[Effects of body-satisfaction on social anxiety and self-disclosing behavior in adolescence].

The present study investigated effects of body-satisfaction on social anxiety and self-disclosing behavior, focusing on the relationship between body and self in adolescence. Thirty-six male and thirty-six female undergraduates talked with an interviewer of the same-sex or opposite-sex. Their interactions were video-taped. The following results were obtained: (1) body-satisfaction level had an effect on social anxiety and on the style of self-disclosing behavior, but not on the contents of disclosure, (2) the high body-satisfaction group was shy and passive with an interviewer of the opposite-sex, and this seemed to be common among late adolescents, (3) the low body-satisfaction group was anxious, shy, tense, and passive with an interviewer of the same-sex, (4) the low body-satisfaction group was both tense and active with an interviewer of the opposite-sex (these results suggest that a relationship exists between body-satisfaction and gender identity confusion), (5) body-satisfaction level had no effect on interpersonal distance.

[Studies on activated charcoal concerning the determination of sulfate in food coal-tar dyes].

It is very difficult to determine the content of sulfate in food coal-tar dye by the method designated in the "Japanese Standards of Food Additives(V)", because the endpoint on titration is difficult to gauge. We determined the concentration of sulfate in food dye nephelometrically after decolorization with charcoal but obtain different values depending on the kind of charcoal used. Because the adsorbed or eluted sulfate in the various charcoals were dispersed widely, so we tested the sulfate contents in five kinds of charcoals. It was found that all kinds of charcoals eluted or adsorbed sulfate in varying degrees. Therefore no charcoal was found to be satisfactory for use in the sulfate test.

[Severe pancytopenic episodes in two cases with mosquito-allergy].

Two young females were admitted one by one because of peculiar skin eruptions suggesting mosquito-allergy and severe pancytopenia with normo-hypercellular bone marrow. The outcome of the first case was fatal on account of severe pancytopenia, immune deficiency and marked jaundice 14 months after the initial pancytopenic episode. In the post-mortem examination, HMR (histiocytic medullary reticulosis) was highly suggestive since hepatosplenomegaly with increase of Kupffer cells and large histiocytes including some atypical cells were detected. The second patient was treated with splenectomy because the past treatment were unsatisfactory in the first case and another cases in literatures. After the operation, the patient sustained almost normal blood cell level for 11 months without any treatment. As for the histopathological findings of resected spleen and lymphnodes, some erythrophagocytic mature histiocytes were found, but no atypical cells was detected. The relationship with pancytopenia, mosquito-allergy and HMR was discussed on the two cases and literatures.

[A case report of rheumatoid arthritis which showed acute renal failure, nephrotic syndrome and drug-related lupus-like syndrome caused by D-penicillamine].

The patient was a 74 years-old male who had suffered arthralgia since April 1986. Gold therapy was performed from June 1986 based on a diagnosis of rheumatoid arthritis (RA). This treatment was ineffective and administration of D-penicillamine (D-Pc) was started from March 1987, which alleviated the arthralgia. However, proteinuria appeared, and the nephrotic syndrome (NS) and acute renal failure (ARF) gradually developed. Four items, not including the renal symptoms, fulfilled the diagnostic criteria for systemic lupus erythematosus (SLE). Thereafter, D-Pc was withdrawn, and the symptoms were improved by hemodialysis and steroid therapy. This case was considered to be NS and ARF caused by treatment of RA with rather small doses of D-Pc (18.3 g in total), but the involvement of other factors could not be ruled out. Since the four items conforming to the SLE diagnostic criteria were alleviated by steroid therapy, the case was considered to be drug-related lupus-like syndrome. In recent years, D-Pc has frequently been used in the treatment of RA and its effects have been confirmed. However, side effects often appear, and considerable caution is required in the presence of drug-related SLE as well as proteinuria. When drug-related SLE is suspected, withdrawal of the drug concerned and steroid therapy appear to be useful.

Application of an anterior eye segment analysis system in clinical cataract research.

Clinical application of a newly developed ANTERIOR EYE SEGMENT ANALYSIS SYSTEM was introduced. By this system, a Scheimpflug and a retroillumination image of the anterior eye segment are obtained through a CCD camera, separately. To overcome difficult problems experienced with previous types of lens documentation systems, several kinds of new mechanisms were built into this system. Slit and retroillumination images of some cases with cataracts and some examples of anterior eye segment analysis were described.

[Effect of induced hypotension on arterial blood-gases].

