[Non-surgical treatment of impotence].

Non-surgical treatment of impotence comprises psychotherapy, drug oral administration and injection, drug intracavernous injection and penile vacuum extraction. Concerning psychotherapy, functional impotence is indicated, but urologists find difficulty in treating patients by themselves, thus requiring the co-operation of a psychiatrist, psychosomatic physician, psychologist and others. Concerning drug therapy, functional impotence is the main target, but is practically unbenefit with the reserve of endocrinally effective hormone. For drug intracavernous injection, papaverine hydrochloride and prostaglandin E1 are mainly used. At our Department, 51 patients have so far had successful self-injections according to this method. However, the self-injection requires strict control of drugs, syringes, etc. and proper selection of indicated cases and that includes an incidence of priapism of ca. 10%, making it necessary to take quick countermeasures. This method is targeted mainly for organic impotence, but is also indicated partially dysfunctional impotence. Penile vacuum extraction is indicated for all kinds of impotence and has no side effect, but is actually too expensive (ca. 130,000 yen) though noteworthy.

Visual load of working with visual display terminals--introduction of VDT to newspaper editing and visual effect.

In the Japanese newspaper industry, since the late 1970s, computerization has been started in large scale. The system, called Computerized Typesetting System (CTS), has been introduced. With this system, all information in the newspaper is input into a computer, called up on a cathode ray display (CRT) to edit, and then output to film or printing plates by CTS. Thus, the work of newspaper production has undergone a major transformation from industrial work to clerical work. It was reported that newspaper production staff working with VDTs complained more than staff working with the older key system about visual and physical problems which affected their job and their daily life. The complaint rates were highest for editors, who used VDT constantly throughout the day. This study was performed to clarify the health impact of VDT use and to prevent health disorders caused by the introduction of CTS. It is concluded that 5 m corrected vision and sphere refraction of CTS workers who used CTS over 2 or 4 hr daily, significantly became worse in a year after the introduction. Therefore, it is indicated that the control of CTS work time is important to prevent the visual disorder.

[Endogenous uveitis in disseminated intravascular coagulation induced by endotoxin].

A study was made of endogenous uveitis in experimental disseminated intravascular coagulation (DIC) in rabbits. Endotoxin was injected intravenously twice with a 24-hour interval. The time courses of the following were examined: 1) aqueous flare using a laser flare-cell meter 2) the number of leukocytes in the peripheral blood 3) the tumor necrosis factor (TNF) activity in the serum and 4) histopathological changes in the eye, lung, liver and kidney. Aqueous flare increased at 1 hour and was maximal at 6 hours, accompanied by a rapid increase in TNF activity at 1 hour following the first endotoxin administration. The number of leukocytes decreased to 963 +/- 266 cells/mm3 at 1.5 hours with subsequent leukocytosis within 12 hours. After the second injection of endotoxin, the aqueous flare peaked in 30 minutes and was twice as high as the first peak. Leukocyte number and TNF activity showed the same behavior. However, TNF activity was 20% that of the first peak. Histopathological examination indicated fibrin formation in the small vessels of systemic organs within 3 hours following the second administration of endotoxin. Endotoxin induced uveitis was induced in experimental DIC, and leukocytes and TNF activity may thus perform important roles.

Natural history of colorectal polyps and the effect of polypectomy on occurrence of subsequent cancer.

To elucidate the natural history of colorectal polyps and to observe the influence of endoscopic polypectomy on the incidence of colorectal cancer, we conducted a retrospective cohort study of all patients who had undergone colonoscopic examination at the Center for Adult Diseases, Osaka, between April 1974 and December 1985. The study subjects consisted of a control group (760 non-polyp patients) and a polyp group (648 polyp patients, including 136 treated by polypectomy at the initial examination). These subjects were followed up until the end of 1987 by record linkage with the Osaka Cancer Registry's file to observe the occurrence of colorectal cancer. The O/E (observed/expected numbers derived from the general population) was 5.1 (95% confidence interval = 2.5-9.4) and 1.0 (0.1-3.6) for the polyp and control group, respectively. When subjects in the polyp group were categorized into polypectomy and non-polypectomy sub-groups, the O/E was 2.3 (0.1-12.6) and 8.0 (3.4-15.8) respectively. The relative risk of undergoing polypectomy to developing subsequent cancer was estimated at 0.3 (0.1-2.1). These results suggest an increased risk of developing cancer among polyp patients and the possibility of prophylactic effect of polypectomy against subsequent cancer. A large-scale and long-term follow-up study is required to confirm these findings.

