[The clinical evaluation of hochuekkito for symptoms of malignant neoplasm patients].

Hochuekkito was administered in 2.5 g doses three times a day to 162 patients who complained of anorexia or lassitude because of genitourinary cancer. The efficacy rate was 63.0%. The rate of effectiveness on anorexia was 48.4% and that on lassitude was 36.6%. Side effects were observed in 12 patients (7.4%), but most of them were mild gastrointestinal disorders. No severe adverse effects were noted.

[A clinical study of radiotherapy with HCFU for advanced and recurrent breast cancers. Radiation and HCFU Study Group for breast cancer].

We have investigated the usefulness of combination therapy with radiation and HCFU for advanced and recurrent breast cancer according to a clinical controlled multicenter trial from 1982 to 1984. One hundred cases were registered and 82 of them were available. Treatment sites were the lymph nodes, skin, bone and lung, and the overall response rate was 58% in CR and 19% in PR, while the duration of remission was 18 weeks in CR. Side effects were found in 10% of the patients. Combination therapy with radiation and HCFU may be useful in multimodal treatment for advanced recurrent breast cancer.

[Conservative treatment of operable breast carcinoma by radical radiotherapy].

Treatment of operable breast carcinoma by radical irradiation instead of total mastectomy was discussed in conformity with our experiences and reports. No significant differences were obtained between conservative management and radical mastectomy in the rate of locoregional control and survival. The cosmetic results were satisfactory in almost all of patients. This method must be considered and established on concepts based on the multimodal treatment of breast carcinoma in Japan.

[Intraoperative radiotherapy of unresectable pancreatic carcinoma].

Twenty-eight cases of unresectable pancreatic carcinoma were treated with intraoperative radiotherapy (IOR) with or without postoperative radiotherapy (POR) from 1975 to 1982 at Tokyo Women's Medical College. The results were as follows. IOR was effective for pain relief, and the response rate was 93%. Twenty-five Gy of IOR, considering sequential POR, was a reasonable dose in about an 8 X 8 cm to 10 X 10 cm radiation field. Precise staging and marking of the tumor edge at operation were important for the treatment planning after IOR. Radiotherapy should be considered in the multimodal treatment of pancreatic carcinoma.

[Multiple primary malignant cancer and radiation induced cancer--a point of view of radiation oncology].

The cases of multiple primary malignant neoplasm and radiation induced cancer were discussed based on the experiences of Tokyo Women's Medical College in which radiotherapy has been mainly used for cancer. These cases have been increasing in number and it seems to be that the problems of them shall be gradually developed in future, because survival time of cancer patients become longer due to improvement of cancer treatment. But if the mechanisms of cancer etiology on which radiation and other many things influence, would not be clear, radiotherapy have been still principal post in cancer treatment. Systemic multidisciplinary treatment of cancer must be considered and we are looking forward to establishing it.

[Patterns of recurrence and survival in breast cancer].

Sixty-nine patients have shown recurrence after curative (standard, extended and modified) mastectomy from 1969 to 1978 in our department. Recurrence patterns and prognostic factors were analyzed using life-table and log-rank methods. Thirty-four patients (49%) developed their first recurrence within two years after surgery and died within two years after that. This period seemed to be high-risk after surgery and first recurrence. The disease-free interval and survival after first recurrence seemed to be shorter with the more advanced presenting Tnm stage. The disease-free and recurrence-to-death intervals of patients with first loco-regional recurrence seemed to be longer than those with first distant metastases.

[Concept of therapeutic ratio in radiation combined with chemotherapy].

Significance of combined therapy should be based in improvement of treatment results and decrease of side effects not only early but also late injuries. In other word, T.R. should be estimated whether direction of it is improvement or not. There are many different factors and difficult subjects which are decision of optimum dose, timing of combination, mechanism in synchronous combined therapy, distribution of tumor microcirculation. It is very important in order to improvement of T.R. clinically that we have been watching reactions after treatment closely and accumulating their datas.

Studies on (n, alpha) recoils of 6Li and 10B in the reactor UTR-Kinki by gas phase electrodeposition.

The energies of recoil particles from (n, alpha) or (n, 2 alpha) reactions of Li and B were investigated by solid state detector and gas phase electrodeposition as previously reported on 228Th series. Nuclear reactor of UTR-Kinki was used as thermal neutron source. Particles from 6Li (n, alpha) 3T and 10B (n, alpha) 7Li were collected on a sheet of triacetate film by gas phase electrodeposition at atmospheric pressure. The film was etched with 6M-NaOH at 50 degrees C. The tracks thus obtained were taken as photographs with a microscope (X600). By counting the number of tracks on the film at various etching times, the energies of alpha-particles were evaluated according to the calibration curve of 227Ac series. Among various peaks of tract density, the two peaks having the evaluated values of 1.85 MeV for 6Li and 1.48 MeV for 10B agreed approximately with the theoretical values of 2.05 MeV and 1.47 MeV of alpha-particle energies for respective reactions.

[Studies on the safe limit of variable decelerations during labor].

Ninety neonates who presented variable decelerations during labor were divided into two groups, according to the one-minute Apgar scores of greater than or equal to 8 and 7 less than or equal to. Only neonates of singleton pregnancies and cephalic presentations of 34 weeks gestation or more were included in the study. Several parameters of this fetal heart rate (FHR) pattern and the presence or absence of meconium staining were compared. Followings were found to be statistically significant. 1) The duration of the variable deceleration (less than 2 hours). 2) The FHR deceleration/uterine contraction ratio (less than 80%). 3) The absence of meconium staining. An effort was made to establish safe limits of variable decelerations. These are shown in parentheses. Based on both these data and findings of others, a multifactorial scoring system named a variable deceleration score was proposed for the proper management of this FHR pattern. Five variables were selected for the system. These were: The duration of variable decelerations, the FHR deceleration/uterine contraction ratio, the duration and amplitude of dips, and the presence or absence of meconium staining. The system scores from 0 to 2 for each of five factors, and was designed to be able to predict depressed neonates with the numerical increase of the score.
