Correlation between morphometric differences and norepinephrine content in benign prostatic hyperplasia.

Tissue norepinephrine (NE) content and quantitative morphometric analysis of benign prostatic hyperplasia (BPH) were evaluated in 30 patients with symptomatic BPH. BPH specimens were obtained by transurethral resection, and NE content was evaluated by high-performance liquid chromatography. The proportions of smooth muscle fibrous, and glandular elements were determined by the light microscopic stereological method. Norepinephrine content of the prostate correlated well with the proportion of smooth muscle component (r = 0.749, P < 0.0001). The percentage of fibrous tissue element was positively correlated with prostate size (r = 0.459, P = 0.0099). Norepinephrine content and histological components did not correlate with subjective symptom score. Morphometrical findings and NE content did not correlate with uroflowmetry parameters and postvoid residual urine rate. In conclusion, NE content of the prostate was probably determined by the amount of smooth muscle element in BPH tissue. The fibrous tissue element was increased in large hyperplastic tissue. The severity of BPH could not be explained by differences in histological composition alone.

Relative and total volume of histological components in benign prostatic hyperplasia: relationships between histological components and clinical findings.

To investigate the correlation between histological differences and clinical findings in patients with benign prostatic hyperplasia (BPH), quantitative morphometrical analysis on prostate was performed on 26 specimens obtained by TURP. The relative and total volume of each histological component were obtained. In this series, the proportion of fibrous tissue element showed a weak positive correlation with prostatic volume (r = 0.499, P = 0.0087). The relative volume of the smooth muscle component showed an inverse correlation with prostatic volume (r = -0.488, P = 0.0105). Patient age and symptom score showed no relationship with the relative volume of histological components. The total volume of all histological components increased with prostatic volume. However, an increase in the fibrous tissue element appeared to be predominant in prostatic enlargement (r = 0.970, P < 0.0001). Although the total volume of all histological components increased with prostatic size, fibrous tissue appeared to play a major role in prostatic enlargement in BPH.

Modified van Vaals-Bergman coaxial cable coil (lambda coil) for high-field imaging.

An easily constructed, low-capacitive coupling volume coil based on the van Vaals-Bergman coaxial cable coil for high field imaging is described. The coil (designated "lambda coil") was constructed using two 5/4 length 50 omega coaxial cables matched to a 50 omega transmission line with LC bridge balun. The standing wave on the single 5/4 lambda length coaxial cable provides two points of current maxima in oppositional direction. Therefore, the four current elements necessary for effective B1 field generation can be obtained by two 5/4 lambda length coaxial cables arranged analogous to 1/2 lambda T-antenna. Capacitive coupling between the coil elements and conductive samples (i.e. animals) is minimized by simply retaining the shield of the coaxial cable for the area of voltage maxima. The lambda coil exhibited excellent performance as a volume coil with a high quality factor and highly homogeneous rf fields. Because of its dramatically simple architecture and excellent performance, the lambda coil configuration appears to be an economical alternative to the original van Vaals-Bergman design, especially for research facilities with a high field magnet and limited bore space.

Direct effect of amezinium on rabbit urethra: effect of estrogen and progesterone treatment.

The effect of amezinium, a new anti-hypotensive agent, and hormonal treatment on the female rabbit urethra was investigated. Cumulative dose responses were obtained for amezinium and norepinephrine on strips of muscle from the urethras of ovariectomized female rabbits by means of the tissue-bath system. Amezinium enhanced the response to electrical field stimulation and showed a direct contractile response on the urethra. These responses were only about 20% of the maximum norepinephrine response. The contractile response to amezinium was completely blocked by prazosin. When rabbits were pretreated with estrogen, with or without progesterone, for 4 weeks, the response to amezinium increased to 40% of the maximum norepinephrine response. Although amezinium enhances muscle contractile responses to electrical stimulation, this effect is strong when amezinium is used alone; concurrent estrogen treatment improves the effects of amezinium.

Chromosome spreading techniques for primary gastrointestinal tumors.

