Squamous cell carcinoma of the cervix with extensive superficial spreading to almost whole genital tract and associated with endometrial stromal sarcoma.

A case of simultaneous squamous cell carcinomas of the uterine cervix, endometrium, left fallopian tube, left ovary, vagina and vulva and a stromal sarcoma of the endometrium is reported. Histologic examination revealed that a squamous cell carcinoma of the cervix mechanically displaced almost whole genital tract in a superficial spreading pattern. The superficial spreading lesions showed fairly well arranged stratification for a carcinoma.

Production of beta-human chorionic gonadotropin by germ cell tumors in vivo and in vitro.

Production of the beta-subunit of human chorionic gonadotropin (beta-hCG) by human germ cell tumors was studied in surgical specimens, cultured cells and xenografted tumors. beta-hCG was identified most frequently in the syncytiotrophoblastic and intermediate trophoblastic components of choriocarcinoma, and it was also detected in syncytiotrophoblastic giant cells, occasionally in association with embryonal carcinoma, and less frequently with yolk sac tumor, seminoma and dysgerminoma. Our experimental data suggest that beta-hCG production by syncytiotrophoblastic giant cells in association with embryonal carcinoma is a result of trophoblastic differentiation of embryonal carcinoma. Moreover, it was proved that the production of beta-hCG by human embryonal carcinomas is regulated by more factors than is the case for human choriocarcinomas.

[Squamous cell carcinoma with a sarcomatoid variant of the vagina].

A case of squamous cell carcinoma with a sarcomatoid variant, usually called a spindle cell carcinoma, located in the vagina of a postmenopausal woman is reported. Although a cytologic examination of a smear of the scraped tumor showed the coexistence of sarcomatous spindle and keratinizing carcinomatous cells, the first bioptic specimen consisted only of sarcomatous tissue. Further histologic, immunohistochemical and ultrastructural examinations succeeded in making a definite diagnosis of a squamous cell carcinoma with a sarcomatoid variant. We would like to emphasize that combined cytologic and histologic examinations led us to make a correct diagnosis after discussing the differential diagnosis.

High production of type VI collagen in multiple fibromatosis with multiple articular dysplasia.

A patient with multiple fibromatosis occurring at the sites of multiple cartilagenous dysplasia was described. Collagen types solubilized with pepsin from the fibromatous tissue were fractionated by a different salt concentration and analyzed by SDS-polyacrylamide gel electrophoresis, which indicated that the tissue produces predominantly "short-chain" collagen. Western blotting of the subunits indicated a cross reaction with antisera of the type VI collagen. The results of rotatory shadowing electron microscopy confirmed the characteristic short-chain structure.

Human testicular germ cell tumors in vitro and in athymic nude mice.

Four cell lines derived from pure and mixed types of human testicular germ cell tumors in vitro and in nude mice were examined by light and electron microscopies. The NEC8 and NEC15 cell lines in vitro were composed of embryonal carcinoma cells with potentiality of trophoblastic differentiation. All of the tumors formed in nude mice by both cell lines were pure embryonal carcinomas. On the other hand, the NEC14 and ITO-II cell lines showed morphological differentiation from embryonal carcinoma cells to trophoblastic cells and to yolk sac tumor cells in vitro. In nude mice, these cell lines formed mixed tumors which consisted of embryonal carcinoma, yolk sac tumor, immature teratoma or trophoblastic cells. Our data suggest that some embryonal carcinomas have multipotentiality of morphological differentiation but that others have little such potentiality.

Production of alpha-fetoprotein by human germ cell tumors in vivo and in vitro.

The production of alpha-fetoprotein (AFP) by human germ cell tumors was studied in surgical specimens, cultured cells and transplanted tumors in nude mice. AFP was detected most frequently in yolk sac (endodermal sinus) tumors, and it was also detected, though only occasionally, in embryonal carcinomas and in teratomas. AFP-positive cells in teratomas presented a glandular or hepatoid appearance. Our experimental data suggest that AFP production in embryonal carcinomas is due to the functional expression of yolk sac differentiation preceding its morphologic expression. Although the transplanted tumors maintained the productivity of AFP, the cultured cells were not successful in maintaining a continuous AFP production.

The role of chloride on deoxycorticosterone acetate-salt hypertension.

Selective sodium loading attenuated the development of hypertension in the deoxycorticosterone acetate (DOCA) treated rat. The DOCA treated rat fed a diet equimolar in sodium to a 7% sodium chloride diet and in chloride to a standard diet, differed in various parameters from the DOCA treated rat fed a 7% sodium chloride diet: it had higher sodium concentration in both erythrocytes and muscles, a higher erythrocyte ouabain sensitive 22Na efflux rate constant (Kos), and a lower norepinephrine turnover rate in the heart and the spleen. These results suggest that the suppressed sympathetic nervous system activity and the activated cell membrane sodium pump contribute in part to the mechanism for the suppression of the development of hypertension in the DOCA-selective sodium loaded rat.

Collagen species in human aorta: with special reference to basement membrane-associated collagens in the intima and media and their alteration with atherosclerosis.

Collagen species, especially basement membrane or short chain types, of human aorta extracted by limited and repeated pepsin digestions were investigated as to their type dependent solubility by sodium dodecyl sulfate-polyacrylamide gel electrophoresis after differential salt precipitation and cyanogen bromide cleavage. Basement membrane-associated collagens, i.e., types IV, V and VI, were more extractable in the earlier limited pepsin digests than in the later repeated ones, whereas type I collagen was continually extracted. The content of collagen type IV as well as that of type V increased with the proliferative fibrotic process in the intima including the media. Basement membrane collagen type IV showed heterogenous molecular sizes, including alpha 1(IV) and alpha 2(IV) chains. The alpha 3(V) chain of type V collagen was found in addition to the alpha 1(V) and alpha 2(V) chains. Short chain collagen type VI was detected in the intima and media at a position corresponding to 145 Kd and was reduced to fragments of around 45 Kd.

