Cloning and sequencing of trehalose biosynthesis genes from Rhizobium sp. M-11.

The two genes encoding maltooligosyl trehalose synthase and maltooligosyl trehalose trehalohydrolase, which are related to biosynthesis of alpha, alpha-trehalose, were cloned from Rhizobium sp. M-11. Sequence analysis showed that the synthase gene composed of 2316 bp was connected with the hydrolase gene of 1788 bp by an overlap of one nucleotide. The deduced amino acid sequences of both enzymes have several regions common to amylolytic enzymes belonging to an "alpha-amylase family".

The production of colony-stimulating factors by thyroid carcinoma is associated with marked neutrophilia and eosinophilia.

Cancers producing colony-stimulating factors and associated with marked leukocytosis are relatively rare. We report here a case of a thyroid cancer producing both granulocyte colony-stimulating factor (G-CSF) and granulocyte-macrophage colony-stimulating factor (GM-CSF). A 72-year-old woman had a thyroid carcinoma with significant neutrophilia and eosinophilia without any evidence of infection. The serum concentrations of both G-CSF and GM-CSF were elevated significantly in this patient, which might have induced the leukocytosis. Furthermore, the G-CSF concentrations in thyroid tumor tissue and metastatic lesions in the lung and skin examined at autopsy also were extremely high.

Dicentric Y chromosome without evidence of mosaicism in an azoospermic male.

A phenotypically male of dicentric Y chromosome with no evidence of sex chromosomal mosaicism is reported. Staining of fluorescence in situ hybridization using Y chromosome centromere-specific DNA probe showed the presence of two bright fluorescent spots in Y chromosome. Testicular histology revealed maturation arrest at the stage of primary spermatocyte. It is suggested that structural anomaly of the Y chromosome is related to the disturbance of spermatogenesis.

[Early steroid withdrawal after kidney transplantation].

BACKGROUND: The side effects of steroid are serious problems in renal transplant patients. However, withdrawal of steroid has been controversial. We evaluated the benefits and risk of early steroid withdrawal after renal transplantation. PATIENTS AND METHODS: The outcomes of early steroid withdrawal from triple immunosuppressive drug therapy were analyzed in four living related and one cadaveric renal transplant recipients. The dosage of steroid was gradually reduced and the time of steroid withdrawal after transplantation was 5 to 7.5 months. RESULTS: Four Patients have been able to be free from steroid and maintained stable graft functions and normal urinary findings for 14 to 33 months after withdrawal. The findings of rejection were not observed in the graft specimens obtained by serial biopsies. One patient who received a living related graft developed an increase in serum creatinine level and proteinuria two weeks after discontinuation of steroid. The serum creatinine level returned to that before withdrawal and proteinuria disappeared by steroid readministration. Long term side effects of steroid were not observed in 4 patients with successful steroid withdrawal. CONCLUSION: These results suggest that steroid withdrawal about 6 months after transplantation can be accomplished without jeopardizing graft function in selected renal transplant recipient and the withdrawal in the early stage is preferred for reducing the side effects. Careful and long-term follow up are required to assess the further risk and benefit of steroid withdrawal on immunosuppressive morbidity.

Role of dopamine in the striatum, renin-angiotensin system and renal sympathetic nerve on the development of two-kidney, one-clip Goldblatt hypertension.

