Chemical restraint of African lions (Panthera leo) with medetomidine-ketamine.

Effects of a combination of medetomidine-ketamine as a chemical restraint and antagonistic effects of atipamezole on this combination were investigated in 5 lions. The medetomidine (47.6-58.4 micrograms/kg) and ketamine (1.9-5.7 mg/kg) combination provided complete immobilization with good analgesia and muscle relaxation in 4 lions, while one lioness was poorly sedated by medetomidine, and additional injections of medetomidine and ketamine were required. The duration of anesthesia seemed to be much longer than one hour in 4 of the lions. Atipamezole, at four times the preceding dose of medetomidine, provided a smooth recovery and animals were able to stand up 17-61 min after its injection. Side effects were limited to vomiting after walking in 3 of 5 lions.

Chloropeptins, new anti-HIV antibiotics inhibiting gp120-CD4 binding from Streptomyces sp. I. Taxonomy, fermentation, isolation, and physico-chemical properties and biological activities.

Chloropeptins I and II, which are gp120-CD4 binding inhibitors, were isolated as pale yellow-brown powders from the mycelia of a soil actinomycete, Streptomyces sp, WK-3419. Their physico-chemical properties showed that they are chlorinated peptides. Chloropeptin I (C61H45N7O15Cl6) is a novel compound, but chloropeptin II was identified as complestatin. Both compounds inhibited gp120-CD4 binding (IC50: 1.3 and 2.0 microM, respectively), the cytopathic effect of HIV in MT-4 cells (EC50: 1.6 and 1.7 microM, respectively) and syncytium formation in co-cultured HIV-1-infected and uninfected MOLT-4 cells (IC50. 0.5 and 1.1 microM, respectively). Chloropeptin I was synergistic in the inhibition of the cytopathic effect when combined with other anti-HIV drugs such as zidovudine (AZT), didanosine (ddI), zalcitabine (ddC) and nevirapine.

[Surgical treatment of malignant hemangioendothelioma originated from the right atrium: a case report].

A 32-year-old man was admitted to our hospital because a cardiac tumor had been pointed out by a physician. CT scans showed a mass lesion with irregular patterns in the right atrium. The patient had the diagnosis of a malignant cardiac or pericardial tumor and an operation was performed. The mass originated from the right atrium included massive coagulated blood, and was resected with the pericardium and the right pleura. The histological diagnosis was malignant hemangioendothelioma. We present this case because only 35 patients with cardiac malignant hemangioendothelioma were reported in Japan.

[Ultrastructural study of lens in rat hereditary cataract by quick-freezing and deep-etching].

The cytoskeleton of lens fiber cells in rats with hereditary cataract (ICR/f) was studied by quick-freezing and deep-etching. Two kinds of filamentous structure with diameters of 5 and 10 approximately 15 nm were observed in the cortical fiber cells. They showed meshwork structures which were buried in globular particles. These filamentous structures were mostly absent in the nucleus fiber cells, but aggregated and fused globular particles with diameters of 10 approximately 20 nm were observed. Small cavities were sometimes observed in their cytoplasm. Between lens fiber cells, many globular structures were also seen. These changes might represent the degeneration of the lens fiber cells in cataract lenses.

[Hemorheological effects of autologous blood storage before surgery for cardiac valvular diseases].

The hemorheological effects of autologous blood storage with or without the use of erythropoietin were examined before surgery for valvular disease. There was no rheological difference between patients with aortic (16 cases) or mitral (10 cases) valve disease. Before storage, the levels of hematocrit, whole blood viscosity, and especially coefficient of rheology, were lower (p < 0.05) in the blood stored with erythropoietin, but this difference disappeared after storage. The plasma viscosity of both groups did not change before and after storage. The viscosity of blood was equalized after the storage of blood, irrespective of the use of erythropoietin.

Distal posterior interosseous nerve syndrome.

