[Hepatic arterial infusion chemotherapy using implantable reservoir in colorectal liver metastasis].

Hepatic arterial infusion chemotherapy was performed in 47 cases with colorectal liver metastasis. Methods of hepatic arterial cannulation were as follows: operative insertion of polyvinyl chloride catheter in 12 cases, angiographic insertion of polyethylene catheter via the left high brachial artery in 20 cases and operative implantation of reservoir in 15 cases. Mean durations of chemotherapy were 31 days, 25 days and 115 days, respectively. Chemotherapies were discontinued because of complications and catheter troubles in 7 cases (58%), 8 cases (40%) and 6 cases (40%), respectively. Regression of tumor size was observed only in the group with reservoir. Prolonged therapy is achievable by use of the reservoir. But catheter troubles, including obstruction, infection, extravasation and pseudoaneurysm, are still observed in the course of chemotherapy. Radionuclide study using 99m Tc macroaggregated albumin is useful to detect catheter troubles. We stressed that the proper management of reservoirs is important.

[Assessment of infarct-related coronary arteries using the bull's eye view method and unfolded surface mapping in thallium-201 myocardial tomography].

Infarct-related coronary lesions and collaterals were assessed by the bull's eye view and the unfolded surface map derived from thallium-201 myocardial tomography (Tl-201 SPECT) in 25 patients with anterior myocardial infarction. The patients were categorized in six groups according to their cine-angiographic findings: locations of stenosis (proximal or distal portions of the first diagonal branch or distal site of the first septal branch), and the presence or absence of collaterals. The anterior half of the left ventricle on the bull's eye view map was divided into eight regions from the anterior septum to the lateral wall and from the apex to the cardiac base. Tl-201 SPECT images were expressed as functional maps using maximum-count circumferential profile analysis: an "extent image" showing the extent of perfusion defect, and a "severity image" demonstrating the degree of hypoperfusion. The following results were obtained: 1. Perfusion defect of the "extent image" at the basal portion of the anterior septum reflected poor collaterals, with a sensitivity of 81.3%, a specificity of 77.8%, and a diagnostic accuracy of 80.0%. 2. Coronary stenosis at segment 6 (AHA classification) was differentiated from that at segment 7 by detecting hypoperfusion at the basal septum on the "severity image", with a sensitivity of 71.4%, a specificity of 77.8%, and a diagnostic accuracy of 75.0%. 3. Perfusion defect at the basal section of the proximal portion of the anterior wall on the "extent image" indicated stenosis at the proximal segment 6, with a sensitivity of 57.1% and a specificity of 88.9%.(ABSTRACT TRUNCATED AT 250 WORDS)

A case of pseudothrombophlebitis due to inguinal synovial cyst.

A case of pseudothrombophlebitis of the whole right leg by an inguinal synovial cyst that had a compressed femoral vein is presented. Differential diagnosis is important in order to avoid hemorrhagic complications. The usefulness of ultrasonography and necessity of phlebography are emphasized. Radical excision of the cyst is recommended.

Mosaicism in the expression of tumor-associated carbohydrate antigens in human colonic and gastric cancers.

Serial sequential sections from a single tumor were examined by immunohistological staining with several monoclonal antibodies directed, respectively, to different tumor-associated carbohydrate epitopes. Staining patterns were compared with those of conventional staining with hematoxylin-eosin or periodate/Schiff's reagent. Each tumor showed different areas of staining with different antibodies, and the combined staining map shows a clear mosaicism of antigen expression within the same tumor. For example, some areas of a given tumor were stained by FH4 (defining dimeric Le(x)), while other complementary areas were strongly stained, in a mutually exclusive manner, by SH1 (defining Le(x)), AH6 (defining Le(y)), FH6 (defining sialosyl dimeric (Le(x)), or TKH2 (defining sialosyl-Tn). Some areas were stained by two or three of these antibodies. Comparisons of the mosaic-staining patterns with cytohistological properties of tumor cells within specific areas suggested that the pattern of antigen expression is correlated with degree of differentiation; e.g., poorly-differentiated cells with severe dysplasia did not express high levels of Le(x) or Le(y) but did express sialyl-Le(x) or dimeric Le(x); on the other hand, moderately or well-differentiated tumor cells in some areas expressed high levels of Le(x) or Le(y) but lower levels of sialyl-Le(x). Areas showing strong expression of sialyl-Tn in their secretions were consistently correlated with presence of well-differentiated tumor cells, whereas secretions from normal mucosae were consistently characterized by lack of sialyl-Tn expression. It is postulated that the original in situ tumors (which had homogeneous glycosylation patterns) evolved into several spatially discrete cell populations displaying different degrees of glycosylation, reflecting stages of tumor cell differentiation and progression.

A new compact and cell dense continuous culture system.

