[Clinical use of combined radiation and hyperthermia therapy at the National Cancer Center Hospital].

At the National Cancer Center Hospital, Tokyo, hyperthermia combined with radiation was started in February 1982. At first BSD-1000 was introduced into clinical practice, followed by Thermotron RF-8 which utilizes radio frequency and then Aloka HMS-020 which utilizes microwaves. This study involved a total of 116 evaluable patients (128 lesions) from a series of patients treated by hyperthermia during a 4-year period up to March 1986. The sites of the tumors were the lower extremities in 7 patients, upper abdomen in 18, pelvic cavity in 18 patients and superficial in 82. Among 82 superficial lesions 34 were classified as showing CR and another 23 as showing PR, with a response rate of 69.5%. In 7 patients with tumors of the lower extremities none was classified as having CR or PR, the response rate being 0%. Eighteen upper abdominal tumors showed a response rate of 16.7%, with 1 of them being categorized as showing CR and 2 as showing PR. Eighteen pelvic tumors showed a response rate of 27.8%, with 1 of them being categorized as showing CR and 4 as showing PR. In 3 patients with tumors of thoracic cavity none was classified as having CR or PR, the response rate being 0%.

[A chronological study on gastric cancer originating from the fundus gland mucosa].

A chronological analysis of gastric cancer originating from fundic gland mucosa was made by measuring the tumor size with reference to the serosal invasion and peritoneal dissemination. As for serosal invasion by cancer cells there were significant differences in the group with tumors exceeding 4 cm in diameter, and peritoneal dissemination was liable to be seen in the group with those exceeding 5 cm. This suggests that gastric cancer originating from fundic gastric mucosa must be detected before it reaches 4 cm in diameter and that the intraperitoneal administration chemotherapeutic agents is recommendable, considering residual microcancer dissemination.

Para-aortic nodal irradiation in the treatment of carcinoma of the uterine cervix: a report of 22 cases.

Twenty-two patients with previously untreated carcinoma of the uterine cervix were treated by para-aortic nodal irradiation for the macroscopical metastases found at the operation with or without pathological confirmation, at the National Cancer Center Hospital from 1962 to 1980. The irradiation was given with 6 MeV X rays. Total doses ranged from 42 to 99 with an average of 69 in Time Dose Fractionation factors. All of the patients died of the tumor within 42 months. The initial site of relapse was intrapelvic for two patients, extrapelvic for 18, and both intra- and extrapelvic for two. Of 12 patients upon whom autopsies were performed, only one of the patients who had multiple metastatic tumors of less than 1 cm in maximum diameter had the tumor controlled in the irradiated region.

Postnatal sudanophilic leukodystrophy in two siblings.

Reported here are two siblings with sudanophilic leukodystrophy occurring in early infancy. Soon after birth, high fever, distension of the abdomen and spasticity of the extremities were noted. Neurologic disorders, frequent convulsive seizures, and malnutrition caused both siblings to die at the age of 3 and 6 months, respectively. Neuropathologic examination of the brain revealed diffuse demyelination in the cerebral and cerebellar white matter as well as a disturbance of the myelination, especially in the descending pathway. Numerous sudanophilic lipids and glial fibers proliferated in the demyelinated areas. Neurochemically, there was a marked decrease in all lipid components, cholesterol, phospholipids, and glycolipids and an increase in cholesterol ester. Fatty-acid composition of cerebroside and sulfatide, especially that of long-chain acids, was within normal range, unlike the fatty-acid composition found in connatal Pelizaeus-Merzbacher disease. The brain damage in postnatal infant caused by frequent convulsive seizures and the severe demyelination in the cerebral and cerebellar white matter in both patients are discussed. The cases of sudanophilic leukodystrophy and Pelizaeus-Merzbacher disease occurring in children under 5 years of age are reviewed.

23Na and 39K NMR studies of ion transport in human erythrocytes.

