The patient was a 27-year-old man who developed spastic quadriplegia and cerebral disorders. Initial signs were gait disturbance, spastic paraplegia, and sphincter disturbance; these occurred when he was 21. Upon autopsy, the white matter of the brain and spinal cord showed diffuse demyelination, and the adrenal glands and right testis were atrophic. Cytoplasmic striations seen by light microscopy and trilamellar inclusions seen by electron microscopy were found in ballooned adrenocortical cells. Trilamellar inclusions were also observed in macrophages of the affected cerebral white matter. Biochemical analysis disclosed a high ratio (0.27) of hexacosanoic acid (C26:0) to docosanoic acid (C22:0) in cerebrosides and sulfatides of the cerebrum. The histological features as well as the result of biochemical analysis were those of classical adrenoleukodystrophy. However, the time of the onset of clinical signs and the duration of the disease were different in the present case from classical adrenoleukodystrophy. The case presented here was diagnosed as adrenoleukomyeloneuropathy, which is a variant of adrenoleukodystrophy.
The clinical usefulness of measuring serum beta 2-microglobulin (BMG) and squamous cell carcinoma-related antigen (SCC-Ag) was studied, and it was found that both were elevated in patients with squamous cell carcinoma of the lung. In squamous cell carcinoma, serum BMG was high in patients who were more than 65 years old, whereas it was not raised in those with other histological types or with benign respiratory diseases. This increase in BMG, however, was thought to correlate with the clinical stage rather than with aging. The rise of BMG was suggested, similar to the rise of SCC Ag, to be specific to squamous cell carcinoma of the lung.

We performed both free and albumin-bound bilirubin removal therapy using the Evaflux-2A and evaluated its clinical effects. The removal rates of total bilirubin in the first and second sessions were 31.4%, and 31.0%, respectively. Our clinical data suggested that plasma exchange using Evaflux-2A and an albumin-added electrolyte solution as the substitute fluid was useful in the treatment of hyperbilirubinemia.
Sulpiride, which has been utilized as an antipeptic ulcer and antidepressant in Japan, is one of the dopamine antagonists, and is considered to have anti vomiting activity. We designed a comparative controlled study of Sulpiride and Domperidone, against nausea and vomiting during cancer chemotherapy. A total of 69 cases were entered into this study, 34 cases being treated with Sulpiride at a dose of 100 mg i.v. or i.m. (S group), and 35 cases being treated with Domperidone at a dose of 10 mg i.v. (D group), before and after chemotherapy. Three cases from the S group and 4 cases from the D group were excluded from this study because of differences in the administration schedule. There was no difference in patient characteristics between the two groups, such as age, sex, original disease and antitumor agents. According to our criteria 27 of 31 cases in the S group showed effectiveness of the agent against nausea and vomiting (87%), while in the D group, 24 cases showed effectiveness (77%). There was no statistical difference between them, and no side effects were found in either group.

The glutathione S-transferases (GSTs) of hepatic cytosol of rats, mice, guinea pigs, rabbits and hamsters were simultaneously investigated with respect to substrate specificity, subunit composition and elution profile of GSTs. The activity towards 1-chloro-2,4-dinitrobenzene was the highest in hamsters, followed by rabbits, guinea pigs, mice and rats. On the analysis of SDS-PAGE, Ya subunit band of GST was detected in only rats, not others. There was also seen a marked species difference in elution profile of GST activity on S-sepharose column. The elution patterns from rats, mice and rabbits were quite different from hamsters and guinea pigs.
A case of convexity cavernous hemangioma associated with sellar meningioma with parasellar extension is presented. A 61-year-old female who had complained of left blepharoptosis and diplopia was admitted to our hospital. On admission she showed left oculomotor nerve palsy. Plain CT revealed an isodense mass in the sellar and parasellar region. Computed angiotomography demonstrated that this mass was enhanced heterogeneously and filled the sellar turcica and extended superiorly. And homogeneously enhanced mass in the convexity without mass effect was observed. Angiogram revealed no tumor stain in any phase. With these findings, this case was diagnosed as the multiple meningiomas preoperatively. Left frontotemporal craniotomy was carried out, and subtotal removal of sellar and parasellar tumor and total removal of convexity tumor were performed. Pathologically the sellar and parasellar tumor was diagnosed as meningioma, convexity tumor as cavernous hemangioma derived from dura mater. Convexity cavernous hemangioma derived from dura mater is very rare, only one case was reported previously. Only 2 cases of cavernous hemangioma associated with meningioma have been reported in the literature. Correlation between cavernous hemangioma and meningioma is discussed. It is possible to owe this complication to the developmental abnormality concerning with inner factor like genetic factor and so on, because both tumors are mesodermal origin. However it is proper to regard that this complication occurred by chance.

