Residual masses on follow-up computed tomography in patients with mediastinal non-Hodgkin's lymphoma.

During the last 4 years we have treated four patients with extensive mediastinal non-Hodgkin's lymphoma (NHL). None had extrathoracic disease and they were clinically stage I-II. All received intensive combination chemotherapy and radiotherapy resulting in good response. Nevertheless, when treatment was completed, computed tomography (CT) showed a residual mediastinal mass in all four patients, who have remained alive and well for more than 19 to 45 months after completion of therapy and without any supplemental treatment. The residual masses gradually regressed or resolved on follow-up CT, suggesting that they did not contain malignant components.

Evaluation of gamma-enolase as a tumor marker for renal cell carcinoma.

To evaluate whether serum gamma-enolase is a useful marker for renal cell carcinoma alpha and gamma-enolases in tissues of 36 renal cell carcinomas and 13 normal kidneys, and in sera of 103 renal cell carcinoma patients were determined with an enzyme immunoassay system. Tissue gamma and alpha-enolase levels were 34 and 2.3 times higher, respectively, in renal cell carcinoma than in normal renal cortex. The tissue gamma enolase-to-total enolase value of renal cell carcinoma (5.3 per cent) was significantly higher than that of normal cortex (0.29 per cent) and medulla (0.51 per cent). Over-all serum gamma-enolase levels were elevated (more than 6.0 ng. per ml.) in 53 of 103 patients (51 per cent) with renal cell carcinoma. In regard to stage the positive rates were 34 per cent (12 of 35) of patients with stage I, 22 per cent (2 of 9) with stage II, 80 per cent (12 of 15) with stage III, 61 per cent (22 of 36) with stage IV and 61 per cent (5 of 8) with recurrent disease. The mean value of serum gamma-enolase in renal cell carcinoma (8.0 +/- 5.7 ng. per ml.) was significantly higher than that of normal subjects (3.1 +/- 0.9 ng. per ml., p less than 0.001). The mean value of serum gamma-enolase in patients with high stage tumors (III and IV, 9.9 +/- 6.8 ng. per nl.) was significantly higher than that of low stage tumors (I and II, 5.8 +/- 3.0 ng. per ml., p less than 0.001). In 39 patients treated by complete surgical excision serum gamma-enolase was significantly reduced postoperatively (p less than 0.01). Furthermore, 7 of 8 patients whose serum gamma-enolase levels were determined serially had levels within the normal range postoperatively that increased when distant metastases appeared. These results indicate that serum gamma-enolase could be a useful tumor marker to stage disease and monitor treatment in patients with renal cell carcinoma.

Increased clearance of plasma cholesterol after injection of apolipoprotein E into Watanabe heritable hyperlipidemic rabbits.

Apolipoprotein E (apoE) is known to play an important role in lipoprotein metabolism. We have studied the effect of apoE on the metabolism of plasma cholesterol by injecting apoE intravenously into rabbits deficient in low density lipoprotein receptors [Watanabe heritable hyperlipidemic (WHHL) rabbits]. Approximately 30 mg of apoE was injected per rabbit; a total of five WHHL rabbits were used. One hour later, plasma cholesterol levels fell 8.3% (from 488 +/- 192 to 446 +/- 174 mg/dl). After 3 hr, cholesterol levels had fallen by 19% (to 392 +/- 152 mg/dl). The reduced levels were maintained for at least 8 hr after injection of apoE. Cholesterol in very low density lipoproteins (VLDLs) and intermediate density lipoproteins fell rapidly during the first 2 hr after injection, followed by a reduction in the low density lipoprotein cholesterol level. Changes in apolipoprotein B levels in each lipoprotein fraction were very similar to those of cholesterol. Plasma apoE levels 3 min after injection were elevated 3-fold to 22.8 +/- 6.3 mg/dl and returned to initial levels 8 hr after injection. The rate of removal of intravenously injected 125I-labeled VLDL that had been incubated with apoE was 3-fold higher than that of unmodified VLDL. From these results, we conclude that the injected apoE is incorporated into VLDLs and that VLDL particles carrying more apoE are removed from the blood more rapidly, resulting in reduced formation of low density lipoprotein and lowered cholesterol levels.

Anti-hypercholesterolemic effect of melatonin in rats.

