[FIO2 and SpO2 during cardiac surgery in neonates and infants].

Fractional inspired oxygen (FIO2) applied before and after a repair or palliative procedure for cardiac defects along with SpO2 measured by pulse oximeter were reviewed from anesthetic records of 62 neonates and infants. Fentanyl was used for anesthesia, except in cases without i.v. route in which sevoflurane with nitrous oxide in oxygen was used for induction of anesthesia. FIO2 was adjusted using air and oxygen. The lower FIO2 was applied to the patients for a closure of ventricular septal defect, a reconstruction of coarctation or interrupted aortic arch, and pulmonary artery banding, while the higher FIO2 was used for systemic-to-pulmonary artery shunts and the repair of tetralogy of Fallot. In the congenital heart disease with the intracardiac shunt, the magnitude of the shunt flow and hemodynamics can be altered by changing systemic and pulmonary vascular resistance which could be induced by various ways. Since alveolar oxygen tension is a known determinant of pulmonary vascular resistance, an appropriate FIO2 should be applied to each patient with different pathophysiology. A low FIO2 should be set for the cases with nonrestrictive left-to-right shunting, since a high FIO2 may cause a torrential pulmonary blood flow. A high FIO2 is preferable for the cases with right-to-left shunting and a concomitant decreased pulmonary blood flow.

[An experimental study on a new technique of regional liver hyperthermia using femoro-hepatic arterial thermal bypass].

This study was undertaken to evaluate the efficacy of a new method using the femoro-hepatic arterial (FA-HA) thermal bypass for regional liver hyperthermia. Using mongrel dogs (n = 8), a 12 Fr catheter was introduced into the abdominal aorta through the right femoral artery. A hepatic arterial catheter (8 Fr) was placed through the gastroduodenal artery. These catheters were connected to a system containing a centrifugal pump and a heat exchanger. The heat exchanger consists of warmer coils and a water bath kept at a temperature of 47 degrees C. The bypass was run at an average blood flow rate of 153 +/- 33 (mean +/- SD) ml/min. The temperature was continuously monitored at four measuring points, including the water bath, the inflow blood, the liver parenchyma and the rectal cavity. The temperature of the liver parenchyma promptly reached 40.0 +/- 1.7 degrees C as early as 5 minutes and reached its highest value of 42.8 +/- 0.8 degrees C 30 minutes after the start of hyperthermia. The rectal temperature was 39.3 +/- 1.2 degrees C at 30 minutes. The temperature of the liver parenchyma (38.1 +/- 1.4 degrees C) became similar to that of the rectum (38.1 +/- 1.3 degrees C) 30 minutes after the end of hyperthermia. These results indicate that the FA-HA thermal bypass is a simple and highly effective method for the performance of regional liver hyperthermia.

[Clinical study of cyclic intra-arterial chemotherapy using mitoxantrone for advanced hepatocellular carcinoma].

Cyclic intra-arterial infusion chemotherapy using Mitoxantrone via reservoir system was prescribed for 8 patients with advanced hepatocellular carcinoma. According to the clinical staging system, two patients were in Stage III and the other six patients in Stage IV-A. Vp-3 portal invasion was observed in six out of the eight patients. Mitoxantrone (5-8 mg/body) was administered intra-arterially via port-a-cath device every 4 weeks. For tumor response, there were two PR cases, four NC cases, and two PD cases. Thus, the response rate was 25%. The mean survival period was 6.5 months, and 1-year survival rate was 13%. In one of two PR cases, the survival time has been more than two years so far. Gastrointestinal toxicities such as nausea and vomiting were not severe. Critical myelosuppression requiring medication was not observed. Although a temporary increase of serum bilirubin was found in two cases, no severe damage to hepatic function was observed. From our preliminary data, this arterial infusion chemotherapy using Mitoxantrone via reservoir system can be safely performed on non-hospitalized patients with advanced hepatocellular carcinoma, and may be a useful treatment modality.

[A study on the distribution of metastases at autopsy in 70 patients with ovarian cancer].

