Application of aspartic acid racemization to forensic odontology: post mortem designation of age at death.

The extent of racemization of aspartic acid in the dentin of healthy, carious and impacted teeth was used to define the ages of individuals at the time of their death. These determinations were performed with an error of +/- 4 years. This method holds the promise of simplifying procedures in, and extending the application of forensic odontology. The current error range of this method may be reduced by analyzing teeth of the same positional types from deceased individuals of known age. Using this method, it appears that post mortem preservation of up to 10 years has a negligible effect on the values determined for the age of individuals at death.

Neurophysiologic dysfunction in hypomelanosis of Ito: EEG and evoked potential studies.

To evaluate neurological involvements in hypomelanosis of Ito (HI), we conducted electroencephalographic and evoked potential studies in 3 children with HI. Patient 1 presented with the West syndrome, and was severely handicapped. Patient 2 had mild mental retardation. Patient 3 appeared neurologically normal, but chromosome analysis showed a 46,XX/46,XX,12p + mosaic karyotype. EEGs of Patient 1 initially showed asymmetric hypsarrhythmia, and later diffusely increased fast waves, occipital dominant alpha rhythm plus multifocal spikes. Patients 2 and 3 had minimal and no EEG abnormalities, respectively. Brainstem auditory evoked potentials showed prolongation of wave I latency as well as I-III and I-V interpeak latencies in Patient 1, and prolonged wave I latency in Patient 3. Flash visual evoked potential studies disclosed a significant difference between the right and left latencies of wave IV in Patient 2. The fact that there were no specific EEG or evoked potential findings for HI seems to be consistent with the pathogenetically heterogeneous nature of this disorders. We emphasize the usefulness of neurophysiologic examinations in evaluating diverse CNS dysfunctions in HI patients.

Effects of superior colliculus inhibition on three-dimensional visual motion processing in the lateral suprasylvian visual area of the cat.

PURPOSE: To determine whether visual inputs from the tectothalamocortical pathway influence three-dimensional motion processing within the lateral suprasylvian (LS) area of the cat. METHODS: Tungsten microelectrodes were used for recording visual-evoked potentials (VEPs) from the LS area of 4 cats. Random dot stereograms were used as visual stimuli. Three-dimensional, motion-triggered VEPs were recorded from the LS area. Each motion sequence consisted of an abrupt onset of motion disparity with a 2 degrees amplitude followed by an abrupt offset and a stationary phase of 900 ms. The velocity of the motion disparity was varied in eight steps from 10 degrees to 400 degrees per second. The onset of motion disparity was used as the trigger for recording the VEPs. Single or multiple injections (two to three) of muscimol were made mainly into the rostral superior colliculus (SC). The amplitudes of the VEPs before and after the muscimol injection were compared. RESULTS: A large negative wave ( N1) with an implicit time of 92.7 +/- 13.5 ms (mean +/- SD, n = 98) was recorded consistently. The amplitude of N1 was significantly larger on stereovision of motion disparity than on either binocular vision of two-dimensional lateral motion or monocular vision, indicating that N1 contains neurons sensitive to motion disparity. The amplitude of N1 was not altered by muscimol injection into the SC at velocities < or =50 degrees/s. On the other hand, the amplitude of N1 was reduced to 66-71% of that observed before muscimol injection at velocities > or =75 degrees/s. CONCLUSIONS: These findings suggest that the LS area processes three-dimensional motion inputs via two parallel pathways, the geniculostriate pathway and the tectothalamocortical pathway, at velocities of motion disparity > or =75 degrees/s, while the three-dimensional motion inputs project to the LS area only via the geniculostriate pathway at velocities of motion disparity < or = 50 degrees/s.

Changes in glucose transport activities in mammary adenocarcinoma of dogs.