The effects on plasma lipids of daily intraperitoneal injections of 4 mg of melatonin (N-acetyl-5-methoxytryptamine) for 10-27-day periods were examined biochemically and morphologically in rats fed regular and high-cholesterol (1% cholesterol, 0.5% cholic acid) diets. Melatonin administration had no significant effect on plasma lipids and lipoproteins in the rats on a normal diet but blunted the effects of a high-cholesterol diet on these parameters. No effects of melatonin on lipase activity were noted. Melatonin also diminished the fatty infiltration in the liver of animals on the high-cholesterol diet. The high-cholesterol diet produced major increases in VLDL and LDL cholesterol and protein content, and decreases in HDL cholesterol and protein. Melatonin decreased the extent of this plasma lipoprotein increase, although it did not completely prevent the phenomenon. Therefore, the effect is thought to be quantitative and not qualititative in nature.

Multivariate evaluation of determinants affecting regional lymph node metastasis and survival in bladder cancer patients who underwent radical cystectomy.

To elucidate the relative importance of clinicopathologic factors affecting regional lymph node metastasis and survival in bladder cancer patients, multivariate analyses by the logistic regression model and proportional hazards model were performed for 86 patients who underwent radical cystectomy between 1978 and 1988. Clinicopathologic factors included in the analysis were sex, age, prior tumor history, time from onset of symptoms to cystectomy, and tumor characteristics (size, number, growth pattern, grade, stage, infiltration pattern, lymphatic invasion, lymphocytic infiltration around tumors, and lymph node metastasis). Nineteen of eighty-six patients (22%) had regional lymph node metastasis. Univariate analysis showed that lymph node metastasis was related to stage (p = 0.0006), lymphatic invasion (p = 0.006) and infiltration pattern (p = 0.02). Multivariate analysis revealed that stage is the only determinant of statistical significance for lymph node metastasis. High-stage tumors (pT3a-pT4) had 15 times higher risk for lymph node metastasis than low-stage tumors (pTis-pT2). The 5-year survival rates were 25 and 75% for patients with and without lymph node metastasis, respectively. Proportional hazards model revealed that stage is the most significant determinant (p = 0.0001) for survival, followed by lymph node metastasis with borderline significance (0.005 less than p less than 0.1). A two-factor model consisting of stage and lymph node metastasis yielded corrected hazard ratios of 14.7 for stage and 2.3 for lymph node metastasis. The present study quantitatively confirms previous univariate analyses of factors affecting lymph node metastasis and survival in patients undergoing radical cystectomy.

Comparison of the effects of various inducers on 7-alkoxycoumarin O-dealkylase activities in liver microsomes.

The effects of six inducers and malotilate on 7-alkoxycoumarin O-dealkylase activities in rat liver microsomes were examined. Phenobarbital (PB) (100 mg/kg) was administered intraperitoneally to rats for 6 days; 3-methylcholanthrene (3-MC) (40 mg/kg), beta-naphthoflavone (beta-NF) (40 mg/kg), isosafrole (150 mg/kg) and polychlorinated biphenyls (PCB) (100 mg/kg) were administered intraperitoneally for 3 days; isoniazid (INH) (50 mg/kg) was administered intraperitoneally for 10 days; and malotilate (500 mg/kg) was administered orally for 3 days. The O-dealkylase activities toward 7-methoxycoumarin (7-MC), 7-ethoxycoumarin (7-EC) and 7-propoxycoumarin (7-PC) were examined 24 hr after the final administration of the drugs. The ratios of 7-EC O-deethylase and 7-PC O-depropylase to 7-MC O-demethylase activity in the control and six inducer-treated groups were compared. The ratios in the groups treated with the six compounds, each of which induces a different form(s) of cytochrome P-450 (P-450), were clearly different from each other. Therefore, the measurement of 7-alkoxycoumarin O-dealkylase activities should be extremely useful for the routine determination of the molecular species of P-450. On the other hand, the ratio in the malotilate-treated group was different from that in any other inducer-treated group, so that there might be a possibility that malotilate induced a form(s) of P-450 that is different from any of the already known species.

Teratoma of the spinal cord. A case report with CT scans.

Teratomas in the spinal cord are rare. We report a case in a 54-year-old man. CT scans revealed tumours of different densities within the spinal canal; this heterogeneity may help to differentiate teratoma from other spinal cord tumours. After resection of the tumours under microscopy, the result was excellent. Histological examination showed a variety of tissues, including elements of all three germinal layers.

Maleness as risk factor for slowly progressive IDDM.