A total of 6,457 autopsy protocols were reviewed to identify 70 women who died of ovarian cancer. 1. Epithelial tumors were the most common tumor (74%). Twenty% were germ cell tumors. There were two cases each of sarcoma and stromal cell tumors. 2. The patients with germ cell tumors were younger than those with epithelial tumors. The duration of the epithelial tumor disease was longer than that of germ cell tumor. 3. The most common mechanism of death was carcinomatosis (69%). Eight of 70 patients (11%) had respiratory failure. One case of subacute cor pulmonale was due to tumor embolization. One case had a rupture of the portal vein. 4. The peritoneum was most frequently involved in every tumor (87%). The liver was the second most frequent site (73%). Other frequent metastatic sites included the lungs (44%), and spleen (47%). Lymph nodes including the paraaortic, pelvic and mesenteric were also very frequent metastatic sites. 5. CDDP containing regimens were used in 33 cases (47%). FAMT regimen was the second most common treatment (15.7%). Another common treatment is whole abdominal radiotherapy (14%). The patients treated with the CDDP containing regimen had a significantly longer survival time (23.4 months) than those treated with FAMT (19.2 months) and others (13.6 months). 6. For metastasis to the lungs, spleen and mesenteric lymph nodes the patients treated with the CDDP containing regimen were more numerous than those treated without CDDP. For metastasis to the kidneys, the patients treated without CDDP were more numerous than those treated with CDDP containing regimens.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of allergic diseases and specific IgE antibodies in different parts of Japan.

We investigated the differences of allergic diseases and specific IgE antibodies to inhaled or ingested allergens between two cities in a Japanese pediatric population. One of the two cities is Itoman, which is a subtropical city in Okinawa prefecture; the other is Gifu, which is a Temperate Zone city in Gifu prefecture. The prevalence of asthma or wheezy bronchitis was significantly higher in Itoman; however, the prevalence of atopic dermatitis was significantly higher in Gifu. Furthermore, the positive percentages of specific IgE antibodies to inhaled allergens such as house dust, and Dermatophagoides farinae were higher in Itoman. The positive percentages of specific IgE antibodies to ingested allergens such as egg white, cow milk, and soybean were higher in Gifu. These results suggest that the differences in prevalence of allergic diseases and allergens may depend on differences in the triggering environmental factors between different parts of Japan.

[Clinicopathoradiological studies in 15 cases of megadolichobasilar anomaly].

We clinicopathoradiologically assessed 15 angiographically diagnosed megadolichobasilar anomalies. Nine of the patients were male and 6 were female; their average age was 61 years. Eleven patients presented with cerebral ischemic attacks, other two complained of trigeminal neuralgia and the remaining two suffered severe headaches. Twelve of the patients had severe hypertension. Vertebral angiography revealed marked elongation of the basilar artery with severe tortuousity and dilatation. The average distance from the dorsum sellae to the basilar artery bifurcation on the lateral view was 24.7 mm, and the average maximum diameter of the basilar artery was 8.6 mm. Aneurysmal dilatation of the basilar artery was also observed in four cases. In 14 of the 15 patients CT scans revealed characteristic findings, such as tubular high density mass with evident contrast enhancement extending from the ventral medulla to the interpeduncular cistern. The outcome was extremely poor, with five deaths and four patients with severe dementia. In the two autopsy cases, enlarged internal lumens could be observed despite severe atheroscrelotic changes, such as intimal thickening by atheromas.

High density lipoprotein conversion mediated by human plasma phospholipid transfer protein.

Phospholipid transfer protein (PLTP) was purified from lipoprotein-free human plasma, obtained upon treatment of plasma with dextran sulfate and Ca2+, by employing a series of column chromatography. Upon sodium dodecyl sulfate-polyacrylamide gel electrophoresis, the purified PLTP showed a single main band, corresponding to the molecular mass of 78 kDa. However, isoelectric focusing of the purified preparation gave multiple bands with pI ranging from 4.3 to 5.1, indicative of microheterogeneity. Purified PLTP was shown to possess not only phospholipid transfer activity, but also high density lipoprotein (HDL) conversion activity (Tu, A.-Y., Nishida, H. I., and Nishida, T. (1990), FASEB J. 4, A2148; Jauhiainen, M., Metso, J., Pahlman, R., Blomqvist, S., van Tol, A., and Ehnholm, C. (1993) J. Biol. Chem. 268, 4032-4036). Isolated HDL3 was enlarged to the size of HDL2b upon incubation with purified PLTP for 6 h at 37 degrees C at the PLTP/HDL3 molar ratio of approximately 1:45. Both the HDL conversion and the phosphatidylcholine transfer activities of purified PLTP were effectively inhibited by rabbit anti-PLTP immunoglobulin G. The primary importance of PLTP in the HDL enlargement that occurs in human plasma upon incubation at 37 degrees C was shown by the strong inhibitory effect of the anti-PLTP immunoglobulin G. The process of PLTP-mediated HDL enlargement was accompanied by the release of apoproteins, primarily apoA-I. HDL3 enlargement mediated by PLTP was effectively inhibited by the addition of free fatty acids.