The requirement for well spread out chromosomes for the cytogenetic analysis of primary gastrointestinal tumors led us to develop new techniques. These techniques involved two main procedures: (1) preliminary incubation with culture medium in the presence of collagenase, Dispase, and colcemid, for 3 h, and (2) treatment with an extremely hypotonic solution (0.044M KCl) for 30 min. The techniques were applied to 11 gastrointestinal malignancies (including 1 early gastric cancer and 1 metastatic liver lesion of colon cancer) and significant increases (P < 0.01) in the number of metaphases of analyzable karyotypes were obtained, compared with a previous method in which the standard hypotonic molarity of KCL (0.075 M) was employed. The mean value for metaphase numbers of the analyzable karyotypes was 37.0 +/- 3.7% in the 5 gastric cancers and 44.7 +/- 4.8% in the 5 colon cancers and 1 metastatic lesion. These values were three times and more than twice, respectively, the values obtained by the previous method. A fluorescence in situ hybridization (FISH) study was carried out on one cologenic tumor, the alpha-satellite centromere-specific probe 17 being used. Deletion of the long arm of chromosome 17 was demonstrated. The method proposed here could yield a sufficient number of metaphases without the use of tissue culture that might cause alteration of karyotype. It can be employed with small biopsy specimens and in studies utilizing the FISH technique.

Norepinephrine contents of human prostatic hyperplasia: differences between pathological subtypes.

Tissue norepinephrine content in benign prostatic hyperplasia (BPH) was evaluated to detect possible histological differences in BPH subtypes and to investigate the correlation between norepinephrine levels and age, prostatic weight and clinical symptom score, respectively. Specimens were obtained from 28 patients who underwent transurethral resection of the prostate. Pathologically, 18 out of 28 specimens were classified as fibromyoadenomatous hyperplasia and the remaining 10 as fibromuscular type. Norepinephrine content in the fibromyoadenomatous type was 133.1 +/- 23.1 ng/g, whereas it was 340.3 +/- 60.5 ng/g in the fibromuscular type. Norepinephrine level in the former group was significantly lower than that in the latter group (p < 0.001). In both groups, there was no correlation between norepinephrine content, age and clinical symptom score, while the norepinephrine content had a reverse correlation with prostatic weight only in the former group (p < 0.05). In conclusion, norepinephrine levels were dependent upon histological differences, especially upon the amount of smooth muscle elements, in the evaluated specimen. Severity of prostatism and patients' age showed no correlation with tissue norepinephrine content.

Chromosomal anomalies in cryptorchidism.

Chromosome analysis was performed in 160 patients with cryptorchidism. Chromosomal anomalies were found in 7 patients (4.4%). The incidence of chromosomal abnormalities was not significantly different between patients with bilateral or unilateral cryptorchidism. Of 7 patients, 1 had sex chromosomal aberration, 2 had marker chromosome and 4 had autosomal anomalies. Additional congenital anomalies were observed in 1 with sex chromosomal aberration, 2 with marker chromosome and 2 with autosomal anomaly. These facts indicate that we had better perform chromosome analysis in all patients with bilateral or unilateral cryptorchidism.

Successful embolization of a renal artery aneurysm with preservation of renal blood flow.

A 76-year-old man with renal artery aneurysm was successfully treated by transcatheter embolization with complete preservation of renal blood flow. Embolization was performed with detachable platinum coils. Two months later, follow-up angiography showed completely packed aneurysm and preserved blood flow of the branch. It may be reasonable to consider that embolization of the aneurysm is the primary treatment modality for some of the patients with renal artery aneurysm.

Prostate-specific antigen in patients with relapsed prostate cancer following endocrine treatment.

We assessed the relationship between changes in PSA and prognosis, as a possible reflection of tumour growth in patients who relapse following primary endocrine therapy. In 8 patients in whom no therapeutic change was attempted after PSA relapse, the PSA level increased exponentially. Their PSA-DT had a close positive relationship to the duration of survival after relapse (r = 0.79, p = 0.02). In 6 patients who received chemotherapy after relapse, PSA-DT between PSA relapse and initiation of chemotherapy also had a relationship to the duration of survival after relapse (r = 0.87, p = 0.05). It appears to be reasonable to conclude from these findings that the PSA-DT value is regarded to be a factor associated with prognosis in cases with an exponential increase in PSA after replase.