Stimulation of all T cells bearing V beta 1, V beta 3, V beta 11 and V beta 12 by staphylococcal enterotoxin A.

To determine the molecular mechanisms of T cell stimulation by staphylococcal enterotoxin A (SEA), we examined the expression of T cell receptor (TcR) V beta on the T cells from four strains of mice stimulated in vitro with SEA, using flow cytometric analysis for the number of T cells bearing V beta 3, V beta 6, V beta 8, V beta 11 and RNA blotting analysis for the amount of transcripts of V beta 1, V beta 5 and V beta 12. The number of T cell blasts bearing V beta 1, V beta 3, V beta 1 or V beta 12 were increased in the T cell blasts proliferating in vitro in response to SEA in C57BL/6 mice. In AKR/J mice, which contain few V beta 11- or V beta 12-bearing T cells due to a tolerance to the self-MHC class II IE-antigens, T cells bearing V beta 1 or V beta 3 responded to SEA. SEA enriched only V beta 1-bearing T cells in BALB/c mice carrying Mls-2a which lack Mls-1a-reactive V beta 3-bearing T cells as well as V beta 11- and V beta 12-bearing T cells. In spite of the presence of V beta 1-bearing T cells, C3H/He T cells exhibited a very low responsiveness to SEA. T cell repertoires skewed by clonal deletion of self-reactive T cells may in part account for the different sensitivity to SEA among the different strains. A tolerance to SEA can be established in C57BL/6 mice which have been primed i.v. with SEA and treated i.p. with 200 mg/kg of cyclophosphamide 2 days later. All mature T cells bearing V beta 3 or V beta 11 were virtually abolished in the periphery of tolerant mice. These results suggest that most T cells reactive to SEA bear V beta 1, V beta 3, V beta 11 or V beta 12 and that clonal deletion of mature T cells reactive to SEA may account for the cellular mechanisms for cyclophosphamide-induced tolerance to SEA.

The range of albumin concentrations in the single-void first morning urine of 1090 healthy young children.

To predict microalbuminuria easily in the diabetic outpatient clinic, we determined the range of albumin concentrations in the single-void first morning urine in 1090 healthy subjects of age 6-11 by the modified turbidimetry immunoassay (TIA) method, which uses 96-well microplates and a microplate reader to perform the assay quickly and conveniently. As the correlation coefficient of albumin concentration by the TIA uncorrected for creatinine versus corrected for creatinine was r = 0.79 (P less than 0.005), the values uncorrected for creatinine were used. When the distribution of values from 1090 healthy young children was simulated by using the chi-square test (k = 1), the 95% confidence interval was 0-18 micrograms/ml.

The efficacy of prazosin HCl in the treatment of urinary flow obstruction due to prostatic hypertrophy.

Prazosin HCl was administered to 20 patients with urinary obstructions associated with prostatic hypertrophy. Significant reductions were recorded in the nighttime and the mean 24-hour urination frequencies. Patient-assessed symptoms diminished significantly, and uroflowmetry revealed significant increases in average and maximum urine flow rates. Residual urine volume decreased, the residual urine ratio declined, and bladder compliance tended to increase. The prazosin HCl therapy produced no significant effects on blood pressure or pulse rate, and no side effects appeared. The therapy was judged to be 'extremely beneficial' in 10%, 'beneficial' or better in 50%, and 'marginally beneficial' or better in 85% of all the patients.

[Carcinoma of the renal pelvis and ureter. An investigation of 384 registered cases in the Tokai Urological Cancer Registry].

