A point-score system made from sympatoms, electrocardiographic findings, auscultation, and chest X-ray has been presented for the differential diagnosis among congestive cardiomyopathy, hypertrophic cardiomyopathy and ischemic heart disease. This system involved a multivariate analysis of 188 cases in which the diagnoses were established by typical echocardiographic findings, invasive examinations or autopsies. The accuracy of diagnosis by this point-score system was 91.5%. This point-score system appears to be useful as a screening test in differentiating idiopathic cardiomyopathy from ischemic heart disease.
Vectorcardiogram (VCG) was studied in four patients with hypertrophic cardiomyopathy (HCM) with RSR' pattern in right chest leads. In these patients there ws an anterior QRS loop with a slow inscription in the initial part of the QRS loop, the pathognomonic initial pig-tail curl pattern was also observed in two patients. Though mid-terminal conduction delay was also observed in two patients, the slowly inscribed terminal QRS vector oriented anteriorly and to the right, thus characteristic of right, thus characteristic of right bundle branch block (RBBB), was not observed. The initial conduction delay was attributed to the conduction disturbance as the result of hypertrophy and disarray of the muscle fiber, particularly in the septal area. The recognition of this VCG pattern provides a pertinent clue in the clinical detection of HCM.

Since 1973, the balloon catheter test with a Swan-Ganz, Dotter-Lukas or Fogarty catheter has been attempted in our department in cases of atrial septal defect (ASD) and patent ductus arteriosus (PDA) during cardiac catheterization and operation. Accurate informations concerning size, number and location of ASD were obtained during cardiac catheterization studies by using the balloon catheter. The difference between the size of the defect measured at catheterization and at operation was less than 1 mm in diameter. Size of the defect ranged from 13 to 44 mm in diameter (25.7 mm on the average) and bacame larger with patient's age. Measurement of left-to-right shunt ratios before and after the temporary balloon occlusion of the defect was of help in making a preoperative diagnosis of ASD with multiple defects. Size of foramen ovale ranged from 3 to 13 mm in diameter (5.2 mm on the average) and was not related to age. Size of the ductus in cases with PDA was measured during cardiac catheterization by using the balloon catheter. The difference between the size of the ductus measured with the balloon technique at catheterization and with a measuring rule at operation was less than 4 mm in diameter. The difference revealed the extensibility of the ductus. The temporary balloon occlusion of the ductus aided in determining presence of other cardiac anomalies. In cases of isolated PDA, the murmur became inaudible with the occlusion of the ductus. A balloon catheter inflated with radiopaque dye was used for measurements at cardiac catheterization and at operation. Persistent arrhythmias or other adverse effects of this procedure were not observed.
The acute effects of acetaldehyde upon the performance of isolated rat papillary muscle were studied. At high concentrations (50 and 100 mg/100 ml), acetaldehyde demonstrated a negative inotropic effect and abbreviated relaxation time. Pretreatment with either propranolol or reserpine unmasked a direct negative inotropic effect of acetaldehyde at concentrations as low as 5 mg/100 ml. The shortening of relaxation was also blocked by propranolol and reserpine. These findings are compatible with both a direct myocardial depressant effect and an indirect beta-stimulatory effect of acetaldehyde. In the range of concentrations encountered clinically, however, acetaldehyde is not likely to have a significant acute effect upon cardiac muscle performance.
Farming by means of greenhouses has spread in different parts of Japan during the past 20 years. Low back pain, an important health problem for farmers, affects those working inside a greenhouse as a result of their taking up particular postures. In order to analyze ergonomic problems of greenhouse farming relevant to low back pain, localized fatigue complaints and parts of the body where fatigue was felt during fruit picking were studied among 49 female farmers engaged in greenhouse strawberry culture and 53 female farmers engaged in greenhouse eggplant culture. Furthermore, the bowing posture for strawberry picking was compared with that of eggplant picking using newly devised posture-pattern recording equipment. More than 50% of strawberry or eggplant farmers complained of fatigue in the lower back and shoulders. The prevalence of low back pain was significantly higher among strawberry farmers than among eggplant farmers, probably due to the deep bowing posture of the farmer during picking work. The new posture-pattern recording equipment proved useful for investigating changes of work postures.