Cholesterol ester transfer mediated by lipid transfer protein as influenced by changes in the charge characteristics of plasma lipoproteins.

The relationship between the cholesterol ester (CE) transfer activity of lipid transfer protein (LTP) and its affinity with lipid and lipoprotein particles was investigated. The study of the effects of chemical modification of low density lipoprotein (LDL) amino groups and carboxyl groups on the CE transfer activity showed that the maximal activity is obtained upon succinylation or acetylation of approximately 7% of LDL amino groups. Further increases in the extent of modification progressively reduced the transfer activity. The treatment of LDL with fatty acids gave results comparable to the chemical modification of LDL amino groups. The addition of low concentrations of fatty acids was stimulatory, while that of high concentrations was inhibitory. Although increases in the positive charges of LDL by the carboxyl group modification did not appreciably influence the CE transfer, the addition of cationic detergents gave a profound effect on the CE transfer. A maximal CE transfer activity was obtained upon addition of very small amounts of the detergents, with the higher concentrations sharply reducing the transfer activity. We also studied the effects of the concentrations of phosphate buffer and various salts on the CE transfer as well as the affinity of LTP for very low density lipoproteins, low density lipoproteins, high density lipoproteins 3, and high density lipoproteins 2. It appeared that the affinity of LTP for various lipoproteins is governed by a delicate balance of electrostatic and hydrophobic interactions. Optimal degrees of the interaction of LTP with both donor and acceptor particles seem to be required for the maximal degree of CE transfer.

Role of urokinase type plasminogen activator (u-PA) in corneal epithelial migration.

The role of plasminogen activator (PA) in the migration of corneal reepithelialization was studied. Rabbit corneal blocks were cultured, and both the extent of epithelial migration over the exposed corneal stroma and the activity of PA released into the culture media were measured. A significant, direct correlation between epithelial migration and PA activity in the medium was observed, even when the migration was stimulated by fibronectin or EGF, or was inhibited by cytochalasin B or cycloheximide. Zymography confirmed that the PA released into the culture medium was of the urokinase type (u-PA). Immunohistochemical studies showed that u-PA and plasmin(ogen) were present at the leading edge of the migrating epithelium. Studies of corneal cell cultures indicated that epithelial cells rather than endothelial cells or fibroblasts were the source of the u-PA. The addition of antihuman u-PA IgG or protease inhibitors retarded the migration of the corneal epithelium in a dose-dependent manner, indicating that u-PA activity is essential for the migration of the corneal epithelium. These findings suggest that the migration of corneal epithelial cells requires not only cell attachment to the extracellular matrix through the fibronectin but also degradation of the fibronectin by the release of cellular u-PA.

Concurrent ovarian serous carcinoma and osteogenesis imperfecta.

Because epithelial malignancies in patients with osteogenesis imperfecta are rare, it has been postulated that these patients may have "cancer-protection". A case of stage III b ovarian serous carcinoma in a woman with congenital osteogenesis imperfecta is now presented and discussed.

The structure of recombinant human carboxy-terminal-truncated macrophage colony-stimulating factor derived from mammalian cells.

