[Ultrastructure of glioma vessel--morphometric study for vascular permeability].

In this study we investigated the ultrastructure of human glioma capillaries in operated sample of low grade astrocytomas and malignant gliomas. Electronmicrographs of a total of 58 vessels were analyzed with computer assisted morphometry for ultrastructural evidence of permeability routes. All of these vessels were present in the marginal area of tumors devoid of necrosis, less than 10 micron in diameter and containing one nucleus at least on axial section. We found that: (1) The number of pinocytic vesicles was significantly higher in capillaries from malignant glioma (an average of 8.1 per 1 micron (2) cytoplasm) than those from low grade astrocytoma (an average of 4.0 per 1 micron (2) cytoplasm). In capillaries from malignant glioma, most of the pinocytic vesicles were arranged in the abluminal side of endothelium and some of them were fused each other. (2) Abnormal endothelial intercellular junctions which were defined as short tight junctions (less than or equal to 0.25 microns) were equally but infrequently seen in low grade astrocytomas and malignant gliomas. (3) Fenestrations in the endothelium were not seen. Therefore we suggest that the high vascular permeability and resultant brain edema in malignant gliomas is likely to increased pinocytic vesicles and rare but abnormal inter endothelial junctions.

The effect of radiation therapy combined with natural killer cells against spontaneous murine fibrosarcoma.

The effect of radiation therapy combined with lymphoid cells against spontaneous murine fibrosarcoma (FSa-II) was investigated both in vivo and in vitro. In the in vivo experiment, syngeneic C3H mice were divided into 3 groups. Animals in the first group were injected with 1 x 10(5) tumor cells into the right hind leg. Animals in the second and third groups were injected with 1 x 10(5) tumor cells mixed with 1 x 10(7) normal lymphoid cells (NLC) or effector lymphoid cells (ELC), respectively. ELC were obtained from spleen and lymph nodes of FSa-II-bearing mice and incubated in vitro for 40 hr to eliminate suppressor T cell function. NLC were obtained from normal mice and incubated in the same way. Irradiation was given using 137Cs unit 3 days after cell inoculation. 12 out of 14 mice (85.7%) inoculated with tumor cells mixed with NLC did not show any tumor growth at 60 Gy local irradiation. 12 out of 21 mice (57.1%) inoculated with tumor cells alone and 6 out of 10 (60%) with tumor cells mixed with ELC rejected tumors at the same radiation dose. This synergistic effect with NLC was not observed when NLC was inoculated after irradiation, indicating that lymphoid cells should be in contact with tumor cells before irradiation. In the 51Cr release assay, lymphoid cells obtained from whole body irradiated (WBI) mice showed 17.8% lysis without irradiation and 28.8% lysis at 5 Gy irradiation. Untreated NLC showed almost no cytotoxic effect at the same radiation dose. This synergistic effect disappeared when WBI lymphoid cells were treated with anti asialo GM1 and complement.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of cultural conditions on hexavalent chromium uptake and the cytotoxicity thereof in KB cell culture].

To reveal the effects of cultural conditions on the cytotoxicity of hexavalent chromium, the uptake of sodium chromate (Na2CrO4) by KB cells and the colony-forming efficiency of the cells were examined under various cultural conditions. The results were summarized as follows: 1) The chromium uptake by the cells after a certain period of incubation with hexavalent chromium was inhibited with the decrease of the temperature (3 degrees, 20 degrees, 37 degrees C), increase of the serum concentration (0, 10, 20, 30%) and increase of pH (6.8-8.2) of the medium. In particular, low temperatures inhibited the chromium uptake by the cells remarkably. However, in relation to the serum addition, no marked effect was found. 2) The chromium uptake by the cells increased with the volume of the medium containing an identical concentration of chromium (2 ppm) and then reached saturation when it was about 0.23 microgram per 10(6) cells. On the other hand, the chromium uptake positively correlated with the concentration of chromium and the total chromium in the medium. 3) The difference of chromium uptake by the cells in different culture media was more marked at acidic pH than that at alkaline pH. However, there was no effect of calcium chloride and glucose concentrations on the uptake of chromium. The chromium uptake by the cells in Ca-Mg-free phosphate-buffered solution (PBS(-] was higher than that in other culture media. Consequently, the above results suggested that the chromium uptake by the KB cells might be affected by the various cultural conditions, especially by temperature, pH and medium volume.

[Zinc and copper concentrations in breast milk and maternal serum in the postpartum period].