The activities of D-glucose transport (D-GT) and cytosolic enzymes were significantly higher in mammary adenocarcinoma of dogs than in mammary gland from normal dogs. The activities of D-GT in adenocarcinoma were over three-and-a-half times higher than in the controls. The K(m) value of the D-GT activity for glucose in both the adenocarcinoma and normal mammary gland was approximately 0.9 mM. The activities of the key glycolytic enzymes, hexokinase and pyruvate kinase, in the adenocarcinoma were also more than three-and-a-half times higher than in the controls. The increased activities of D-GT are considered to be accompanied by an acceleration of glucose utilisation in the adenocarcinoma of dogs.

Teratogenic mechanisms of longitudinal deficiency and cleft hand.

In order to better understand the teratogenic mechanisms of congenital defects of the digits, we analyzed clinical cases and induced similar types of congenital hand anomalies in rat fetuses by oral administration of busulfan. In clinical cases, radial and ulnar deficiencies had common characteristic features. We induced radial and ulnar deficiencies in rat fetuses with the same drug. Radial and ulnar deficiencies induced in rats have similar clinical manifestations and these anomalies might be caused by the same teratogenic mechanism. Then, the formation of the digital rays was examined histologically. The results of histological examination suggested that these deficiencies were not caused by localized damage of the limb bud. They also suggested that the cause of missing digits in longitudinal deficiency is closely related to a deficit of mesenchymal cells in the limb bud. Cleft hand is considered to be one of the types of longitudinal deficiency. However, several investigators have suggested that the abnormal induction of finger rays in the process of formation of fingers induced central polydactyly, osseous syndactyly and also cleft hand. X-rays of the clinical cases and skeletal changes of the anomalies induced in rats appear to demonstrate that cleft hand formation proceeds from osseous syndactyly and central polydactyly. The results of our experimental study show that the critical periods of central polydactyly, osseous syndactyly and cleft hand are the same. They also suggest that central polydactyly, syndactyly and cleft hand might be induced when the same teratogenic factor acts on embryos at the same developmental stage in the human being. Because they have a similar causation, cleft hand, syndactyly and central polydactyly should be classified into the same entity, that is, abnormal induction of digital rays. Based on these clinical and experimental studies, we modified the Swanson classification. In our modified classification, typical cleft hand, syndactyly and polydactyly are included in the same category of abnormal induction of digital rays as the fourth new category.

Clinical studies on peripheral hemodynamics in arterial hypertension.

Digital blood pressure and flow were measured in both normal subjects and patients with essential hypertension by means of the apparatus which have been recently devised, and peripheral vascular resistance was calculated according to Poiseuille's law in order to compare them. 1. Digital blood pressure is higher, the digital blood flow is less and the peripheral vascular resistance is stronger in patients with essential hypertension than in normotensives. 2. Hypertensive patients with ischemic heart disease, calcification of the aortic arch, left ventricular hypertrophy and heart size greater than 53% of the cardio-thoracic ratio were higher in the digital pressure, less in the digital flow, and stronger in the peripheral vascular resistance than those without these findings of the heart and the aortic arch. 3. The curve of relationship obtained from many cases of both normotensives and hypertensives between the digital blood flow and the peripheral vascular resistance forms a hyperbola. 4. Peripheral vascular resistance in both normotensives and hypertensives increased gradually with age up to 50-60 years, and decreased slightly thereafter. The resistance was stronger in patients with essential hypertension than in normotensives through all ages.

Digital ischemia in baseball players.

Eight baseball players developed digital ischemia as a result of repetitive ball impact. Symptoms and signs were coolness, numbness, cyanosis, paleness, and a positive reaction to the digital Allen's test. Seven of the eight players developed digital ischemia between the ages of 16 and 17. Angiograms of four patients with an occluded index digital artery are presented. Thermograms of four patients with a cool area of the left index finger are also presented. We investigated the incidence of digital ischemia by administration of a questionnaire. The respondents were 578 players belonging to clubs in junior high schools, high schools, and colleges. No digital ischemia was found in 207 junior high school baseball players. The incidence increased in high school (66 of 299) and college (29 of 72). The probability of developing digital ischemia corresponded to the accumulated playing time. Digital ischemia occurred characteristically in the left index finger.

