Xanthine calculi in the patient with the Lesch-Nyhan syndrome associated with urinary tract infection.

A Japanese boy with Lesch-Nyhan syndrome who passed xanthine calculi is reported. After pyelolithotomy for a left renal stone, made up of ammonium urate, associated with urinary tract infection, a high dose of allopurinol was given because of the persistence of pyuria. In the present case, the administration of a high dose of allopurinol, given for the prevention of ammonium urate stone formation in infected urine, induced xanthine calculi formation and we had difficulty in the management of this patient with Lesch-Nyhan syndrome associated with urinary tract infection. However, we believe it a basic necessity to cure our patient of his urinary tract infection and prevent recurrent ammonium urate stone formation because of the risk of renal deterioration.

Promotion of the selective lymphatic delivery of cyclosporin A by lipid-surfactant mixed micelles.

The absorption of an immunosuppressive drug, cyclosporin A (CsA), with the aid of lipid-surfactant mixed micelles from the rat gastrointestinal (GI) tract and lymphatic delivery were studied. The administration of CsA in oily solution, sesame oil or linolic acid, into the rat duodenum indicated a small amount of CsA both in the plasma and lymph for about 6 h. The administration of CsA in the mixed micellar solution composing of linolic acid and HCO-60, polyoxyethylated (60 mol) hydrogenated castor oil, accelerated the absorption of CsA from the GI tract, and CsA was delivered into the lymphatics with an extremely high selectivity.

Changes in epidermal growth factor concentrations of submandibular gland, plasma and urine of normal and sialoadenectomized female mice during various reproductive stages.

The concentrations of epidermal growth factor (EGF) were measured by radioimmunoassay in the submandibular gland, plasma and urine of adult female C3H/HeN mice whilst virgin and during pregnancy, lactation and after lactation. During gestation there was a significant increase in the submandibular EGF concentration which was five to seven times higher than that found in virgin mice. The level of EGF in the gland remained high during the period of lactation and even several weeks after lactation. Plasma EGF levels were also increased during the periods of pregnancy, lactation and after lactation when compared with those of virgin mice. These increases were, however, apparent only between 24.00 and 08.00 h, because of circadian variations in circulating EGF. The level of plasma EGF was significantly (P less than 0.05) higher during the 24.00-08.00 h period than during the 12.00-20.00 h period in all stages examined. Concentrations of EGF in the urine of virgin, pregnant, lactating and primiparous mice remained relatively constant, and the levels were much higher than those in the plasma. Similar studies using sialoadenectomized pregnant and lactating mice indicated that the plasma levels of EGF were below the level of sensitivity of the assay (less than 16.5 pmol/l (less than 0.1 ng/ml] even during the 24.00-08.00 h period, whereas urinary EGF remained at high levels which were similar to those of normal pregnant and lactating mice. These results suggest that submandibular EGF contributes to the increase in plasma EGF which occurs after gestation, but is not the major source of the urinary EGF.

Experimental infection of rabbits with human T-cell leukemia virus type I.

Rabbits were successfully infected with human T-cell leukemia virus type I (HTLV-I) and produced antibodies to adult T-cell leukemia-associated antigens (ATLA) on intravenous inoculation of the HTLV-producing human cord T-cell line MT-2, or autologous cell lines established by cocultivation with MT-2 cells. Lymphocytes taken from the rabbits between 4 and 14 days after the inoculation of MT-2 cells, but not lymphocytes obtained in earlier or later periods, could be immortalized in vitro and expressed ATLA and type C virus particles. However, lymphocytes harvested during an early culture period (5 to 8 days) were found to be negative for ATLA. The transformed cells had the karyotype of a normal male rabbit. Two rabbits inoculated with the autologous HTLV-producing cell lines did not allow their growth in vivo, but some peripheral blood lymphocytes could be immortalized in vitro. One of these transformed cell lines was examined for the integration of HTLV-I provirus genome. The transformed cells were found to contain the provirus genome and also to be monoclonal with respect to the integration site of the provirus genome, unlike the inoculated cells.

[Randomized controlled trial of postoperative adjuvant chemotherapy with bestatin for gastric cancer].

The effectiveness of Bestatin on the prognosis of gastric cancer patients was investigated in a randomized controlled trial of postoperative adjuvant chemotherapy. One hundred and sixty-two cases of gastrectomized patients were divided into two groups; a Bestatin group (83 cases, 1/2MF'C [mitomycin C 2 mg and cytosine arabinoside 20 mg i.v. twice a week for 3 weeks, FT-207 400 mg/day i.v. for 3 weeks] followed by Bestatin 30 mg/day and FT-207 0.6 g/day p.o.) and a control group (79 cases, 1/2MF'C followed by FT-207 alone). No significant differences in background factors between the two groups were found. Bestatin was slightly effective in improving the three-year survival rates of gastric cancer patients, but this was not significant. However, survival rates in the Bestatin group in Stage II, curatively operated cases, and n (-) ps (+) cases were significantly higher than those of the control group during the same postoperative period. Bestatin prolonged the disease-free interval in curatively resected cases. Thus, efficacy of Bestatin in improving survival rates was observed in the early stages of disease, but it is suggested that combination a stronger chemotherapeutic regimen may be effective in advanced cases.
