Hepatic gluconeogenesis from alanine following surgery.

To clarify alanine metabolism in the liver with special reference to gluconeogenesis, catheters were placed in hepatic, portal, femoral, and external jugular veins of six male mongrel dogs. A trace amount of 14C-alanine was administered as a single pulse into the external jugular vein on the first postoperative day, and the blood samples were taken from each vein for the subsequent two hours to measure 14C-glucose radioactivity. Cumulative radioactivity after 14C-alanine injection showed that 74 per cent of the radioactivity in whole protein-free serum was that of 14C-glucose. Therefore, it is considered that the metabolic pathway of alanine in the liver after surgery is mainly through gluconeogenesis to glucose. Our in vivo experiment clearly showed that hepatic gluconeogenesis from alanine is one of the important factors related to hyperglycemia after surgery.

Effects of gaseous ammonia on Mycoplasma pulmonis infection in mice and rats.

Effects of gaseous ammonia on Mycoplasma pulmonis infection in mice and rats were investigated with special reference to growth of the organism in the respiratory organs and formation of pneumonic lesions. Multiplication of the organism was enhanced in both mice and rats exposed to 50 and 100 ppm ammonia, but the growth enhancement of the organism was not evident in the animals exposed to 20 ppm ammonia. The enhanced multiplication of the organism was especially remarkable in the lung of mice, showing a difference of about 10(4) organisms/g lung tissue between the exposed and unexposed mice. However, no formation of pneumonic lesions was observed in exposed and unexposed groups of both species of animals.

Immunofluorescence for detection of viral antigen in mice infected with Sendai virus.

Sendai virus infection transmitted by contact from cagemates was followed by virus titration and immunofluorescence. The virus grew in the respiratory tract and caused macroscopic lesions in all contact mice. The virus grew to a higher titer in the lung than in the trachea. Tracheal smears, however, were found to be the most suitable for the diagnosis of Sendai virus infection by immunofluorescence, since they contained a large number of cells with intense fluorescence. Diagnosis of Sendai virus infection was made by immunofluorescence within a few hours after autopsy made at early stages of infection.

[Multihospital randomized study on the adjuvant chemotherapy with mitomycin-C and Futraful for gastric cancer V. Estimation of 5-year survival rate].

The addition of Futraful to MMC therapy of gastric carcinoma was evaluated. Nationwide 297 institutions participated in a randomized controlled study and cumulative five-year survival rate was calculated. A total of 1,673 cases was studied. These were divided in two groups: Group A, without Futraful, and Group B, with Futraful. Futraful was administered postoperatively for 3 months. The MMC treatment was done by moderate intermittent administration (Method I) or by large dose administration (Method II). Futraful improved survival rates regardless of the method of MMC treatment. Especially, Group B' (more than 60 g of Futraful) showed significantly better results in patients with metastases and positive serosal invasion in both methods, and beta type infiltration in Method I. Furthermore the Group B' patients had higher survival than Group A patients in Method II for patients in stage III and gamma type infiltration. Method II, a large dose MMC, plus Futraful was suggestive of particular effectiveness in preventing peritoneal metastases in curatively resected cases.

[Effect of postoperative long-term chemotherapy of stomach cancer using mitomycin C and futraful--the secondary study (1)].

The Cooperative Study Group of Surgical Adjuvant Chemotherapy for Gastric Cancer in Japan with the participation of 334 institutes nation-wide had conducted the secondary study in order to investigate the usefulness of futraful in long term treatment, based on the results obtained by the primary study. The following method was taken for the study and patients were randomly divided into three groups: A group-given a high dose of MMC immediately after operation; B group-given futraful for one year after MMC administration; C group-given futraful alone for long-term. The number of eligible cases for data analysis was 3,o30. Side effects observed during the long term treatment with futraful were considered to be minor. Although the occurrence of hematocytopenia was slightly frequent because of possible increase of influence by the concomitant use of MMC, no tendency of hepatic disorders and any subjective side effects to be strengthened by MMC combination was observed at all. As for 2-year survival, an elevated survival was found in the group of concomitant use of MMC and futraful; Especially, a significantly high survival was found in the cases of stage III and n (+) . ps (+). Furthermore, the survival was elevated in proportion to the increase of total dosage of futraful. This suggested the usefulness of futraful for long term treatment.

Experimental studies on the effect of artificial respiration after lightning accidents.

Many suggestions have been made regarding protective methods against lightning, without any quantitative tests of safety. As a fundamental study on lightning protection, the lethal threshold energy for the artificial lightning impulse was investigated, using 14 rabbits. The voltage and current impulse had a 1 microsecond wave front and 40 microsecond wave tail. The imposed energy was measured by simultaneous photographic recording of both voltage and current waves on separate oscilloscopes. The impulse voltage was applied to head, neck, upper chest, and lower chest while the right hind limb was grounded. Artificial lightning current impulses were fed through the skin of the animals by making the surface flash over so that the surface flash was monitored carefully to delete the data of this case which gives an erroneous energy value from the statistical analysis. The lethal threshold energy was found to be 157.6 +/- 54.1 Joule (J), and the percentage of survival was 25% when 160 J impulse energy was given. Impulses of 160 J were then applied to the heads of 21 other rabbits maintained on an artificial respirator. Under these conditions the percentage of survival increased significantly to 47.6%.

Hepatoma in children--a clinical analysis on 15 cases.

In ten boys and five girls with hepatoma, evaluations were made as to the sex, age, pathologic findings, operation and prognoses. Ages at the time of operation ranged from 1 month to 12 years. In twelve, the tumors were successfully removed; 4 underwent left lateral segmentectomy, 4 right hepatic lobectomy and 4 trisegmentectomy. At least at the time of completion of this paper, eight of these 12 patients (66%) were alive including one patient with pulmonary metastases. Thirteen patients had hepatomas of massive type and 2 had nodular type. Histologic type of these hepatomas was hepatoblastoma in 14 cases and hepatocarcinoma (liver cell carcinoma) in one. These hepatoblastomas included 10 of the fetal type and 4 of the embryonal type. In five of 12 patients pulmonary metastases were detected after the hepatectomy. Four of these 5 patients underwent pulmonary surgery; one girl is alive with no sign of recurrence for over 8 years after surgery on both lungs. Histological examination of pulmonary lesions disclosed hepatoblastomas of an embryonal type, and such were not related to the histological types of hepatic lesions.

Neurotensin and substance P and dumping syndrome.

To investigate the pathophysiological relation between releases of gut hormones and dumping syndrome, plasma radioimmunoassayable neurotensin, substance P, glucagon-like immunoreactivity (GLI), insulin and blood sugar were measured in both gastrectomized patients and control subjects after 50 g oral glucose tolerance tests. Remarkable rises of radioimmunoassayable neurotensin and GLI were found in all gastrectomized patients, but not in control subjects. In contrast, plasma radioimmunoassayable substance P responses were not detected in either gastrectomized patients or control subjects. There were three patients with symptoms of dumping syndrome in the early stage of the test. Plasma radioimmunoassayable neurotensin responses in two out of these three were higher than those in other patients, though the other patient with symptoms had the same degree of neurotensin elevation as patients with no symptoms. In view of the pharmacological effects of neurotensin, it could not be ruled out that a part of the early symptoms of dumping syndrome may result from the remarkably enhanced plasma neurotensin release in some patients, although the enhanced neurotensin responses did not always accompany symptoms of dumping syndrome.
