An extracellular quinoprotein oxidase that catalyzes conversion of enacyloxin IVa to enacyloxin IIa.

A new extracellular quinoprotein oxidase named enacyloxin oxidase (ENX oxidase), which is involved in biosynthesis of ENX IIa, a congener of ENX, was found in the culture supernatant of Frateuria sp. W-315 and purified as a homogeneous protein on SDS-PAGE. ENX oxidase was shown to have a molecular mass of 73 kDa by SDS-PAGE and 79 kDa by gel filtration. The enzyme was inhibited by various carbonyl reagents and the activity was stimulated by addition of PQQ. This is the first report on a quinoprotein oxidase that is secreted into the culture medium in the logarithmic growth phase, and acts for biosynthesis of the antibiotic.

Calcified intracranial metastatic tumor mimicking meningioma--case report.

A 60-year-old female presented with mild cerebellar dysfunction due to a calcified tumor attached to the undersurface of the tentorium cerebelli demonstrated by cranial computed tomography, and a lung mass on a chest x-ray film. The calcified nature and location made preoperative differentiation between metastatic brain tumor and meningioma difficult. Operation subsequently revealed that the brain tumor was a metastasis from lung adenocarcinoma. Metastatic brain tumors can be calcified, and should be considered in the differential diagnosis of calcified intracranial lesions.

Effects of long-term enalapril treatment on persistent micro-albuminuria in well-controlled hypertensive and normotensive NIDDM patients.

OBJECTIVE: To determine whether long-term treatment with an angiotensin-converting enzyme (ACE) inhibitor has a beneficial effect on the urinary microalbumin excretion and renal function in non-insulin-dependent diabetes mellitus (NIDDM) patients, enalapril (5 mg/day) was administered for 48 months. RESEARCH DESIGN AND METHODS: -Fifty-two patients with NIDDM who had persistent microalbuminuria in the range of 20-300 mg/24 h, serum creatinine < 106.1 microM (1.2 mg/dl), supine systolic blood pressure (BP) < 150 mmHg, supine diastolic BP < 90 mmHg, and HbA1c < 10% were divided into four groups. Twenty-six patients with normotension were divided at random into two groups; one group received enalapril (5 mg/day) (NE group), the other did not receive enalapril (NC group). In the same way, 26 other patients who were already well-controlled with nifedipine (30 mg/day) over a long-term period (4-6 years) were divided at random into two groups; one received enalapril (5 mg/day) (HE group), the other did not receive enalapril (HC group). RESULTS: After 48 months, urinary albumin excretion (UAE) was markedly reduced in group NE from 102.4 x/divided by 1.3 to 55.5 x/divided by 1.3 mg/24 h (P < 0.005), whereas no significant change occurred in group NC. In the well-controlled hypertensive groups, a significant reduction in UAE occurred in group HE (P < 0.05), whereas no significant change occurred in group HC. No changes in creatinine clearance, BP, or blood glucose control were seen during the study. CONCLUSIONS: Treatment with enalapril for 48 months may have a beneficial effect on the decline of microalbumin excretion in NIDDM patients.

Lung cancer containing growth hormone-releasing hormone associated with hypertrophic osteoarthropathy. Case report.

In a 49-year-old man, large-cell carcinoma of the lung was accompanied by raised serum levels of growth hormone (GH) and growth hormone-releasing hormone (GHRH) and hypertrophic osteoarthropathy. Immunohistochemically the tumour contained GHRH but not GH. The osteoarthropathy disappeared after resection of the primary tumour and did not reappear after its recurrence. The high serum GH level presumably was due to ectopic GHRH production in the tumour. The hypertrophic osteoarthropathy was not clearly attributable to these hormones.

Hispidospermidin, a novel phospholipase C inhibitor produced by Chaetosphaeronema hispidulum (Cda) Moesz NR 7127. I. Screening, taxonomy, and fermentation.

A novel phospholipase C inhibitor, hispidospermidin, was discovered from a fungal culture broth. The producing fungus, NR 7127, formed abundant pycnidia on banana leaf agar under near UV light. The ostiolate pycnidia were dark colored with a short beak possessing numerous protruding setae. The conidiogeneous cells were phialidic. The conidia were hyaline, 1 septate, smooth and spindle-shaped. From these distinctive characteristics, this strain was identified as Chaetosphaeronema hispidulum (Cda) Moesz of the Coelomycetes. Hispidospermidin was produced in a 50-liter jar fermentor containing 2% glucose, 2% potato starch, 2% Toast soya, 0.5% yeast extract, 0.25% NaCl, 0.0005% ZnSO4.7H2O, 0.0005% CuSO4.5H2O, 0.0005% MnSO4.4H2O, 0.32% CaCO3, and 0.3% Nissan disfoam CA-115. Fermentation was conducted at 27 degrees C at an aeration rate of 30 liters/minute and agitated at 500 rpm for 95 hours. Maximum production yield of hispidospermidin was observed after 72 hours. Hispidospermidin inhibited rat brain phospholipase C at 16 microM of IC50. This is the first recorded discovery of a secondary metabolite from the genus Chaetosphaeronema.

