Plasma-cell dyscrasia with polyneuropathy and endocrine disorders associated with dysfunction of salivary glands.

A case of plasma-cell dyscrasia with polyneuropathy and endocrine disorders that showed dysfunction of the salivary glands is reported. A 49-year-old Japanese man noticed swelling of the cervical lymph nodes and numbness in the lower extremities in May 1983. Histological examination of the enlarged cervical lymph nodes revealed many follicles penetrated by radial capillaries and proliferation of capillaries and plasma cells in the interfollicular area, forming Castleman disease-like lesions. The patient complained of a dry mouth and noticed swelling of the submandibular salivary gland in April 1984. Microscopic examination of this gland revealed an angiofollicular lymphoid lesion resembling that in the lymph nodes. He also suffered from an endocrine disturbance characterized by increased serum adrenocorticotropic hormone and impotence. This is the second reported case of plasma-cell dyscrasia with polyneuropathy and endocrine disorders that showed dysfunction of exocrine secretion. This case indicates that attention must be paid to organs of exocrine secretion as well as of endocrine secretion in this disease.

Multiple efferent arterioles of the human kidney glomerulus as observed by scanning electron microscopy of vascular casts.

About three thousand resin vascular casts of human renal glomeruli were examined with a scanning electron microscope, and two extremely rare glomeruli with quadruple or quintuple efferent arterioles were found. Each of these multiple efferent arterioles arose independently from the glomerular capillary tuft supplied by a single afferent arteriole. In addition, 34 glomeruli possessed double or triple efferent arterioles and two showed double afferent arterioles. In one of the former cases, one of the double efferent arterioles had a heterotopic origin near the urinary pole of the glomerulus. No glomerulus with triple or more afferent arterioles was noted.

Regression of hypertrophy in small resistance vessels.

Many studies have documented regression of left ventricular hypertrophy following blood pressure control by some antihypertensive agents but not by others. To determine whether similar differences in regression of wall thickening also occur in resistance vessels during treatment, matched control groups of spontaneously hypertensive rats (SHR) were treated for 12 weeks with either hydralazine or captopril + hydrochlorothiazide and compared with untreated SHR and Wistar-Kyoto rats (WKY). Perfusion pressure was then determined in the hind-limbs of pithed rats under conditions of constant blood flow (4.0 ml/min) and maximal vasodilatation as an index of thickening (hypertrophy) of resistance vessel walls. Treatment with hydralazine or captopril + hydrochlorothiazide controlled blood pressure equally in SHR but had significantly different effects on both left ventricular hypertrophy and resistance vessels. Perfusion pressure was reduced from 37.4 +/- 0.5 mmHg to 33.9 +/- 0.5 mmHg (P < 0.01) with captopril + hydrochlorothiazide but only marginally to 35.9 +/- 0.3 mmHg with hydralazine (P < 0.05 at some levels of flow and P > 0.05 at others). Left ventricular weight was significantly reduced by captopril + hydrochlorothiazide (2.02 +/- 0.02 versus 2.63 +/- 0.05 mg/g, P < 0.01) but only to 2.44 +/- 0.05 by hydralazine. Significant correlations were found both before and following treatment between perfusion pressure and left ventricular weight (r = 0.59, P < 0.01) but not between either of these two parameters and arterial pressure. Thus, despite equal blood pressure control, thickening of resistance vessel walls regressed more with captopril + hydrochlorothiazide than with hydralazine, suggesting that vascular hypertrophy-like left ventricular hypertrophy is not determined by blood pressure levels alone.

Polypoid traumatic neuroma of the gallbladder.

We report a case of polypoid traumatic (amputation) neuroma incidentally found in a gallbladder resected for cholelithiasis. This traumatic neuroma may have developed as the result of injury to the gallbladder during an unsuccessful cholecystectomy performed 20 years previously. Traumatic neuromas are known to arise at the stump of the cystic bile duct after cholecystectomy, but, to our knowledge, there seem to be no previous reports of traumatic neuromas arising in the gallbladder.

[A study of bone metastasis of cervical carcinoma by bone scintigraphy].

