[Effect of enflurane and halothane on hemodynamics during the induction of anesthesia].

The effects of enflurane (E) and halothane (H) on hemodynamics were studied during the inhalation of 2MAC of each anesthetic before the surgery. Mean arterial pressure (MAP), heart rate (HR), cardiac index (CI), systemic vascular resistance (SVRI) and stroke volume index (SVI) were measured noninvasively using automatic blood pressure manometer and ultrasonic Doppler method (Accucom). MAP decreased with both E and H, but the decrement was bigger with E. CI and SVI decreased with H, although increased with E. SVRI was unchanged with H, but decreased with E. These results indicate that enflurane causes the depression of blood pressure mostly by the decrease of afterload during the induction of anesthesia.

[Clinical study of second-look operation (SLO) for primary ovarian cancer].

The results of second-look operation (SLO) were reviewed in 25 patients with primary ovarian carcinoma and following results were obtained. There were 21 patients in whom SLO was performed to examine whether tumors were present or not. There were 17 patients with negative SLO and remaining four patients were positive SLO. There was a higher tendency for positive SLO with patients of stage III.IV, serous cystoadenocarcinoma, and initial incomplete surgery. It is recommended that SLO is done for the patients with above characteristics. However, there were no cases of positive SLO among patients of stage Ia, so it is suggested that it is not necessary to do SLO. Five years and three years survival of negative SLO were 46% and 68%, respectively and those of positive SLO were 50% and 50%, respectively. Patients with negative SLO were better prognosis than those with positive SLO until four years after SLO. Eight out of 17 patients with negative SLO were recurred. Therefore, some maintenance treatments after negative SLO are needed for attaining further reduction of recurrence, especially patients for serous histology, low grade differentiation, initial incomplete pelvic surgery and stage III.IV. Two out of four patients with positive SLO are alive. Patients with initial complete pelvic surgery and microscopic lesion at SLO tended to be better prognosis. There were four patients in whom tumor debulking was attempted at SLO due to tumor presence after multimodality approach. All patients were eventually recurred and died of disease. However, two patient survived for 44 and 52 months after SLO. It was suggested to be effective for prolongation of life by debulking tumors at SLO.

Survival of breast-cancer patients and body size indicators.

The association of breast-cancer survival with various risk factors was investigated using data of 213 breast-cancer patients who underwent surgical operation between 1975 and 1978. They were followed-up until 1987, and a total of 64 deaths including 47 breast-cancer deaths were certified. The 5-year and 10-year relative survival rates were 78.5% and 75.3% respectively. Of the various factors investigated, some anthropometric indicators revealed interesting results; i.e., body weight, Quetelet index, and body surface area at the time of operation turned out to be strong predictors of survival with a statistically significant trend towards lower survival with larger body structure even after adjustment for confounding factors (e.g., clinical stage) using a proportional-hazard model. The estimated survival probability for women with Quetelet index of 20 was about 12% higher than that with Quetelet index of 24 over a 10-year or more follow-up period. Other variables exhibiting prognostic importance were clinical stage, TNM classification, and some histological findings, while height and reproductive life indicators were, as a rule, not significantly related to survival.

Malignant hemangiopericytoma of the omentum presenting as an ovarian tumor.

Malignant hemangiopericytomas are usually found in the musculature of the extremities, the retroperitoneum, and the pelvis. Malignant hemangiopericytoma arising in the omentum is extremely rare. We recently experienced such a case, in which a malignant ovarian tumor was suspected preoperatively on the basis of the sonogram, CT scan, magnetic resonance image, and increased CA-125 value. Microscopically, the tumor showed many mitoses, increased cellularity, and cytologic atypia. The patient was treated with CAP combination therapy following resection of the tumor; however, an intraperitoneal recurrence probably due to implantation was recognized 11 months later.

Serum lipids and colorectal adenoma among male self-defence officials in northern Kyushu, Japan.

Colorectal adenoma is regarded as a precursor lesion of adenocarcinoma. In view of the controversy on serum cholesterol and colorectal cancer, the risk of colorectal adenoma was examined in relation to serum total cholesterol, triglycerides and HDL-cholesterol. In the comparison of 88 men having adenoma and 1055 men with normal colonoscopy, there was no association between serum total cholesterol and colorectal adenoma. An increased risk of adenoma was observed at the highest quartile of triglycerides and at the lowest of HDL-cholesterol. When the three serum lipids were simultaneously examined, only the relation with HDL-cholesterol remained unchanged giving odds ratio of 1.7 at the lowest quartile compared with the upper three combined (p less than 0.05). The present study is consistent with the view that the inverse relation between serum total cholesterol and colorectal cancer is due to the effects of preclinical cancer. Further clarification is needed on low HDL-cholesterol and colorectal adenoma.

