[Gonadotropin and alkaline phosphatase-producing gastric carcinoma with widespread metastasis to the bones].

A case of advanced gastric carcinoma in a 47-year-old man with widespread metastasis of generalized bone and associated with high serum levels of human chorionic gonadotropin (hCG) and alkaline phosphatase (ALP) is reported. Macroscopically, the primary tumor was Borrmann type IV. Microscopically, it showed poorly differentiated adenocarcinoma. Bone metastasis, however, showed well-differentiated adenocarcinoma. These lesions did not manifest conspicuous trophoblastic differentiation. Immunohistochemical studies revealed that hCG-positive cells were observed sporadically in the primary tumor and lymph node metastasis, but remarkably in the bone metastasis. Cancer cells in the lymph nodes were positively stained by modified Burstone's ALP staining. The serum levels of hCG and ALP fluctuated with the clinical course. Therefore, the authors concluded that this gastric carcinoma produced hCG and ALP.

[A case of gastric cancer successfully treated with hyperthermo-chemo-radiotherapy].

A 56-year-old woman was diagnosed as having gastric cancer with lymph node metastasis and invasion to the pancreas head and was admitted to our hospital in September, 1984. She was treated with hyperthermo-chemo-radiotherapy, using radiofrequency-induced hyperthermia (8 MHz) with a daily dose of fractionations at 42-43 degrees C for 60 minutes, 5 fractions per week of irradiation at 180 cGy and systemic chemotherapy. After the total doses of hyperthermia and radiation had reached 11.8 hours and 2500 cGy, the primary tumor and lymph node metastasis measured by X-ray examination were reduced in size. On October 8th, laparotomy was done, and total gastrectomy combined with total pancreatectomy, left hepatic lobectomy and right hemicolectomy was performed. Histologically, almost all of the cancer cells in the primary tumor and lymph node metastasis had disappeared. These results show that this multimodal therapy is effective for local control of gastric cancer, when used preoperatively.

[The significance of the measurement of a serum gonadotropin-like substance in gastric cancer].

The serum concentration of hCG and CEA in 277 gastric cancers was extensively studied. 8.6% for hCG and 18% for CEA were regarded as positive. 9.4% of patients showed elevated serum level for both hCG and CEA, on the other hand 7% of the patients showed high hCG only. All of hCG positive patients had severe lymph nodal involvement. All cases of hCG positive and 85% of CEA positive cases were classified as stage 4. Macroscopically, 85% of hCG positive cases and 40% of CEA positive cases showed generalized type. Histological type was poorly differentiated adenocarcinoma in 72% of tumors with elevated serum hCG level, but in only 35% of tumors with elevated serum CEA level. The content of hCG subunits in serum and tumor showed different levels among patients. The hCG positive cases did not reveal endocrine symptom. The prognosis of these patients with elevated serum hCG and CEA level were poor. One year survival rates were 0% and 30%, respectively. These results showed that both hCG and CEA, especially when used in combination, was potentially useful marker for determination of staging, recurrence and prognosis in gastric cancer.

[The effect of continuous hyperthermic peritoneal perfusion using high-dose cis-diamminedichloroplatinum for peritoneal dissemination of gastric cancers].

A continuous hyperthermic peritoneal perfusion using high-dose cis-diamminedichloroplatinum (CDDP) and its antidote, sodium thiosulfate, was applied to eight patients with peritoneal metastasis from gastric cancers. Two of eight patients showed partial response and the rate of decrease of ascites was 38%. Renal failure and leukopenia were observed in one and two patients, respectively, but no severe or lethal dysfunction was observed and all were tolerated. These results showed that CDDP exerted anti-tumor effects on peritoneal metastasis from gastric cancers when used in high doses in combination with hyperthermia.

Enhancement of mouse immune system by pyrrolomycin B.

Pyrrolomycin B enhanced both humoral immune response and delayed-hypersensitivity against sheep red blood cells in mice. In spleen cell culture it was a weak inhibitor of mitogenesis. However, in combination with concanavalin A, there was stimulation of mitogenesis in spleen cell culture. Pyrrolomycin B also enhanced phagocytosis of yeasts by peritoneal macrophages after in vivo administration to mice. Thus, pyrrolomycin B, formerly isolated as an antibiotic agent, is an immunopotentiator possibly acting on the membrane of lymphocytes or macrophages.

[The pathogenesis of diabetes produced by subtotal pancreatectomy, with special reference to the cell kinetics of the islet cells].