A comparative study of the effect of misonidazole and novel radiosensitizers on glutathione (GSH) levels and related enzyme activities in isolated rat hepatocytes was performed. Incubation of hepatocytes with 5 mM radiosensitizers led to a decrease in the intracellular GSH level. The most pronounced decrease in cellular GSH was evoked by 2,4-dinitroimidazole-1-ethanol (DNIE); after incubation for only 15 min, GSH was hardly detected. DNIE-mediated GSH loss was dependent upon its concentration. DNIE reacted with GSH nonenzymatically as well as with diethylmaleate, while misonidazole and 1-methyl-2-methyl-sulfinyl-5-methoxycarbonylimidazole (KIH-3) did not. Addition of partially purified glutathione S-transferase (GST) did not enhance DNIE-mediated GSH loss in a cell-free system. DNIE inhibited glutathione peroxidase (GSH-Px), GST, and glutathione reductase (GSSG-R) activities in hepatocytes, while misonidazole and KIH-3 did not. GSH-Px activity assayed with H2O2 as substrate was the most inhibited. Inhibition of GSH-Px activity assayed with cumene hydroperoxide as substrate and GST was less than that of GSH-Px assayed with H2O2 as substrate. GSSG-R activity was decreased by DNIE, but not significantly. Incubation of purified GSH-Px with DNIE resulted in a little change in the activity when assayed with H2O2 as substrate.
The tissue distribution of muscle-type alpha beta and beta beta enolases in rats were determined with the sandwich-type enzyme immunoassay method which utilized the purified antibodies specific to the alpha and to the beta subunit of enolase, and beta-D-galactosidase from Escherichia coli as label. All the tissues examined contained detectable levels of both alpha beta and beta beta enolases. The beta beta enolase was found at high levels in the skeletal muscle tissues (tongue, esophagus, diaphragm and leg muscles) and in the cartilages (xipoid process and auricular cartilage). The alpha beta enolase was distributed at a relatively high concentration in the heart and in the above-mentioned tissues. The beta beta enolase in the leg muscles, diaphragm and tongue was present on the day of birth at a concentration higher than that of the alpha alpha and alpha beta enolases, and its concentration further increased in a manner apparently related to the functional state of each tissue. Denervation of the leg muscles by cutting the sciatic nerve in adult rats resulted in a drastic change in the isozymes profile. The concentration of beta beta enolase in the tibialis anterior gastrocnemius lateralis and extensor digitorum longus (about 800 pmol/mg protein) decreased to about a half in a few weeks after denervation. In contrast, the concentrations of alpha alpha (2 pmol/mg) and alpha beta (80 pmol/mg) usually showed a slight increase by the treatment (alpha alpha, 7 pmol/mg; alpha beta, 100 pmol/mg after 2 weeks). As compared with these three muscles, the soleus had normally a low enolase level and the effect of denervation was less drastic. These results seem to suggest that the concentration of beta beta enolase is closely correlated with the functional state of the muscle tissue.
Slowly discharging neurons in the cat dorsal raphe could be classified into 3 types according to the behavior of antidromic spike discharges during repetitive stimulation of the medial forebrain bundle at 10 Hz. In the types 1 and 2, the latency of antidromic discharge was gradually prolonged to reach an asymptote, whereas no marked change occurred in the type 3. The type 2 neurons, which had a slower conduction velocity, showed a greater prolongation than the type 1 neurons. The maximum length of this prolongation was not significantly correlated with the initial latency. During 10 Hz stimulation some neurons showed repeatedly a conduction block after a sequence of initial decrease and later increase in latency. The spontaneous discharge was strongly suppressed during 10 Hz stimulation. During 1 Hz stimulation just after the cessation of 10 Hz stimulation, the prolonged antidromic latency was gradually restored in parallel with the recovery of the spontaneous discharge. Circumstantial evidences seem to be in favor of the idea that hyperpolarization of the axonal and somatic membranes is mainly responsible for the observed behavior of antidromic spikes of type 1 and 2 neurons.