Fusarium merismoides Corda NR 6356, the source of the protein kinase C inhibitor, azepinostatin. Taxonomy, yield improvement, fermentation and biological activity.

Fungal strain NR 6356, Fusarium merismoides Corda, was discovered as the source of the protein kinase C (PKC) inhibitor, azepinostatin. The strain was identified based on its growth on potato sucrose agar, slender conidial shape, characteristic polyphialide and production of abundant chlamydospores. Fusarium aquaeductuum Lagh. IMI 103658 and Fusarium sp. NR 7222 were also found to produce the same inhibitor. After single colony isolation and medium optimization trials, a more than 30-fold increase in the production of azepinostatin over the original culture was achieved. Azepinostatin selectively and potently inhibited rat brain PKC with an IC50 value of 70 nM. Other enzymes utilizing ATP, including hexokinase, were not affected. The Ki of azepinostatin for PKC was 0.5 nM. The inhibition of PKC was competitive with ATP and uncompetitive with histone.

Scanning electron microscopy and energy-dispersive X-ray microanalysis studies of several human calculi containing calcium phosphate crystals.

Human calcium phosphate calculi: two sialoliths, a urolith, a rhinolith, and a tonsillolith were investigated by scanning electron microscopy (SEM) and energy-dispersive X-ray analysis (EDX). The sialoliths and urolith had appositional shells with thick cortices, respectively, around several nuclei composed of calcospherulites and a rubber-film fragment. The rhinolith had a thin cortex with appositional laminations around a glomerulus-like mass of calcified cotton-like strings. The tonsillolith had a rough cortex with appositional laminations. Its porous interior was composed of numerous calcified conglomerates with microorganisms and calcified masses with fine appositional laminations around the conglomerates. The major crystals were identified as biological apatites (AP) with a sand-grain rather than a needle-like shape, and plate-shaped octacalcium phosphate (OCP). The AP deposits of the rhinolith probably were associated with magnesium (Mg) phosphates or contained Mg. No OCP was found in the rhinolith. The AP deposits were mainly formed by extracellular calcification. Hexahedral crystals, identified as Mg-containing whitlockite (WH), were precipitated in the internal spaces of the AP and OCP deposits. The rhinolith nucleus consisted of WH crystal deposits only.

[Pharmacokinetic and clinical evaluation of cefozopran in the pediatric field].

We conducted pharmacokinetic and clinical studies on cefozopran (CZOP, SCE-2787), an aminothiadiazolmethoxyiminoacetamido cephalosporin, and obtained the following results. 1. Concentrations in serum/excretion in urine We studied pharmacokinetic in children upon intravenous bolus injection and 30-minute intravenous drip infusion in single doses of 10 and 20 mg/kg. Upon intravenous bolus injection, mean serum concentrations 30 minutes after administration of 10 and 20 mg/kg were 27.8 and 52.3 micrograms/ml, respectively, and half-lives were 2.01 and 2.02 hours, respectively. Upon 30-minute intravenous drip infusion, mean serum concentrations on completion of the drip infusions of 10 and 20 mg/kg were 36.8 and 70.3 micrograms/ml, respectively, and half-lives were 1.74 and 4.11 hours, respectively. Their urinary recovery rates in the first 8 hours after administration were higher than 67.0% in the former regimen and between 23.2 to 98.0% in the latter. 2. Clinical results 54 patients were treated with CZOP for 32 cases of pneumonia, 11 cases of bronchitis, 3 cases of cervical lymphadenitis, 1 case of purulent tonsillitis, 2 cases of phlegmon, 2 cases of enterogastritis and 3 cases of urinary tract infections. CZOP gave "excellent" or "good" responses in 53 cases. 1 case of urinary tract infection showed fair response. Diarrhea was observed in 1 case. Abnormal laboratory test results were noted in 9 patients including elevations of eosinophils, GOT and GPT. In no cases the treatment had to be discontinued.

[Pharmacokinetic and clinical studies on biapenem (L-627) in the pediatric field].