A new compact and cell dense continuous culture system was developed. The system consisted of a culture bag, which had two compartments separated by a semipermeable membrane, and an external rotator. Cells were cultured in the inner compartment with medium containing serum or the necessary growth factors. The outer compartment was filled with medium without serum or growth factors. The concentration of cells in culture was normally in the order of 10(7). Due to the large number of cells, sufficient carbon dioxide was produced so that use of a CO2 incubator was unnecessary. The medium in the outer part was frequently changed, but no medium changes were necessary in the inner bag. Using this system, a concentrated yield of antibody as well as a large number of cells could be obtained.

A new high-yield continuous cell-culture system for lymphokine-activated killer cells.

We performed basic studies on a new high-yield culture system (concentrate rotary tissue-culture system) for application to adoptive immunotherapy with lymphokine-activated killer (LAK) cells. Using this system, we demonstrated that up to 2 x 10(7) peripheral blood mononuclear cells/ml could be cultured in interleukin-2 with a sufficient recovery rate and cytotoxicity in short-term cultures (6 days). This system can also be used to proliferate LAK cells to four times the initial cell number with sufficient cytotoxicity for 14 days of culture. Thus, this system allows activation of sufficient numbers of cells to conduct clinical trials on humans.

Quantification of area and percentage of infarcted myocardium by single photon emission computed tomography with thallium-201: a comparison with serial serum CK-MB measurements.

In order to quantify the size of the infarcted myocardium, two kinds of data processing techniques were applied to single photon emission computed tomography (SPECT) with thallium-201 and its clinical reliability was evaluated by comparing it with the infarct sizing procedure with the serial serum creatine kinase-MB measurements in 14 patients with acute myocardial infarction. After maximum-count circumferential profile analysis, short axis images were reformatted into an unfolded surface map and a bull's eye view map. The SPECT-determined infarct size was defined as the area or the percentage of hypoperfused myocardium of which the profile count was less than the mean minus 2SD derived from 8 normal subjects. The infarct area was calculated from the number of pixels with an abnormal count and expressed in an unfolded surface map. The percentage was calculated from the number of abnormal profile points and displayed in a bull's eye view map. A high linear correlation was observed between the enzymatically determined infarct size and the infarct area or the percentage (r = .947, r = .872, respectively), despite underestimations in 2 patients with accompanying right ventricular infarction and overestimations in 2 patients with prior anterior infarction. Moreover, a close negative correlation was found between the left ventricular ejection fraction and the infarct area or the percentage (r = .836, r = .821, respectively). Thus, the semiautomatic techniques for processing thallium-201 SPECT images might contribute to the quantitative estimation and display of infarcted myocardium and have high clinical reliability.

Availability of 111In-labeled platelet scintigraphy in patients with postinfarction left ventricular aneurysm.

Eighteen patients with postinfarction left ventricular aneurysms (LVAs) were examined with Indium-111-labeled autologous platelet scintigraphy to identify intracardiac thrombi and to investigate the effect of antithrombotic agents on thrombogenesity within their LVAs. Left ventriculography (LVG), and two-dimensional echocardiography were also carried out to assess the diagnostic ability of the platelet imaging. Indium-111-platelet scintigraphy for the detection of LVA mural thrombi had a sensitivity of 60% and a specificity of 100%. Four of six patients with false-negative scintigraphic studies had been under antiplatelet therapy. Eight of the nine patients who had showed active platelet deposition on initial examination had not received antiplatelet therapy. Thereafter, five of these nine were treated with tichlopidine (300 mg/day) for 29.8 +/- 5.0 days. On the second platelet study, two had resolution and the other three had interruption of intra-aneurysmal deposition, which remained positive. In only one patient of the three, the third platelet study was performed after warfarin therapy. It took two weeks after beginning the therapy to completely interrupt platelet deposition within the LVA in this patient. ECG gated radionuclide ventriculography and Thallium-201-myocardial scintigraphy were also performed to assess left ventricular wall motion of left ventricular ejection fraction (LVEF) and myocardial blood perfusion. Thallium-201-SPECT showed apical or anteroapical perfusion defects and the radionuclide ventriculography correctly identified all 18 apical and anteroseptal aneurysms which were confirmed by LVG methods. The comparison between the thrombus positive group and the thrombus negative group was carried out on both the LVEF and the period from the last myocardial infarction to the initial platelet scanning study. There were no statistical differences in the LVEF and the interval (34.5 +/- 12.5% vs 37.3 +/- 14.6%, 39.6 +/- 52.6 days vs 89.6 +/- 108.3 days) between the two groups. These results suggest that Indium-111-labeled platelet scintigraphy can be a reliable method for the identification of active left ventricular mural thrombi and a practical method of judging antiplatelet and anticoagulant therapy.

Effects of prostaglandin E2 and D2 on the release of vasopressin and oxytocin.

