Chemical and crystallographic studies on 33 cases of calcium carbonate gallstone (so-called limy bile).

Chemical analyses by atomic absorption spectrophotometry and crystallographic studies by the X-ray powder diffraction method and infrared spectrometry (KBr-disk method) were made on 33 cases of calcium carbonate gallstone or so-called limy bile. Chemically, calcium carbonate was the major constituent, ranged from 33.7 to 91.6% and averaged 77.8%. Crystallographically, calcium carbonate has three different polymorphic crystalline forms; calcite, aragonite and vaterite. In nature the most stable calcite (hexagonal) is most commonly found and aragonite (rhombic) is next. On the other hand vaterite, which is unstable hexagonal modification, rarely occurs in biological systems. But in our gallstone series in man, aragonite was most commonly found, with an occurrence rate of 90.6%, while that of calcite was 62.5%. Even vaterite was found in 28.1%. Moreover three cases contained all three forms of calcium carbonate polymorphs; calcite, aragonite and vaterite. This was a very unusual condition. Some environmental factors controlling the growth of these crystals, such as specificity of the bile, are suggested.

Revascularisation of free full thickness skin grafts in rabbits: a scanning electron microscope study of microvascular casts.

Casts of the vasculature of free full thickness skin grafts from rabbits' thighs and of the graft beds prepared on their ears, were obtained by microvascular injection of "Mercox", a methacrylate preparation. After transplantation, similar casts were made of the grafted area. All the casts were examined in a scanning electron microscope. Partial filling of the graft vasculature was observed 3 days after grafting and complete filling 5 to 7 days afterwards. These results confirm that vascularisation of free full thickness skin grafts is accomplished through early anastomoses between the original small vessels of the graft and the bed.

Unusual lysosomes in aortic smooth muscle cells: presence in living and rapidly frozen cells.

Unusual tubular structures have been observed in rat aortic smooth muscle cells (SMC) grown in culture. These tubular structures have several characteristics that strongly suggest that they are lysosomes: they are bounded by a single membrane bilayer, contain densely staining material, and acid phosphatase activity. Furthermore, these structures are present in living cells, as demonstrated by their ability to accumulate the membrane-impermeable fluorescent dye lucifer yellow CH. In ultrastructural preparations they are best seen in samples that are cryofixed by rapid freezing and then freeze-substituted in osmium-acetone solutions. Conventional chemical fixation did not appear to preserve these structures to as great an extent as did rapid freezing. Comparison of SMC in vitro to the same cells in situ revealed differences in lysosome number as well as morphological appearance. Thus, the culturing of rat SMC leads to the formation of unusual tubular lysosomes whose ultrastructural appearance is particularly sensitive to the methods employed for examination.

Glomerulonephritis with focal and segmental distribution of glomerular basement membrane antigen(s).

In this report, the authors describe a case of a 2-year, 11-month-old girl with glomerulonephritis and no family history of renal diseases and deafness. Immunofluorescent studies in the renal biopsy specimens with the use of anti-sera against human glomerular basement membrane (GBM) and P3 antigen (prepared from bovine GBM and inducible of Steblay's type nephritis in rats) demonstrated focal and segmental distribution of the GBM antigen(s). Electron microscopic examination revealed splitting and thinning of the GBM. Indirect immunofluorescence showed that there was no binding of Goodpasture's anti-GBM antibodies to the glomeruli. These findings are similar to those in patients with hereditary nephritis. The immunofluorescent examination of the fixation of the various anti-sera, including anti-types IV and V collagens, laminin, fibronectin, and actomyosin sera on the GBM, revealed normal reactivity. The abnormalities observed in this case may be a part of the spectrum of primary GBM defects.

Portal blood flow measured by duplex scanning during mesenteric infarction.

The portal blood flow of a 70-yr-old man with suspected massive mesenteric infarction was measured using duplex scanning. The main portal flow was 109 ml/min, while cardiac output was 4.2 L/min. Laparotomy revealed a massive mesenteric infarction from proximal jejunum to rectum. Duplex scanning may be a useful method for studying the course of mesenteric infarction.

Hemodialysis using gabexate mesilate (FOY) in patients with a high bleeding risk.

The efficacy of gabexate mesilate (FOY), a synthetic serine proteinase inhibitor, was compared with that of heparin in preventing the acceleration of bleeding after hemodialysis. Transfused blood volume (TBV) was measured 24 h before and after 24 dialyses in 14 bleeding patients with impaired hemostatic function. The predialysis TBV did not differ significantly between heparin and FOY groups; however, TBV was significantly (p less than .05) larger after heparin dialysis than after FOY dialysis. After dialysis, TBV was increased in eight of nine heparin patients, compared to only three of 15 FOY subjects (p less than .01). FOY is an effective agent and may decrease postdialysis bleeding complications in certain high-risk patients.

Lipid peroxidation in experimental septic rats.