Anthropometric indicators of breast cancer risk in Japanese women in Fukuoka.

This study was based on a case-control study of breast cancer risk in Fukuoka, which was initiated to identify risk factors for the disease. We re-analyzed the data of the study to evaluate the association of breast cancer with anthropometric variables in postmenopausal women. Data were obtained through interviewing 121 cases with histologically confirmed breast cancer and an equal number of age-matched hospital controls as well as neighborhood controls. The variables used for analysis were; height, weight at various ages, shoe size, and brassiere size and body size indices (body surface area and various body build indices) computed from the height and weight data. In a multiple covariance analysis, the results showed that cases were slightly lighter in weight and smaller for some of the indices, but the differences were not statistically significant. The odds ratios, estimated by multiple logistic regression analysis, suggested that the cases were slightly shorter in height and smaller in shoe size without any consistent dose-response gradients. The effect of changes in weight and body size indices with aging were also assessed, but no significant findings were observed. The results reported here do not support the hypothesis of the positive association between obesity or large body size and breast cancer.

Cancer and other causes of death among Koreans in Fukuoka, Japan, 1976-1986.

Mortalities from cancer and other causes among Koreans living in Fukuoka, Japan, between 1976 and 1986 were examined as compared with those of Japanese in the prefecture. Korean males had a marked excess in all-cause mortality, while the excess among females was less prominent. In both sexes, mortalities from liver cancer, liver cirrhosis, accident and suicide were markedly increased in the Korean population. These findings are in agreement with those observed among Koreans in Osaka. Although 20-30% lower-than-Japanese mortality from stomach cancer has been reported for Koreans in Osaka, those in Fukuoka had a risk of this cancer comparable to that of Japanese. A life-style survey of Koreans in Japan might provide a better understanding of the disease patterns observed in this population.

[Influence of the changes in food intake patterns and smoking and drinking habits on stroke--20-year follow-up survey in the Oki-Islands, Shimane Prefecture, Japan].

In 1968 (the first survey), the food intake patterns and smoking and drinking habits of the 3,546 male and 4,350 female inhabitants aged 40-74, of the Oki-Islands, Shimane, Japan were investigated. The response rates were 64% for males and 65% for females. A second survey according the same protocol was carried out from 1987 to 1988. A total of 1,140 males and 1,694 females were randomly selected from the cohort members who were completely followed up from 1968 to 1987. The overall response rates at the second survey were 91.2% for male and 88.9% for female. The results are summarized as follows: 1. 'Saltless' dietary habits significantly advanced in both sexes of all age groups (40-49, 50-59, and 60-74 years old in 1968) and in both hypertensives and normotensives; the frequency of pickle intake significantly decreased, while that of miso soup intake did not change. Nutritional improvement was remarkable in that the frequency of fish, meat, eggs, milk, green vegetable and fruit intakes increased in both sexes of all age groups and in both hypertensives and normotensives. 2. The frequency of smoking significantly decreased in all age groups and in both male hypertensives and normotensives, in contrast with that of drinking. The frequencies of smoking and drinking showed the same tendencies in females. 3. The frequencies of smoking, drinking, and miso soup intake were higher in males who died from strokes than in those who died from other causes from 1968 to 1987, in contrast with the intakes of eggs and milk.(ABSTRACT TRUNCATED AT 250 WORDS)

A rapid diagnosis of anaerobic infection in the oro-maxillary region by gas-liquid chromatography.