Pharmacokinetic and clinical studies on biapenem (L-627), a newly developed carbapenem, were performed and the following results were obtained. 1. Absorption/excretion: Pharmacokinetics of biapenem was studied in 14 children at doses of 6 mg/kg and 12 mg/kg administered through 30 minutes-drip infusion. Peak plasma levels and plasma half-lives of the 2 doses were 22.5, 29.9 micrograms/ml, and 0.84, 0.85 hours, respectively. Their urinary recovery rates were 54.5 to 76.1% and 37.3 to 59.5%, respectively. Cerebrospinal fluid levels of biapenem in two patients with purulent meningitis were 0.88 and 2.72 micrograms/ml, and the penetration rates were 3.7 to 8.3%. 2. Clinical study: Forty-nine patients were treated with biapenem at doses exceeding 90 to 100 mg/kg/day for purulent meningitis and at doses between 15.0 and 36.0 mg/kg/day for other infections. Biapenem gave "Excellent" or "Good" responses in 48 cases, hence an efficacy rate of 98.0% was obtained. Only one patient with pneumonia showed a fair response. No adverse reactions were observed. Abnormal laboratory test results were noted in 7 patients including elevation of GOT, GPT, and eosinophils. In no cases the treatment had to be discontinued.

The effect of probucol on the development of non-insulin dependent diabetes mellitus in Otsuka Long Evans Tokushima Fatty (OLETF) rats.

The effect of probucol which acts as a scavenger of free radicals, on preventing the development of non-insulin dependent diabetes mellitus (NIDDM) in a model rat (Otsuka Long Evans Tokushima Fatty [OLETF]) was examined. Sixteen male OLETF rats aged 5 weeks were divided into two groups. One group were fed standard laboratory chow and the other group were fed the chow containing 1% probucol. At 12, 16, 24 weeks of age, an oral glucose tolerance test (OGTT) was performed. At 24 weeks of age, the cumulative incidences of diabetes mellitus in both groups were 100%. After two weeks, insulin-mediated glucose uptake was measured using a euglycemic clamp technique. There was no significant difference in glucose infusion rates between both groups. At the end of the experiment, plasma lipid concentrations were measured. The serum total cholesterol levels were significantly lower in rats fed probucol containing chow (66.3 +/- 3.7 mg/dl) than those of rats fed standard laboratory chow (78 +/- 6.4 mg/dl). On the other hand, the serum triglyceride level were not significantly different between them. Histological studies on the pancreas of both groups showed enlarged fibrotic islets and mononuclear cell infiltration in the periinsular area, though to a modest extent. These results demonstrated that probucol was not effective in preventing the development of NIDDM in this model animal. Considering these data, inflammatory destruction of pancreatic beta cells by free radical may not be involved in the development of NIDDM.

Myasthenia gravis after allogeneic bone marrow transplantation.

A 37-year-old Japanese man with chronic myeloid leukemia (CML) developed myasthenia gravis 29 months after bone marrow transplantation (BMT) from an HLA one locus-mismatched brother. Proximal muscle weakness and bilateral ptosis occurred along with the exacerbation of chronic GVHD shortly after sudden cessation of cyclosporine (CYA) and prednisolone. The diagnosis of myasthenia gravis was made based on clinical symptoms and elevation of an anti-acetylcholine receptor antibody titer and all symptoms related to myasthenia gravis promptly diminished with the start of treatment for chronic GVHD. In most previously reported cases, the underlying disease was aplastic anemia (6 of 7 cases) and donors were of the opposite sex (6 of 7 cases). The haplotypes HLA B7 (3 of 5 cases), B35 (3 of 5 cases), and DR2 (3 of 3 cases) were common. All cases suffered from chronic GVHD. The present case had only chronic GVHD and HLA B7 as a background for myasthenia gravis after BMT. The abrupt cessation of immunosuppressive therapy may also be related to the development of myasthenia gravis after BMT.

[A case of ventricular septal defect and patent ductus arteriosus associated with absent right pulmonary artery, scimitar syndrome and severe pulmonary hypertension].

A 8 month-old female was diagnosed with ventricular septal defect and patent ductus arteriosus associated with absent right pulmonary artery and scimitar syndrome. Cardiac catheterization revealed severe pulmonary hypertension as follows; 72 mmHg of mean pulmonary artery pressure, 0.56 Qp/Qs and 1.73 Pp/Ps. Temporary closure of the ductus reduced the mean pulmonary artery pressure from 72 to 40 mmHg. PDA was ligated and VSD was closed successfully. However, 4 months after initial operation she was readmitted due to infection of the hypoplastic right lung. Removal of the hypoplastic right lung was performed and postoperative course was uneventful.

