Derivation and validation of a new angiographic score of right coronary artery perfusion.

A semi-quantitative right coronary artery score (RCA score) was derived from the ratio of the number of the major left ventricular branches of the right coronary artery to the total of the right coronary and left circumflex arteries, to stratify the extent of perfusion in patients with right coronary artery dominance. Thirty-seven patients with one-vessel coronary disease involving a dominant right coronary artery proximal to the left ventricular branches were selected for study. Thallium scintigraphy was performed after right intracoronary injection in 11 patients, and 26 patients underwent conventional stress thallium scintigraphy (24 exercise thallium and two dipyridamole thallium scintigraphy). Thallium scores of perfusion region size after right intracoronary thallium injection and perfusion defect size in stress thallium studies were quantitated from planar thallium images. Both the RCA score and the regional thallium scores spanned over a wide range. The RCA score (range 0.23-0.85) correlated best with the posterior (70 degrees left anterior oblique view) plus lateral segment (40 degrees left anterior oblique view) thallium score (r = 0.88 and 0.53 for intracoronary and stress thallium studies respectively). It also correlated with the summed thallium scores in the posterior, lateral, apical and inferior segments (r = 0.73 and 0.54 respectively) but not with thallium scores in the apex or inferior segment alone. The proposed RCA score quantitates the variable posterolateral perfusion territory of the right coronary artery, and could stratify the area of myocardium at risk from coronary stenosis in the majority of patients with right coronary dominance.

Spontaneous remission of acute adult T-cell leukaemia with chromosomal abnormality infiltrating to skin and liver.

Acute type of adult T-cell leukaemia (ATL) has been recognized as highly-progressive T-cell malignancy with a median survival of 7 months. We present a case who had a spontaneous remission from acute type ATL with organ involvement and chromosomal abnormality implying very poor prognosis. Although the mechanism of spontaneous remission remains uncertain, our case was unique with regard to the progression and regression of leukaemia.

Cytokine production by peripheral blood monocytes and T cells during haemopoietic recovery after intensive chemotherapy.

We studied the production of cytokines by peripheral blood monocytes and T cells during the period of haematological recovery following intensive chemotherapy. Twelve adults with haematological malignancies received consolidation chemotherapy of complete remission. Monocytes and T cells were collected during the phase of recovery from intensive chemotherapy, and were incubated for 24 h in a culture medium with 10% FCS. Concentrations of cytokines in the culture supernatant were measured with an enzyme-linked immunosorbent assay. During the recovery phase, concentrations of IL-6, G-CSF and IL-1 beta in the culture supernatant of the collected monocytes significantly exceeded those of the monocytes obtained from normal healthy subjects. Similarly, the concentrations of GM-CSF and IFN-gamma in the supernatant of recovery phase T cells significantly exceeded those of normal T cells. Plasma levels of these cytokines were also elevated. These data suggest that the monocytes and T cells may be activated in vivo to produce haemopoietic cytokines during haematological recovery, and that, during haematological recovery, the monocytes and T cells may be actively involved in the induction of haematopoiesis following the myelosuppression induced by chemotherapy.

Decreased levels of myeloid progenitor cells associated with long-term administration of recombinant human granulocyte colony-stimulating factor in patients with autoimmune neutropenia.

We studied the long-term in vivo effect of recombinant human granulocyte colony stimulating factor (rhG-CSF) on in vitro growth of granulocyte/macrophage colony forming cells (GM-CFC) in bone marrow and peripheral blood obtained from two patients with autoimmune neutropenia, who received rhG-CSF. Along with rhG-CSF treatment for more than 40 d, numbers of GM-CFC-derived colonies from both bone marrow and peripheral blood gradually decreased to a significant level though white blood cells in peripheral blood and nucleated cells in bone marrow were increased in number. This observation suggests that long-term administration of rhG-CSF may preferentially activate a differentiation pathway for granulopoiesis while proliferation of GM-CFC is not induced as expected in response to rhG-CSF.

[The role of the central monoaminergic nervous system relative to mechanisms of stress adaptation].

The difference of amine dynamics in central noradrenergic, dopaminergic and serotonergic systems of adaptive or non-adaptive rats to repeated restraint stress was investigated neurochemically. A single immobilization induced stress responses, such as reductions in growth rate, food intake, water intake and locomotor activity, and an increase in pain threshold. These responses disappeared after repeated immobilization 1 or 2 hr daily for 7 days, but not after 4 hr daily for 7 days. These results suggest that stress adaptation models should include the parameter of repeated exposure to restraint stress 1 or 2 hr daily for 7 days. In the frontal cortex, midbrain-thalamus, striatum, hypothalamus, medulla-pons, cerebellum, cervical cord, thoracic cord and lumbar cord of models with adaptability to repeated restraint stress, a marked increase in serotonin (5-HT) turnover was observed. This neurochemical change was seen in the hypothalamus and lumbar cord of non-adaptive models, but not in the other brain and spinal cord regions. Also, an increase in norepinephrine (NE) turnover in spinal cord regions was observed in adaptive models, but NE turnover was decreased in the frontal cortex, striatum or cervical cord of non-adaptive models. These findings suggest that enhancement of central 5-HT turnover and NE turnover in the spinal cord contribute to the formation of adaptation to repeated restraint stress.