Arterial blood-gases were evaluated before, during and after the vasodilator induced hypotension in patients undergoing mastectomy. Forty-two patients studied were anesthetized with halothane and nitrous oxide in oxygen. They were divided into following four groups according to vasodilators used: trimetaphan (TMP), nitroglycerin (TNG), adenosine triphosphate (ATP), and prostaglandin E1 (PGE1). Significant reduction in PaO2 was observed during induced hypotension in all groups. However, there was no difference in the degree of PaO2 decrease among four groups. A small but significant increase in PaCO2 and a decrease in pH were observed during and/or after hypotension with TNG and PGE1. These findings suggest that induced hypotension may impair the pulmonary gas exchange by decreased cardiac output and/or change in ventilation-perfusion ratio regardless of vasodilators used. Therefore, continuous arterial blood-gas monitoring should be desirable under these conditions.

[Two cases of obstructive jaundice due to extrahepatic carcinoma of the bile duct with marked response to daily oral administration of etoposide].

Two cases of obstructive jaundice due to cancer of extrahepatic bile duct were treated with a combination chemotherapy combined with etoposide. The first case was a 81-year-old male and the second case was a 62-year-old female who introduced to this hospital because of jaundice and admitted. Cholangiography was performed and diagnosed cancer of the extrahepatic bile duct. Daily oral administration of etoposide (25mg/day) added to other immunochemotherapy was carried out. The cholangiography via PTCD tube revealed a passage of obstruction of the choledochus. These data suggest that a combination chemotherapy including oral administration of etoposide was effective in these two cases.

[Evaluation of preoperative chemoembolization using ethiodized oil, cisplatin and gelatin sponge (sandwich therapy) for hepatocellular carcinoma].

The significance of preoperative chemoembolization using ethiodized oil, cisplatin and gelatin sponge (Sandwich therapy) for resectable hepatocellular carcinoma (HCC) was evaluated. One hundred and thirteen patients with solitary and less than 10 cm sized HCC who underwent radical hepatic resection were chosen for this study. Fifty-three patients received Sandwich therapy before surgery (Group A), and the remaining 60 patients under-went surgery without any preoperative treatments (Group B). Any background factors between two groups were not significantly different. The anticancer effects of this therapy were evaluated by histologic examination in 31 patients who had preoperative Sandwich therapy. In 22 of 31 patients (71%), the main nodules were completely necrotic. The ratios of patients with complete necrosis in daughter nodules were 7/12 (58%), in portal vein tumor emboli, 7/10 (70%), in intracapsular invasions, 11/21 (52%), in extracapsular invasions, 4/11 (36%). The 4-year disease-free survival rates in Group A and Group B were 56% and 27% respectively, and the rate of the former was significantly higher than that of the latter (p less than 0.05). The 4-year survival rates in Group A and Group B were 83% and 53% respectively. The rate of Group A was also significantly higher than that of Group B (p less than 0.01). We concluded that preoperative Sandwich therapy was very significant to obtain successful long-term disease-free survival and survival in regard to relatively early stage HCC.

Decollateralization with silicone rubber sheeting for advanced hepatocellular carcinoma: a preliminary report.

A new treatment method, interception of collaterals with a silicone rubber sheet, was applied to three patients with advanced hepatocellular carcinoma in whom arterial chemoembolization was assessed as ineffective because of the developed collateral feeding arteries. This procedure was followed by arterial chemoembolization or intraportal infusion chemotherapy or both. Follow-up celiac angiography confirmed that the long-term decollateralization was achieved by shielding the liver with silicone rubber sheeting. This technique resulted in partial responses without serious complications in all the patients including two who had no responses to chemoembolization before the procedure. The response durations were 5, 21, and 27, months, respectively. One patient died of gastrointestinal bleeding 7 months after the decollateralization. The other two patients are still alive, and the survivals after the procedure are 28 and 30 months, respectively. This therapy is considered promising and may be worth choosing as an adjuvant treatment for advanced hepatic malignancies uncontrolled by arterial chemotherapy or chemoembolization.

Human pancreas scintigraphy using iodine-123-labeled HIPDM and SPECT.