In carrying out bone scintigraphy in 224 cases over the 5 years from June, 1978 to May, 1983 as a part of the post-treatment management of cervical carcinoma, we obtained the following findings concerning bone metastasis. Bone metastases were seen in 12.5% (28 cases) of the subjects, about 6% of the total post-treatment cases of cervical carcinoma in the corresponding period (466 cases). Bone metastases were seen in 9.3% (16/172) of post-operative cases, compared with 23.1% (12/52) of non-operative cases. Bone metastases were not seen in clinical stages Ia through IIa (49 cases) but were seen in IIb or higher stages. Bone metastasis rates by histological type, according to WHO classification, were 12.8% (26/203) in squamous cell carcinoma, 5.9% (1/17) in adenocarcinoma, and 25% (1/4) in adenosquamous carcinoma. Among the squamous cell carcinoma cases, small cell non-keratinizing type had the highest bone metastasis rate (p less than 0.05). Of 172 post-operative cases, 20.8% (11/53) of those with lymph node metastasis exhibited bone metastasis, higher than the 4.2% (5/119) in cases without lymph node metastasis. As to CPL classification, bone metastasis was seen more often in L type (18.8%) than C(0.0%) or P types (6.6%). Our risk classification of 168 cases demonstrated that bone metastasis was not seen in risk I group (74 cases), but was seen in 6.7% (1/17) of risk II group and in 19.0% (15/79) of risk III group. Twenty-eight cases with bone metastasis included 11 cases with local recurrence, 8 with pulmonary metastases, 4 with hepatic metastases and 4 with Virchow's lymph node metastases.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of remission in diffuse pleural mesothelioma induced by anticancer chemotherapy with cisplatin and adriamycin].

A 56-year-old man was admitted to our hospital with complaints of dyspnea and right pleural effusion. Malignant pleural mesothelioma was diagnosed, based on the high level of hyaluronic acid in the pleural fluid and manifestation of malignant cells by pleural needle biopsy. Anticancer chemotherapy with cisplatin and intrapleural injections of adriamycin against this mesothelioma was carried out because of the presence of stage III by Butchart's classification. Regression of the tumor, decrease of pleural fluid and decrease of LDH level in effusions were observed after this chemotherapy. Cisplatin was easily transported into the pleural fluid, and its concentration was maintained for a satisfactorily long period.

Natural killer (NK) and lymphokine-activated killer (LAK) cell activity in healthy volunteers with special emphasis on the familial incidence of cancer.

Natural killer (NK) and lymphokine activated killer (LAK) cell activities in 133 healthy volunteers were analyzed with regard to the volunteer's sex, age, smoking history and the familial incidence of cancer. None of these factors had any influence on NK and LAK cell activities. It was concluded that identifying individuals with increased risk of cancer development by examining NK and LAK activities would be difficult.

Co-storage and co-secretion of somatostatin and catecholamine in bovine adrenal medulla.

Co-storage and co-secretion of somatostatin (SRIF) and catecholamine (CA) were demonstrated using bovine adrenal medulla. In the experiment of retrograde perfusion system of isolated adrenal gland, the basal concentration of immunoreactive (IR)-SRIF was 20 pg/4 ml/min, but a significant increase of the value (142 pg/4 ml/min) was observed at 1-2 min after a brief infusion of acetylcholine (10 mM) and the marked release continued for over 6 min. Similar result was obtained for CA release. On gel-filtration chromatography of a soluble lysate of chromaffin vesicles prepared by differential and discontinuous sucrose density centrifugation, IR-SRIF was separated into three components with molecular sizes corresponding to prepro-SRIF (76.8% of the total immunoreactivity), SRIF28 (14.1%) and SRIF14 (9.1%).

Nucleotide sequence of cucumber pale fruit viroid: homology to hop stunt viroid.

Double stranded cDNA of cucumber pale fruit viroid ( CPFV ) has been cloned by the method of Okayama and Berg (Mol.Cell.Biol.2,161-170 (1982] and the complete nucleotide sequence was established. The covalently closed circular molecules of single-stranded CPFV RNA consists of 303 nucleotides. The nucleotide sequence of CPFV was compared with the previously established sequence of hop stunt viroid (HSV), which consists of 297 nucleotides ( Ohno et al. Nucleic Acid Res.11,6185-6197 (1983]. CPFV differs from HSV in the nucleotide sequence at 16 positions which include 8 exchanges, 7 insertions and 1 deletion. Both viroids share about 95% sequence homology. Considering the pathogenic properties of both viroids together, it is concluded that CPFV is a cucumber isolate of HSV.

A revised replication cycle for viroids: the role of longer than unit length RNA in viroid replication.

Longer than unit length plus and minus strand RNAs were detected in hop stunt viroid (HSV) infected cucumber leaf tissues by Northern blot hybridization analysis using strand-specific probes. To elucidate the role of these longer than unit length RNAs in the viroid replication cycle, we synthesized tandemly repeated plus and minus strand HSV RNAs in vitro from cloned HSV cDNA and assayed their infectivities. Two and four unit tandemly repeated plus strand RNAs were infectious, but one unit plus, and one, two and four unit minus strands were noninfectious. Taking these data into consideration, we propose a revised rolling circle model for viroid replication.

Primary Burkitt-type lymphoma of the central nervous system.

A primary Burkitt-type lymphoma of the central nervous system (CNS) is presented. The right temporoparietal tumor in a woman of 55 was diagnosed histologically as a Burkitt-type lymphoma. Primary Burkitt-type lymphoma of the CNS is extremely rare and has been reported previously only four times. This is the first case reported in Eastern Asia.