Nonoperative treatment of osteochondritis dissecans of the humeral capitellum.

We examined 24 patients with osteochondritis dissecans of the humeral capitellum to determine the results of nonoperative treatment. The average age of the patients at the initial examination was 13.3 years (range, 11 to 16). All the patients were advised to stop heavy use of the elbow for 6 months. At the last examination, at a mean follow-up period of 5.2 years, 4 patients (17%) had no residual elbow pain, 7 (29%) had pain only with heavy activities, and 13 (54%) had pain with activities of daily living. Final radiographs were obtained for 15 lesions, of which 3 lesions were assessed as healed, 3 as improved, and 9 as not improved. Five of 11 lesions in the early stage and all 4 advanced lesions failed to show radiographic improvement. These results suggest that osteochondritis dissecans of the capitellum has only a slight tendency to heal, and that instability can cause failure of the lesion to heal.

Results of a non-controlled trial of hyperthermia combined with radiation for superficial tumours.

Fifty-four patients with 65 superficial malignant lesions were treated by local hyperthermia combined with radiation therapy at the National Cancer Center Hospital, Tokyo. Hyperthermia was delivered with an Aloka Model HMS-020 (2450 MHz) or with a horn-type applicator of BSD-1000 (80-90 MHz). Relatively small tumours, those less than 4 cm in thickness, were treated by using 2450 MHz while 80-90 MHz delivered through the horn-type applicator was used for tumours exceeding 5 cm in thickness. The radiation dose was 4 Gy twice a week or 2 Gy five times a week, the total dose being 40-60 Gy. A total of six to 10 hyperthermia treatments ranging from 40 to 60 min each, with the tumour heated to more than 42.5 degrees C, were given twice a week within 1 h following radiation therapy. Complete response was achieved in 16 of the 30 patients (55 per cent) treated with the 2450 MHz microwave, and partial response in seven others (23 per cent). Tumours treated with the BSD-1000 achieved complete response in 10 out of 33 patients (30 per cent) and partial response in nine others (27 per cent). In five out of nine patients classified as partial responders, however, complete disappearance of tumour cells was noted by post-treatment histological examination. Complete plus partial response rates were thus essentially the same with the Aloka HMS-020 and the BSD-1000, though the rate of complete response was apparently higher with the Aloka unit, probably because it was used on smaller tumours.

Characteristics of the response of soft tissue sarcoma to hyperthermia: the correlation between temperature distribution, radiological examination and histology.

Nineteen patients with soft tissue sarcoma were treated by a combination modality of hyperthermia and radiation or chemotherapy. There were 26 treatment sites. The size of the tumours ranged from 2.5 x 2 cm to 24 x 26 cm. Hyperthermic treatments were given twice a week, for a total of five to 14 sessions. Twenty-one tumours were treated by hyperthermia combined with radiotherapy, 2 Gy daily, five times a week, for a total of 40-78 Gy. Three tumours were treated by hyperthermia and arterial infusion of adriamycin, 100-120 mg in five or six treatments. For the superficial tumours the complete response rate was 40 per cent, and for the deep-seated tumours 6.2 per cent. Among the 12 tumours with no response, nine in which the treatment was evaluated as effective histologically (necrosis of the tumour) and by X-ray CT findings (development of a prominent hypodensity area) were included. Six cases were studied to correlate the X-ray CT findings, angiography and histological findings before and after hyperthermic treatment. The data were also used to interpret the thermal curve. The increased hypodensity area was roughly proportional to the development of necrosis, but there was one case in which hypodensity was not correlated with the necrosis. On the contrary, even in the contrast-enhanced area around the tumour in which the presence of residual tumour was strongly suspected clinically, no tumour cells were visualized. Clinical evaluation of the effect by size of the tumour can be supplemented by CT findings and histology, but should be cautiously adopted.

