The disappearance rate of indocyanine green (K.ICG) and the maximum removal rate (Rmax) usually correlate with each other. However, in some cases it was shown there was a dissociation between them. We investigated the relationship between the two rates in 146 subjects. K.ICG and Rmax correlated strongly with a correlation coefficient of 0.749 (p less than 0.001). Sixty-six cases were included in the limits of 95% confidence, and the other 80 cases outside the limits were defined as dissociated cases. Among them a lower Rmax rate as compared to the K.ICG rate was found in many cases of obstructive jaundice. Particularly a lower K.ICG rate compared to the Rmax rate was found in many cases of liver cirrhosis accompanied by esophageal varices and idiopathic portal hypertension. On the other hands, we performed multiple regression analysis on 12 other liver function tests. K.ICG was strongly related to platelet count, circulatory blood volume, and albumin, all factors relating to portal hypertension. Rmax largely depended on LCAT, A/G ratio, and cholinesterase, which are Therefore, the dissociation between K.ICG and Rmax was caused by differences in the characteristic of each disease.
Four monoclonal antibodies (MoAbs) reactive with carcinoembryonic antigen (CEA) were obtained by hybridizing mouse myeloma cells (P3-X63-Ag8-U1) with spleen cells from nude mice (BALB/c, nu/nu) that had rejected transplanted human colonic adenocarcinomas Co-3 and Co-4 following intraperitoneal injection of spleen cells from immunocompetent mice (BALB/c). By solid-phase RIA with purified CEA and its related antigens, NCC-CO-413 (IgG2a, kappa) was shown to react with NCA and BGP-I as well as with CEA, whereas the reactivities of three other MoAbs, NCC-CO-308 (IgG1, kappa), -432 (IgG1 lambda), and -411 (IgG1, kappa) were limited to CEA. Immunohistochemical reactivities of these MoAbs to colonic carcinomas, granulocytes, and liver bile canaliculi on acetone-fixed paraffin-embedded sections ("AMeX" sections) confirmed the specificities of these MoAbs shown by the solid-phase RIA. By competition solid-phase RIA, the epitopes recognized by NCC-CO-308 and -432 were shown to be shared or located close to each other, whereas the other MoAbs were shown to recognize different epitopes. Thus, two epitopes specific to CEA and one shared by NCA and BGP-I as well as CEA were identified. Furthermore, reactivities of MoAbs with the two CEA-specific epitopes were easily abolished by heat denaturation or reduction of CEA, as revealed by solid-phase RIA and SDS-PAGE-immunoblotting, indicating that these two CEA-specific epitopes are based on the conformational structure of the CEA molecule.
1. Macrophages are an important source of the lipid mediators arachidonic acid (AA) and its metabolites that are produced during inflammation. 2. Previously, we reported that insoluble glucans from Streptococcus mutans in dental plaque could induce macrophages to secrete PGE2 and TXB2. 3. Studies were undertaken to identify the phospholipid substrates that can serve as a source of AA in macrophages exposed to the insoluble glucan. 4. When macrophage cell prelabelled with [3H]AA, stimulation with insoluble glucan resulted in a loss of label mainly from phosphatidylcholine (PC) and phosphatidylinositol (PI). 5. In addition, the PC-, and PI-specific phospholipase A2-mediated mechanisms for AA release may be activated in guinea peritoneal macrophages exposed to the insoluble glucan from S. mutans.
A single s.c. injection of diethyldithiocarbamate (DDC, 1 g/kg) induced not only gastric but also duodenal mucosal damage in rats. DDC induced marked decreases in gastric acid output, gastro-duodenal mucosal blood flow and transmucosal potential difference prior to the development of mucosal lesions. Superoxide dismutase activity in the gastro-duodenal mucosa was also inhibited, while catalase and glutathione peroxidase activities gradually increased after the administration of DDC. These results suggest that a decrease in mucosal defensive mechanisms plays an important role in the development of DDC-induced gastro-duodenal mucosal damage and that oxygen-derived free radicals may also participate in the development of this mucosal damage.
A fact-finding survey was conducted to ascertain the treatment of 298 patients with acute myocardial infarction (AMI) who were referred to our coronary care unit (CCU) from other medical facilities between May 1978 and December 1987. The controls consisted of 169 patients with AMI who were admitted directly to our CCU during the same period. The mean time from onset of AMI to admission to our CCU was 21.7 +/- 67.9 hours (mean +/- SD) for patients from other medical facilities, and it took longer than that of the controls (11.7 +/- 34.9 hours). However, the mortality was 19.8%, being lower than that of the controls (26.0%). By the Killip's classification, there was no significant differences between both patient groups who belonged to Killip groups I, II and IV, but the mortality of cases in Killip group III was 3.7%, which was lower than that of the controls (7.15%). Among the 298 cases, 169 (56.7%) had received some kind of emergency treatment, and their mortality was 29.9%. However, the mortality of the remaining 129 cases (43.3%) who had received no emergency treatment was only 17.1%. The reason for this contradictory result is attributed to the fact that the former group included relatively severe cases. The number of patients receiving emergency treatment has gradually increased recently; however, the overall results achieved were not satisfactory even with appropriate therapy. Since April 1984, conferences with local practitioners have been held concerning emergency treatment for ischemic heart disease. This resulted in better understanding of CCU among the practitioners, less time delay until admission, and increased frequency and higher quality of emergency treatment. However, the mortality in the CCU did not decline, probably because of the relatively high rate of severe cases. To reduce mortality of AMI, a communication network should be established between practitioners and the CCU.

