Patients with a giant intracranial aneurysm usually present as having an intracranial space-taking lesion, but no subarachnoid hemorrhage. The authors describe a 61-year-old man with a completely thrombosed giant aneurysm at the pericallosal artery. Computed tomography scan showed a large, round, mottled, high-density area in the paramedian frontal region that had ringlike enhancement with contrast media. On cerebral angiography it was demonstrated to be an avascular mass lesion. These findings led to an initial diagnosis of falx meningioma. However, final diagnosis at operation was completely thrombosed giant aneurysm. The literature is reviewed and the pathogenesis of intracranial thrombosed giant aneurysm, in regard to its growth and thrombus formation, is discussed.

Aneurysms of the extracranial carotid artery are rare. Neurological manifestations in patients with these lesions may be quite varied. This paper presents a unique patient who showed both hoarseness due to left recurrent laryngeal nerve paralysis as a local sign of compression, and ischemic syncopal attack as an embolic or flow-related symptom. Little has appeared in the otolaryngologic literature, despite the fact that many patients have symptoms that may initially lead them to see an otolaryngologist. A misleading needle biopsy of the mass in the neck was performed by a local otolaryngologist before the diagnosis was confirmed. The literature on the subject is briefly reviewed.
Butanol-extracted water-soluble adjuvant (Bu-WSA) obtained from Bacterionema matruchotii was mitogenic to murine splenic B lymphocytes, but not T lymphocytes. When murine splenic cells were cultured in the presence of Bu-WSA and concanavalin A (Con A) together, [3H]thymidine uptake of the culture cells synergically increased. The mechanism of the synergy of Con A and Bu-WSA and the participation of interleukin (IL) 1 and 2 in the synergy were studied. The proliferation cells in the synergy were Lyt-1+23- lymphocytes. Ia-positive accessory cells were required for the response. When separated cell populations and Marbrook-type culture vessels were used, a mixed cell population of T lymphocytes and B lymphocytes or macrophages (M phi) produced some active factor(s) after co-stimulation by Con A and Bu-WSA, and the factors enhanced DNA synthesis of another Con A-activated T lymphocyte population. Supernatants obtained from the spleen cell cultures or the mixed cell cultures with T lymphocytes and M phi in the presence of Con A and Bu-WSA contained greater amounts of IL-1 and IL-2 than those from cultures containing Con A or Bu-WSA alone. An addition of exogenous IL-1 or IL-2 to spleen cell cultures with Con A resulted in a proliferative response like that obtained through co-stimulation by Con A and Bu-WSA. These results suggest that the synergistic effect of Con A and Bu-WSA on the proliferative response in murine splenic cells is sustained by the enhancement of production of these T-lymphocyte growth factors.
We studied the effects of positive end-expiratory pressure (PEEP) ventilation in ten patients with acute myocardial infarction (nine in Killip class III, one in Killip class IV; pulmonary capillary wedge pressure greater than 24 mmHg) and of extracorporeal ultrafiltration method (ECUM) in seven patients with refractory heart failure due to acute myocardial infarction and others. Application of PEEP resulted in significant increases in PaO2 and SaO2 and decrease in PaCO2. Significant reduction in mean pulmonary arterial and pulmonary capillary wedge pressures and heart rate was observed, while stroke work index increased significantly. There was a significant correlation between changes in stroke work index and PaO2 after the application of PEEP. The use of ECUM removed fluid of 1416 +/- 662 ml (680-2800 ml) with the ultrafiltration flux rate being 478 +/- 223 ml/hour. Significant decreases in mean pulmonary arterial, pulmonary capillary wedge and central venous pressures were observed, while PaO2 increased significantly. BUN and serum creatinine levels increased significantly, and total protein and serum albumin tended to increase. There was a significant correlation between fluid removed and change in PaO2 after the use of ECUM. Thus, PEEP and ECUM are beneficial for patients with refractory heart failure. The mechanism(s) are: reduction in preload due to an increased intrathoracic pressure and a decreased systemic venous return with PEEP, or due to removal of excess fluid with ECUM, and improvement of the oxygenation of the blood.

Investigations were made to determine the roles of interleukin (IL)-1 and IL-2 in the synergistic enhancement of DNA synthesis by concanavalin A (Con A) and bacterial lipopolysaccharide (LPS) or butanol-extracted water-soluble adjuvant (Bu-WSA) from Bacterionema matruchotii in cultures of thymic cells taken from hydrocortisone (HC)-treated C3H/HeN (LPS-responsive) and C3H/HeJ (LPS-non-responsive) mice. When the C3H/HeNCrj cells were cultured in the presence of Con A and LPS or Bu-WSA together, [3H]thymidine [( 3H]TdR) uptake of the cells was enhanced synergistically in comparison with those cultured with either one of the mitogens alone. The synergistic effect on thymic cells was dependent on Ia-positive accessory cells, since a previous treatment of the cells with anti-Iak serum and complement inhibited the response, and the inhibition could be relieved by the addition of either purified peritoneal exudate macrophages (Mø) or splenic B lymphocytes. The co-stimulation of cells with Con A and LPS or Bu-WSA also enhanced their production and release of thymic cell growth factor(s) into the culture medium. The amounts of IL-1 and IL-2 in the culture supernatants were sufficiently high to explain the activities of the growth factor(s). On the other hand, enhanced IL-2 production without significant increase in IL-1 was seen in the co-cultures of anti-Ia+ cell deprived thymic cells and purified splenic B cells prepared from C3H/HeNCrj mice in the presence of Con A and LPS or Bu-WSA, and it was seen in the cultures of C3H/HeJ thymic cells with Con A and LPS. These results suggest that the synergistic effect of Con A and LPS or Bu-WSA on the proliferative response of HC-treated thymic cells is mainly due to the enhanced production of IL-2 and its action to increase cell growth, and there are two pathways by which the enhancement of IL-2 production by Con A and LPS or Bu-WSA can occur: an IL-1-dependent pathway, or an IL-1-independent one.