The effects of prostaglandin (PG) E2 and D2 on the plasma levels of arginine-vasopressin (AVP) and oxytocin (OXT) in rabbits, and on the release of the both hormones from the isolated posterior pituitary of rats were examined. An intraventricular administration of PGE2 to a rabbit raised the plasma levels of the both hormones. An intraventricular injection of PGD2 also increased the plasma level of OXT but not that of AVP. The release of AVP and OXT from fragments of the posterior pituitary of a rat was not influenced by perfusion with Eagle MEM medium containing 10(-6) or 10(-5) M PGE2 and D2.

Human superoxide dismutase failed to limit the size of myocardial infarct after 20-, 30-, or 60-minute ischemia and 72-hour reperfusion in the rabbit.

The effect of superoxide dismutase (SOD) on the size of the myocardial infarct resulting from various durations of ischemia and a 72-hour reperfusion was examined in the rabbit. A coronary branch of the circumflex artery was occluded for 20, 30, or 60 min and then reperfused. Seventy-two hours after the coronary occlusion, the infarct size and the size of the area at risk (vascular bed of occluded coronary artery) were determined by histology (hematoxylin-eosin and Mallory's staining) and by fluorescent particles, respectively. Human SOD (45,000 units/kg) was injected intravenously as a bolus in SOD-treated rabbits, while only saline was administered to control rabbits. The percentage of the area at risk which actually infarcted (%I/AAR) was 25.5 +/- 12.9% (mean +/- S.D.) in the 20-min ischemia control group (n = 9), 19.7 +/- 10.2% in the 20-min ischemia SOD group (n = 9), 44.8 +/- 9.0% in the 30-min ischemia control group (n = 9), 41.0 +/- 6.3% in the 30-min ischemia SOD group (n = 9), 74.2 +/- 13.8% in the 60-min ischemia control group (n = 9), and 76.6 +/- 8.2% in the 60-min ischemia SOD group (n = 7). The %I/AAR was not significantly different between the control and SOD groups for any duration of ischemia. Heart rate, blood pressure, and the size of area at risk were comparable in all six groups. These findings suggested that oxygen-free radicals produced during initial moments of reperfusion were unlikely to contribute to myocardial necrosis regardless of the duration of ischemia in the rabbit.

Progression of myocardial infarction in a collateral flow deficient species.

The effect of a varying period of ischemia on the development of myocardial infarction was investigated in the rabbit. Radiomicrosphere measurements confirmed that the collateral blood flow is almost zero (0.02 +/- 0.01 ml/min/g) and without a significant transmural gradient in the rabbit heart (n = 15). A coronary branch of the left circumflex artery was occluded for 5, 10, 15, 30 or 60 min and then reperfused. The coronary branch was occluded permanently in another group of rabbits. Three days after the coronary occlusion, the infarct size was determined by hematoxylin-eosin and Mallory's staining and the ischemic zone size was determined by fluorescent particles. The results showed that the percentage of the ischemic zone infarcted (% infarction) vs the log of duration of ischemia yielded a sigmoid curve which could be linearized by probit analysis: Probits of % infarction = 3.05 x log (ischemic duration in minutes) + 0.33, r = 0.83, p less than 0.01. The regression equation indicated that 50% of the ischemic myocytes necrotized after 34 min of coronary artery occlusion. Unlike in the dog heart, the infarct of the rabbit heart first appears in the midmyocardium and then progresses towards both the endocardium and epicardium.

[Autologous bone marrow transplantation as a measure against myelosuppression in cancer chemotherapy].

One hundred seventy-three patients were treated 256 times with chemotherapy supported by autologous bone marrow transplantation during the past 9 years. The two most commonly used protocols were (cyclophosphamide 1,600-2,400 mg/m2 + adriamycin 80 mg/m2 + ACNU 3 mg/kg) and (cyclophosphamide 1,600-2,400 mg/m2 + adriamycin 100 mg/m2 + CDDP 100-150 mg/m2). Among 115 patients in the therapeutic setting in which two courses were usually given, 75 were evaluable and the overall response rate was 42.7% with 12.0% CR rate. Breast and pediatric groups responded well; the response rate in breast cancer was 67.9% with 21.4% CR rate. Two patients with breast cancer who had multiple distant metastases and 2 pediatric patients are now alive with NED after 5 years of the treatment and seem to have been cured. The results in adjuvant settings have also been quite promising, e.g., 79.4% 5-year survival probability among breast patients mainly in stage III. Although drops of blood cell counts to the nadirs (WBC counts: less than 100-300) could not be prevented, the periods of myelosuppression appeared to have been effectively shortened so that the patients could be safely managed with intensified general supportive measures. Platelet counts are usually less affected, but the recovery is slower than for WBC counts. There were 13 patients who died within 10 weeks of the initiation of the treatment. Two of them succumbed to sepsis, and progressive disease was the cause of death in 8 patients whose terminal phases were undoubtedly affected by some infectious problems. We have shown that there are inverse relationships between infused numbers of CFU-GM and marrow recovery judged by the duration of neutropenia and the time required for neutrophils to recover over 500. Our recent laboratory experiments testing CFU-E, BFU-E and CFU-Mk in addition to MNC counts and CFU-GM showed that vulnerabilities of marrow progenitors seem to differ from cell lineage to cell lineage. This must therefore be taken into careful consideration in pursuing marrow transplantation.