[Overview of clinical trials on adjuvant chemotherapy for curatively resected gastric cancer].

Reports of the results of clinical trials on adjuvant chemotherapy for curatively resected gastric cancer were reviewed. All but two trials did not show its efficacy. In spite of this, understanding some results of subset analysis as definite proof, a consensus was achieved in Japan on its efficacy due to lack of proper knowledge of medical statistics among surgeons. Recently it has been recognized that this consensus was groundless and that evaluation of adjuvant chemotherapy should be carried out again. The results of the trial of JCOG, comparing adjuvant chemotherapy versus control (surgery alone) are therefore awaited. For the moment, all clinical trials on adjuvant chemotherapy for curatively resected gastric cancer should have an arm of surgery alone as control.

A preliminary investigation of a method of detecting temporomandibular joint sounds.

This study established a method of detecting temporomandibular joint sounds based on signal-to-noise ratios. After comparing the temporomandibular joint signals obtained from three different sites over the skin, the articular eminence was found to be the best site for detecting temporomandibular joint sounds; this site provided the highest mean amplitude in the time domain waveform. However, using an electret condenser microphone at the intra-auditory meatus provided a broader, 20-decibel signal-to-noise bandwidth, which resulted in minimized artifacts. This method may be useful for recording temporomandibular joint sounds in the differential diagnosis of various temporomandibular joint conditions using spectral analysis.

Intracytoplasmic granulofilamentous inclusion bodies in a meningioma. Immunohistochemical and ultrastructural studies.

A case of cerebellar tentorial meningioma, with abundant, large, eosinophilic intracytoplasmic inclusions, was studied immunohistochemically and ultrastructurally. These intracellular inclusions were periodic acid-Schiff negative and ultrastructurally appeared as a condensation or aggregation of numerous intracytoplasmic intermediate filaments. These inclusions were not related to microvillus-lined ductules, indicating that these inclusions were distinct from pseudopsammoma bodies. In addition, these inclusions were intensely immunopositive for vimentin, whereas staining for glial fibrillary acidic protein, epithelial membrane antigen, cytokeratin, neurofilament, desmin, actin, or type IV collagen was negative. To our knowledge, this is the second reported case of meningioma with such unusual inclusions, but the first one to demonstrate that the intracytoplasmic intermediate filaments and their related inclusions were mainly composed of vimentin.

Immunoblastic lymphadenopathy in a five-month-old girl: successful treatment with immunosuppressants.

A five-month-old girl developed high fever, erythema, hepatosplenomegaly and generalized lymphadenopathy. Laboratory examinations revealed elevated peripheral leukocyte counts, C-reactive protein, lactate dehydrogenase and serum ferritin level. Pathologic examination of the lymph nodes revealed immunoblastic lymphadenopathy (IBL) on the basis of the complete effacement of the normal architecture, replacement by a diffuse infiltrate composed of immunoblasts, plasmacytoid cells and small lymphocytes, and an abortive proliferation of blood vessels. B-cells and T-cells were nearly equally mixed throughout the lymph nodes. No rearrangements of the B-cell immunoglobulin and T-cell receptor genes were detected. The patient was initially treated with alpha-interferon with dramatic efficacy. After relapse, however, the disease was well controlled with cyclosporin A (CyA) and subsequently with combination regimens of CyA, deoxyspagarin and azathioprine with fair success. An alternating regimen of 6-mercaptopurine, cyclophosphamide and methotrexate was then instituted which continued the complete remission for 12 months. The effects of immunosuppressants in the treatment of IBL merit investigation.

[A case of sarcoidosis with elevated SCC antigen in the serum and broncho-alveolar lavage fluid which was difficult to distinguish from lung cancer].

A case of sarcoidosis with a high level of SCC antigen in the serum and broncho-alveolar lavage fluid (BALF) which was difficult to distinguish from lung cancer is reported. A 66-year-old female was admitted with a fever, sputum and coughing. Chest X-ray on admission showed bilateral hilar swelling and infiltrative shadows in the left lower lung. Chest CT showed bilateral lymph node enlargement and a localized nodular mass. The level of SCC antigen in the serum and BALF was high. Initially she was suspected of having lung cancer, but she was subsequently diagnosed as having sarcoidosis from a bronchial biopsy specimen, BALF and gallium scintigraphy. It was difficult to distinguish this case showing lymph node enlargement and mass shadow from lung cancer. Although this is a rarely reported case of sarcoidosis with a high level of SCC antigen in the serum and BALF, it is possible that SCC antigen may be elevated in patients with benign inflammatory lung disease due to various interactions of cytokines.
