[Chronic empyema: key-point and results of our one-step operation].

A new operative method named as "Extraperiosteal air plombage" against chronic empyema were performed to 80 patients and its cure rate by the first operation was 93.8% which is a good and high rate. Know-how to perform the operation without failure is (1) to complete the excoriation of the pleural debris, (2) to seal completely the leakage such as bronchiolar fistula, (3) to perform the extraperiosteal abbration rather over much to attach the lung to the muscle layer, (4) to close the chest wall completely, and (5) to wash out thoroughly the extraperiosteal cavity with brush and enough saline solution and to use the effective antibiotica.

[A case of malignant schwannoma of the mesentery in a patient with von Recklinghausen's disease].

A 23-year-old man with Von Recklinghausen's disease was admitted because of abdominal pain. The preoperative diagnosis was malignant mesenteric tumor. Laparotomy was done and the tumor was located in the mesentery of the ileum which showed numerous tortuous cords or bulbous projections from the surface. Partial resection of the involved ileum and the mesentery was performed. Histological examination revealed plexiform neurofibroma and malignant Schwannoma of the mesentery. It is very difficult to preoperatively diagnose early associated lesions of Von Recklinghausen's disease. Therefore, careful examination of the GI tract is important so that earlier therapy can be carried out.

[Sellar fibrosarcoma following radiotherapy for prolactinoma].

A 51-year-old house woman visited the Department of Neurosurgery, Osaka University Hospital for examination of the head injury on Oct. 7, 1975. Neurological examination was normal. Endocrinological examination showed galactorrhea. The patient had a past history of premature menopause. Plain skull films revealed enlargement of sella turcica and CT scan showed sellar tumor with suprasellar expansion which was enhanced by contrast medium. The serum prolactin (PRL) level was 3,290 ng/ml. Diagnosis of PRL secreting pituitary adenoma (prolactinoma) was made. Though surgical removal of the tumor was recommended, it was refused by the patient. Therefore, careful observation was continued as an out-patient until May 1979 when she noticed a temporal hemianopsia of her left eye. She was admitted and had partial removal of the tumor via frontal route and subsequent irradiation (total dose of 5,000 rad by Lineac). The tumor was verified to be a prolactinoma by the immunohistochemical staining. Postoperative course was uneventful and she lead a normal life. In Oct. 1981, severe faceache began and she was readmitted. Sella was destructed extensively and CT scan revealed a hugh sellar tumor with multi-directional extrasellar extension which was less enhanced than that of the first study. The serum PRL level was 588 ng/ml and the regrowth of prolactinoma was suspected. High dose bromocriptine (40 mg/day) therapy was started. The serum PRL level rapidly fell to the negligible value, however, shrinkage of the tumor was not observed. On Jan. 20, 1982, suddenly she developed a left hemiparesis and her level of consciousness gradually deteriorated. On Mar. 11, 1982, the second operation was performed and a solid firm tumor in the base of the skull was partially removed. The tumor was histologically verified to be a fibrosarcoma. After the second operation bromocriptine therapy was discontinued, however expected elevation of the serum PRL level was not recognized. She died on Apr. 4, 1982.

Inhibition of oxidative metabolism in rabbit polymorphonuclear leukocytes by triphenyltin chloride.

Treatment of rabbit polymorphonuclear leukocytes (PMN) with triphenyltin chloride (TPTCl) inhibited chemiluminescence generation stimulated by particulate stimulus, zymosan, or soluble stimuli, concanavalin A + cytochalasin D. Superoxide anion (O-2) production was also inhibited, indicating that the inhibition involved inhibition of early oxidative metabolic process(es). The direct inhibition of the activation process of the oxidative burst was established by the experiments showing that a) chemiluminescence generated by xanthine oxidase-acetaldehyde system was not inhibited by TPTCl, b) washing the PMN after the treatment with TPTCl did not affect the results of chemiluminescence, and c) there was no change in cell viability after the treatment with TPTCl.

Acquired B antigen and polyagglutination in a patient with gastric cancer.

Erythrocytes from a patient with blood type of A1 became nongenetically reactive with A (anti-B) serum, following the development of gastric cancer. Transient polyagglutinability was also apparent. The in vitro acetylation of the erythrocyte antigens abolished both the acquired B antigen and the polyagglutination. Although incubation of the heterologous type A1 erythrocytes with the patient's serum did not produce B antigen, deacetylase activity in the serum seems to relate to the acquired B antigen. In this case, either ileus as a result of metastatic adenocarcinoma of the stomach, or urinary tract infections could be the cause of the acquired B antigen.