PURPOSE: The possible role of the striatum and renin-angiotensin (R-A) system for the development of two kidney-one clip (2K-1C) Goldblatt hypertension is examined. MATERIALS AND METHODS: Monoamine contents of the striatum were determined by brain dialysis techniques, and plasma and kidney levels of the R-A system were measured by radioimmunoassay in sham-operated rats (experiment 1), 2K-1C hypertension (experiment 2), left renal denervation only (experiment 3), renal denervation of left clipped-kidney (experiment 4) and left renal artery clipping with renal denervation of right nonclipped kidney (experiment 5) in the acute phase. RESULTS: Plasma angiotensin I (ANG I) and plasma angiotensin II (ANG II) were initially increased between the first day and the first week, were suppressed at the second week, then increased again at the third week. Renal denervation of clipped or nonclipped kidneys did not influence the plasma R-A system in 2K-1C rats. The levels of striatum dopamine (DA), dihidroxyphenylacetic acid (Dopac) and homovanillic acid (HVA) in hypertensive 2K-1C rats were higher than those in normotensive sham-operated rats. There were significant positive relationships between these monoamines and systolic blood pressure. Renal levels of ANG I, II and angiotensin I converting enzyme activity (ACE) in the clipped-kidney were significantly higher than in sham-operated and nonclipped kidneys of 2K-1C rats. CONCLUSION: The enhanced R-A system in clipped-kidney and high levels of monoamines described above in the striatum appear to participate in the development of 2K-1C hypertensive rats.

Increased vascular collagen and noncollagenous protein synthesis contributes to sustain chronic phase of two-kidney, one-clip renovascular hypertension.

PURPOSE: Although the enhanced renin-angiotensin (R-A) system responsible for two-kidney, one-clip (2K-1C) hypertension is well known, there may be a shift with time so that this, hemodynamic factor plays a less important role and increased vascular resistance is predominant in sustaining hypertension. While increased vascular protein synthesis has been demonstrated in genetically hypertensive rats, we evaluated the possible relationship between vascular protein synthesis and 2K-1C hypertension with special reference to the R-A system. MATERIALS AND METHODS: Two-kidney one-clip rats were treated with splanchnicotomy, beta-aminopropionitrile (collagen inhibitor), or captopril (angiotensin converting enzyme inhibitor) in the acute or chronic hypertensive phase. 3H-proline was injected into rats, and incorporation rates of 3H-proline into vascular collagen, noncollagenous protein and elastin were counted. The plasma level of the R-A system was assayed. RESULTS: In the acute phase of 2K-1C hypertensive rats whose R-A system was enhanced, captopril treatment further enhanced plasma renin activity and plasma angiotensin I and suppressed plasma angiotensin II while reducing blood pressure. Synthesis of the vascular proteins was almost identical. In the chronic phase of 2K-1C hypertensive rats whose R-A system was within normal limits, increased incorporation rates of 3H-proline into noncollagenous protein or collagen of mesenteric arteries were decreased by splanchnicotomy or beta-aminopropionitrile and hypertension was lowered. Captopril failed to reduce protein synthesis. CONCLUSIONS: An enhanced R-A system participates in the pathogenesis of the acute phase of 2K-1C hypertension while increased noncollagenous protein and collagen syntheses of small arteries appear to play some role in the etiology of the chronic phase of hypertension.

A case of strangulated transomental hernia diagnosed preoperatively.

A patient with an unusual case of internal hernia was treated successfully with surgery for strangulated transomental hernia, diagnosed preoperatively by abdominal computed tomographic examination (CT). The herniation occurred through congenital defects in the gastrocolic omentum. Herniation of this nature is extremely rare, and abdominal CT examination is very useful for preoperative diagnosis.

Suppression of sympathetic nervous system attenuates the development of two-kidney, one-clip Goldblatt hypertension.

PURPOSE: This study tried to assess the possible contribution of the sympathetic nervous system to the onset of two-kidney, one-clip (2K-1C) Goldblatt hypertension. MATERIALS AND METHODS: The effect of chlorisondamine administration with or without subsequent splanchnicotomy on the development of hypertension was examined in 2K-1C rats with special reference to norepinephrine synthesis. RESULTS: The 2K-1C rats were treated either with chlorisondamine or chlorisondamine plus subsequent splanchnicotomy, so that the development of hypertension was effectively arrested for 4 weeks. An apparently high rate of release of norepinephrine in 2K-1C rats was reduced by treatment with chlorisondamine plus splanchnicotomy. A similar trend was also seen in plasma norepinephrine concentration and norepinephrine clearance, to a lesser extent. There were significant positive relationships between percent change of systolic blood pressure and apparent rate of release of norepinephrine, plasma norepinephrine concentration and norepinephrine clearance in 2K-1C rats and 2K-1C + chlorisondamine + splanchnicotomy rats. There were no significant relationships in these parameters in sham-treated rats. CONCLUSION: Increased sympathetic innervation appears to participate in the development of 2K-1C Goldblatt hypertension.