This study reports the contents of Zn and Cu in the breast milk and serum of postpartum mothers, 17 primiparas and 20 multiparas, at one week and at one month after delivery. Results were as follows. 1. The mean content of Zn in the breast milk was 5.44 micrograms/ml at 1 week after delivery, and it decreased significantly (p less than 0.01) to 2.73 micrograms/ml at 1 month after delivery. 2. The mean content of Zn in serum was 0.66 microgram/ml at 1 week, and it increased significantly (p less than 0.01) to 0.84 microgram/ml, close to the normal level, at 1 month. 3. The milk Zn level at 1 week after delivery was about 8 times as high as the Zn in serum. There was a significant (p less than 0.05) negative correlation in Zn contents between milk and serum at 1 week after delivery, and there was no significant correlation in Zn contents between milk and serum at 1 month after delivery. 4. The mean content of Cu in breast milk was 0.55 microgram/ml at 1 week after delivery, and it decreased to 0.44 microgram/ml at 1 month after delivery. 5. The mean content of Cu in serum was 2.14 micrograms/ml at 1 week after delivery, and it decreased significantly (p less than 0.01) to 1.35 micrograms/ml, close to the normal level, at 1 month after delivery. 6. Concerning the Cu contents of milk and serum, there was not a significant correlation at 1 week after delivery, but a significant (p less than 0.05) positive correlation was found at 1 month after delivery.

[Silicon concentrations in maternal serum and breast milk in the postpartum period].

The silicon (Si) concentrations in maternal serum and breast milk were determined for 38 healthy postpartal women, 19 primiparae and 19 multiparae, by using inductively coupled plasma-atomic emission spectrometry (ICP-AES). The results are as follows: 1. The geometric mean (95% confidence interval) of the Si level in maternal serum was 9.7 mumol/l (8.8-10.8 mumol/l) at 1 week after delivery, and 8.1 mumol/l (7.1-9.2 mumol/l) at 1 month after delivery. The Si level at 1 week after delivery was significantly (p less than 0.05) higher than that at 1 month after delivery. 2. The geometric mean of the Si level (95% confidence interval), in breast milk was 6.3 mumol/l (5.4-7.3 mumol/l) at 1 week after delivery, and 6.1 mumol/l (5.1-7.2 mumol/l) at 1 month after delivery. 3. At both 1 week and 1 month after delivery, a significant correlation was not found between the Si concentrations in maternal serum and in breast milk.

[Effects of sodium chloride, acetic acid and citric acid on the dissolution of aluminum from aluminum cooking utensils].

In order to investigate the effects of sodium chloride and organic acids on aluminum elution from cooking utensils made of aluminum, the quantities of aluminum elution were measured in solutions with various concentrations of sodium chloride, acetic acid and citric acid by flameless atomic adsorption spectrophotometry. The increase of the aluminum elution rate from a pudding cup, an aluminum pan and an alumite pan could be clearly distinguished by the coexistence of acetic acid or citric acid and sodium chloride. The elution was low in the presence of sodium chloride at room temperature, but it was distinctly accelerated by heating. Although alumite treatment had the effect of protection against aluminum elution, such elution obviously increased, as shown by the existence of acid and sodium chloride at high temperatures. Aluminum elution rates from surface-untreated cooking utensils made of aluminum were increased by heating by a factor of several thousand.

[Stereotactic aspiration using coordinate software for hypertensive intracerebral hematomas].

Stereotactic aspiration for intracerebral hematoma has become widely used due to improvements in computed tomography (CT). CT-guided stereotactic aspiration for hypertensive intracranial hematomas was performed in 46 cases. Stereotactic aspiration with coordinate software was carried out among 16 cases. We compared the clinical results of conventional stereotactic aspiration with results of the new method using coordinate software. Using coordinate software, we can examine the route for the target point, as well as the target point of the hematoma before surgery. Therefore we can avoid obstacles such as ventricles and internal capsules. Further, we can aspirate hematoma by using only this one procedure. Mean hematoma aspiration percentage has risen from 44.9 to 87.2% by using coordinate software. Rate of drainage tube insertion after operation has decreased from 90 to 50%. The period of placement of the drainage tube and the frequency of infection due to the drainage tube has become lower, too. Therefore we can say that the outcome of treatment of hypertensive intracranial hematomas is improved by using coordinate software.

[Instrumentation surgery for tumor of the spine].