Glutamate inhibitors in the crayfish neuromuscular junction.

1. The effects of chlorisondamine and TI-233 on the crayfish neuromuscular junction were investigated in order to compare the action of glutamate with that of the excitatory transmitter. 2. The glutamate-induced synaptic current was inhibited by both of these two drugs. Excitatory junctional potentials were significantly reduced by chlorisondamine, whereas they were increased by TI-233. 3. It is suggested that chlorisondamine and TI-233 are powerful non-competitive antagonists for glutamate. 4. A quantum analysis of extracellular EJPs demonstrated that chlorisondamine did not possess presynaptic action in the crayfish neuromuscular junction. Chlorisondamine shortened the decay phase of extracellular EJPs, and the decay was frequently fitted by a double exponential in relatively low concentrations. 5. Semilogarithmic plots of the decay phase of the glutamate current evoked by a short glutamate pulse were nearly linear, but they shifted from linearity to some extent in the presence of chlorisondamine, showing prolongation of the glutamate current tails. 6. When TI-233 was added to the bathing solution at a concentration of 0.1 mM, the quantum content of extracellular EJPs was increased by about two times, but the average unit size was not changed. 7. There was no change in the rise time and the decay phase of the glutamate potential in the presence of TI-233. 8. Pharmacological difference between glutamate responses and EJPs was revealed in the presence of chlorisondamine and TI-233. Unless this difference can be explicated with a reasonable explanation on the glutamate transmitter hypothesis, it is difficult to confirm that glutamic acid is an excitatory transmitter at the crayfish neuromuscular junction.

Receptors for peanut agglutinin isolated from cell lines of human Burkitt lymphoma, lung squamous cell carcinoma and gastric signet ring cell carcinoma.

By affinity chromatography on peanut agglutinin-agarose, receptors for peanut agglutinin were isolated from three lines of human malignant cells labeled with [3H]-galactose. The cell lines examined were a Burkitt lymphoma line Daudi, a lung squamous carcinoma QG-56 and a gastric signet ring carcinoma KATO-III. The isolated receptors from the three sources had similar properties and behaved as glycoproteins with apparent molecular weights of 160,000 to 280,000 upon sodium dodecyl sulfate gel electrophoresis. However, they showed molecular weights of only around 70,000 upon gel filtration, suggesting that they contained large amounts of carbohydrates. Pronase digestion depolymerized the glycoproteins, though significant amounts of the products were large enough to be excluded from a column of Sephadex G-50. The large molecular weight products were not detected when the receptors were treated with mild alkali and NaBH4 prior to pronase digestion. Major components of the radioactive oligosaccharides and glycopeptides released in the latter case had molecular weights in the range from 3,500 to less than 1,000. These results indicate that the glycoprotein receptors had clusters of oligosaccharide chains of small and medium sizes.

Effect of imipramine on central 5-hydroxytryptamine turnover and metabolism in rats.

The levels of [3H]-5-HT and [3H]-5-HIAA in P2-fraction from rats treated with a single dose of imipramine were lower at 2, 5 and 10 min after intraventricular administration of [3H]-5-HT, compared with control animals, though these levels were statistically insignificant. Thereafter, however, the values became higher than in the controls. After long-term administration of imipramine, the levels of [3H]-5-HT and [3H]-5-HIAA in P2-fraction were, at any time after intraventricular administration of [3H]-5-HT, significantly lower than those in the fraction from controls. The endogenous contents of tryptophan and 5-HIAA were not altered either with a single dose or by long-term administration of imipramine, however, the content of endogenous 5-HT was decreased by long-term administration of imipramine. These results indicate that the rate of disappearance of 5-HT from presynaptic neurons was accelerated by long-term administration of imipramine.

In vitro and in vivo anti-influenza A virus activity of antisense oligonucleotides.

We have demonstrated that antisense phosphorothioate oligonucleotides (S-ODNs) inhibit influenza virus A replication in MDCK cells. The liposomally encapsulated and the free antisense phosphorothioate oligonucleotides with four target sites (PB1, PB2, PA, and NP) were tested for their abilities to inhibit virus-induced cytopathogenic effects by a MTT assay using MDCK cells. The liposomally encapsulated S-ODN complementary to the sites of the PB2-AUG initiation codon showed highly inhibitory effects. Therefore, the antiviral effects of S-ODN-PB2-AUG and PA-AUG were examined in a mouse model of influenza virus A infection. PB2-AUG oligomer treated i.v. significantly prolonged the mean survival time in day (MDS) and increased the survival rates with does dependent manner.