Reproducibility of short echo time proton magnetic resonance spectroscopy using point-resolved spatially localized spectroscopy sequence in normal human brains.

The reproducibility of short echo time proton MR spectroscopy (1H-MRS) in normal human brains was examined. Thirteen healthy volunteers were studied, and each underwent three MRS examinations. Second and third measurements were done on the same day, about two months after the first measurement, and interday and intraday reproducibility were evaluated. MRS was performed with proton brain examination/single voxel (PROBE/SV) and point-resolved spatially localized spectroscopy (PRESS) (repetition time = 2000 ms, echo time = 30 ms). Five metabolite ratios were computed; N-acetyl-aspartate (NAA)/creatine (Cr), choline (Cho)/Cr, myo-inositol (mI)/Cr, NAA/(NAA + Cr + Cho), and NAA/Cho. Their normal range and reproducibility were measured. For each metabolite ratio, there was no significant difference between interday difference and intraday difference, suggesting that the interval of two months has minimal effect on MRS measurements. MRS may be utilized for the observation of central nervous system diseases.

Intraoperative and conformal external-beam radiation therapy in patients with locally advanced pancreatic carcinoma; results from a feasibility phase II study.

BACKGROUND/AIMS: Chemoradiation therapy is widely indicated to patients with locally advanced pancreatic carcinoma, though the capability of radiotherapy alone is not assessed enough. The purpose of this study is to clarify the efficacy and safety of a more intensive radiotherapy for those patients. METHODOLOGY: Fifteen patients were enrolled in a feasible phase II study of treatment with intraoperative radiation therapy (25 Gy), followed by conformal external-beam radiation therapy (40 Gy in 20 fractions, 5 times/week). The antitumor effect was evaluated on the early phase of dynamic computed tomography image. RESULTS: The full irradiation dose was feasible in 12 of 15 patients. The overall response rate was 40% (1 complete and 5 partial responses). Grade 3 toxicity was observed in 2 patients (13%) with nausea/vomiting or anorexia. One patient developed gastric ulcer and died of gastrointestinal bleeding 12 months after intraoperative radiation therapy. The median survival time was 11.1 months, and the 2-year survival rates were 13%. Survival for more than 2 years was observed in 2 of the 6 responders. CONCLUSIONS: The above radiotherapy is considered to be active for the locally advanced pancreatic cancer with acceptable toxicity, when the gastrointestinal tract is excluded from the radiation field. This should be further assessed in late phase II studies involving a large number of patients.

Hepatic arterial infusion of 5-fluorouracil for liver metastases from pancreatic carcinoma: results from a pilot study.

BACKGROUND/AIMS: Liver metastasis is a common progression of pancreatic carcinoma, but an effective chemotherapy has not been established. The purpose of this study was to examine the efficacy and safety of a hepatic arterial infusion of 5-FU in patients with liver metastasis from pancreatic carcinoma. METHODOLOGY: Thirteen patients were enrolled in a pilot study of a hepatic arterial infusion of 5-FU therapy. They received 5-FU for 5 days at a dose of 500 mg/m2/day by continuous hepatic arterial infusion every 4 weeks. RESULTS: One patient showed a partial response, while 6 showed no change. Of these 6 patients, 2 showed a minor response. The overall response rate was 8% (95% confidence interval: 0-22%). Nausea and vomiting were the most common types of toxicity. Three patients (23%) had hepatic arterial occlusion. There were no life-threatening toxicities or complications. The overall median survival time was 15.9 weeks. CONCLUSIONS: Hepatic arterial infusion of 5-FU in patients with liver metastasis from pancreatic carcinoma is tolerable but is minimally effective at this dose and schedule. The schedule of administration should be modified.

Dynamic human glycogen and glucose metabolism detection utilizing in vivo 13C NMR.