Chemo-endocrine therapy in patients with stage D2 prostate cancer.

There have only been a few studies of chemo-endocrine therapy compared with endocrine therapy alone in newly diagnosed prostate cancer patients. We assessed the effects of these two therapies by comparing long-term survival rates. One hundred and twenty-nine patients were entered in this study between November 1977 and March 1992. Seventy-seven patients were treated with endocrine therapy alone. Other 52 patients received chemo-endocrine therapy, which included orchiectomy and/or diethylstilbestrol diphosphate (DES-DP) plus Cisplatin, with or without other cytotoxic agents. All patients had bone metastasis at the beginning of the study. There was a significant difference in survival between patients who received endocrine therapy and chemo-endocrine therapy (P = 0.0078). That is, survival rate was superior for the chemoendocrine therapy patients throughout the entire follow-up period. These data suggest that early chemo-endocrine therapy containing Cisplatin, with or without maintenance chemotherapy, is a potentially effective treatment for newly diagnosed metastatic prostate cancer and is worth further investigation via a randomized trial.

Effects of hypothermia and hyperpotassium on alveolar fluid clearance in the resected human lung.

The effect of hypothermia and hyperpotassium on alveolar fluid clearance in the resected human lung was examined by instilling an isosmotic albumin solution with a potassium concentration of 0.3 mEq/l or 20 mEq/l into one segment of a resected lobe within 10 min of surgical removal for bronchogenic carcinoma. The experiments were carried out at 37 degrees C, 25 degrees C, and and 8 degrees C over 4 hr, after which the alveolar fluid was aspirated. Alveolar fluid clearance was calculated by a simple equation using the changes in the albumin concentration of the alveolar fluid. It was found that although hypothermia at 8 degrees C abolished alveolar fluid clearance completely, alveolar fluid clearance at 25 degrees C was not different from that at 37 degrees C. Moreover, although the potassium concentration increased in the alveolar fluid at 37 degrees C and 8 degrees C, hyperpotassium did not affect the alveolar fluid clearance. These findings indicate that the net transport of potassium leans to influx from the alveolar epithelial cells into the alveolar spaces when the alveolar potassium concentration is low, and to efflux from the alveolar spaces when the alveolar potassium concentration is high. Thus, we conclude that hypothermia abolishes alveolar fluid clearance in resected human lungs, but that the potassium concentration in alveolar fluid does not affect alveolar fluid clearance.

Use of acetylcholine (Ach) for spreading metaphase chromosomes and application to the cytogenetic analysis of human stomach cancers.

Cytogenetic studies of biopsy specimens endoscopically obtained from gastric cancers were performed in our laboratory. The necessity for well-spread chromosomes for analysis has resulted in the development of a new technique, in which culture medium containing acetylcholine (Ach) is used; with this new technique, the number of metaphases of analyzable karyotypes was significantly increased (P < 0.01 compared with a previous method in which Ach was not used). The mean ratio of metaphase numbers of analyzable karyotypes in four cases in which Ach was used was 38.1 +/- 8.1%, a value more than four times the number in the seven cases in which Ach was not used. It is possible that Ach may decrease the viscosity of the cytoplasm in gastric cancer cells by disrupting microfilaments.

Hyperbaric oxygenation therapy for cyclophosphamide-induced haemorrhagic cystitis.