The pathological entity of intracranial giant cell tumor remains controversial, which has been considered a glioma (giant cell glioblastoma) by some and a sarcoma (monstrocellular sarcoma) by others. The authors present a case in which CT scan shows a wedge-shaped high density area at the left parietal lobe. This tumor expands explosively only a month on serial CT scan. The microscopic speciemen demonstrates the bizarre irregular giant cells with large vesicular nuclei, and glial proliferation and abundant reticulin fibers. Then we reviewed and discussed with emphasis on the historical aspect of giant cell tumor and also radiologic, therapeutic problem.
The interaction between two cell lines derived from the human salivary gland (HSG), neoplastic epithelial duct HSG cells and myoepithelial human pleomorphic adenoma (HPA) cells, was studied morphologically and immunohistochemically in nude mice tumors produced by inoculation of HSG and HPA cells. Transplantation of HSG cells into nude mice resulted in the production of adenocarcinoma which contained carcinoembryonic antigen (CEA). The nude mice tumors induced by HPA cells were interpreted as myoepithelioma in which the presence of S-100 protein and myosin were identified. On the other hand, the occurrence of squamous cell nests was frequently noted in the nude mice tumors produced by inoculation of a mixture of HSG and HPA cells. The tumor cells present in the squamous cell nests had abundant tonofilaments in the cytoplasm and were attached with tight junction and distinct desmosomes. In addition, the presence of keratin in the tumor cells composing squamous cell nests was demonstrated. When the mixture of HSG and HPA cells treated with polyethylene glycol (PEG) was transplanted into the nude mice, the tumors produced consisted almost entirely of areas showing the histologic features of anaplastic carcinoma, and did not contain all of the specific cell markers observed in the HSG or HPA tumors. The nude mice tumors induced by PEG-treated HSG or HPA cells were interpreted as adenocarcinoma and myoepithelioma, respectively, and giant cells were occasionally observed in the tumor sections. These findings indicate that neoplastic cells showing differentiation stages other than those of the original two cells can be induced in nude mice by utilizing HSG and HPA cells.
Human fibroblast interferon (HuIFN-beta) was topically administered to 20 premalignant lesions histopathologically showing epithelial dysplasia such as leukoplakia and lichen planus which arose in the oral mucosa. HuIFN-beta was prepared in the water-soluble gel form containing 2% carboxymethylcellulose, 45% glycelin, 0.1 M citrate buffer (pH 4.5) and 0.2% SDS as stabilizing agents. This preparation was found to be effective for herpetic gingivostomatitis and zostal lesions arising along the intercostal nerve. Thus, the HuIFN-beta preparation (10(4) to 5 X 10(3) IU) was applied to the oral mucosal lesion for 1 h twice a week. The lesion with topical administration of HuIFN-beta was covered tightly with the mucosal bandage which was coated with carboxymethylcellulose, glycelin and CaCl2 on vinyl acetate matrix. The 14 oral lesions with erosion or ulcer formation accompanied by severe pain by touch, had complete remission after approximately 10 successive applications of this preparation. Although subjective symptoms such as irritation pain in the other 6 patients with severe hyperkeratotic lesion subsided, white coatings and streaks could not be completely removed by this therapy. No other side-effects excluding slight pain and reddish swelling which occurred intermittently during HuIFN-beta administration were observed.
Murine B cell mitogens such as bacterial lipopolysaccharide (LPS), butanol-extracted water soluble adjuvant (Bu-WSA), dextran sulfate (DS), synthetic muramyl dipeptide (MDP), and its analog MDP-Lys (L18) do not show any mitogenic ability in vitro on human peripheral blood lymphocytes or mixed cell populations of purified T and B cells obtained from the lymphocytes in an ordinary culture system. However, these mitogens are capable of enhancing the mitogenic effect of concanavalin A (Con A) in the cultures. In the presence of one of these mitogens, the activity of interleukin 2 (IL 2), but not interleukin 1, in the supernatants obtained from cultures containing Con A-stimulated T cell and B cell populations was higher than that of control cultures. The role of the newly produced IL 2 in the synergistic effect of the mitogens in human lymphocyte cell cultures was discussed.
