UNLABELLED: The repetitive ventricular response (RVR) to three stimulation techniques (bipolar, cathodal and anodal) was investigated in 35 patients. 26 patients suffered from coronary heart disease and 9 patients from dilative cardiomyopathy. The stimulation study was performed at a ventricular driving rate of 120/min with one and two premature ventricular extrastimuli. We used rectangular impulses of 1.8 ms duration at double diastolic threshold strength. RVR was scored as follows: 0: no RVR, 1: one nonstimulated RVR, 2: two nonstimulated RVR, 3: three nonstimulated RVR, 4: four to ten nonstimulated RVR, 5: more than ten nonstimulated RVR lasting less than 2 minutes, 6: sustained ventricular tachycardia or ventricular fibrillation. We found that with unipolar anodal stimulation the diastolic threshold was significantly greater and the effective refractory period of the right ventricle was significantly shorter as compared to the other stimulation techniques. Between the three different electrode configurations there were no significant differences concerning the number of consecutive ventricular depolarizations following premature stimulation. CONCLUSION: the phenomenon of RVR is not influenced by the stimulation technique (bipolar, cathodal and anodal) at double diastolic threshold.
The electrophysiological properties of N-allyl-clonidine (ST 567) were studied in 18 patients. Four of these patients were pretreated with intravenous propranolol, 0.2 mg/kg, and atropine, 0.04 mg/kg, to induce autonomic blockade. ST 567 produces a sustained bradycardiac effect at a dose of 40 mg, i.v., in patients without (group I) and with (group II) autonomic blockade. The mean increase in Group I was 23% and in group II, 42%. The sinus node recovery time increased in both groups, from 1,151 +/- 73 to 1,310 +/- 81 msec in group I, and from 995 +/- 145 to 1,599 +/- 248 msec (means +/- SEM) in group II. The corrected sinus node recovery time was unaffected by ST 567 in Group I and increased from 165 +/- 50 to 427 +/- 117 msec in group II. The sinoatrial conduction time was unaffected by ST 567 in group I. The effective refractory period of the right atrium, the atrioventricular node and the right ventricle, as well as the functional refractory period of the atrioventricular node, were unaffected by ST 567 in patients without autonomic blockade. Intra-atrial, atrioventricular, and intraventricular conduction were also not altered by this drug. Thus, the electrophysiological profile of ST 567 differs from other bradycardia-inducing agents such as beta-blockers or calcium antagonists.

We have presented two cases of traumatic diaphragmatic hernia which have been diagnosed and operated in late stages, 6 months and 4 years, respectively, after the abdominal and thoracical traumatism causing diaphragmatic injure. The diagnosis was based on clinical and imaging results, while the treatment was meant to reduce the herniated organs in the abdomen and the closing up of the diaphragmatic defect, followed by good postoperative results. We have discussed the peculiarities of both case, the difficulty of early diagnosis and the techniques used to treat these cases. We have also presented data regarding posttraumatic diaphragmatic hernia and their treatment techniques.
The authors present two cases of congenital choledochal cysts treated by cyst excision with Roux-en-Y hepaticojejunostomy, followed by good postoperative results. One of the patients has been treated by early excisional procedure, while the other has initially undergone an enteric drainage by cystoduodenostomy, followed by complications, which required reoperations after 18 months. We have evaluated the peculiarities of both cases, the present data revealed by literature regarding biliary carcinogenesis related to congenital choledochal cysts, their classification according to pancreatobiliary malunion, as well as the treatment of choice in choledochal cysts, meaning excisional procedure with Roux-en-Y hepaticojejunostomy to prevent the risk of postoperatory complications which might appear after plain enteric drainage of the cyst.

The paper reports on a new way of cardiological, radionuclidic investigation of the patients, a technique used for the first time in Romania. The technique and investigation method are presented in detail, with emphasis on some more important stages. As resulting from the description of the methodology, the performances of the left ventricle consist of three types of radioactivity/time curves at: first passage, beat by beat, and their summation. Of the advantages offered by the nuclear stethoscope, mention should be made that, besides the evaluation of systolic and diastolic parameters, it monitors the left ventricle functions in the rhythm disturbances, beat by beat, and being mobile, it permits the investigation at the patient's bed. Finally, the paper presents several cases investigated by the authors, and their special clinical applicability.
A 22-year old female patient with psycho-motor retardation, statural hypotrophy and clinical picture characteristic of the 9p deletion syndrome, i.e., trigonocephalia (corrected surgically), epicanthus, hypertelorism, long filtrum, micrognathia, low inserted and malformed ears, filiform fingers, is presented.

After a short survey of the data of vertebrobasilar anatomy and physiology, the authors present their conclusions drawn from the study of 110 patients with vertebrobasilar circulatory insufficiency accompanied by severe vestibular disturbances. The paper reports on the clinic disturbances present in the patients studied, with emphasis on the importance of the interdisciplinary consultation and of the repeated otologic examination for application of an adequate therapeutics.
The paper reports on three cases of phlebectasia of the internal jugular vein, two in adults and one in a child, admitted and operated in the Cluj-Napoca Clinic of O.R.L. The surgical technique had to be adapted to each case separately, according to the extension of phlebectasia, the patient's age, and other related diseases.