Comparative studies on the inhibitory effects of calcium antagonists on cytosolic Ca2+ levels increased by high-potassium or glutamate in cultured rat cerebellar granule cells.

The inhibitory effects of calcium antagonists on high-potassium or glutamate (Glu) enhanced intracellular calcium ion ([Ca2+]i) levels were studied in cultured cerebellar granule cells. Dosages between 0.5 and 10 microM of flunarizine, nicardipine, SM6586 and SM12565 reduced the rise in [Ca2+]i induced by 50 mM KCl in a dose-dependent manner, although diltiazem, verapamil and nifedipine showed less effects on such [Ca2+]i increases. SM6586, SM12565 and flunarizine at dosages below 10 microM each reduced the magnitude of the [Ca2+]i increase induced by 25 microM Glu, but the other examined calcium antagonists were less effective. These results suggest the dissimilar efficacy of calcium antagonists on the inhibition of [Ca2+]i levels increased by high-potassium and Glu.

Enzymatic synthesis of a novel trisaccharide, glucosyl lactoside.

Cyclomaltodextrin glucanotransferase from Bacillus stearothermophilus produced a series of oligosaccharides by the transglycosylation reaction with cyclomaltohexaose as a glycosyl donor and D-lactose as its acceptor. The content of oligosaccharide A as the main transfer product was 22.1% of the total sugar. The content was increased to 34.1% by hydrolysis with glucoamylase. Oligosaccharide A was isolated by column chromatography and crystallized. By chemical and enzymatic analyses, oligosaccharide A was found to be a novel non-reducing trisaccharide; O-beta-D-galactopyranosyl-(1-->4)-O- beta-D-glucopyranosyl alpha-D-glucopyranoside.

[Nutrition for total gastrectomy. Especially the usefulness of enteral hyperalimentation].

A large number of reports have dealt with comparative studies of total parenteral nutrition (TPN) and enteral hyperalimentation (EH). However, there have been few reports on comparative studies in which patients were stringently selected for identical observation periods. We divided patients who underwent total gastrectomy for stomach carcinoma into two groups. The two groups were given TPN and EH, respectively, for three weeks to investigate immunological competence and nutritional conditions. There were ten patients in the TPN group and ten in the EH group was IgG, IgA, IgM, and complements (C3 and CH50) selected as the immunological indices. At the same time, TP, Alb, Tf, PA and RBP were used as the nutritional indices. Immunological competence dropped from one to three days after surgery in both groups, but gradually increased thereafter. Up to three weeks after surgery, immunological indices showed preoperative values or above though there were no significant differences between the two groups. The nutritional indices were low from three to four days after surgery but gradually increased thereafter and returned to preoperative levels or above, no significant differences were noted between the two groups. This was probably because malignant tumors were resected, and nutritional was supplemented from three to four days after surgery which led to the improvement of immunological and nutritional indices. Since the patients in both groups experienced uneventful clinical courses without developing any complications, there were no significant differences in these indices between the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

A patient with congenital plasminogen deficiency manifesting primary pulmonary hypertension.

A 25-year-old Japanese man was diagnosed as having primary pulmonary hypertension, on the basis of the findings of cardiac catheterization and ventilation-perfusion scintiscans. The plasma level of plasminogen in this patient was found to be reduced to 43% by a functional assay and 61 mg/l by an antigenic assay. Based on the family study, the patient was considered to have a heterozygous congenital plasminogen deficiency. Accordingly, it is suggested that the defective fibrinolysis in this patient may have played an important role in the development of primary pulmonary hypertension through microthrombosis.

Pituitary adenoma combined with Rathke's cleft cyst--case report.

A rare pituitary adenoma associated with Rathke's cleft cyst was discovered incidentally in a 44-year-old male admitted after head trauma. Neurological and physiological examination found no abnormalities, except for panhypopituitarism. Computed tomography and magnetic resonance imaging demonstrated a solid mass in the sellar cavity with suprasellar extension, associated with a cystic mass extending into the third ventricle. The tumor was removed subtotally by the transcranial approach. Light microscopy demonstrated that the cyst wall was composed of ciliated columnar cells, cuboidal cells, and goblet cells, and the solid part indicated chromophobe pituitary adenoma. Immunohistochemistry demonstrated that a few adenoma cells were positive for prolactin and the cyst wall cells were positive for cytokeratin and negative for S-100 protein.