The relationship between volatile fatty acids (VFAs) in pus and infecting bacterial species was examined in order to establish a rapid identification system for anaerobic microorganisms in purulent inflammation in the oro-maxillary region. VFAs were detected by the direct injection of pus into gas-liquid chromatography (GLC). Bacterial examination was carried out by anaerobic culture using blood agar plates. The bacterial identification was carried out mainly according to the VPI manual. Analysis of the direct VFA patterns of each sample resulted in 5 groups. The following bacterial species were the main isolates in each group: Streptococcus intermedius in Group A, Peptostreptococcus micros in Group B, Fusobacterium nucleatum in Group C, Bacteroides gingivalis in Group D, and Peptostreptococcus anaerobius in Group E. The profile of VFAs produced in the PYG culture medium of the above isolated bacteria was compared with the direct VFA patterns. Agreement ratios between direct and PYG VFA patterns were as follows: Group A, 47.1%; Groups B and C, 45.0%; Group D, 87.5%; and Group E, 62.9%. The acetic acid concentration was more than 14 x 10(-4) meq/ml in Group B, the butyric acid concentration was more than 7 x 10(-4) meq/ml in Group C, and the iso-caproic acid concentration was more than 14 x 10(-4) meq/ml in Group E. In these cases, it was found that the agreement ratios between the direct and PYG FVA pattern were high. In Group D, irrespective of the concentration of iso-valeric acid detected, the agreement ratio was very high. The antibiotic susceptibility of the isolates was studied. Efficiency rates of ABPC, PIPC, CCL, CEZ, CMZ, SBT/CPZ, JM, CLDM, MINO and GM were relatively low and resistant rates were high for the gram-negative rods.

Analysis of the chewing movement in temporomandibular disorders.

Chewing and border movements at the mandibular incisor points were recorded three dimentionally by the mandibular tracking device (Saphon). Data were recorded from 71 patients (16 males and 55 females) with temporomandibular (TM) disorders and 20 normal subjects (15 males and 5 females). In TM disorders, a narrowed range of motion in various border movements, as well as an irregularity in chewing rhythm, narrowed the range of motion, and reduced reproducibility in the pattern of chewing movements, were observed.

Clinical findings in temporomandibular disorders.

71 patients (16 males and 55 females) with temporomandibular (TM) disorders were examined for clinical symptoms of mandibular dysfunction. The frequency of TM disorders in the present study was higher in females (55 females, 77.5%) than in males (16 males, 22.5%); the ratio of females to males was about 3.4:1. The most frequent chief complaint was pain; limitation of opening movement was the next most common. Many patients had several of the major symptoms simultaneously. TM joint sounds were noted in 47 patients, including reciprocal clicking in 35 patients and crepitation in 12 patients. Tenderness with palpation of the TM joint and muscles were found in 46 patients; most of them complaining at two positions or more. Occlusal interferences were noted in various occlusal positions, and occlusal wear was found in 30 patients (42.3%). In recording the frequency of parafunction and bad habits, grinding of the teeth was found in 6 patients (8.5%), clenching of the teeth in 10 (14.1%), and unilateral mastication in 24 (33.8%).

[A single year birth cohort analysis of mortality in Japan, 1950 to 1984, statistical method and all cause mortality].

Mortality of single birth year cohort were calculated using the data tape of Information Service on World Health Statistics, WHO, 1950-1984. The data tape gave population and number of deaths for each year of age under 5 years of age and by 5 years of age for ages from 5 to 84 years. Population and number of deaths by age for each year of age in each calendar year were estimated by interpolation. Populations for each year of age were interpolated by B-spline interpolation. Number of deaths for each year of age were calculated by application of "two dimensional semi-Hermite method" after estimation of number of deaths for age 80, 81,......84, using Sprague interpolation factors. Age-specific cohort mortality curves were lower for each successive cohort, demonstrating the so-called "cohort phenomenon", clearly. Ratios of mortalities at the same age, for each successive 5 year interval cohort, which indicate cohort-by-cohort changes of mortalities, were thus less than 1.0 in almost all age groups. In recent years the ratios have gradually been leveling off at 0.8-0.9. Cohorts born in the early Showa Era (around 1930) showed higher ratios than all other cohorts, indicating relatively poor improvement in mortalities compared to the other cohorts. Ratios of cohort mortalities for successive ages within the same cohort, which indicates mortalities increase with advance of age, were greater than 1.0 after adulthood. These particular results showed that the increase in mortality with age accelerated after 30 years of age and that a difference in trend was observed between male and female after age 50. This new method of cohort analysis, single birth year cohort, gives clearer results than those obtained by methods heretofore utilized in cohort studies and is useful for more detailed analysis of cohort mortality.

[Coronary spasm during laryngoplasty].

A 63-yr-old man weighing 56kg was scheduled for pharyngoplasty under general anesthesia. The patient had no history of ischemic heart disease. Preoperative ECG showed incomplete right branch block. We administered thiopental and succinylcholine for intubation. Anesthesia was maintained with enflurane, nitrous oxide, oxygen and pancuronium bromide. Thirty minutes after the start of incision, the patient developed a severe hypotension and ECG revealed ST elevation and complete AV block. We administered ephedrine hydrochloride, phenylephrine hydrochloride and atropine sulfate. The ECG returned to sinus rhythm but ST segment was depressed this time. We considered it due to coronary spasm, so we started continuous intravenous administration of nitroglycerin (0.5 microgram. kg-1. min-1). One hour later, ST segment returned to normal. ECG showed no remarkable changes and no symptoms were seen after the operation. We found it important to suspect coronary spasm when ECG showed PVC-like abnormal waves with ST elevation. We consider that continuous administration of nitroglycerin at a rate of 0.5 microgram. kg-1. min-1 was effective for the treatment of coronary spasm in this case.