The pancreatic affinity of iodine-123-labeled HIPDM (N,N,N'-trimethyl-N'-(2-hydroxy-3-methyl-5-iodobenzyl)-1,3-propane diamine) ([123I]HIPDM) was studied in 18 cases (5 normal volunteers, 7 cases with pancreas cancer, and 6 with chronic pancreatitis). In the normal cases, the pancreas was visualized in the planar images as early as 3 hr, and again at 20 hr postinjection. Single-photon emission computed tomography (SPECT) performed following 3-hr planar scintigraphy, provided excellent pancreas images without an overlap of activity in the liver or spleen. The mean pancreas-to-liver (P/L) ratio was 1.26 +/- 0.22 in normal controls. With the exception of one case of massive calcification in the pancreas, the entire pancreas could be observed in the cases with chronic pancreatitis, but the P/L ratio was 0.74 +/- 0.15, significantly lower than that of normal cases. Defective areas of the distal portion of the pancreas were clearly seen in those with cancer of the pancreas. The results of our study indicate that [123I] HIPDM may have clinical potential as a human pancreas imaging agent.

Modification of lymphokine-activated killer cell accumulation into tumor sites by chemotherapy, local irradiation, or splenectomy.

The effects of chemotherapy or local irradiation on lymphokine-activated killer (LAK) cell accumulation into tumor sites were investigated. Lymphokine-activated killer cells labeled with 111In-oxine were injected into the caudal vein of C57BL/6 mice that had been previously transplanted with 3LL cancer. An adoptive transfer of LAK cells was carried out 4 days after treatments. Twenty-four hours after the transfer, tumor tissues were excised, and the accumulation of labeled LAK cells in the tumor was measured. In two different experiments, LAK cell accumulation in tumor in the nontreated group was 2.15% and 1.58% of the administered dose per gram of tissue. The accumulation in the groups of mice treated with cyclophosphamide, nimustine hydrochloride, or Adriamycin increased fourfold (7.38% dose/g, 6.61% dose/g), threefold (6.47% dose/g) and twofold (4.46% dose/g), respectively, as compared with the nontreated group. These agents induced significant tumor regression. In the group treated with bleomycin, which showed no significant effect on tumor growth, LAK cell accumulation in tumor remained unaltered (1.57% dose/g). However, the group treated with local irradiation, which induced significant tumor reduction, showed no increase in LAK cell accumulation into tumors. These results suggest that some antitumor drugs enhance LAK cell accumulation into tumor sites and that this increase is due to tumor modification by antitumor drugs.

[A comparative study of myocardial scintigraphy with 99mTc-SQ-30,217 and 201Tl in cases with myocardial infarction].

A comparative study of myocardial scintigraphy with 99mTc-SQ-30217 (SQ) and 201Tl was performed in 6 patients having ischemic heart diseases which were confirmed by the coronary angiography. The SQ study was done 1 week after the 201Tl study. Both studies were performed in exercise and resting state. Degree of perfusion defect was compared between SQ and 201Tl SPECT by means of scoring 0-2 (0: no defect, 1: equivocal, 2: definite defect) by five doctors (two physicians and three radiologists). Ability of this agent to detect the ischemic lesions was similar to that of 201Tl except for two regions. Effect of the liver image on the evaluation of the inferior wall of the left ventricle was small on this study, although the hepatic accumulation of the tracer was prominent. This agent is a promising tracer for the evaluation of myocardial perfusion in the cases with myocardial infarction.

The results of arthrodesis of the ankle for leprotic neuroarthropathy.

Twenty-four patients who had arthrodesis of one or both ankles for leprotic neuroarthropathy were followed for an average of nine years and five months. At operation, after the removal of cartilage, joint debris, and sclerotic bone, the ankle joint was transfixed with a Küntscher intramedullary nail, and staples or Kirschner wires were used to control rotation. Fusion of bone was obtained in nineteen (73 per cent) of the twenty-six ankles. Failure to obtain fusion was due to postoperative infection in four patients, deficiency of the site of arthrodesis in one patient, and refracture through the site of fusion in two patients. When arthrodesis was successful, additional neuroarthropathic destruction of the mid-tarsal joint was halted, and the preoperative clinical symptoms of dull pain, local warmth, swelling, and instability were relieved.

[Pharmacokinetics and clinical studies on aztreonam in neonates].