Spinal instrumentation surgery has come into wide use recently, especially for tumor of the spine. It is very effective for fixing the spine for relieving the patient from pain. But the decision about the indication of instrumentation surgery for tumor of the spine must be made carefully. In cases of double malignant tumors including spinal tumor, it is hard to decide how to treat tumor of the spine. We report a case of 78-year-old male with multiple myeloma of the cervical spine and cancer of the prostate. The patient had been in good health until one year previously, when he started to complain of nuchal pain. He became bedridden with loss of voluntary control of the right upper extremity for the last two months. Then he was admitted to our hospital and radiological examination was carried out. It revealed tumor of the cervical spine and it seemed to be bone metastasis. Further examination revealed that the patient was suffering from multiple myeloma and cancer of the prostate. The tumor of the spine was diagnosed as either multiple myeloma or metastasis of cancer of the prostate. Spinal instrumentation surgery using Luque-rod was carried out as one of the comprehensive treatment plans for double malignant tumors. The instrumentation surgery was effective for fixing the spine and improving the patient's quality of life. In operative procedures, intraoperative spinal cord monitoring was carried out. Intraoperative spinal cord monitoring was indispensable for preventing spinal cord injury. The tumor of the spine was diagnosed as multiple myeloma by pathological examination.(ABSTRACT TRUNCATED AT 250 WORDS)

[Study by I-123-IMP-SPECT before and after surgery for craniosynostosis].

Single photon emission computed tomography with N-isopropyl-p-[I-123]iodoamphetamine was performed in 13 cases of craniosynostosis before and after surgery. Of 13 cases, 8 cases (62%) showed focal low perfusion area on preoperative study. Four of seven cases (57%) with brachycephaly showed low perfusion areas in either of frontal lobes, occipital lobes, and cerebellum. Besides, two patients with scaphocephaly and one with plagiocephaly showed low perfusion area in unilateral cerebral hemisphere. Two Crouzon disease cases showed no focal low perfusion area, but an Apert disease showed low perfusion areas in both frontal lobes, cerebellum, as well as left occipital lobe. Corresponding CT and MRI showed no focal abnormality in any of these cases. These low perfusion areas were diminished or disappeared after surgical treatment in 6 cases (75%). We concluded that the I-123-IMP-SPECT is considered to be a useful index for the evaluation of functional recovery after surgery in cases with craniosynostosis.

[Demonstration of a vascular lesion by intraoperative portable DSA in a case of intracerebral hematoma with impending herniation].

A nine year old boy with sudden onset of loss of consciousness was brought to the emergency room where he was in deep coma with bilateral decerebrated rigidity, and fixed dilated pupils. Computerized tomographic scan showed an intracerebral hematoma in the left temporal lobe associated with signs of uncal herniation. He was taken to the operating room immediately and craniotomy was started. After aspirating a small amount of hematoma through a burr hole, left carotid angiography was carried out by using a portable digital subtraction angiography (DSA) unit. The procedure took approximately ten minutes, and it showed arteriovenous malformation (AVM) supplied by a temporal branch of the middle cerebral artery. Using this angiogram as a guide, the intracerebral hematoma and the AVM were completely removed. The patient recovered well and returned to school after rehabilitation. This case illustrates a method of treatment for intracerebral hematoma with impending herniation. Although, in general, preoperative angiography is required before the operation of AVM, it could be argued that time spent for angiography may result in irreversible damage to the brain stem in the presence of uncal herniation. We chose to perform intraoperative portable DSA during evacuation of the hematoma instead of wasting time on preoperative angiography. With this method, we were not only able to remove the hematoma without delay, but were also able to extirpate the AVM safely. Whether or not our method can be used in general remains to be seen, and further experience is required.

Pathological studies on nephrotoxic serum nephritis accelerated with rabbit gamma-globulin in mice.

In order to characterize nephrotoxic serum nephritis accelerated with rabbit gamma-globulin in mice, histopathological studies were carried out 15 days after NTS injection, the time when increases in urinary protein and serum cholesterol and a decrease in serum albumin were apparent. Characteristic changes were widespread thickening of glomerular capillary walls and widening of mesangial areas, owing to deposits of mesangial matrixlike substances. The mesangial interposition into subendothelial areas and the resultant narrowing of the capillary lumen were shown ultrastructurally. In severely affected glomeruli, a hyaline nodular lesion was observed. Visceral epithelial cells demonstrated fusion of the foot processes, microvilli formation, occasional proliferation, and enlargement. Parietal epithelial cells proliferated, forming a cellular crescent. Based on these characteristics, it appears this nephritic model shares a common pathology with human membranoproliferative glomerulonephritis type 1 and crescentic glomerulonephritis and can be considered an appropriate model for producing severe nephritis for short periods.

A small temporal lobe glioma detected only by magnetic resonance imaging.