The dynamic detection studies of glucose-glycogen metabolism of the human liver were performed with normal volunteers. A single tuned surface coil of 20-cm diameter was used for 13C (at 16.04 MHz). Natural glucose 100 g was administered for the natural abundance 13C MR dynamic study and 13C enriched (99%) 1-13C-D-glucose 1 g mixed 75 g glucose was administered to study the 13C enrichment effect. The total scan time of this dynamic study was 1 hr and each single scan time was 20 min. We did not use proton decoupling for the human 13C MR study. Localized (PRIME) proton shimming was performed and the shim value was less than 20 Hz. Glucose and glycogen spectra were clearly visible at 120 to 80 ppm and about 3 hr later both peaks almost disappeared. These data demonstrate that dynamic hepatic glucose-glycogen metabolism should be possible to detect which should allow for the direct diagnosis of hepatic glucose-glycogen metabolic disorders.

Local effect of hyperthermia for superficial and shallow-seated tumors.

In approximately seven years, 134 patients with 161 tumors were treated by hyperthermia combined with radiation or chemotherapy at our department. The primary tumors were breast cancer, head and neck cancer, and soft tissue tumors in most patients. Adenocarcinoma was the most frequent, followed by squamous cell carcinoma and soft tissue sarcoma. The local response rates for primary inoperable advanced, metastatic, and local recurrence of breast cancer were 88% (7/8), 50% (10/15), and 86% (18/21), respectively. The local response rate of 39 tumors of neck lymph nodes was 49% (19/39). A total of 26 tumors of bone and soft tissue were treated. Five tumors showed CR and six PR, for a total response rate of 42%. Among 20 patients with malignant melanoma, CR and PR were 25% (5/20) and 30% (6/20), respectively. The local response rate for all patients with superficial and shallow-seated tumors was 58% (94/161). In some tumors classified as showing NR, complete disappearance of tumor cells was demonstrated by a post-treatment histological examination. The efficacy of hyperthermia, when evaluated solely on the basis of tumor size, is likely to be underestimated.

Computed simulationgraphy: concept and method of clinical application.

The beam's eye view is one of the methods for transferring a patient's computed tomography information to the actual treatment position. We have developed a system for producing beam's eye projections through axial computed tomography data. The treatment field boundary, whose display is applicable to the multi-leaf collimator system, is superimposed upon the image. Data for the treatment set-up and coordinate transfer are also displayed on the image. Conventional X-ray simulators have fallen into disuse for marking the treatment field on the patient's skin. The process and the image completely take the place of simulation and the simulation film made by a conventional X-ray simulator; consequently, we have named the process computed simulationgraphy.

Classification of the 16,471 patients treated by radiation therapy from 1962 to 1986, and crude five-year survival rates of patients in each category of radiotherapy treatment from 1962 to 1978 in the National Cancer Center Hospital.

The summation and classification of patients registered for planned radiation therapy in the 24-year period from 1962 to 1986, in the Department of Radiation Therapy, National Cancer Center Hospital, are reported. Patients numbered 16,471, and total sessions of treatment were 26,175. The ratio of the two figures, 1.58, is the average number of treatment sessions per patient. Peak age was 61-65 years' old and 56-60 years old, respectively for males and females. The most frequent primary site of disease for radiation therapy was the head and neck, followed by trachea, bronchus and lung, crevix uteri, breast, and esophagus. Frequency of squamous cell carcinoma was 38.0%; that of adenocarcinoma was 20.9%. Radical treatment was performed in 32.0% of patients in the first session, but this figure decreased to 24.7% for all sessions. There were many cases of secondary and primary palliative treatment, i.e., 31.8%. Radiotherapy was done in 70% of patients by megavoltage X-ray, with 9.4% treated by electron beam. Crude 5-year survival rates for each classification of malignant disease respective to the category of treatment policy from 1962 to 1978 were obtained. Among the radical treatment group, head and neck tumors, skin cancer, and Hodgkin's disease showed 5-year survival rates greater than 50%.