Ion transport in human erythrocytes was studied by 23Na and 39K NMR with an anionic paramagnetic shift reagent, Dy(P3O10)2(7-). The intra- and extracellular 23Na and 39K NMR signals were well separated (over 10 ppm) at 5 mM concentration of the shift reagent. The NMR visibility of the intracellular Na+ and K+ was determined to be 100% in human and duck erythrocytes. The intracellular ion concentrations were 8.1 +/- 0.8 mM Na+ (n = 7) and 110 +/- 12 mM K+ (n = 4) for fresh human erythrocytes. The ouabain-sensitive net Na+ efflux was 1.75 +/- 0.08 mmol/hr per liter of cells at 37 degrees C (n = 3). The gramicidin-induced ion transport in human erythrocytes was also studied by 23Na and 39K NMR or by simultaneous measurements of 23Na NMR and a K+-selective electrode. The time courses of the Na+ and K+ transport induced by the ionophore were biphasic. The initial rapid fluxes were due to an exchange of Na+ for K+, which were found to occur with a 1:1 stoichiometry. The subsequent slow components were the net Na+ and K+ effluxes rate-limited by the Cl- permeability and accompanied by a reduction in cell volume. The Cl- permeability determined from the NMR measurements of these slow fluxes was 3.2 +/- 0.5 X 10(-8) cm/sec at 25 degrees C (n = 4).

Immunochemical characterization of pyrimidine kinase induced by varicella-zoster virus.

Thymidine kinase (TK) induced by varicella-zoster virus (VZV) was precipitated with ammonium sulphate and purified by Sephadex G-150, QAE-Sephadex and Blue Sepharose column chromatographies. The purified TK fraction also contained deoxycytidine kinase (dCK) activity and a 35000 mol. wt. (35K) polypeptide as a major component. The TK and dCK activities were both neutralized by anti-VZV serum. Antiserum to an extract of cells infected with a bromodeoxyuridine (BUdR)-resistant mutant virus contained no antibody to the viral TK and dCK activities or to the 35K polypeptide. Antiserum to the purified viral TK fraction was prepared and absorbed with a lysate of BUdR-resistant mutant virus-infected cells. The resulting absorbed antiserum (anti-vTK serum) neutralized the viral activities of both TK and dCK, and specifically immunoprecipitated a 35K polypeptide from the lysate of parental virus-infected cells, but did not immunoprecipitate any detectable polypeptide from cells infected with BUdR-resistant mutant virus. Anti-vTK serum stained mainly the nuclei of cells infected with the parental virus strain, but did not stain those infected with BUdR-resistant mutant virus by an indirect fluorescent antibody test. These results suggest that the 35K polypeptide produced in VZV-infected cells is responsible for the viral TK and dCK activities, and that the TK and dCK are mainly localized in the nuclei of infected cells.

Complex partial seizures in childhood.

A clinical and electroencephalographic study was undertaken on 215 children with complex partial seizures as selected on the basis of the International Classification of Epileptic Seizures (1981). Complex partial seizures were noted in 7.8% of the epileptic children. The ictal symptoms of complex partial seizures closely resembled those of psychomotor triad described by Lennox. Interictal EEG revealed seizure discharges from the temporal or frontal focus in 57.2%. There existed a group with automatism as a main symptom having both diffuse slow spike-waves and focal temporal spikes. This type should be interpreted to be one of secondary generalized epilepsies and be a variant of the Lennox syndrome.

NMR measurements of intracellular sodium in the rabbit proximal tubule.

The present study evaluated the use of nuclear magnetic resonance (NMR) spectroscopy to monitor directly and continuously intracellular sodium levels in rabbit renal cortical tubule suspensions. When the paramagnetic shift reagent dysprosium tripolyphosphate was added to the extracellular medium it was possible to resolve signals from intracellular and extracellular sodium without adversely affecting cellular viability. An efflux of intracellular sodium against a significant concentration gradient was observed when sodium-loaded cells were warmed from 4 to 37 degrees C. At 37 degrees C in steady state, inhibition of Na+-K+-ATPase activity by ouabain increased intracellular sodium content in a dose-dependent and time-dependent manner. A biphasic time course of increased intracellular sodium following ouabain (10(-3) M) suggested that the sodium permeability of the plasma membrane may decrease following pump inhibition, thus limiting sodium influx. Nystatin, an agent known to facilitate sodium entry across cell membranes, increased intracellular sodium fivefold. In another series of experiments several maneuvers were performed to ascertain the fraction of intracellular sodium that was NMR visible. Quantitative assessment of either an efflux or influx of sodium indicated that the NMR visibility of the transported sodium was 100%. Furthermore, disruption of the cell membranes with Triton X-100 showed that the entire pool of intracellular sodium was 100% NMR visible.(ABSTRACT TRUNCATED AT 250 WORDS)

[Flow cytometric dual-parameter analysis of human leukemic cells using monoclonal anti-BrdUrd antibody].