Pharmacokinetic and clinical studies on aztreonam (AZT) were performed in neonates. Serum concentrations and urinary excretion of AZT were determined in 12 neonates with ages between 0 and 7 days (birth weights were between 1,260 and 3,500 g) upon intravenous injection or 1 hour drip intravenous infusion of AZT at 20 mg/kg. Serum concentrations of AZT at 1 hour after i.v. administration were 54.0 +/- 12.5 micrograms/ml, and half-lives were 6.01 +/- 0.70 hours. Serum concentrations of AZT reached their peaks at the end of drip infusion with levels of 42.1 +/- 17.6 micrograms/ml in the d.i.v. group and half-lives were 6.40 +/- 1.88 hours. Urinary recovery rates in the first 12 hours after administration were 28.5 +/- 6.4% for the i.v. group and 32.3 +/- 13.9% for the d.i.v. group. AZT was administered to 12 neonatal patients (2 cases of sepsis, 2 cases of suspected sepsis, 3 cases of pneumonia, 2 cases of urinary tract infection and 3 cases for prophylaxis), and clinical effectiveness, bacteriological efficacy and adverse reactions were evaluated. Clinical efficacies in 9 cases except 3 cases with prophylactic use were excellent in 1 case, good in 5 cases, fair in 1 case, poor in 1 case and unknown in 1 case, thus the efficacy rate was 75%. Bacteriological effects in 3 strains with Gram-negative bacilli were eradicated in 2 strains and unchanged in 1 strain, hence the bacteriological eradication rate was 66.7%. Increased GOT and GPT were observed in 1 cases as abnormal laboratory test results, but the abnormality was not serious.(ABSTRACT TRUNCATED AT 250 WORDS)

[Ventricular fibrillation after administration of aclacinomycin emulsion into mesenteric lymph nodes in a patient anesthetized with enflurane, nitrous oxide and oxygen].

A 57-year-old man was given 40 mg of activated carbon aclacinomycin (ACM) emulsion into mesenteric lymph nodes during anesthesia with enflurane, nitrous oxide, oxygen and epidural anesthesia. He had no complications preoperatively. Immediately after the injection, his skin turned to red, and 5 minutes later, sinus tachycardia, R wave amplitude reduction, T wave amplitude elevation, QT prolongation and PVCs were noted, and then, ventricular fibrillation (Vf) occurred 15 minutes after the injection. We succeeded in electrical defibrillation within about 180 seconds. At that time, both arterial blood gas and electrocytes were normal. Serum ACM concentration was remarkably elevated 60 minutes after the administration, and remained high 22 hrs later. Postoperative course was uneventful and he was discharged on the 17th postoperative day. It has been said that ACM has relatively low cardiotoxicity compared with adriamycin because of rapid distribution and metabolism. However, it might cause cardiac complication such as ECG abnormality, heart failure, pericarditis, though the effects are transient and reversible. Therefore we should be ready for its rapid treatment. Coenzyme Q10 could counteract cardiotoxicity of ACM.

Defective homologous pairing and proficient processive unwinding by the recA430 mutant protein and intermediates of homologous pairing by recA protein.

The recA protein promotes the formation and processing of joint molecules of homologous double- and single-stranded DNAs in vitro. Under a set of specified conditions, we found that the substitution of a single amino acid in the recA protein (recA430 mutation) depresses its activity for the homologous pairing to about 1/100 of that by the wild type protein when compared by the rate for the first 2-3 min of the reaction, but that the mutation only slightly, if at all, affects its ability to bind progressively to double-stranded DNA to unwind the double helix ("processive unwinding"). This is in striking contrast to an anti-recA protein monoclonal IgG, ARM193, which severely inhibits the processive unwinding but not the homologous pairing, providing further support for our conclusion that the homologous pairing and processive unwinding are functionally independent of each other. Antibody ARM193 caused the breakdown of spontaneously formed filaments of the recA protein, but the recA430 mutation did not affect the self-polymerization of the protein. The recA430 protein was apparently proficient in the functional binding to a single-stranded DNA and in the hydrolysis of ATP. However, we found that under the above conditions the mutant protein was defective as to homology-independent conjunction of DNA molecules to form a "ternary complex" (of macromolecules). These results suggest that (i) only one DNA-binding site is sufficient for the recA protein to promote the processive unwinding (the ability of the protein to form spontaneous filaments is closely related to this process) and that (ii) two DNA-binding sites on each of the recA polypeptides or those composed of a dimer (or oligomer) of the polypeptide are required for the recA protein to promote both the conjunction of parental DNA molecules and the homologous pairing (the ability to form the spontaneous filaments is not essential to this process). (iii) The simultaneous inactivation of the activity to promote the homologous pairing and that to form the ternary complex by the single substitution of the amino acid provides a physical support for the conclusion that the ternary complex is an indispensable intermediate in the homologous pairing.
