[A case of retrocecal appendicitis occurrence during effective chemotherapy in lung cancer].

Reported is the case of 61-year-old male patient who developed an abscess, secondary to a ruptured retrocecal appendicitis, while undergoing effective chemotherapy for lung cancer. Patients undergoing cancer chemotherapy are especially susceptible to infectious complications because chemotherapy can cause myelosuppression. To our knowledge, has been no previous reported case of retrocecal appendicitis occurring during chemotherapy for the lung cancer. Retrocecal appendicitis is a dangerous form of complication occurring in patients being treated with chemotherapeutic drugs.

[Determination of various tumor markers, with special reference to sialyl SSEA-1 antigen in lung cancer].

Tumor markers including sialyl SSEA-1 antigen (SLX) were measured in 77 patients with lung cancer and 116 patients with benign respiratory diseases. Increased levels of serum SLX were observed in 57.6% (19/33) of adenocarcinoma of the lung, and an overall positive rate was 42.9% (33/77) in lung cancer cases. The false positive incidence of SLX in the sera from the patients with non-malignant diseases was as low as 8.6%. Among other tumor markers determined, serum CA 19-9, CEA had a relatively high positive rate, but the false positive incidence were not low. In cases of poor prognostic cases of with adenocarcinoma of the lung, SLX levels became levels higher than the previous ones, but not in cases of other cell types were not. Not only in the patients with lung cancer but also in those with benign respiratory diseases such as bronchiectasis and lung fibrosis, high levels of SLX seemed to suggest poor prognosis. These results indicate that SLX may contribute for monitoring and prognosis of the cases of lung cancer and other respiratory diseases.

Cardiac performance and coenzyme Q10 in thyroid disorders.

To investigate the relationship between serum levels of Coenzyme Q10 and cardiac performance in thyroid disorders, we studied the cardiac performance and assessed serum levels of thyroid hormones and Coenzyme Q10 in 20 patients with hyperthyroidism, 5 patients with hypothyroidism and 10 normal subjects. A significant inverse correlation between thyroid hormones and Coenzyme Q10 levels was found by performing partial correlation analysis. Because low serum levels of Coenzyme Q10 were found in thyrotoxic patients and congestive heart failure may occur as a result of severe hyperthyroidism, 120 mg of Coenzyme Q10 was administered daily for one week to 12 hyperthyroid patients and the change in cardiac performance was assessed. Further augmentation of cardiac performance was found in hyperthyroid hearts, which were already augmented, after the administration of Coenzyme Q10. It appears, therefore, that the Coenzyme Q10 dose actually has a therapeutic value for congestive heart failure induced by severe thyrotoxicosis.

[Dopaminergic control of aldosterone secretion in man (author's transl)].

To evaluate the dopaminergic control of aldosterone secretion, the following experiments were performed on 10 normal subjects (3 men and 7 women, aged 21 approximately 69 yrs.), 16 diabetics (8 men and 8 women, aged 20 approximately 74 yrs.) and 7 patients with untreated hyperthyroidism (2 men and 5 women aged 16 approximately 58 yrs.). Blood samples were withdrawn from an intravenous cannula indwelled in an antecubital vein at 0, 15, 30, 45, 60, 90 and 120 min after intravenous injection of 10mg metoclopramide with a volus. Plasma aldosterone levels and plasma renin activities (PRA) were measured by radioimmunoassay. In normal subjects, plasma aldosterone levels were significantly increased from basal levels of 111.8 +/- 1.3 Opg/ml (Mean +/- S.E.) to 183.4 +/- 23.3pg/ml 15 min after an intravenous injection of metoclopramide and were sustained for about 90 min. This increase induced by metoclopramide was, however, abolished by pretreatment with 2.5mg of bromocriptine. It is suggested that metoclopramide and bromocriptine are in a competitive relationship at the level of dopaminergic receptor. In diabetics, the mean plasma level of aldosterone was as low as 66.3 +/- 8.7pg/ml, which was significantly lower than that in normal subjects (p less than 0.01), and aldosterone response to metoclopramide was significantly diminished. Although this tendency was more apparent in diabetics with such complications as neuropathy or retinopathy, aldosterone response to metoclopramide implied that aldosterone secretion was under dopaminergic inhibition in this hypoaldosteronemic state. While aldosterone responded well to metoclopramide, PRA was not significantly altered in this treatment in normal subjects and diabetics. In patients with untreated hyperthyroidism, aldosterone response was similar to that in normal subjects.

[Role of the intramural ganglion cells played in the motility of the large intestine of the tortoise, Geoclemys reevesii].

The development of the intramural plexuses in the tortoise, Geoclemys reevesii was inferior to that observed in the fowl: the meshes of both Auerbach's and Meissner's plexuses were coarser, and consisted of less number of nerve fibers. Occasionally, a small ganglion was seen at the crossing points of the meshes. The average number of nerve cells contained in the ganglion was estimated to be 1500/cm2, being about half as much as that estimated in the fowl. When the mucosa of a colon in vitro was stimulated mechanically by stroking or chemically with 1/10 N HCl, the excitability of the colonic muscle was raised at the region oral to the stimulated spot, but lowered at the region anal to it. These effects were abolished by applying a ganglion-blocking agent, hexamethonium (10(-6) g/ml), to the mucosa. When the intraluminal pressure of a colon in vitro was raised from 0 to 3-12 cm H2O, the colon was distended at first but soon contracted powerfully. The contraction became extremely reduced or was abolished when hexamethonium (3.5X10(-7) approximately 1X10(-5) g/ml) was added to the saline solution in which the colon was immersed. The results indicate that the intramural ganglion cells serve as the center of the intrinsic mucosal reflex.

Clinical evaluation of CYFRA 21-1 in malignant pleural fluids.

The diagnostic value of the novel tumor marker CYFRA 21-1 in malignant pleural fluid was assessed in comparison to carcinoembryonic antigen (CEA). CYFRA 21-1 and CEA were measured in pleural fluid obtained from patients with 108 malignant and benign diseases. The levels of pleural fluid CYFRA 21-1 in malignant diseases (median: 84.5 ng/ml) were statistically higher than those in benign diseases (median: 13.9 ng/ml; p < 0.01). The CYFRA 21-1 test was able to discriminate significantly between squamous cell lung cancer and pneumonia (p < 0.01), while pleural fluid CEA levels could not. Receiver operating characteristic (ROC) curve analysis showed that the CYFRA 21-1 test has an advantage over CEA because of its higher specificity. These results indicate that measurement of CYFRA 21-1 in pleural fluid is a new tool, in addition to cytologic examination, to discriminate between malignant and benign diseases.