The structure of recombinant human carboxy-terminal-truncated macrophage colony-stimulating factor expressed in CHO cells was investigated. The bioactive protein ([-32-153]M-CSF), expressed from a nucleotide sequence that encoded a signal peptide of 32 amino acids and N-terminal amino acids numbers 1-153, was heterogeneous in terms of molecular mass, as analyzed by SDS-PAGE, because of the presence of N-linked sugar moieties. The primary structure of the polypeptide was determined by sequence analysis and amino acid analysis of the fragments obtained from lysylendopeptidase digests of reduced and alkylated M-CSF, and from pepsin digests of the intact molecule. A sugar chain was located only at Asn-122 of the two putative sites of N-glycosylation that were present per subunit. The homodimeric structure appeared to have seven disulfide bonds, formed by inter- or intra-molecular linkages, since there were no free thiol groups in the molecule. The assignment of disulfide bonds by sequence analysis using peptide fragments indicated the combinations of Cys7-Cys90, Cys48-Cys139, and Cys102-Cys146. Gel-filtration analysis of Ser31[-32-153]M-CSF, in which the remaining Cys31 was replaced by Ser and which was expressed in COS cells, suggested that the mutein existed as a monomer. Our study shows that the disulfide-bond pairings of [-32-153]M-CSF that is expressed and post-translationally modified in mammalian cells are identical to those of Escherichia coli-derived [3-153]M-CSF with only one intermolecular disulfide bond, namely, Cys31-Cys31.

Serum manganese superoxide dismutase (MnSOD) and histological virulence of ovarian cancer.

The serum level of a scavenging enzyme, manganese superoxide dismutase (MnSOD), was examined in 78 patients with ovarian cancer. Tumors included 27 stage I, 6 stage II, 37 stage III and 8 stage IV diseases. They were classified as 43 serous, 18 mucinous, 10 clear cell and 7 endometrioid neoplasms, and grouped into 25 grade I, 27 grade II and 26 grade III cancers. Factors affecting the MnSOD level were estimated by analysis of variance and Bonferroni t-test. From these, both the stage and histology were less-effectively affected by the level, while the histological grade was estimated as effective with p < 0.05. The MnSOD value increased significantly in accordance with the escalation of the tumor grade with p = 0.02 for grade I vs II, 0.005 for II vs III, and 0.0001 for I vs III respectively. MnSOD determination may be unique as a predictor of the histological virulence of ovarian carcinoma.

Purification and characterization of recombinant human macrophage colony-stimulating factor expressed in Chinese hamster ovary cells.

We expressed human macrophage colony-stimulating factor (M-CSF) in Chinese hamster ovary (CHO) cells by introducing an expression plasmid coding for a 554-amino-acid M-CSF precursor and dihydrofolate reductase (DHFR) gene, and by amplifying the sequence. A cell line was obtained that secreted approximately 200,000 units/ml after 6 days in culture. The expressed recombinant human M-CSF (rhM-CSF) primarily consisted of two molecular species, a main 80-90 kD M-CSF as a homodimer and a molecular form higher than 150 kD. Purification of a main rhM-CSF gave an apparently homogeneous protein disulfide-bonded from 42-kD subunits, but one of the purified rhM-CSFs was composed of two subunit species with molecular masses of 44 and 42 kD. These purified rhM-CSFs had substantially the same specific activity (1 to 4 x 10(7) units/mg protein). Deglycosylation experiments with the latter rhM-CSF using chemical (trifluoromethanesulfonic acid) and enzymatic methods found a terminal neuraminic acid in addition to N- and O-glycosylation, but the two subunit species did not coalesce into a single molecule.

Strumal carcinoid of the ovary.

A strumal carcinoid arising in a benign cystic teratoma of the left ovary was reported in a 41 year-old woman. The solid tumor was histologically a trabecular carcinoid tumor associated intimately with thyroid follicle-like structures. By Grimelius staining, argyrophil granules were found in the cytoplasm of the tumor cells. The final diagnosis of strumal carcinoid, however, was established by the confirmation of thyroid tissue.

Effects of antimicrobials on corneal epithelial migration.

