the computed tomography (CT) number as an index was deviced. Basic and clinical studies were performed and its clinical applicability was evaluated. The CT number of vertebrae was shown to be highly reproducible; thus, the variations of two measurements between 1 and 336 days observed on the same vertebral body of 13 cases were averaged 3.1. By immersing the isolated autopsy lumbar vertebral bodies in water and then removing the air by aspiration the comparison of CT number showed that in vivo condition could be stimulated. The CT number of the cancellous bone examined in this phantom model with such treatment was found to closely correlate with the Ca content. The CT numbers of the cancellous and cortical bone of lumbar vertebral bodies were investigated from 200 subjects and the CT numbers were decreased gradually with age. Furthermore the tendency of the decline in CT number of cancellous bone at ages over 50 was more distinct in females than males, and in the cortical bone the presence of bone spiculae tended to be associated with a higher CT number. The changes with age were noted in the distribution pattern of CT number in the cancellous bones from the same 200 subjects and the possibility that osteoporosis begins at the periphery of cancellous bone was suggested. This method was used clinically for sequential chronological evaluation of the mineralization status of lumbar vertebrae. The situations sometimes develop that it is impossible to continue the evaluation on the specific vertebral body. In such cases, an adjacent lumbar vertebral body could instead as the target.

Non-invasive thymoma with widespread blood-borne metastasis.

A 49 year-old woman with multiple coin lesions in both lungs died three months after admission to hospital for progressive neurologic manifestations due to increasing intracranial pressure. At autopsy, neoplastic lesions were found in the anterior mediastinum, lungs, spleen, cerebrum, liver, right kidney and right ovary. Microscopically, the tumor was composed predominantly of epithelial cells which were arranged in an hemangiopericytoma-like pattern. Rosette formation was also prominent. This case was diagnosed as a thymoma with widespread blood-borne metastasis. Previous reports of metastasizing thymomas are very uncommon.

Scanning and transmission electronmicroscopy of leukemic lymphoma cells without T- and B-cell surface markers.

The lymphoma cells from a patient with leukemia lymphoblastic sarcoma (Kiel classification) were observed by scanning and transmission electron microscopy. These cells were also examined by E, EA, EAC rosette-formation tests and by the indirect immunofluorescence technique for surface immunoglobulins. The malignant cells showed failure of rosette-formation or absence of surface immunoglobulins. Scanning electron microscopy revealed that many uniform protrusions were present on the cell surfaces. These surface protrusions were different from those seen on E-or EAC-rosette-forming cells. Ultrastructurally, the malignant cells were characterized by long profiles of rough surfaced endoplasmic reticulum with regular, narrow cisternae which radiated from Golgi area to the periphery of cytoplasm. These appearances differed from those observed in T-or B-lymphoma cells.

Relation of occupations to the regional differences of lung cancer motality in Fukuoka Prefecture.

Geographic pattern of lung cancer mortality in Fukuoka Prefecture showed elevated mortalities among males in the Chikuho district where many coal-mines had long been operated as one of the biggest coal-mining areas in Japan. The analysis in relations of occupations to lung cancer mortality revealed that consistently significant correlations exist between lung cancer mortality, and mining and quarrying occupations in every census year after World War II. No other occupations showed consistent relations to lung cancer though a few significant correlations were found only in the recent years. The results appear to suggest that elevated risk of lung cancer among coal-mining workers may exist and deserve further analytical study.