Registration of renal pelvic and ureteral tumor was done from 1980 through 1986, in the Tokai Urological Tumor Registry. Among the 404 cases of the carcinoma, 384 cases (210 renal pelvis, 174 ureter) were subjected to the present study. A total of 319 cases (83.1%) showed a complete cure, while 20 cases (5.2%) were considered as partial cure, and 45 cases (11.7%) were classified to progressive disease group. Chemotherapy was performed on 151 patients including systemic administration to 141 patients (33.9%) and 11 patients received combined intravesical instillation therapy. Irradiation was performed on 49 patients (12.8%). Degree of histological and morphological differentiation of renal pelvis and ureter tumor were studied in 341 cases of transitional cell carcinoma (88.8%); G0, 0.3%, G1, 12.9%, G2, 49.3%, Gx, 5.2%. Stage of the tumor were T1, 33.6%; T2, 12.5%; T3, 14.6%; T4, 13.3%; Tis, 0.3%; Tx, 24.7%. Five-year relative survival for carcinoma of the renal pelvis was 46.6%, and for the ureter it was 53.1%. Five-year survival according to histological differentiation of transitional cell carcinoma and according to the stage of the tumor were as follows; G1, 91.6%; G2, 58.5%; G3, 27.2%; Ta, 84.6%; T1. 74.2%; T2, 48.5%; T3, 24.1%, T4, 7.3%. The rate of survival was correlated with histological differentiation of the tumor cells. The survival was poor in cases who had the components of squamous cell carcinoma and/or adenocarcinoma. Overall survived case were 177, 122 patients died of cancer, 20 patients died of other causes than cancer, and 1 patient died from immediate surgical effect.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of alpha-1-blocking agent in the treatment of detrusor sphincter dyssynergia].

Effects of adrenergic alpha-1-blocking agent, prazosin, in the treatment of detrusor external-sphincter dyssynergia (DSD) were evaluated in both experimental and clinical aspects. Experimentally, in the urethral pressure profile in dogs, the maximum urethral closing pressure was depressed after intravenous injection of 1 mg prazosin. When experimental DSD was obtained in dogs by stimulating electrically the unilateral 2nd sacral root, intra-venous injection of 1 mg prazosin inhibited contraction of the external urethral sphincter. Clinically, 74 patients with DSD based on neurogenic bladder from cerebral vascular attack (CVA) (13 cases) and spinal cord injury (61 cases) were retrospectively surveyed in terms of therapeutical effects of prazosin for DSD. Spinal cord injury was subdivided to 4 groups for clinical evaluation; cervical cord injury (C) with complete paralysis, thoracic cord injury (Th) with complete paralysis, lumbar cord injury (L) with complete paralysis and spinal cord injury with incomplete paralysis. Patients with CVA and spinal cord injury with incomplete paralysis showed good response rates in subjective improvement, 69% and 60% respectively. However, those with spinal cord injury with complete paralysis showed a poor response (28% for C, 23% for Th and 14% for L). The amount of residual urine significantly decreased after treatment, in all the groups except that of lumbar cord injury with complete paralysis. In all the groups, however, even after the drug treatment the amount of residual urine ranged from 80 to 170 ml and the rates of needing clean intermittent catheterization unchanged.(ABSTRACT TRUNCATED AT 250 WORDS)

Mechanism of action of a new macromolecular antitumor antibiotic, C-1027.

C-1027 is a new antitumor protein antibiotic containing a non-protein chromophore. The active moiety and the mechanism of action of this antibiotic were studied. C-1027 and its chromophore inhibited the growth of KB carcinoma and L1210 leukemia cells, even at extremely low concentrations. C-1027 inhibited DNA synthesis of L1210 cells and cleaved cellular DNA in a drug concentration-dependent manner. C-1027 and chromophore caused directly DNA single strand breaks in the purified DNA without any supplement of reducing agents. These results suggest that C-1027 chromophore may inhibit cell growth by causing DNA breakage with subsequent inhibition of DNA synthesis.

Effects of adenine nucleotides on choriogonadotropin alpha and beta subunit synthesis.

The alpha subunit of human chorionic gonadotropin (CG) contains a discrete cAMP response element in the 5' flanking region of the gene. Since cAMP also stimulates the synthesis of the CG beta subunit the presence of a cAMP cis element in the CG beta gene was examined. Deletion mutants bearing various lengths of CG beta 5' region in front of the chloramphenicol acetyl transferase (CAT) gene were transfected in placental tumor cells. No discrete cAMP response element could be identified. Unexpectedly we also observed that AMP and adenosine not only stimulated CAT activity driven by CG beta promoter sequences but also enhanced synthesis of CG alpha and beta subunits in cultured choriocarcinoma cells. GMP, CMP, guanosine, and cytosine were inactive at comparable concentrations. These data suggest that the response of the CG alpha and beta genes to the non-cyclic adenine derivatives occurs by a mechanism that differs from cAMP.