The authors were able to demonstrate a very small glioma (1.0 x 0.8 cm in size) of the temporal lobe using MRI. They discuss the relative merits of CT and MRI in the diagnosis of gliomas in terms of their size. CT fails to detect tumours less than 2.0 x 1.5 cm in size which are not calcified. So far, MRI has revealed a tumour as small as 1.0 x 0.8 cm. Therefore, MRI is superior to CT for detecting small gliomas.

Congenital arteriovenous malformation of the scalp--case report.

A case of congenital arteriovenous malformation (AVM) of the scalp containing a complex vascular network is described. The AVM was totally removed, together with its feeding artery and draining veins. Congenital AVM of the scalp is rare; only 21 other cases have been reported since 1973. All involved complex vascular networks, and about one half of the patients had red or purple birthmarks. These clinical features clearly differ from those of traumatic arteriovenous fistulae of the scalp. Although many treatment approaches have been tried, total excision appears to be the most appropriate.

[Cloverleaf skull syndrome with occipital bone cristae. Report of two cases].

Two cases of cloverleaf skull syndrome are reported. The patients were born with trilobed deformity of the skull associated with synostosis of the lambdoidal, coronal, and sagittal sutures in Case 1 and of the lambdoidal suture in Case 2. Both patients had hydrocephalus, low-set ears, and ocular hypertelorism. Ventriculoperitoneal shunting had been performed for hydrocephalus in both cases in early infancy. Thereafter, the skull anomaly caused mental and physical retardation and elevated intracranial pressure. In both cases, surgery was successful in terms of cosmetic and neurological improvement. Both cases involved prominent midline crista formation of the skull over the posterior fossa, which steadily progressed during the first year of life and produced hydrocephalus, apparently by compressing or obstructing the aqueduct and/or the ostia of the fourth ventricle. Twenty-four cases of surgery for cloverleaf skull syndrome have been reported to date. Only Partington's types II and III were treated surgically, and patients with type III had more favorable outcomes than those with type II. However, these reports provide no clear-cut data that surgery significantly influences the outcome. The experience with the two cases reported here suggests the benefits of early shunting followed later by radical surgery.

[Barbiturate therapy in traumatic cerebral vascular disease: report of two cases].

We report two cases of traumatic cerebral vascular disease which were treated successfully with barbiturate. The first case sustained blunt trauma to the bilateral vertebral arteries, resulting in complete occlusion of both arteries. After ligation of the injured vertebral arteries, multiple cerebral infarction appeared. Cerebral angiography revealed dissection and stenosis of the bilateral internal carotid arteries. We treated this case with barbiturate (Thiamylal) in combination with administration of heparin. The second case sustained cerebral contusion and traumatic subarachnoidal hemorrhage as a result of a motor cycle accident. This patient deteriorated and cerebral angiography showed diffuse cerebral arterial vasospasms. When this was treated with induced hypertension, he developed recurrent subarachnoid hemorrhage. In order to protect the brain from ischemia without elevating blood pressure, we employed barbiturate therapy and the patient recovered without major neurological deficit. The condition of severe head injury with cerebral ischemia is complicated. Therefore it has been hard for neurosurgeons to cure the patient with this condition. But we treated it with barbiturate successfully. Barbiturate therapy in severe head injury with cerebral ischemia may decrease the mortality in that group of patients considered difficult to treat with the usual therapeutic modalities.

Marked cerebrospinal fluid collection associated with hepatic failure.

We report two cases with abnormal CSF accumulation in the cranium. One case had marked dilatation of the ventricular system, for which a ventriculoperitoneal shunt was carried out. The shunt was not beneficial and it turned out that the patient was suffering from ornithine transcarbamylase deficiency. The second case developed marked subdural effusion after fulminating hepatic failure and septicemia, for which drainage of the subdural space and subsequent insertion of Ommaya reservoirs were carried out. In spite of successful CSF control, the outcome was bad, both cases being incapacitated due to severe encephalopathy probably caused by hyperammonemia. The results of serial computed tomographic follow-up are presented.

Studies on iridoid-related compounds. V. Antitumor activity of iridoid dervs. periodate oxidation products.

In the course of a modification study of iridoid glucosides to investigate their antitumor and antimicrobial activity, we found that about all metaperiodate oxidation products of iridoid glucosides which had no antitumor activity showed potent activity against the leukemia P388 in mice. They were found to be more active than the corresponding aglycones obtained by enzymic hydrolysis of iridoid glucosides. Among them, periodate oxidation product of sweroside showed the most potent activity, of which the maximum total/control (T/C) value was 198% at 200 mg/kg.