We previously reported increased lipid peroxide and decreased alpha-tocopherol levels in the blood of critically ill septic patients. To clarify these results, we investigated lipid peroxidation in experimental septic rats and severely traumatized rats. In septic rats, both platelet count and plasma alpha-tocopherol decreased significantly, while lipid peroxide in plasma and major organs significantly increased. Serum transaminases also increased significantly. Traumatized rats had a significant but transient decrease in platelet count and a continuous decrease in plasma alpha-tocopherol; lipid peroxide did not change significantly in the plasma but increased significantly in the lung and kidney. Serum transaminases of traumatized rats showed transient increases. Thus, although traumatic stress caused lipid peroxidation similar to sepsis in the major organs, plasma lipid peroxide did not change.

Plasma norepinephrine in sleep apnea syndrome.

In 8 male patients suffering from sleep apnea syndrome, plasma norepinephrine (NE) levels were examined. At 22.00 and at 6.30 blood samples were obtained. In 3 cases, plasma NE levels displayed little changes between 22.00 and 6.30. In 2 cases, plasma NE levels at 6.30 increased 20% compared with those at 22.00. In 3 cases, plasma NE levels at 6.30 increased more than 40%. No significant correlation between apnea index and plasma NE levels was observed. Total time under 90% arterial oxygen saturation (SaO2) significantly correlated with the ratio of plasma NE levels at 6.30 to those at 22.00. In 2 cases of the 8 patients blood samples were drawn hourly (22.00-6.00) and at 6.30. In these 2 cases, when SaO2 decreased, plasma NE levels tended to increase. It is concluded that in sleep apnea syndrome plasma NE levels increased during sleep and did not correlate with apnea index but with oxygen desaturation.

Kasabach-Merritt syndrome with disseminated intravascular coagulopathy treated by exchange transfusion and surgical excision.

Kasabach-Merritt syndrome with consumption coagulopathy occurred in a 5-week-old female infant with a hemangioma on her back. Disseminated intravascular coagulopathy was improved by an exchange transfusion. Administration of steroid and radiation therapy was attempted as a treatment for the hemangioma and thrombocytopenia; however, it was unsuccessful. Complete recovery was obtained by surgical excision. Enhanced computerized tomography was advantageous to determine the size of the hemangioma prior to operation.

LAV/HTLV-III antibodies in patients treated with intravenous gammaglobulins.

The test sera from 50 patients treated with intravenous gammaglobulins (IVG) were examined for LAV/HTLV-III antibody by ELISA tests in order to evaluate the risk of transmission of LAV/HTLV-III virus via administration of IVG. The three ELISA-positive sera were negative for the antibody by both indirect immunofluorescence assay (IFA) and Western blot assay (WBA). Thus, all of the test sera were negative for LAV/HTLV-III antibody. Two of the ELISA-positive/IFA-negative/WBA-negative samples had HLA-DR4 antibodies causing positive ELISA results.

The effects of HA compound calcium antagonists on delayed cerebral vasospasm in dogs.

The authors have examined the effects of the HA compounds HA1004(N-(2-guanidinoethyl)-5-isoquinolinesulfonamide) and HA 1077(1-(5-isoquinolinesulfonyl)homopiperazine), which are intracellular calcium antagonists, on delayed cerebral vasospasm from subarachnoid hemorrhage (SAH). The modes of action of these compounds were compared with those of the more commonly used calcium entry blockers. Calcium ionophore A23187 (4.8 X 10(-6) M)-induced contraction of a canine basilar artery strip was completely antagonized by the HA compounds (10(-5) M) but not by the entry-blocking calcium antagonists nicardipine, diltiazem, and verapamil (10(-5) M), suggesting that the HA compounds act differently. Delayed cerebral vasospasm was induced by a "two-hemorrhage" canine model. The magnitude of the vasospasm and the effects of the HA compounds were determined angiographically. On SAH Day 7, a significant vasospasm was observed in every dog. The diameter of the basilar artery had diminished to 59% +/- 2% (mean +/- standard error) of the control value obtained before SAH (on Day 1). The intravenous administration of HA 1004 caused a mild dilation of the basilar artery of 10% and 11% at doses of 3 and 10 mg/kg, respectively; however, HA 1077 produced a more marked dilation of 19% and 27%, respectively, at the same doses. Both of these drugs lowered mean arterial blood pressure to about 80% and 50% at doses of 3 and 10 mg/kg, respectively. Intracisternal administration of the HA compounds (6 mg) completely reversed cerebral vasospasm without much effect on the blood pressure. The intracellular calcium antagonists of the HA compound group appear to be promising agents for the treatment of intractable cerebral vasospasm.

Enrichment of antitumor effector cells that are effective in vivo from spleen cells of tumor-bearing mice through the use of Dolichos biflorus lectin.

Spleen cells of X5563 tumor-bearing mice were fractionated with Dolichos biflorus lectin (DBA) into DBA+ (agglutinable with DBA) and DBA- (nonagglutinable with DBA) fractions. The DBA- cells showed antitumor activity specific for X5563 cells in vivo when injected into mice together with X5563 cells (Winn assay) or injected i.p. into mice previously inoculated s.c. with X5563 cells (adoptive transfer) with simultaneous administration of interleukin-2. Both DBA+ and DBA- cells have the cell surface phenotype of Lyt-1-2+, but only DBA- cells exhibit antitumor activity in vivo, while DBA+ cells have stronger cytolytic activity against X5563 cells in vitro than DBA- cells. This cell separation method involving the use of DBA is simple and gives reproducible results and high yields of viable cells.