[Comparison of intravenous buprenorphine or fentanyl and epidural buprenorphine for pain relief after upper abdominal surgery].

In three groups [(1) intravenous buprenorphine (0.1mg) or (2) fentanyl (100 micrograms.hr-1) and (3) epidural injection of buprenorphine (0.1mg diluted with 10ml normal saline)], we determined the effects of postoperative pain relief in patients after upper abdominal surgery. There are no differences in postoperative analgesia in the three groups, but respiratory depression was seen in some patients who had intravenous buprenorphine or fentanyl. We conclude that epidural injection of buprenorphine is a useful method for postoperative analgesia because of little adverse effect. As respiratory depression caused by buprenorphine was reversed with naloxone, it is not necessary to employ fentanyl instead of buprenorphine.

[Seven cases of primary adenocarcinoma of the fallopian tube].

Seven patients with primary adenocarcinoma of the fallopian tube were treated at Kyushu University Hospital from 1978 to 1987. They were 0.59% of all patients with gynecologic malignancies in our hospital. Their mean age was 56.0 years old. Four of 7 patients presented with abnormal genital bleeding. Pelvic mass was detected in 6 of 7 patients. One patient showed abnormal cervical cytology and 3 of 6 patients had positive endometrial cytology and/or histology. No diagnosis of tubal cancer was made preoperatively. Primary surgical treatments were performed in all patients. Four patients, who underwent dissection and/or biopsy of pelvic and paraaortic lymph nodes, had no metastasis. However, at the time of recurrence, paraaortic and/or neck lymph nodes metastases were present in 3 of 4 patients. Following surgery, combination chemotherapy with cyclophosphamide, doxorubicin and cisplatin (CAP) was administered to 6 of 7 patients. Partial responses were obtained in 2 patients with recurrent and evaluable lesions. The mean survival time of 7 patients as yet was 36.0 months after diagnosis. In conclusion, the difficulty of preoperative diagnosis of tubal cancer was reconfirmed. CAP combination chemotherapy seemed to be effective for the treatment of tubal cancer and further study with more cases is warranted.

[Clinical application of subrenal capsule assay in ovarian cancer].

The 6-day subrenal capsule assay (SRC) in normal immuno-competent mice was used to test the responsiveness of ovarian cancer to combination chemotherapy and assess the usefulness of SRC in clinical application. A total of 18 different patients in 22 different assays were studied. Twenty-one assays (95%) were evaluable. The predictive sensitivity was 50%, the predictive resistance 75% and the predictive total accuracy 55% respectively. Sensitive drugs in repeated assays in four patients were not changed. Etoposide, cyclophosphamide, cis-platin and adriamycin were sensitive. Etoposide was the most sensitive of four drugs. The response rate was 50% in the patients treated with etoposide as the first line chemotherapy and the second line chemotherapy, respectively. Patients who were treated with sensitive drugs in SRC survived longer than the patients treated with cis-platin combined chemotherapy without etoposide. The median survival time was 19 months and the mean progression free interval was 15 months. In the results of this study, the value of SRC was supported by the results of chemosensitivity tests and etoposide seems useful in chemotherapy in ovarian cancer.

[A primary gastric adenosquamous carcinoma with remarkable lymphatic metastasis diagnosed by the stomach and lymph node biopsy].

This report describes the case of a 74-year-old female, who had been admitted to hospital because of epigastralgia and appetite loss. An ultrasonogram and a CT scan of the abdomen revealed a remarkable lymph node metastasis. Through an upper gastrointestinal tract (UGI) X-ray, a Borrmann III type gastric carcinoma was detected. Under endoscopic guidance, a gastric and a lymph node specimen were taken and biopsied, revealing a keratinous, well-differentiated squamous cell carcinoma and a poorly differentiated adenocarcinoma, leading to a diagnosis of a primary gastric adenosquamous carcinoma with a remarkable lymphnode metastasis. After chemotherapy, a CT scan, a UGI X-ray, and an endoscopic examination revealed distinct tumor reduction.