T lymphoid/myeloid bilineal crisis in chronic myelogenous leukemia.

We describe 2 cases of "bilineal" crisis in chronic myelogenous leukemia (CML) with T cell and myeloid phenotypes. In both cases, morphocytochemically distinct myeloid and T lymphoid blast populations proliferated simultaneously in the phase of blastic crisis--myeloperoxidase (MPO)-positive, CD7+/CD33+ myeloblasts in the peripheral blood, and MPO-negative, periodic acid Schiff (PAS)-positive lymphoblasts in the lymph nodes. In each case, common karyotypes containing Ph1 translocation were demonstrated in both the peripheral blood and the lymph node samples. In Case 1, the lymph nodes were occupied by > 90% lymphoblasts, which were positive for CD2, cytoplasmic CD3 (cCD3), CD5 and CD7 and terminal deoxynucleotidyl transferase (TdT), but negative for myeloid antigens. Myeloblasts and T lymphoblasts showed an identical rearrangement of the bcr gene by Southern blotting analysis, although the clonal rearrangement of the T cell receptor (TcR)-delta gene was seen only in T lymphoblasts. In Case 2, simultaneous proliferation of myeloblasts and lymphoblasts was documented morphocytochemically in the lymph node, and a flow cytometric analysis revealed the coexistence of CD7+/CD33+ and CD7+/CD33- blast populations. Each blast population was enriched by antibody-conjugated immunomagnetic beads; the former was positive for MPO by 64% but negative for cCD3 and TdT, whereas the latter was positive for cCD3 and TdT but negative for MPO (< 1%). CD7+/CD33+ myeloblasts and CD7+/CD33- lymphoblasts showed an identical rearrangement of the bcr gene. Neither TcR-beta, TcR-gamma nor the TcR-delta gene was clonally rearranged in either population. These observations clearly indicate that T lymphoid and myeloid blasts share common Ph1-positive progenitors, and that Ph1-positive T lymphoid/myeloid progenitors are probably involved in the development of blastic transformation in some percentage of CML patients.

[Cytomegalovirus antigenemia in the differential diagnosis of pulmonary infiltrates after allogeneic bone marrow transplantation].

For the differential diagnosis of pulmonary infiltrates after bone marrow transplantation, cytomegalovirus (CMV) antigenemia was evaluated in 9 episodes of pneumonia which developed in 7 allogeneic marrow transplant patients between 9 and 495 days after transplant. The diagnosis of lung infiltration was made based on clinical findings including histological, cytological or microbiological examinations using bronchoalveolar lavage fluid specimens, sputum or lung tissue. The CMV antigen-positive leukocytes were detected with a direct immunoperoxidase technique using a peroxidase-labeled monoclonal antibody (HRP-C7) against CMV immediate early antigen. The episodes included 2 CMV pneumonias, 1 pneumocystis carinii pneumonia, 1 adenovirus pneumonia, 1 bacterial pneumonia, 1 bacterial and fungal pneumonia, 2 idiopathic pneumonias and 1 capillary leak syndrome associated with hyper acute GVHD. The CMV antigenemia became positive only in two patients with CMV pneumonia and the number of CMV antigen-positive leukocytes exceeded 10 per 50000 WBCs. The CMV antigenemia test required only 24 hours to obtain results. These observations suggest that the detection of CMV antigenemia is of great value in the differential diagnosis of pulmonary infiltrates in marrow transplant patients.

[A case of subtotal esophagectomy for esophageal basaloid carcinoma after mitral valve replacement].

We reported a rare case of subtotal esophagectomy for esophageal basaloid carcinoma after mitral valve replacement. Up to present, only this case had been printed in Japan. A 51-year-old male who had been performed MVR for mitral stenosis at the 44 complained of dysphagia. On esophageal X-ray and esophagoscopic findings showed round, irregular, and stenotic lesion (type-3) in the middle thoracic esophagus. We performed subtotal esophagectomy, esophagogastrostomy through the posterior mediastinal pathway and lymph node dissection (R2). Despite normal cardiac function before operation, the patient followed lung edema and perioperative measurement of Extra Vascular Lung Water Index (EVLWI) was stable within high level. On microscopic inspection, we revealed lots of basophilic basal cells mainly occupied submucosal layer coexisted with pseudoglandular formation and poorly differentiated squamous cell carcinoma. And the tumor cells strongly invaded into the lymph ducts and blood vessels. We need more detailed exploration for more cases about factors causing poor prognosis.

Role of calcium ion in Schistosoma mansoni cercarial tail loss induced by unsaturated fatty acids.