The effect of sex on longitudinal changes in serum C-peptide immunoreactivity (CPR) response to the oral glucose tolerance test (OGTT) was examined up to 48 mo in 30 islet cell antibody-positive (ICA+), non-insulin-dependent diabetes mellitus (NIDDM) subjects (15 men, 15 women) who were matched for age, duration of diabetes, and mode of treatment. The subjects were recruited from 2858 NIDDM patients screened for ICA between 1980 and 1984. In male NIDDM subjects, CPR levels to OGTT decreased insidiously, and 8 of 15 men developed the insulin-dependent state with abolished CPR. Only 2 female NIDDM subjects progressed to the insulin-dependent state (P less than .05, women vs. men). Thus, CPR in female subjects tended to decrease less than in male subjects. There were no significant differences between the two groups in human leukocyte antigens (HLA) or titer of ICA during the follow-up period. These results suggest that maleness is a major risk factor for slowly progressive beta-cell dysfunction in adult-onset insulin-dependent diabetes mellitus (IDDM).

[Prognosis of the prostatectomy of benign prostatic hyperplasia].

Surgery of benign prostatic hyperplasia has been performed in 484 cases during a period of 10 years from January 1978 to December 1987; these 484 cases comprised 345 to transurethral resection, 130 of subcapsular extirpation and 9 of cryosurgery. Their period of prognosis was examined; 10-year survival rate was 88.9, 95.7, 60.4, 55.8 and 42.8% for 55-59, 60-64, 65-69, 70-74 and 75-79 years, respectively, 8-year and 7-year survival rates being 31.7 and 66.7% for 80-84 85-89 years, respectively. Survival rate by age bracket was compared in terms terms of electrocardiogram (ECG), pulmonary function and possible anomalies and operative blood transfusion. The group of patients with abnormal ECG and pulmonary function showed a significant decreasing survival rate with aging. The aged patients showed no difference in survival rate according to possible blood transfusion. Examination of the cause of death revealed predominant involvement of cardiopathy and cerebrovascular disorder, with comparatively less due to cancers. In terms of postoperative conditions of life of these patients and a long period prognosis of their urination, more 75% of them are living in good condition and 70% are in a state of satisfactory urination.

[Treatment of gonococcal urethritis with norfloxacin].

Fifty six males with gonococcal urethritis were administered 600 mg of norfloxacin (NFLX) daily for 7 days, before the effects were examined. The pharmaceutical effects were studied on the third and the seventh day after administration. Pronounced effectiveness of 77.8%, effectiveness of 19.4% and ineffectiveness of 2.8% were found on the third day after administration, and pronounced effectiveness of 94.4%, effectiveness of 5.6% and ineffectiveness of 0% were found on the seventh day after administration. Effectiveness of 100% was obtained up to the seventh day. Minimum inhibitory concentration (MIC) of NFLX and ampicillin (AMPC) were determined for 14 strains. The peak of MIC of NFLX was found at 0.05 micrograms/ml and all strains were 0.78 micrograms/ml or lower. The peak of MIC of AMPC was 0.1 microgram/ml. MICs of both drugs were 0.78 micrograms/ml or lower, and resistant bacteria were not found. In view of the analyses of age distribution of patients and source of infection, the peak was found in the twenties and the source of infection in 64.7% were, so-called, professional women.

[IgM myeloma without depression in serum IgG and IgA--a case report].

A 64-year-old woman with a monoclonal gammopathy was admitted to Nagoya National Hospital with the complaint of occasional hemoptysis. On examination, there was no hepatosplenomegaly or no lymphadenopathy. The hemoglobin was 10.1 g/dl; platelets 22.5 X 10(4)/microliters; white blood cells 4.9 X 10(3)/microliters, with 4% of atypical lymphocytes. Immunoglobulin analysis of the serum by immunodiffusion revealed an IgG of 1,459 mg/dl, an IgA of 219 mg/dl, and an IgM of 5,091 mg/dl. Serum viscosity was 4.9. Serum immunoelectrophoresis demonstrated atypical precipitant arcs reacting with mu and kappa antisera. Urine immunoelectrophoresis showed a positive reaction against kappa antiserum. Radiologic studies of the bones revealed generalized osteoporosis with multiple punched out lesions of the skull. Thirty percent of bone marrow nucleated cells was atypical plasma cells, the presence of which was verified by electron microscopy. Although they were positive mainly for cytoplasmic mu and kappa chains by immunoperoxidase studies, cells positive for gamma, alpha, or lambda chains were occasionally found, indicating that normal immunoglobulin synthesis was not suppressed in this case of IgM myeloma.

Composition of very-low-density lipoproteins in non-insulin-dependent diabetes mellitus.

