Sex difference of barbiturate-protection in experimental cerebral ischemia in spontaneously hypertensive rats (SHR).

Survival time following bilateral carotid occlusion was significantly longer in spontaneously hypertensive rats (SHR) anesthetized with barbiturate than in those with ether. Under barbiturate anesthesia, female SHR survived longer (16.6 h) than did males (10.9 h), whereas such sex difference was not found in those with ether anesthesia. The present results indicate sex difference of barbiturate-protection in cerebral ischemia.

Hypophosphatemia induced by intravenous administration of Saccharated iron oxide.

Forty milligrams saccharated iron oxide was given intravenously, daily, to nine iron-deficient patients with moderate to severe anemia. During the 14-42 days of treatment, levels of serum inorganic phosphorus (Pi) decreased significantly and in a stepwise manner (before vs 1 week, P less than 0.005; 1 week vs 2 weeks, P less than 0.01). Seven of the nine patients became hypophosphatemic within 2 weeks and the other two within 4 weeks. In parallel with the decline in serum Pi, the phosphate clearance increased and tubular reabsorption of phosphate decreased. Reversion to normal levels was delayed in patients treated for the longer period.

Acute electrocardiographic effects of captopril in the initial treatment of malignant or severe hypertension.

We evaluated electrocardiographically the acute effect of captopril in the initial treatment of malignant or severe hypertension and the findings were compared with the data obtained with diazoxide. The captopril group included seven patients treated with captopril or with a combination of diuretics. The diazoxide group included seven patients initially treated with diazoxide. A significant and similar degree of reduction in blood pressure was observed the second day after treatment in both groups. From 15 to 60 minutes after treatment, however, the reduction in mean blood pressure was more pronounced in the diazoxide group and was followed by a reflex tachycardia. In the diazoxide group, two patients went into shock and another three had nausea and vomiting, whereas in the captopril group there were no complications in any of the patients. In the captopril group ECG ST-T findings in leads V5 and V6 did not deteriorate, while in the diazoxide group there was a significant deterioration (p = 0.01). Thus captopril may be an effective and safe compound for the initial treatment of malignant or severe hypertension.

Relationship of urinary kallikrein excretion to renal water and sodium excretion.

The circadian rhythms of urine volume and urinary excretion of sodium, potassium, kallikrein, and aldosterone were analyzed by a multivariate method (cosinor method) in 20 healthy Japanese women on an ordinary diet. The relationship of urinary kallikrein and aldosterone excretion to urine volume and urinary sodium and potassium excretion was studied by assessing the correlation of the circadian rhythms. The acrophases in the circadian rhythms of urine volume (16:51) and urinary sodium excretion (16:55) appeared after the acrophase of urinary kallikrein excretion (15:28). There was a highly significant correlation between the circadian rhythm of urinary kallikrein excretion and the circadian rhythms of urine volume (r = 0.948) and urinary sodium excretion (r = 0.921). These results suggest that the renal kallikrein-kinin system participates in the regulation of renal water and sodium excretion in persons on an ordinary diet. A highly significant relationship between the acrophases in the circadian rhythms of urine volume and sodium excretion (r = 0.935) also suggests that water and sodium excretion may have a mutual influence on the kidneys. There were positive correlations between the circadian rhythms of potassium excretion and kallikrein excretion and potassium excretion and sodium excretion; and the latter relationship was relatively closer than the former. The acrophase in the circadian rhythm of aldosterone excretion did not correlate well with the acrophases of the other urine variables including sodium excretion.

Acid protease activity in thyroid gland from patients with Graves' disease.