(+/-)-(R*)-2,6-Dimethyl-4-(m-nitrophenyl)-1,4-dihydropyridine-3,5- dicarboxylic acid (R*)-1-benzyl-3-piperidinyl ester, methyl ester hydrochloride (benidipine hydrochloride, KW-3049) has been developed as antihypertensive and antianginal agent. The absorption, distribution and excretion were investigated after single oral administration of 14C-labeled KW-3049 using Wistar rats and beagle dogs. The results were summarized as follows: 1. After oral administration of 14C-KW-3049 to rats, the plasma radioactivity reached the maximum at 0.5 h, showed the second peak at 4 h and decreased biphasically with the biological half-lives of about 6 h and 38 h. 2. After oral administration of 14C-KW-3049 to dogs, the plasma radioactivity reached the maximum at 1 h and decreased biphasically with the biological half-lives of about 2 h and 25 h. 3. After oral administration to rats and dogs, the excretion of the radioactivity in feces and urine during 72 h (rats) and 96 h (dogs) were 74% and 19%, 66% and 25%, respectively. The radioactivity excreted in the bile was 34% during 48 h and was followed by partial reabsorption from the gastrointestinal tract. 4. High radioactivities were observed in the digestive organs, mesenteric lymphnodes, liver, pancreas, urinary bladder, fat tissue, kidney and spleen after oral administration to rats.
The absorption, distribution and excretion of (+/-)-(R*)-2,6-dimethyl-4-(m-nitrophenyl)-1,4-dihydropyridine-3,5-dicarb oxylic acid (R*)-1-benzyl-3-piperidinyl ester, methyl ester hydrochloride (benidipine hydrochloride, KW-3049) were studied after repeated doses to rats. The results were summarized as follows: 1. After oral repeated administration once a day for 3 weeks, the radioactivities in blood and tissues increased up to the 14th day, and thereafter they tended to show the steady state. 2. The radioactivity in the tissues at 1 h after last dosing was compared with that at 1 h after single administration. Both tissue distribution patterns were not markedly different, but the concentrations in the liver, testicles, skin and descending aorta significantly increased, showing approximately the 2-fold radioactivities. 3. The radioactivities were excreted in the urine (6%) and feces (88%). The excretion of radioactivity in urine and feces did not change markedly during the repeated administration period.
Peripheral blood mononuclear cells (PBMC) from 30 patients with definite or classic active rheumatoid arthritis who were on no remittive drugs were studied for spontaneous and pokeweed mitogen (PWM)-stimulated immunoglobulin plaque-forming cell frequency (IgPFC), spontaneous IgM-rheumatoid factor (IgM-RF) secretion, and in vitro proliferative responses to soluble recall antigens. Rheumatoid spontaneous total (IgG + IgM + IgA) IgPFCs were higher than those of normal controls when assayed after 7 days in culture. Spontaneous and PWM-stimulated IgM-PFCs, in contrast, were significantly less than normal regardless of when assayed. Spontaneous synthesis of IgM-RF was observed in 56% of the RA patients, but absolute amounts produced were widely heterogeneous. Spontaneous IgM-RF production by RA PBMC was associated with low or absent spontaneous IgM-PFC production. Moreover, a strong association was found between the median amount of IgM-RF secreted and depressed proliferative responses to soluble recall antigens. Our results define several abnormalities of immunoglobulin production in a clinically homogeneous and highly active rheumatoid population and delineate methodologic variations that can complicate the interpretation of similar data in the literature. In addition, our findings suggest that subgroups of rheumatoid patients that show distinct cellular and humoral immune abnormalities can be identified.

Experimental chemotherapy with cisplatin and carboplatin was performed against nine human tumor xenografts serially transplanted into nude mice. Tumors used for the experiment were seven gastric (St-4, St-15, St-40, H-111, SC-2-JCK, SC-6-JCK and Exp-4), one breast (MX-1) and one colon (Co-4) carcinomas. Cisplatin 9 mg/kg and carboplatin 100 mg/kg were administered intraperitoneally (ip). Carboplatin 25 mg/kg was also given ip 4 times every 4 days. The efficacy rates of cisplatin and carboplatin by bolus injection were 77.8% and 66.7% respectively with no statistically significant differences. However, carboplatin was found more effective when given by bolus. The antitumor spectra of both drugs were similar. From these results, these two platinum compounds seemed to be effective against human gastric carcinomas.

Two patients with co-existing meningeal and pulmonary cryptococcosis were successfully treated by pulmonary resection and chemotherapy. Under cover of miconazole and 5-fluorocytosine, pulmonary mass lesions were successfully removed despite the fact that the patients had meningitis. The patients recovered from meningitis immediately after the surgical procedures and show no recurrence of meningeal symptoms more than 2 years after the operation. It appears that early removal of a pulmonary cryptococcal focus combined with antifungal agents may give a quite favourable outcome for meningitis.

From a retrospective analysis of 38 consecutive cases of twins the authors have reached a conclusion that in the management of twins the following policy is applicable: 1. For every pregnancy, a routine ultrasonic examination should be done for the detection of twin earlier than 20 weeks of gestation. 2. Elective hospitalization or bed rest at home should be advised from a period of 30 weeks of gestation. 3. After 32 weeks of gestation, an increased antenatal assessment (including serial ultrasonography) and biophysical monitoring (such as simultaneous NST) should be done.