Five patients presenting with chronic dorsal wrist pain and diagnosed as distal posterior interosseous nerve syndrome are reported. Clinical examination revealed point tenderness of the fourth extensor compartment. The symptoms were reproduced by extreme wrist extension in all patients and by extreme flexion in three of the patients. The pain was relieved in all patients by a selective lidocaine block of the terminal branch of the posterior interosseous nerve at the wrist joint. All the patients failed to respond to nonoperative treatment and underwent surgical exploration of the nerve. The operative findings were an enlarged nerve in three patients and adhesion of the nerve to the joint capsule in five patients. A 2 cm section of the nerve proximal to the extensor retinaculum was resected. Four of the five patients had excellent pain relief and returned to full asymptomatic activity. One patient had improvement in pain and was satisfied with the outcome. A diagnosis of distal posterior interosseous nerve syndrome should be considered if the usual sources of dorsal wrist pain are eliminated.

[A long-term surviving case with invasive thymoma undergone 4 times operations for tumor recurrences].

We reported a long-term survivor with invasive thymoma associated with myasthenia gravis. The patient underwent operations 4 times for these 13 years. At the third and fourth operation, the tumor was resected after preoperative neo-adjuvant chemotherapy with cisplatin and adriamycin, and postoperative radiotherapy was added. Her postoperative course was uneventful, and she has been alive. Our strategy for invasive thymoma is that tumors which include both primary and recurrent lesions are removed as completely as possible, and that postoperative radiotherapy is added for stage II, III and IV in Masaoka's classification, and preoperative neo-adjuvant chemotherapy for stage III and IV. We believe that these multidisciplinary treatments and long term follow-up lead good results without harm to quality of life.

High sensitivity to streptozotocin in herbivorous voles, Microtus arvalis, compared to mice.

Herbivorous voles, Microtus arvalis, have characteristics similar to herbivores in that their hepatic glycolytic enzyme activities are relatively low. The effects of a single low dose (100 mg/kg body weight) of streptozotocin (STZ) in voles were studied and the difference in sensitivity to or toxicity of STZ in voles and C57BL/6 mice was compared. In voles which received STZ, the cumulative incidence of glycosuria reached 53% by 4 weeks after administration. The diabetic voles showed marked increases in their blood glucose and plasma free fatty acid concentrations and a significant decrease in plasma immunoreactive insulin concentrations. Their hepatic hexokinase, glucokinase, glutathione peroxidase and glutamate dehydrogenase activities decreased significantly and lesions were widely observed in the liver, kidney and pancreas. The activities of glutathione peroxidase, a scavenger of H2O2, decreased significantly in their liver and pancreas. These changes were not observed in C57BL/6 mice which received STZ. The higher sensitivity to and toxicity of STZ in voles than in mice are considered to be caused by the characteristically low activities of glycolytic enzymes and glutathione peroxidase in the tissues of voles. Voles may be a good model for studying the mechanisms of cytotoxicity by STZ in herbivorous animals.

Long-term results of surgical treatment of thumb polydactyly.

One hundred thirteen hands exhibiting thumb polydactyly were treated and followed up for an average of 49 months. Of these, 109 hands were treated by resection of a supernumerary hypoplastic thumb. Radial thumbs were resected in 107 hands and ulnar thumbs in 2 hands. Four hands were treated using a modified Bilhaut procedure. According to a modified Tada's evaluation, the results were evaluated as good in 97 hands, fair in 12 hands, and poor in 4 hands. Patients and/or their parents were satisfied with the results in 100 hands and dissatisfied with the results in 13 hands. The factors that influenced the surgical results were analyzed. The incidence of unsatisfactory results was relatively high in Wassel types 3, 5, and 6 and triphalangeal-type thumb polydactyly. It was higher when the ulnar digit was removed than when the radial digit was removed. The results for those patients treated between 1983 and 1991 were better than for those treated between 1976 and 1982. The type of deformity, type of procedure, and skillfulness of the surgeon were factors in the results after surgery.

Oxidative damages by iron-chelate complexes depend on the interaction with the target molecules.