Linoleate (C18: 2) and oleate (C18: 1), but not stearate (C18: 0), induced tail removal in cercariae. Linoleate stimulated tail loss more strongly than oleate did. Tail loss induced by linoleate was significantly suppressed by incubating cercariae with ethyleneglycol-bis-(beta-aminoethyl ether) N,N,N',N'-tetraacetic acid (EGTA). Preincubation of cercariae with EGTA for 5 min caused further inhibition of the tail loss. Calcium ionophore A23187 (A23187) increased the cercarial tail-loss rate. When A23187 was combined with linoleate at 0.03 mM, an additive effect on tail loss appeared, whereas the ionophore in combination with linoleate at 0.3 mM had no such effect. EGTA almost completely abolished cercarial tail loss induced by linoleate at both 0.03 and 0.3 mM in the presence and absence of A23187. Linoleate at 3 mM provoked cercarial tail loss even in the presence of EGTA, although the effect of oleate at 3 mM disappeared. Under these conditions, the effect of linoleate was synergistically enhanced by the combination with A23187. A similar, but not significant, synergism took place in cercariae stimulated by oleate. These findings suggest that unsaturated fatty acids enhance calcium influx into cercariae, resulting in triggering tail loss, and, furthermore, that the fatty acids have other potentiating effects on cercarial tail loss. Protein kinases play an insignificant role in fatty acid-induced cercarial tail loss, because a protein kinase C inhibitor, 1-(5-isoquinolinylsulfonyl)-2-methylpiperazine (H-7), and an inhibitor of various protein kinases, staurosporine, had little or no effect on cercarial tail loss induced by linoleate at 3 mM.

[A case of simultaneous association of an early esophageal carcinoma and an early gastric carcinoma with radical operation].

We encountered a case of simultaneous association of an intraepithelial esophageal carcinoma and an intramucosal gastric carcinoma with radical operation. Up to present, only four cases including this report had been printed in Japan. A 67-year-old with chronic cholecystitis was pointed out the esophageal lesion (O-IIa) in the Im area by FGS after it had demonstrated the gastric lesion (early I) at the posterior wall of the prepyrolus. We performed subtotal esophagectomy, total gastrectomy, pyroloplasty with esophagoileostomy through the retrosternal tunnel and lymph nodes dissection (R2). On gross inspection, there was no obvious lesion in the esophagus. Microscopic examination revealed no invasion of cancerous cells to lymph nodes, lymph ducts, and vessels. The patient followed post operative respiratory disorders which not lung edema but atelectasis caused because perioperative measurement of Extra Vascular Lung Water Index (EVLWI) was stable within normal values.

[Study on the inhibitory effect of oral granisetron against nausea/vomiting induced by cytosine arabinoside containing chemotherapy for tumors in the hematopoietic organs].

We investigated the antiemetic effect, safety and usefulness of granisetron tablet on nausea/vomiting induced by cytosine arabinoside (Ara-C) in the chemotherapy for tumors in the hematopoietic organs. Out of 52 cases with malignant tumors in the hematopoietic organs including acute leukemia, 30 in granisetron group had no antiemetic treatment, were evaluated for the clinical efficacy of granisetron and 22 in control group. Their chemotherapies were combination therapy with Ara-C and daunorubicin (DNR), Ara-C and mitoxantrone (MIT), or Ara-C and etoposide (VP-16). In the trial, the dosage of granisetron tablet was 2 mg once a day, and the drug was given before each chemotherapy for 6 consecutive days. In clinical efficacy the effective rate of granisetron (the percentage of cases in which the trial drug was assessed as "Remarkably effective" or "Effective") was more than 80% on each day of administration. There was no adverse event. As the abnormal laboratory test value, only 1 case tested positive in urine protein, whose causal relation to the trial drug was judged as "Unassessable". Granisetron was judged as "Safe" in 31 out of 32 cases (96.9%). In terms of usefulness, the drug was rated "Extremely useful" or "Useful" in 26 out of 30 cases (86.7%). The above results have shown that granisetron tablet, when administered orally once daily at a dose of 2 mg, has an excellent antiemetic effect, and is a safe and useful drug.

[A case of penetrating head injury caused by nail-gun].

A rare case of penetrating head injury caused by a nail-gun was described. A 24-year-old male was admitted to our hospital due to head injury. He had handled a nail-gun at a construction site. On admission the patient was fully conscious with no neurological defects. A small wound was observed at the left front-temporal region. Skull films showed a large nail embedded in the skull cavity. A computed tomographic (CT) scan, changing level of window, demonstrated intracerebral hematoma, fragment of skull bone and nail. Cerebral angiography (CAG) showed extravasation at the left frontal region. Emergency operation was performed and his recovery was uneventful. Twenty-two craniocerebral injuries caused by nail-guns have been reported in world medical literature but this was the first report in Japan. The characteristics of craniocerebral nail-gun injuries were less damage and better prognosis compared with gunshot injuries. However intracranial infection and vascular injury were possible lethal complications. In this case, preoperative examination, such as CT scan and CAG was valuable and the early operation for the sake of safety was very effective.