Although the activity of lysosomal protease in Graves' thyroid is considered to be increased, there has been no quantitative method to estimate the protease activity in the thyroid tissue due to the contamination of thyroglobulin (Tg) which varies in susceptibility to the protease. In the present study, the proteolytic activity (PA) of thyroid lysosomal protease preparation (P25) separated from Tg was assayed using 125I labeled rat Tg. More than 95% of 125I-Tg was hydrolyzed at pH 4.0 without deiodination, and the pattern of liberated iodoamino acids resembled that of pronase digest except for a higher T3/T4 ratio. Thirty-seven Graves' thyroids and 15 paranodular thyroid tissues were assayed. PA, the specific PA (SPA; calculated as PA per mg P25 protein) and PA per DNA content in Graves' thyroids were significantly higher than those in controls. There were good correlations among PA, SPA and PA per DNA content of the thyroid tissue. PA and SPA in the thyroid from 29 patients with Graves' disease treated with propylthiouracil or methimazole did not correlate with serum thyroid hormone level immediately before surgery. In 8 patients treated with KI, PA from 5 patients whose serum thyroid hormone levels had been normalized were significantly lower than those from 3 patients who were still thyrotoxic.(ABSTRACT TRUNCATED AT 250 WORDS)

The BTPABA pancreatic function test in giardiasis.

The test for exocrine pancreatic function using N-benzoyl-L-tyrosyl-p-aminobenzoic acid (BTPABA test) was assessed in 7 patients with giardiasis and 7 healthy controls. Cumulative percent p-aminobenzoic acid (PABA) recovery in 6 hr was significantly lower in patients with giardiasis, compared with the control group. When an equivalent dose of free PABA was given, there were no differences in PABA recovery between the groups. In patients with giardiasis, the post-treatment values of BTPABA test were significantly higher than the pretreatment values and no differences were found in PABA recovery between patients with giardiasis after eradication and healthy controls. These findings indicate that Giardia lamblia interferes with the action of pancreatic chymotrypsin. It is noteworthy that giardiasis could cause an abnormal BTPABA test.

Bleomycin in verrucous squamous cell carcinoma of the oesophagus.

A case of verrucous squamous cell carcinoma of the oesophagus treated with bleomycin is reported. Bleomycin brought prompt reduction of the tumour and relief of symptoms. There have been no previous reports on experience with anti-cancer agents for verrucous carcinoma of the oesophagus. Administration of bleomycin might be an effective treatment for verrucous carcinoma of the oesophagus.

Sudden unexpected deaths in a Japanese community--Hisayama study.

Six hundred and fifty-eight autopsy cases of both sexes in a general population of Hisayama town (autopsy rate 85%) were examined to determine the frequency and causes of sudden and unexpected death. Seventy-three sudden deaths (11.1%) were identified when sudden death was defined as death occurring within 24 hours after the onset of clinical symptoms. Thirty-nine deaths (54%) were due to intracranial hemorrhage (cerebral and subarachnoid) and 17 (23%) were due to coronary heart disease. An instantaneous death occurred within one hour was observed in 20 of the 73 cases (27%), of which 8 were attributed to coronary heart disease (40%) and one (0.5%) to subarachnoid hemorrhage. Pathologic evidence of recent myocardial infarcts or coronary thrombosis was found less frequently in cases who died within one hour than in those who died between one and 24 hours. An annual incidence of sudden death due to various etiologies, calculated from the population study on Hisayama residents who were aged 40 or over, was 1.7/1,000. Deaths from coronary heart disease in this community were much less common as compared with those reported in Western communities.

Recurrent pheochromocytoma associated with glucagonoma. A case report.

We treated a hypertensive patient with recurrent pheochromocytoma (paraganglioma) associated with glucagonoma. No clinical symptom of glucagonoma was found and it could be detected only by a slight elevation of plasma immunoreactive glucagon (IRG) while the extirpated pancreatic tumor contained much IRG. This case could not be classified as either multiple endocrine neoplasia (MEN) type I or type II.

Comparison of circadian rhythms of the renin-angiotensin-aldosterone system and electrolytes in clinically healthy young women in Fukuoka (Japan) and Minnesota (USA).

Circadian rhythms of the renin-angiotensin-aldosterone (R-A-A) system and electrolytes were evaluated in young healthy Fukuokan (Japan) and Minnesotan (USA) women. A similar protocol was used and each group remained on its usual diet. Blood was taken for 24 h at 4 h intervals from 8 a.m. during which time urine samples were also collected. The determinations in both groups were made in a laboratory in Japan. Plasma renin activity, plasma aldosterone concentration and aldosterone excretion rate showed similar circadian rhythms in both groups. However, those values at each sampling time and each span were always significantly higher in the Americans while the urinary Na and Cl excretions and blood pressure were higher in Japanese. The significant differences in the R-A-A system and blood pressure levels between these two groups may be due to the large amount of salt consumed by the Japanese.