Immediate and early postoperative evaluation of results of cardiac surgery by transesophageal two-dimensional Doppler echocardiography.

In cardiac surgery significant residual lesions increase postoperative morbidity and mortality. Although intraoperative epicardial real-time two-dimensional Doppler echocardiography (two-dimensional Doppler) is an accurate and efficient technique for assessing the presence and severity of a residual lesion, it requires placement of a transducer in the operating field and consequent obstruction of the operative procedure. Transesophageal two-dimensional Doppler echocardiography (transesophageal two-dimensional Doppler), which can be applied intraoperatively and postoperatively without such problems, was performed in 35 patients during cardiac surgery (12 patients) and/or at an intensive care unit within 6 hr after cardiac surgery (30 patients). In those with constrictive pericarditis, an extensive pericardiectomy was performed with effective monitoring by intraoperative transesophageal two-dimensional Doppler. Abnormal posterior wall motion was observed in the case of left ventricular rupture (type III) after mitral valve replacement 3 hr before clinical manifestation. After prosthetic valve replacement (18 St. Jude Medical valves, one Carpentier-Edwards valve, one Björk-Shiley valve), no perivalvular leakage was detected, but minor physiologic transvalvular leakage was noticed in 11 patients with St. Jude Medical valves. In two patients with congenital heart disease, a small residual shunt was detected. In a patient in which a composite valve graft with direct coronary artery reattachment (Bentall's operation) was performed, reattachment was confirmed to be satisfactory. In conclusion, intraoperative and early postoperative monitoring of cardiac function by transesophageal two-dimensional Doppler echocardiography can improve the results of cardiovascular surgery by providing accurate information on cardiovascular structure and blood flow dynamics.

Comparison of seven cell lines derived from human gastric carcinomas.

In an attempt to elucidate various histological features of gastric cancers, seven human gastric adenocarcinomas were studied in vitro and in nude mice. Growth pattern of each cultured cell line in vitro corresponded well to the histological type of parent tumor. The cell lines, MKN7, MKN74, and MKN28 derived from differentiated carcinomas showed morphological characteristics of intestinal differentiation in cell polarity and microvilli with core-filaments in vitro as well as in nude mice. However, they gradually diminished the characteristics in course of time. The cell lines, MKN 45 and OKAJIMA, derived from undifferentiated carcinomas, had natures of not only ordinary gastric mucosa but also intestinal metaplastic mucosa. They seem to have multipotentiality for differentiation, and preserved well the natures for long periods of culture. The KWS-I cell line composed of undifferentiated cells in vitro displayed the potential for differentiation in nude mice. However, the differentiation of KATO-III cells derived from a signet-ring cell carcinoma was suppressed in nude mice. The common abnormality of chromosome was not found, and the growth rate in vitro was not dependent on the histological type of parent tumor.

Embryonal rhabdomyosarcoma in nude mice and in vitro.

A transplantable tumor strain designated YNnu and a cultured cell line designated YN were established from the human paratesticular rhabdomyosarcoma of a 15-year-old boy. In vitro the cells showed ultrastructural features of immature mesenchymal cells, and a few cells were labeled by antibodies to myoglobin and/or desmin. In nude mice, the cells became to contain numerous myofibrils with Z-bands, and considerable number of cells reacted with anti-myoglobin and/or anti-desmin antibodies. We conclude that spindle-shaped or small round cells are the most immature rhabdomyoblast with multiplicity for cellular differentiation.

Characterization of human embryonal carcinoma cell lines derived from testicular germ-cell tumors.

Four human embryonal carcinoma (EC) cell lines (ITO, NEC 8, NEC 14, NEC 15) derived independently from testicular germ-cell tumors were established in vitro. In their xenografted tumor tissues, all of them exhibited histological characteristics consistent with EC. The cell-biological characterization of these human EC cell lines was investigated with reference to well-known murine EC cell lines. This included examination of their morphology, growth, tumorigenic potential, karyotype, cell-aggregate formation, HLA expression, large glycopeptides, AFP and HCG production, plasminogen-activator secretion, and LDH profiles. Three (ITO, NEC 14, NEC 15) of these human EC cell lines shared cell-biological characteristics consistent with typical EC, but one of them (NEC 8) differed from the others with respect to its rapid growth, high tumorigenic potential, formation of solid cell aggregates, and less differentiated, solid histological pattern. Thus, it is suggested that there are several developmentally different types of human EC cells. The relationship between the properties of these human EC cell lines and their differentiation potential is discussed.

Endodermal sinus tumor of the stomach.

This is a case report of an endodermal sinus tumor arising in the stomach of a 72-year-old Japanese woman. The tumor showed Borrmann 2 type at the mucosal surface, and the main mass occupied the subserosal layer with outgoing extension. Microscopically, the tumor showed typical characteristics of the endodermal sinus tumor, associated with embryonal carcinoma, poorly differentiated adenocarcinoma and well differentiated tubular adenocarcinoma. Alpha-fetoprotein was detected in the serum of the patient by radioimmunoassay and within tumor tissue by immunohistochemical techniques. Ultrastructurally, a characteristic basement membrane-like material has been identified in intercellular spaces.