[A new high-yield continuous cell culture system and its clinical application in adoptive immunotherapy with LAK cells].

We developed a high-yield culture system, consisting of a culture bag on a rotator. The culture bag has two compartments, an inner compartment separated from an outer compartment by a semipermeable membrane. Cells in an appropriate culture medium are placed in the inner compartment, and medium without serum is placed in the outer compartment. The bag is rotated at an angle of 45 degrees between 0.5 and 5 rpm in a 37 degrees C incubator. The medium is changed in the outer compartment at intervals when needed. The outer compartment acts as the feeder as well as allows catabolites to diffuse through the culture. In addition, since the system is a closed-system, sterile conditions can easily be maintained. Using this system, we demonstrated that up to 20 X 10(6) cells/ml of PBMC could be cultured in IL-2 with sufficient recovery rate and cytotoxicity.

[A case of right aortic arch with the stenotic left subclavian artery at its origin complaining of left nuchal pain].

A 25 year-old male was transfered to our department with a complaint of left nuchal pain. Angiography revealed a right aortic arch with the stenotic left subclavian artery at its origin and poor left vertebral artery. The anomaly was classified an intermediate type of Stewart type (2) and (3). It was very rare and we have found only 3 same cases in literature. The complaint disappeared after the ascending aorto-left subclavian artery bypass.

[A case of WPW syndrome with slow Kent documented by ATP injection].

A 46-year-old woman with chest tightness and palpitation at exercise was admitted to Sapporo Medical College Hospital for the evaluation of the ST-T changes on stress electrocardiogram. In this patient, PQ time was 0.14 second and pre-exitation was not clearly documented on electrocardiogram at rest. Bolus injection of 10 mg of adenosine-5'-triphosphate (ATP) demonstrated deltawave through the elongation of antegrade conduction of atrio-ventricular (AV) node. Electrophysiological study also showed left lateral accessory pathway with slow antegrade conduction, slow Kent. Stress 201-T1 myocardial scintigraphy using bicycle ergometer did not show the existence of ischemic region in spite of the ST-T changes on electrocardiogram. In this case, it seemed that a false positive ST-T changes might be caused by ventricular pre-exitation through slow Kent fiber. From these findings, it was suggested that the transient interruption on conduction through AV node by ATP bolus injection may be a useful diagnostic method in borderline pre-exitation syndrome.

[A rare case of left atrial thrombus after left upper pulmonary lobectomy].

A 75-year-old man was operated for left atrial thrombus. He had received left upper pulmonary lobectomy for epidermoid carcinoma of the lung, 6 months previously. Thrombus, 5.2 X 1.5 X 0.8 cm in size, was grown up from the inside of the left upper pulmonary vein into the left atrial cavity. It was removed and left upper pulmonary vein orifice was sutured and closed. There was no abnormal findings in the endocardium of the left atrium and mitral valve. A case of the left atrial thrombus after the pulmonary lobectomy is very rare and this may be the first report in world literature.

Formation of gamma-amino-beta-hydroxybutyric acid from 2-hydroxyputrescine in rat brain.

gamma-Amino-beta-[3H]hydroxybutyric acid ([3H]-GABOB) was formed in rat brain from 2-[3H]-hydroxyputrescine that had been chemically synthesized from 2-oxoputrescine and [3H]sodium borohydride. After the injection of 2-[3H]hydroxyputrescine into the lateral ventricle of a rat brain, the rat was killed and then the brain was removed. [3H]GABOB in the brain was identified by a combination of ion-exchange chromatography, high-voltage paper electrophoresis, and recrystallization of the radioactive compound with authentic GABOB.

Assay of 2-hydroxyputrescine in various regions of rat brain by gas chromatography-mass spectrometry.

2-Hydroxyputrescine in seven regions of single rat brains was measured with a sensitive, specific assay by gas chromatography-mass spectrometry. The regions were the cerebral cortex, cerebellum, medulla oblongata, hypothalamus, striatum, hippocampus, and midbrain. The level of 2-hydroxyputrescine was very high in the cerebral cortex and cerebellum, high in the medulla oblongata, hypothalamus, and hippocampus, and low in the striatum and midbrain. The level of 2-hydroxyputrescine in the cerebellum was significantly higher than in the striatum and midbrain.