Expression of biologically active human follitropin in Chinese hamster ovary cells.

To study the structure-function relationships of follitropin (FSH), we expressed the hormone in a heterologous cell system. A genomic clone bearing a 3.7-kilobase FSH beta insert containing the entire coding sequence was transfected alone or together with the alpha subunit gene into Chinese hamster ovary cells and stable lines expressing either FSH beta or FSH dimer were selected. Pulse-chase experiments revealed that, when transfected alone FSH beta was very slowly secreted similar to lutropin beta and thyrotropin beta but unlike choriogonadotropin beta which is efficiently secreted. However, cotransfection of the FSH beta and alpha subunit genes resulted in "rescue" of the beta subunit and rapid secretion of dimer. These data support the hypothesis that the glycoprotein hormones of pituitary origin have determinants for secretion that differ from those on the placental hormone, choriogonadotropin. Recombinant FSH stimulated steroidogenesis comparable to purified human FSH isolated from pituitaries in an in vitro rat granulosa cell assay and appears more homogeneous by chromatofocusing. Human FSH produced by this cell line provides a source of bioactive FSH for experimental and clinical use.

Imbalance of deoxyribonucleoside triphosphates, DNA double-strand breaks, and cell death caused by 2-chlorodeoxyadenosine in mouse FM3A cells.

The mechanism of the cytotoxic action of 2-chlorodeoxyadenosine in mouse FM3A cells was investigated. Imbalance of the dNTP pools occurred within 3 h of treatment with 20 microM 2-chlorodeoxyadenosine; the dATP and dGTP pools were depleted and the dTTP pool increased. 2-Chlorodeoxyadenosine added to the culture medium broke mature DNA strands, giving fragments of 100-200 kilobase pairs as found by orthogonal-field-alternation gel electrophoresis. DNA strand breaks, measured by this technique, were observed in the treated cells about 12 h after the addition. The cells also lost viability at about 12 h. Breaks in the single and double strands of DNA, as measured by alkaline and neutral filter elution, became evident 18 h after treatment with 20 microM 2-chlorodeoxyadenosine; there were as many single-strand breaks as would be caused by 130 rads of gamma-ray irradiation. Double-strand breaks were equivalent to those caused by 2180 rads of gamma-ray irradiation. Comparison of the ratio of single- and double-strand breaks caused by 2-chlorodeoxyadenosine to that following radiation suggested that 2-chlorodeoxyadenosine broke only double strands. Cycloheximide inhibited the breakage of DNA double strands and the cell death caused by this compound. Flow cytometric studies of cytostasis brought about by 2-chlorodeoxyadenosine in FM3A cells showed that cells accumulated in the earlier part of the S phase. 2-Chlorodeoxyadenosine decreased DNA synthesis more than RNA or protein synthesis. The breaks in double strands of DNA were probably important in the cell death caused by 2-chlorodeoxyadenosine. The intracellular dNTP imbalance may trigger these events.

Effectiveness of endoscopic treatment of carcinoid tumors of the rectum.

The effectiveness of endoscopic polypectomy in the treatment of carcinoid tumors of the rectum was investigated. Flat tumors of less than 1.5 cm in the largest diameter, of normal or yellow color, consisting histologically of solid nodular nests or trabecular or ribbon-like structures were "early" tumors, in which the tumor cells were confined to the mucosa and submucosa, and did not involve deeper layers. Eight patients with "early" carcinoids were treated by endoscopic polypectomy. Subsequently, two of them were submitted to surgery, while the other six were followed-up endoscopically. Histological examination of the specimens obtained at operation showed no evidence of residual tumor cells at the polypectomy site. Endoscopic follow-up studies also failed to reveal local recurrence in any of the patients during the average observation period of 1.3 years. The wall of the rectum was significantly thicker than that of the sigmoid colon, so that deeper local excision was possible. These findings indicate that even though "early" carcinoid tumors of the rectum showed submucosal invasion, they were completely cured by local endoscopic excision.
