Clinicopathological study of the heart and coronary arteries of autopsied cases from the community of Hisayama during a 10-year period. Part IV. QS waves in the precordial leads.

During a ten-year period, from November 1, 1961, to October 31, 1971, 339 residents aged 40 years or over at death were nonselectively autopsied in a Japanese community, Hisayama town (mean autopsy rate: 84%). One or more standard 12-lead ECGs plus V4R and V7 electrocardiograms taken at periodic medical examinations were available for 308 of them. In 46 persons, QS waves were localized in one or more leads from V1 to V4. By transverse sectioning of the hearts, old myocardial infarction extending into the interventricular septum was found in nine of these 46 persons. Frequency of myocardial infarction cases in each category for QS localization was as follows: Lead V1 to V4, three of three; Leads V1 to V3, six of nine; Leads V1 and V2, three of 15; Leads V2 and V3, none of two; Lead V3, none of one; Lead V2, one of eight; and Lead V1, one of 25.

Differences and similarities among circadian characteristics of plasma renin activity in healthy young women in Japan and the United States.

A circadian rhythm of plasma activity (PRA) was demonstrated for both Japanese and North American women, the latter mostly Caucasians of mixed ethnic origin. The results were based on blood samples withdrawn at 4-hour intervals during a 24-hour span (in March 1978) from 20 subjects from Fukuoka (average age 20.4 +/- 0.1 years) and 16 subjects from Minneapolis (average age 20.2 +/- 0.4 years). The rhythms in the two populations showed similarities in some characteristics and differences in others. The timing of high values, i.e., of acrophases, objectively assessed by curve-fitting (and of corresponding 95 per cent confidence limits) was at 07(36) (05(00), 10(16) and 06(32) (03(00), 10(00) for Japan and USA, respectively. As objective measures of the extent of predictable rhythmic change mean amplitudes, in nanograms per milliliter per hour (ng/ml/hour), were similar (0.31 and 0.32); a statistically significant difference (P less than 0.05) was found in mean amplitudes expressed as percentage of the rhythm-adjusted average. Mean rhythm-adjusted average values (mesors) were lower in women from Japan than in those from the United States: (1.64 +/- 0.14 and 2.39 +/- 0.23 ng/ml/hour, respectively; P less than 0.01). A statistically significant difference in dietary salt, indicated by differences between the Japanese and North American women in the urinary excretion of sodium and chloride (P less than 0.05), almost certainly contributed to these results.

Cerebral and aortic atherosclerosis in Hisayama, Japan.

This study of autopsy cases in the general population of the town, Hisayama, describes the incidence and severity of aortic and cerebral atherosclerosis in Japan. Atherosclerosis was more severe in the aorta than in the cerebral arteries of all age groups and its disparity became more conspicuous with age. In hypertensive cases, atherosclerosis was more severe in both the aorta and the cerebral arteries from and beyond the 6th decade of age. The severity of atherosclerosis in the aorta in those with systolic hypertension was lower under the age of 79 and higher after the age of 80 than in diastolic hypertension; the cerebral arteries were afflicted similarly by the two forms of hypertension. The serum cholesterol level correlated better with the severity of aortic than cerebral atherosclerosis.

Brain blood flow and mean transit time as related to aging.

Brain blood flow and mean transit time were measured in 41 normotensive control subjects aged from 21 to 76 years by using the intravenous 131I-RISA external counting method. The results showed a significant prolongation of mean transit time and a tendency to increase in brain blood volume relating to advancing age but no change in the blood flow. This indicates that prolonged transit time in the aged is probably due to the increased brain blood volume.

Influence of unilateral adrenalectomy on renin-angiotensin-aldosterone system in primary aldosteronism.

In a study of 14 patients (11 Japanese and 3 white North Americans) with aldosterone-producing tumor carried out within 2 to 4 weeks after unilateral adrenalectomy, both plasma renin activity (PRA) and plasma aldosterone concentration (PAC) increased almost normally with short-term sodium depletion and upright posture. Aldosterone excretion rate (AER) also increased significantly with sodium depletion, and was normally suppressed by sodium loading. Highly significant correlations were observed between PRA and PAC (r=0.89, p < 0.001) or AER (r=0.88, p < 0.001) ater operation, whereas there had been no such correlation before operation. Either physiological or laboratory findings were normalized or markedly improved in all cases although pathohistological diagnosis was made as adenomatous hyperplasia in 10 out of 11. These results indicate that within 4 weeks of operation PRA and the remaining adrenal gland can almost normally respond to stimuli such as sodium depletion and upright posture regardless of the amount of spironolactone given preoperatively.

Spontaneous disappearance of fundic gland polyposis: report of three cases.

Three cases are reported in which multiple sessile polyps of the stomach disappeared spontaneously during the follow-up period from 9 to 34 mo. All of the patients were middle-aged females, and the distribution of the polyps was limited to the portion of the stomach with fundic glands. Histolgoic examination revealed simple hyperplasia of the fundic glands with microcysts. The macroscopic and histologic pictures were the same as the gastric lesions ("fundic gland polyposis") found in cases with familial adenomatosis coli; however, colonic polyposis was not observed in our cases. The possible cause of the spontaneous disappearance of the polyps was discussed.

Comparison of hemodynamics induced by conventional acetate hemodialysis, bicarbonate hemodialysis and ultrafiltration.

Six patients on conventional acetate hemodialysis who had a marked decrease in total peripheral resistance index (TPRI) and hypotension after dialysis were selected for study. Systemic hemodynamic factors such as cardiac output, blood pressure, hematocrit and heart rate were measured, and the values were compared to those found after bicarbonate hemodialysis (Bi-HD) or ultrafiltration without simultaneous dialysis (UF) in the same patients. After Bi-HD and UF, the hemodynamic indices remained stable. Hematocrit level was significantly elevated after Bi-HD (+2.7%, P less than 0.05). Heart rate was insignificantly increased after each procedure. We concluded that Bi-HD can prevent unpleasant side effects induced by the decrease in TPRI with acetate hemodialysis. The stability of TPRI resulting from the increase in hematocrit level during Bi-HD may contribute to the prevention of cardiovascular instability.

Circadian variation in urinary melatonin in clinically healthy women in Japan and the United States of America.

Urinary melatonin excretion is lower in East-Asian (Japanese) than in North-American (whites of mixed ethnic origin) women. Moreover, a statistically significant circadian rhythm is demonstrated by population-mean cosinor in the data pool from both groups of women. Furthermore, statistical significance characterizes interactions of effects from geographic differences (between ethnic groups) with temporal factors. Such spatio-temporal interactions await further scrutiny with a view inter alia of carcinogenesis as it is influenced by a spectrum of intermodulating rhythms.