The slowed healing rates observed by some investigators may be caused by vehicles or preservatives in the antimicrobials preparations tested. To determine whether antimicrobials directly inhibit corneal epithelial wound healing, we cultured blocks of the rabbit cornea in media containing various concentrations of antibiotics or antimicrobials (at 1, 10, or 100 micrograms/ml); after 24 hours, we measured the distance of epithelium that had migrated down the side of each block. The higher concentrations of fluoroquinolones (ofloxacin; 74 +/- 5.8% of control at 100 micrograms/ml, p < 0.05, ciprofloxacin; 4.4 +/- 1.5% of control at 100 micrograms/ml, p < 0.01, or norfloxacin; 71 +/- 7.0% at 10 mu g/ml, p < 0.01, and 1.5 +/- 0.4% of control at 100 mu g/ml, p < 0.01) and the highest concentrations of peptides (polymyxin B; 64 +/- 3.0% of control at 100 micrograms/ml, p < 0.01, or colistin; 67 +/- 5.7% of control at 100 micrograms/ml, p < 0.01) or fosfomycin (79 +/- 6.2% of control at 100 micrograms/ml, p < 0.05) had an inhibitory effect on corneal epithelial migration. Among aminoglycosides tested, sisomicin (85 +/- 10.0% of control, not significant), dibekacin (76 +/- 11.6% of control, p < 0.05) and streptomycin (77 +/- 9.4% of control, not significant) were inhibitory at 100 micrograms/ml, but tobramycin had no effect. Penicillins (aspoxicillin, sulbenicillin or ampicillin), cephalosporins (cefmenoxime or cefminox), oxytetracycline, erythromycin and chloramphenicol did not affect epithelial migration at all. These results demonstrate that some antimicrobials are inhibitory at high concentrations, but penicillins, cephalosporins, oxytetracycline, erythromycin or chloramphenicol has no inhibitory effect on corneal epithelial migration.

[Comparison of endoscopic therapy and conventional surgery for the treatment of early gastric cancer in elderly patients].

The first choice for the treatment of gastric cancer is surgery. Presently, endoscopic therapy offers the possibility of cure for some types of early gastric cancer, especially among aged or poor surgical risk patients and has obtained excellent results. During the past 13 years and 6 months from April 1978 to September 1991, 55 lesions in 52 patients aged over 75 years old were treated by endoscopic therapy. During this same period, 57 patients aged over 75 years old were surgically treated for early gastric cancer. In this study we evaluated the efficacy of endoscopic therapy for the treatment of early gastric cancer, and furthermore we compared endoscopic therapy with surgery in relation to prognosis, complications and quality of life. The analysis of endoscopic treatment showed that in 72.7% of our cases local cure was obtained. Generally, early gastric cancers less than 20 mm, of well differentiated type, restricted to the mucosa and without ulceration are suitable for endoscopic therapy. Local cure could be achieved in more than 90% of these cases. Among the 52 patients treated by endoscopic therapy, there were residual lesions after treatment in in 4 patients. They died of advanced disease or metastatic liver tumor. These patients were poor surgical risks and for this reason surgery was contraindicated. Analysis by the Kaplan-Meier method showed that in comparison with conventional surgery, endoscopic therapy showed better results in the first two years of follow up and almost the same results after three years.(ABSTRACT TRUNCATED AT 250 WORDS)

An immunohistochemical survey of peptide-containing nerves in the copulatory apparatus of several male birds.

Distribution of peptide-containing nerves in male copulatory apparatus as well as rectal and cloacal ganglia were examined immunohistochemically in cock, guinea fowl, duck and muscovy duck. Neurofilament protein-immunoreactive nerves were also examined to investigate general distribution of nerves. Substance P (SP)-, vasoactive intestinal polypeptide (VIP)-, calcitonin gene-related peptide (CGRP)-, and neuropeptide Y (NPY)-immunoreactive (IR) nerves were detected in all four species of birds whereas leucine enkephalin (LENK)-IR nerves were detected only in duck and muscovy duck. In the rectal and cloacal ganglia, peptide-IR nerves were detected in varying degrees of abundance. Most noticeable distribution was found in the vascular body, where SP-IR nerve fibres were more abundant in duck and muscovy duck, VIP-IR fibres in muscovy duck and CGRP-IR fibres in duck, than in other regions and other species, respectively. A few peptide-IR nerve fibres were detected in other cloacal regions, although a peculiar pattern in distribution unique to a certain region could be not found. The present results suggest that the peptide-containing nerves participate in the regulation of tumescence, erection and detumescence of male copulatory apparatus of birds and their regulatory mechanisms are different depending on the species.
