[Primary carcinoma of the fallopian tube: its clinical aspects and problems in the postoperative staging system].

Clinical features of primary carcinoma of the fallopian tube were studied in 13 patients treated in Osaka National Hospital between 1961 and 1987. The relationship between clinical staging and prognosis in each patient is discussed. The incidence of this disease among all cases of malignant tumor in the gynecological department during the same 27-year period was 0.18%. The mean age of the patients was 50.7 years. Twelve patients (92%) were within the age range from 41 to 59 years, the most common age of onset. Six patients (46%) were nulligravida and 9 (69%) were nulliparous, suggesting infertility as a possible important risk factor for the disease. None of the six who underwent cervical cytodiagnosis showed tumor cells. Three were positive for cytodiagnosis by aspiration biopsy. The concomitant use of many drugs, including CDDP given postoperatively to 2 patients with advanced disease, was markedly effective. The radiotherapy carried out in 4 patients was ineffective. When the patients were staged according to the classifications of Dodson et al. and Schiller et al., there was no difference between these classifications in advanced cases. However, Dodson et al.'s classification was considered unsuitable for patients with early-stage cancerous lesions localized in the fallopian tube, while that of Schiller et al. was considered to be superior in evaluating prognosis.

Base liberation from nucleotides by superoxide and intramolecular enhancement effect of phosphate group.

The reaction of nucleotides with superoxide gave the corresponding nucleobases in good yield. The base-release reaction of nucleotides due to superoxide was examined and compared to that of nucleosides. Hence, the phosphate moiety greatly enhances the yields, in particular those of adenine from adenosine monophosphates. Superoxide in combination with the phosphate moiety has been revealed to be more active than superoxide alone. The phosphate peroxy radical, generated in situ from superoxide and phosphate, seems to be the species responsible for the formation of the free bases.

Periungual fibroma.

Two cases of periungual fibroma of the hand are described. Lesions caused by periungual fibroma place pressure on the nail matrix, which results in marked nail deformities. In both cases, the nail deformities were improved by excision of the fibroma.

Study on left atrial contractile performance--participation of Frank-Starling mechanism.

We estimated the magnitude of the left atrial contraction in 28 patients with ischemic heart disease by calculating the biplane cineangiographic volumes of the left atrium. The left atrial contraction contribution to the left ventricular stroke volume (ml/m2, Y axis) correlated inversely with the left ventricular ejection fraction (%, X axis), yielding an equation: Y = -0.31X + 43.7 (n = 28, r = -0.56, p less than 0.001). The result indicates that the left ventricular filling is compensated by the augmented atrial contraction in case of impaired left ventricular function. We defined the left atrial volume at the beginning of the atrial contraction (LAVd, ml/m2) as the "preload" of the left atrium, and we also defined the volume expelled by the active atrial contraction (delta LAVa, ml/m2) as the stroke volume of the left atrium. The stroke volume (Y) correlated positively with the "preload" (X), yielding an equation: Y = 0.48X - 1.3 (n = 28, r = 0.80, p less than 0.001). We then defined the left ventricular end-diastolic pressure, the peak of the a-wave, as the "afterload" of the left atrium, and we studied the "afterload"-stroke volume relations in the left atrium. However as expected, a reasonable relationship between them was difficult to form. Consequently, we showed that an increase in the "preload" of the left atrium augmented the stroke volume of the left atrium, which means that Frank-Starling mechanism is operative in the left atrium.

[Effects of coenzyme Q10 on ischemic myocardium during coronary artery occlusion--evaluation of the time needed to change irreversible myocardium].

We studied the irreversible change of myocardium on coronary artery occlusion using mongrel dogs. These dogs were divided into 2 groups. One group was administered CoQ10 before and after coronary artery occlusion and the other received placebo. Results are summarized as follows. 1) In both groups the hemodynamic change showed no significant change. 2) In the biochemical studies there was no significant difference between both groups, but in the infarction group, serum GPT and pyruvic acid in a sampling of blood taken from the coronary sinus were significantly higher than in the non-infarction group (P less than 0.05, 0.025). 3) By the triphenyl tetrazolium chloride (TTC) technique for the myocardial infarction, infarction did not appear in the CoQ10 group until 90 minutes after coronary occlusion but in the placebo group infarction appeared as early as 60 minutes. 4) In the ultrastructural studies of myocardium the electron microscopy detected irreversible change of mitochondrias when the TTC technique identified infarction. In conclusion, the administration of CoQ10 has a protective effect for the irreversible change of ischemic myocardium.

Microvasculature of the human testis and excurrent duct system. Resin-casting and scanning electron-microscopic studies.

The vascular microarchitecture of the human testis and excurrent duct system was examined by use of a casting method combined with scanning electron microscopy. In the testicular parenchyma, the capillaries form a fine plexus around the seminiferous tubules. The commonly described rope-ladder-like pattern of the peritubular capillaries is absent in the human testis. The microvasculature of the rete testis consists of a poorly organized plexus. Throughout the epididymis including the ductuli efferentes, the peritubular capillary network encircles each tubule cylindrically. In the cauda epididymidis, a coarse venous network surrounds the capillary network externally. Small arteries of the epididymis not only penetrate the connective tissue septa, but also run along the surface. In the ductus deferens, three distinct microvascular systems are distinguished: (1) a venous network located in the connective tissue surrounding the muscular coat, (2) a microvascular plexus within the muscular coat, and (3) a peritubular capillary network. The plexus within the muscular coat interconnects the venous and the peritubular capillary networks.

High-resolution cytogenetic studies in patients with Prader-Willi syndrome.

We investigated 24 patients with Prader-Willi syndrome by the high-resolution banding technique. Their history and clinical findings were also examined in some detail. Twelve had interstitial deletion of 15q; del(15) (q11.2q13) in 11 cases and del(15) (q11.2q12) in one case. Six revealed normal karyotypes at about 500-850 bands per haploid-set level. In an additional six cases, no deletion was detected. However, we took the results as tentative, as the observed karyotypes were at the 400-bands level. During the course of this study, it was realized that a small deletion in the proximal 15q could be easily overlooked when a mitotic spread around 400-bands or less per haploid-set level was used. There was no distinct difference in the clinical features of patients with interstitial deletion and those with a normal karyotype. Two cases in the latter group lacked some of the typical features of the former group, e.g. poor fetal vigor, neonatal feeding difficulty, hypotonia, and delayed motor development.