Activation of renin-angiotensin system in maintenance of high blood pressure in uncomplicated pheochromocytoma--a case report.

In a patient with an uncomplicated pheochromocytoma, we observed high basal value of plasma renin activity (PRA), 16.2 ng/ml/h, and a remarkable elevation in PRA from 18.2 to 90.4 ng/ml/h and concomitant blood pressure reduction following a single oral administration of 25 mg of captopril. The response was same as that observed in renovascular hypertensives, and disappeared during the period of alpha-, beta-blocker administration or after the surgery. Basal PRA also returned to low normal level. The renin-angiotensin (R-A) system probably had a role in the maintenance of high blood pressure. The mechanism of the activation of the R-A system was thought to be renal vasoconstriction due to excessive circulating catecholamines.

Influence of hypertension on the progression of experimental autologous immune complex nephritis.

In order to determine the influence of hypertension on the progression of chronic glomerulonephritis, we studied the renal lesions in Heymann nephritis (autologous immune complex nephritis) produced in SHR. Nephritic SHR treated by AHD, normal SHR, nephritic WKYR, and normal WKYR served as controls. Induction of Heymann nephritis did not alter the blood pressure in either SHR or WKYR as compared with each untreated control group. Administration of AHD normalized the blood pressure of SHR. Proteinuria, hypoproteinemia, hypercholesterolemia, and reduction in body weight were significantly greater in nephritic SHR than in nephritic SHR treated by AHD or nephritic WKYR, whereas BUN and serum creatinine were unchanged in all the nephritic rats. Histological findings such as glomerular basement membrane thickening, IgG and C3 deposits along capillary walls, and subepithelial electron-dense deposits were similar in all nephritic groups. Glomerular sclerosis and tubulointerstitial changes were more marked in nephritic SHR than in the other nephritic groups. Severe vascular thickening and necrosis, intravascular thrombosis, and perivascular cell infiltration were frequently observed in nephritic SHR. These lesions are characteristic of malignant hypertension. However, they were not found in control SHR, which maintained elevation of blood pressure equivalent to that of nephritic SHR throughout the study. It was concluded that hypertension may aggravate nephritic manifestations such as proteinuria, hypoproteinemia, and hypercholesterolemia but not excretory renal function and that the hypertensive vascular lesions are augmented by Heymann nephritis.

Long-term hemodynamic response to captopril (SQ 14,225) in hypertensive hemodialysis patients.

The long-term effect of treatment with captopril was observed in 7 hypertensive patients on maintenance hemodialysis who had been refractory to combined antihypertensive therapy with propranolol and hydralazine. Captopril showed a potent hypotensive effect in all the subjects throughout the study period of 6 months. The blood-pressure reduction during treatment with captopril was associated with a decrease in total peripheral resistance. Cardiac index increased significantly due to a significant increment in heart rate at the 6th month. Plasma renin activity increased significantly at the 3rd month, whereas it decreased at the 6th month. No significant correlation was observed between the blood-pressure response and the pre-treatment level of plasma renin activity.

Human gastric carcinoma: prognosis in relation to macroscopic and microscopic features of the primary tumor.

The correlation of prognosis with various clinical and pathologic factors was studied by use of multiple stepwise linear regression analysis among 77 patients undergoing resections for gastric cancer. Factors such as depth of penetration, invasion of the duodenum, macroscopic appearance, intramural and extramural growth pattern of the cancer, lymphatic or vascular invasion, and cancer cell-free dense submucosal fibrosis around the cancerous area were significant, and independent factors correlated with the prognosis, in that order. When these factors were taken into account, other factors such as sex and age of the patient, location, size, and histologic classification of the tumor, and folds' convergence were not significant.