To elucidate that in iron-catalyzed oxidative damage the interaction of iron complex with the target molecules is important, the oxidative damage to plasmid DNA, protein and fatty acid has been compared using iron-chelate complexes with nitrilotriacetic acid (nta), citric acid, ethylenediamine-N,N'-diacetic acid (edda) and diethylenetriamine-N,N,N',N",N"-pentaacetic acid (dtpa). In the presence of hydrogen peroxide, plasmid pBR322 strand breaks occurred in the order of Fe-edda > Fe-citrate > Fe-nta > > Fe-dtpa. However, fragmentation of bovine serum albumin and diene conjugation of linoleic acid micelle occurred in the order of Fe-nta > Fe-edda > > Fe-citrate > Fe-dtpa = O, which were similar to hydroxyl radical production by these iron complexes and H2O2. Bleomycin-detectable free radical-promoting irons in these iron complexes were about 85% of iron in Fe-nta, Fe-citrate and Fe-edda, and only about 33% in Fe-dtpa. Not only hydroxyl radical productivity and free radical-promoting iron content in iron complex, but also the interaction of the complex with the target molecules determines the iron-catalyzed oxidative damage.

Congenital anomalies of the hand. The Asian perspective.

To have a better understanding of teratogenic mechanisms of congenital absence of digits, ulnar and radial deficiencies, cleft hand, and symbrachydactyly were analyzed in clinical cases. The same anomalies were induced in rat fetuses by busulfan, and their characteristics were investigated. The formation process of longitudinal deficiency also was observed histologically. There seemed to be 4 teratogenic mechanisms of congenital absence of digits. Ulnar and radial deficiencies have the same clinical features, and the cause of these deficiencies is related closely to a deficit of mesenchymal cells in the limb bud caused by the impairment before the formation of the limb bud. Cleft hand, central polydactyly, and osseous syndactyly were induced by the same treatment at the same developmental stage in rats. Cleft hand formation process from osseous syndactylies and central polydactylies was supposed. The teratogenic mechanism of cleft hand seemed to be failure of induction of digital rays in the hand plate. The sequence of anomalies from brachysyndactyly, or the atypical cleft hand, to the transverse deficiency can be regarded as equivalent to the category of bony dysplasia of the hand. Congenital constriction ring syndrome does appear after the formation of the digital rays.

A novel gene that interferes with the phosphotransfer signal transduction mediated by the EnvZ osmosensor in Escherichia coli.

In Escherichia coli, expression of the major outer membrane proteins, OmpC and OmpF, is regulated in response to the medium osmolarity and other environmental stimuli. A two-component signal transduction system, mediated by EnvZ and OmpR, is crucially responsible for this osmotic regulation of the ompC and ompF genes. In this study, an E. coli gene was cloned, which interferes with expression of both the ompC and ompF genes at the level of transcription, provided that the cloned gene was introduced in E. coli cells by a multicopy plasmid. The gene product was identified as F107, which was previously characterized as a hypothetical protein in E. coli genome databases. F107 containing 107 amino acids appears to be highly hydrophobic, and has a sequence similarity to the eukaryotic type of cytochrome-c oxidase subunit III. The mechanism by which F107 inhibits transcription of ompC and ompF was examined extensively, mainly by using a set of envZ and ompR mutants. These results suggested that F107 interferes specifically with a function of the EnvZ osmosensory kinase. Possible mechanisms by which F107 affects the EnvZ function are discussed.

[Bilateral multilocular cystic renal cell carcinoma: a case report].

A case of bilateral synchronous multilocular cystic renal cell carcinoma is reported. A 52-year-old man was admitted to our hospital for further examination of right renal mass which was found incidentally by ultrasonography. After he was examined by intravenous pyelography (IVP), computerized tomographic (CT) scan, magnetic resonance imaging (MIR) and angiography, right nephrectomy was performed under the diagnosis of right renal cancer. Histological diagnosis was multilocular cystic renal cell carcinoma. CT scan revealed a small left renal cystic lesion at that time. Ten months later enhanced CT scan revealed irregular septum in the cystic lesion. Under the diagnosis of left renal cancer, we carried out simple enucleation. Histological diagnosis was multilocular cystic renal cell carcinoma as in the right kidney. The patient has been followed up carefully. This is the third case of bilateral multilocular cystic renal cell carcinoma, which is regarded as the first synchronous one, reported in Japan.