The pathogenesis of the diabetes produced by the subtotal pancreatectomy was investigated from the pancreatic endocrine function and the cell kinetics of the islet cells. The authors prepared the following two groups in the rats. The first group was 90% pancreatectomized rats and the second group was the 60% pancreatectomized rats. Most of the 90% pancreatectomized rats developed diabetes twelve weeks after operation. In these diabetic rats the insulin secretion was undetectable during glucose infusion. To the contrary, the relative increase of the glucagon secretion was found during arginine infusion in spite of the hyperglycemia. However, non of the 60% pancreatectomized rats developed diabetes during post-operative course up to 6 months. To investigate the cell kinetics of the islet cells, the 3H-thymidine autoradiography was prepared at several intervals after operation. The results of the thymidine labelling index showed two phase of regeneration. The first phase was observed on three to seven days after pancreatectomy and the second phase was on fourteen to twenty-eight days. After the second phase of regeneration, the degeneration of the B-cells was observed in the 90% pancreatectomized rats. On the ninetieth days after operation the atrophy of the islets and the disturbance of the arrangement of A, B and D cells were observed by the immuno-histochemical studies. Meanwhile, the 60% pancreatectomized rats did not show degeneration of the islets. These results show that the onset of diabetes after subtotal pancreatectomy was produced by degeneration of the B cells surpassing their regenerative capacity.

[A case of gastric carcinoid associated with multiple early gastric cancers and atypical epithelium].

A 69-year-old man was admitted to our hospital complaining of the sensation of abdominal fullness and tarry stool. X-ray and endoscopic examination revealed a gastric tumor measuring 6.5 X 7.5 cm in the antrum along the greater curvature. Histologically, the tumor was diagnosed as carcinoid; five lesions of early gastric cancer and one lesion of atypical epithelium were also detected. Through 1982, 24 Japanese patients with gastric carcinoid associated with gastric cancer have been reported.

Inhibition of histidine decarboxylase and tumour promoter-induced arachidonic acid release by lecanoric acid analogues.

Lecanoric acid analogues containing benzanilide structure inhibited histidine decarboxylase and arachidonic acid release from the cell membrane phospholipids induced by a tumour promoter, 12-O-tetradecanoylphorbol-13-acetate. But they did not inhibit cellular binding of phorbol-12,13-dibutylate. Lecanoric acid analogues also inhibited prostaglandin synthetase and delayed-type hypersensitivity responses against sheep red blood cells in mice. Thus, lecanoric acid analogues antagonized several enzymic and cellular effects of the tumour promoter.

Ventilation requirements during high frequency ventilation.

This study was undertaken to investigate ventilatory requirements during high frequency ventilation (HFV). Six anesthetized dogs were ventilated with bird M-2 or Emerson 2-V ventilators at respiratory rates (RRs) ranging from 13-1300 breath/min. PaCO2 was maintained within normal range at all rates by tidal volume (VT) adjustment. Required minute volume (VE) increased linearly with rate while VT decreased exponentially and approached a plateau at rates above 200. Airway pressure was inversely proportional to rate at rates below 80 but increased with rate thereafter. A method is provided to estimate required ventilatory volume during HFV based on the results of this study. It was concluded that gas exchange during HFV can be explained by conventional concepts of ventilation and with an unvented nonrebreathing system no benefit accrues from respiratory rates above 200 inasmuch as neither airway pressure nor VT can be reduced in the face of increasing VE requirements.

Protective effects of beta blockade on pulmonary function when intracranial pressure is elevated.

Intracranial pressure (ICP) was increased by hyperosmolar intracerebral infusion in dogs and the cardiopulmonary and catecholamine (CA) responses followed for 4 h. Increased ICP evoked persistent increases in endogenous CAs, pulmonary vascular pressures, pulmonary blood volume, and venous admixture. Other dogs similarly monitored were treated with a beta-blocking dose of propranolol 25 min after the onset of increased ICP. Although catecholamines were increased, elevated pulmonary pressures and venous admixture returned to control levels. CO and heart rate (HR) were reduced after beta blockade but systemic vascular resistance increased. It was concluded that increased ICP induces sustained increases in CAs which adversely affect pulmonary pressures and shunting. Selective beta blockade reverses these effects and may be useful in patients with evidence of sympathetic overactivity and progressive hypoxemia after head injury.

Vanoxonin, a new inhibitor of thymidylate synthetase.

Vanoxonin, a new inhibitor of thymidylate synthetase, was found in cultured broths of the strain MG245-CF2 classified as Saccharopolyspora hirsuta. Vanoxonin, C18H25N3O9, was obtained as colorless powder. Vanoxonin forms a vanadium complex which exhibits a strong inhibition against thymidylate synthetase. The concentration for 50% inhibition of the enzyme (IC50) was 0.7 micrograms/ml.