We studied the characteristics of very-low-density lipoproteins (VLDLs) composition in patients with non-insulin-dependent diabetes mellitus. VLDLs were isolated from 28 diabetics and 33 nondiabetics with a wide range of plasma lipid concentrations, so we could compare VLDL composition between the two groups at similar lipid concentrations. The mean cholesterol and triglyceride concentrations in plasma of diabetic subjects were 2170 (SD 570) and 2510 (SD 2230) mg/L, respectively, and 2230 (SD 560) and 2210 (SD 1830) mg/L in nondiabetics. Diabetic subjects showed positive correlations between plasma triglyceride concentrations and either preprandial blood glucose or glycated hemoglobin values (P less than 0.02 and less than 0.01, respectively). Concentrations of triglycerides, cholesterol, and apolipoproteins (apo) B, CII, CIII, and E in VLDL were not significantly different between diabetics and nondiabetics. At comparable triglyceride concentrations, the chemical compositions of the VLDL in the two groups were still very similar. Dividing the concentration of each component by the VLDL-apo B concentrations yielded similar contents of each component per unit of VLDL for the two groups. We conclude that the composition of VLDLs in diabetics is much the same as that in nondiabetics at comparable concentrations of triglycerides in plasma.

[Clinical study of estramustine phosphate (Estracyt) on prostatic cancer].

Clinical effect of Estracyt was investigated in prostatic cancer patients. Twenty seven patients had been previously treated and 20 had not received prior treatment. Improvement rate of subjective symptoms was 85% in the previously untreated patients and that of objective findings was 85%, while those rates were 44% and 50% in the previously treated patients, respectively. Most of the adverse reactions were changes in mamma and mammary papilla which were considered to be due to the estrogenic activity.

[Clinical observation of 8 cases of carcinoma in situ of the urinary bladder].

We encountered 8 patients with primary carcinoma in situ of the bladder. All the cases had cystic irritation symptoms, and diagnosis was confirmed by urinary cytologies and transurethral biopsy. Total cystectomy was performed in 5 of the 8 cases during the course of disease. Bacillus Calmette-Guerin (BCG) instillation therapy was instituted in 4 cases, in two cases of which bladder malignancies were abolished after BCG instillation therapy. In one case treated by total cystectomy, histopathological study disclosed malignancies that invaded to the bladder musculature.

[So-called Ph1-negative chronic myelogenous leukemia with a mosaic pattern of trisomy 8 and normal karyotypes--report of 2 cases].

Two cases of Ph1-negative chronic myelogenous leukemia (CML) are described, they were 66-year-old female and 73-year-old male. Both patients shared all of the following features: presence of anemia, thrombocytopenia and leukocytosis with every stage of neutrophilic differentiation, hypercellular bone marrow with hyperplasia of the degranulated neutrophilic series, diminished neutrophilic alkaline phosphatase, elevated serum lysozyme and vitamin B12 level, mosaic pattern of trisomy 8 and normal karyotypes in chromosome analysis, and markedly increased number of CFU-GM. In addition, bcr rearrangement by Southern blot hybridization was not demonstrated in these patients. The diagnosis of chronic myelomonocytic leukemia was not verified, however, because of the absence of monocytosis in peripheral blood. The existence of so-called Ph1-negative CML like these two cases as a diagnostic entity must be further studied.

[An analysis of 30 patients with renal trauma: diagnostic value of radiographic examinations].

To evaluate the diagnostic value of radiographic examinations in renal trauma, 30 patients treated between April, 1985 and March, 1988 were examined. The 25 male and 5 female patients ranged in age from 10 to 74 years, with 17 (57%) less than 30 years of age. Causes of injuries were traffic accidents in 19 patients (63%), falls in 5 (17%), sports in 2 (7%), and others in 4 (13%). Twenty-nine patients (97%) had hematuria and 3 (10%) had shock due to renal injury. The presence of shock indicated severe renal injury, but the degree of hematuria did not correlate to severity of injury. Of the 30 patients, 18 (60%) had renal contusions, 7 (23%) had minor lacerations, 4 (13%) had major lacerations and one had vascular injury. One patient had penetrating trauma and the other 29 had blunt trauma. Two patients (7%) had pre-existing renal anomalies. Associated injuries were present in 16 patients (53%). The presence of associated injuries had no relation to the severity of the renal injury. Computerized tomography (CT) with contrast medium was more sensitive to abnormal findings, especially subcapsular and perirenal hematoma, than excretory pyelography. Findings of renal angiography provided additional anatomical information for deciding immediate surgical treatment and for selecting operative methods for salvage of the kidney.(ABSTRACT TRUNCATED AT 250 WORDS)