A 41-year-old female patient undergoing cyclophosphamide therapy for ovarian tumour suffered from haemorrhagic cystitis. Hyperbaric oxygenation (HBO) (100% of oxygen concentration at 2 atmospheric absolute pressures for 60 minutes a day for 30 days) was performed. After treatment the symptom subsided and haematuria disappeared. Cystoscopic findings demonstrated marked improvement. During the course of therapy no side effect was noted. Thus, HBO treatment appeared to have a beneficial effect on cyclophosphamide cystitis.

Serum concentration of iopamidol after retrograde urethrography in male patients with outlet obstructive symptoms.

Serum concentration of iopamidol was measured in 29 male patients with outlet obstructive symptoms after retrograde urethrography. Although no patient showed extravasation, serum iopamidol level was detectable in 4 (14%) patients. Since severe adverse effects are considerably reduced with iopamidol, we prefer to use this contrast medium for retrograde urethrography in male patients with outlet obstructive symptoms.

Successful steroid withdrawal half a year after kidney transplantation.

We report two kidney transplant recipients with successful steroid withdrawal. They are living related donor transplant recipients. The first patient, a 37-year-old female, received the kidney from her HLA identical father. The second patient, a 44-year-old man, received the kidney from his HLA 1 haploidentical brother. Both patients were maintained on triple immunosuppressive drug therapy prior to withdrawal of steroid and subsequently were maintained on cyclosporine and azathioprine or mizoribine. Acute rejection occurred within the first 1 month and was treated with steroid bolus therapy successfully in both cases. The time of steroid withdrawal after transplantation was 6.5 months in the first patient and 5 months in the second patient. After steroid withdrawal their graft function remained stable and the graft specimens obtained by biopsy 8 months after withdrawal showed no signs of rejection; no side effects of steroid appeared. These results suggest that steroid withdrawal half a year after transplantation can be accomplished without jeopardizing graft function in selected living related donor transplant recipients.

Assessment of adrenal surgery on creatinine clearance in patients with phaeochromocytoma.

Sequential changes in creatinine clearance (Ccr) were determined in 14 patients undergoing operation for phaeochromocytoma. All patients had been given alpha-adrenergic antagonists 2 weeks before surgery. In the younger group (mean age 40 +/- 2 years), Ccr improved following adrenal surgery. Urinary concentrations of epinephrine, norepinephrine and total catecholamines correlated inversely with Ccr. A similar inverse relationship was noted between tissue norepinephrine concentration expressed as microgram/g and Ccr to a lesser extent. In older patients (mean age 69 +/- 2 years) Ccr remained unchanged by this surgical procedure. No significant correlations were found between both parameters in the older group. It appears reasonable to presume that excessive amount of catecholamine plays an important role in the impairment of Ccr in relatively young patients with phaeochromocytoma, but other possibilities as regards the sequence of this phenomenon must be pursued in older patients.

A case of microscopic testicular tumour.

A 34-year-old male was referred to us for further examination of gynaecomastia. Serum tests revealed high levels of alpha-foeto protein, human chorionic gonadotropin, oestradiol and prolactin. He had a history of testicular pain and mass, and had been under a local physician's care. With the physician's record of the course of regression we made a diagnosis of microscopic testicular tumour. Serial histopathological sections of the testis revealed a 0.8 mm lesion of chorionic and embryonal carcinoma. We discuss the course of spontaneous regression with a review of the literature.

Successful treatment of vesicoureteric reflux by subureteric injection of silicone.

Although the technique of subureteric injection has been widely accepted as an alternative to reimplantation in the treatment of vesicoureteric reflux (VUR), the choice of the material to be used is controversial. We report our experience with the endoscopic treatment of VUR by a submucosal injection of silicone. A 24-year-old man who had VUR (Grade II) and renal insufficiency was treated by endoscopic injection of silicone. Cystography after the treatment showed absence of reflux.