A more precise analysis of cell kinetics can be made the simultaneous flow cytometric measurement of cellular DNA content and amount of incorporated BrdUrd, using propidium iodide and a monoclonal antibody to BrdUrd. We applied this technique to the cell kinetic analysis of leukemic cells obtained from a 62-year-old woman with AML. A computer analysis of a DNA distribution histogram provided the percentage of cells in each phase of the cell cycle, i.e., G1: 73.3%, S: 19.6%, and G2M: 7.1%. In contrast, 12.7% of the total cell population incorporated BrdUrd, but more active DNA synthesizing cells comprised only 3%. Some fractions of cells that have DNA content corresponding to that of S phase cells incorporated little BrdUrd. Most of the S phase cells were in the early or late S phase, and their DNA synthesis rate was very low.

[Evaluation of the measurement of serum carcinoembryonic antigen (CEA) levels using monoclonal antibody].

We measured serum carcinoembryonic antigen (CEA) levels in 164 cancer patients, 153 patients with benign diseases and 45 healthy controls using monoclonal antibody and compared CEA levels by monoclonal antibody (m-CEA) with those by polyclonal antibody (p-CEA). There was a good correlation between m-CEA and p-CEA, especially in cancer patients. The positively of m-CEA was almost the same as that of p-CEA in cancer patients. But, false-positive cases by m-CEA were less common than by p-CEA in non-cancerous patients. Thus, the measurement of m-CEA was not less useful than that of p-CEA.

Cubitus varus deformity following supracondylar fracture of the humerus. A method for measuring rotational deformity.

Cubitus varus following supracondylar fracture of the humerus in children consists of varus, hyperextension, and internal rotation deformities of the distal bone fragment of the humerus. There has been no simple clinical method of measuring internal rotation deformity. The authors report here on a method of accurately measuring the internal rotation of the shoulder in a position with the elbow at 90 degrees flexion on the back and the shoulder held at the maximum extension. When a patient with developing cubitus varus attempts to rotate his shoulder internally, there is an apparent increase in the degree of internal rotation at the shoulder of the fractured side. The mean value of the angle formed between the horizontal plane of the back and the mid-line of the forearm was approximately 0 degrees in 217 normal children, whereas an increase of 35 degrees was seen in seven children with cubitus varus. When this angle was corrected successfully, the angle after surgical correction became approximately 0 degrees. In the treatment of cubitus varus deformity, it is necessary to simultaneously correct three-dimensional deformities.

Delayed rupture of a flexor tendon secondary to fracture of the lunate.

Attritional ruptures of flexor tendons following carpal fractures are rare. The authors report a case of delayed rupture of flexor digitorum profundus of the index finger caused by fracture of the lunate. To our knowledge, this is the first report of such a tendon rupture following a fracture of the lunate.

[The metastatic patterns of gastric cancer from its histopathological characteristics in the primary lesion--particularly in relation to peritoneal dissemination and liver metastases].

The purpose of this study was to catagorize the metastatic patterns of advanced gastric cancer into liver and peritoneal metastases from the stromal behavior of the primary lesion in 278 patients. According to our data, peritoneal disseminations were encountered 5.0% of the medullary type, 30.0% of the intermediate type and 38.0% of the scirrhous type, while the corresponding figures for liver metastases were 28.8%, 6.0% and 1.9%, respectively. The differences among the three types were significant with both patterns of metastasis. From the present data, we assume that the histological pattern in the stroma of primary gastric cancer shows the biological behavior of cancer growth and metastatic patterns, particularly in close relation to peritoneal dissemination and liver metastases.