Role of symptom scoring and uroflowmetry in patients with diabetic cystopathy.

Forty-two patients with diabetes mellitus were evaluated by uroflowmetry and the AUA Symptom Index. After urodynamic and physiological investigations 12 patients were diagnosed as having diabetic cystopathy, while 22 patients as being urodynamically normal. In patients with diabetic cystopathy, most of the parameters in uroflowmetry were significantly different from those of normal subjects. Subjective symptom assessment revealed increased total obstructive scoring (p < 0.05). However, irritative symptom indexes in patients with diabetic cystopathy were not different from those of the normal subjects. Although uroflowmetry and the AUA Symptom Index are not specific to identify detrusor impairment, these methods appeared to be useful to decide upon further urodynamic investigations.

Derivative Y chromosome resulting from a t(Y;15) (q12;q11.2) in a boy with Prader-Willi syndrome.

Chromosome analysis of peripheral lymphocytes from a boy with Prader-Willi syndrome showed the presence of 45 chromosomes, including der(Y) resulting from a t(Y;15) (q12;q11.2). In situ hybridization using DYZ3 and D15S11 showed a positive signal and negative signal in derivative Y chromosome, respectively. The deficiency of 15pter q11.2 may not influence the clinical manifestation of Prader-Willi syndrome.

Malacoplakia of the bladder: efficacy of enoxacine therapy.

Malacoplakia of the urinary bladder was seen in a 69-year-old woman with haematuria. We firstly treated her with distigmine bromide, ascorbic acid and trimethro-prim-sulfamethoxazole. Haematuria subsided but the lesion did not change after 6 weeks. Thereafter we tried enoxacine therapy. Eight weeks after the medication the tumorous lesion disappeared. Bladder biopsy performed 4 months after the initiation of the treatment revealed predominantly granulation tissue. We can find no previous case treated successfully with long-term enoxacine.

Comparison of the nephrotoxicity between ioversol and iohexol.

To evaluate the renal toxicity of intravenous administration of ioversol and iohexol, the urinary N-acetyl-beta-glucosaminidase (NAG) to creatinine ratio was measured before and after drip infusion pyelography. Thirty patients received intravenous ioversol and 29 patients received iohexol. The urinary NAG to creatinine ratio tended to increase after respective contrast media: 5% increase in the ioversol-treated, and 8% in the iohexol-treated patients. There were no significant differences in these values; the nephrotoxicity of ioversol appeared to be just as insignificant as that of iohexol.

Electropermeabilization in bladder cancer chemotherapy.

PURPOSE: Electropermeabilization has been used for the introduction of genes into cells. Using this technique, we introduced the cytotoxic drug bleomycin (BLM) into cells and examined whether the technique might be useful for the treatment of bladder cancer. MATERIALS AND METHODS: For electropermeabilization in vitro, we used YTS-1 cells, a human transitional cell carcinoma line. Aliquots of cell suspension were mixed with a solution of BLM and immediately exposed to electric pulses. A high-power pulse generator was used to supply square-shaped pulses of 1250 V/cm (100 micros, eight pulses). After a 2-h post-shock incubation, cells were washed and incubated for one further hour. Then the concentration of BLM in the cells was measured using a bioassay. For electropermeabilization of tissue, we used normal male Wistar rats. The bladder was exposed and 10 mg/kg BLM was injected into the caudal vein. A series of eight pulses with a time constant of 100 micros at an electric field intensity of 1000 V/cm was applied. The bladder, liver and lungs were extracted 1 h later and prepared for quantification of the BLM concentration using the bioassay. RESULTS: Electrotreated cells contained significantly higher concentrations of BLM than nonelectrotreated cells. The concentration of BLM 1 h after electrotreatment in bladder tissue was 2.7 times higher than that in nonelectrotreated bladder tissue. CONCLUSION: The electropermeabilization technique has the potential to serve as a new and effective modality for the treatment of bladder cancer.
