[Measurement of anti-HTLV-I antibody titer in cerebrospinal fluid].

Anti HTLV-I antibody was measured in 590 cerebrospinal fluids (CSFs) employing a gelatin particle agglutination (PA) method. Anti HTLV-I antibody was detected in the CSFs from 24 patients (4.1%). The serum from the 24 patients also showed positive results. Out of the 24 patients, 11 cases (45.8%) had HAMs. Of the remaining patients 3 had ATLs with meningeal infiltration. Only 11 cases out of 24 patients were positive for anti HTLV-I antibodies when an immunofluorescence (IF) method was employed. This discrepancy of the results obtained by two methods may be explained by the presence of the patients whose CSFs contain anti HTLV-I antibodies of low titer. It is concluded that PA method is both sensitive and specific for the detection of anti HTLV-I antibodies in CSFs. Further, PA method is more convenient than IF method. So, it will be a very useful tool in the diagnosis of HTLV-I related neurological disorders and asymptomatic carriers.

[Superselective angiography of lower gastrointestinal bleeding; a report of three case].

Angiography was performed in three patients with lower gastrointestinal bleeding. Extravasation was demonstrated only with superselective catheterization of a branch of superior mesenteric artery. However, this bleeding was not demonstrable with conventional catheterization of superior mesenteric artery. Superselective catheterization is suggested as a useful technique to detect lower gastrointestinal bleeding.

[Phase I study and early phase II of SM-108 (4-carbamoylimidazolium-5-olate) in lung cancer. SM-108 Study Group].

Phase I study with a new oral purine antagonist, SM-108, was conducted in a total of 73 cases in a 5-day consecutive schedule with the dosage ranging from 20-2,500 mg/m2 at II institutes. The incidences of subjective and objective side effects were 20.5% (15/73) and 31.5% (23/73), respectively, however the correlation between these effects and their dosages was unclear. MTD and DLF values were not determined. SM-108 levels in serum reached maximum in 2-4 hours after oral administration of SM-108, and exhibited a dose-response relationship up to the dosage of 2,000 mg/m2/day. Forty to 60% of the administered drug was excreted into urine in 24 hrs; thus, the main excretory pathway was considered to be renal. An early phase II study was undertaken in patients with lung cancer in 5 cancer centers and university hospitals. Each patient had received 400 mg/m2/day (in two divided doses) or 600 mg/m2/day (in three divided doses) for more than 2 weeks. Only one case (adenocarcinoma) showed a minor response out of 27 cases but not reached partial response according to our criteria. In the 400 mg/m2/day group and the 600 m/m2/day group the incidences of subjective side effects (mainly GI tract disturbance) were 33.3% (4/12) and 40.0% (6/15), while objective side effects (hematological changes) were 16.7% (2/12) and 26.7% (4/15) respectively. In conclusion, we could not determine the dose limiting factor and maximum tolerated dose from our phase I clinical study and early phase II study for lung cancer.

[Protective effects of ulinastatin on hepatic oxygen metabolism during halothane anesthesia in the presence of graded hypoxic hypoxemia].

Hepatic oxygen metabolism and the hepatic energy charge were assessed in mongrel dogs receiving 40,000 U.kg-1 of ulinastatin intra-portally during 2 MAC halothane anesthesia combined with graded hypoxic hypoxemia (21-6% oxygen) for the purpose of evaluating the role of ulinastatin in protecting the liver against the deprivation of the hepatic energy charge resulting in halothane-induced hepatotoxicity. Hepatic blood flow was measured using electromagnetic flowmetry; hepatic oxygen delivery and consumption were calculated from measured hepatic blood flow and oxygen content in hepatic arterial, portal venous and hepatic venous blood. In the animals who received ulinastatin during halothane anesthesia (the UST group), nine out of ten survived even at 8% oxygen, whereas six out of eight died in the halothane-alone group. There was no significant difference in total hepatic blood flow between the two groups. PaO2 was higher in the UST group than the halothane-alone group at 15 and 12% oxygen. Therefore hepatic oxygen delivery was significantly higher in the UST group at 15 and 12% oxygen. Hepatic oxygen consumption and the delivery/consumption ratio were also higher in the UST group at the various inspired oxygen levels. The arterial ketone body ratio, which indicates the mitochondrial energy charge level, significantly decreased with the progress of hypoxia in both groups, but it was significantly higher in the UST group at 8% oxygen.(ABSTRACT TRUNCATED AT 250 WORDS)

Peritoneal dialysis as therapy for electrolyte and acid base disorders.

Peritoneal dialysis (PD) does not demand special equipment and its fluid composition can be easily changed according to the individual condition. Nine patients with chronic or acute renal failure presented severe metabolic alkalosis (MA). Hemodialysis (HD) proved virtually ineffective and the MA persisted. Physiological saline solution was adopted as the main component of the PD fluid for the treatment of MA. By this method, Cl- can be shifted from PD fluid to extracellular fluid (ECF) and HCO3- from ECF to PD fluid by ionic gradient. Therefore, pH and base excess (BE) of these patients both improved to the normal range after several fluid exchanges. The lowering effect of BE (delta BE/L) ranged from 0.99 to 2.6. Hyposaline and normo-osmol solution (Na+ 70 mEq/L) were used for one patient with hypernatremia and consciousness disturbance. Serum (S)-Na decreased from 170 to 138 mEq/L, and consciousness became almost clear with the use of 15 L of PD fluid. Hypersaline solution (Na+ 190 mEq/L) was used for two patients with hyponatremia (S-Na 113 and 121 mEq/L). S-Na rose to within the normal range after 16 and 9 L in the two cases. A fluid mixed with distilled water, 10% NaCl and 7% NaHCO3 (HCO3 34-68 mEq/L) was used to treat lactic acidosis in two patients. By this method, HCO3- can be shifted from PD fluid to ECF and lactic acid from ECF to PD fluid. Two patients recovered from prolonged shock, and pH was corrected by 10 L and 4 L, respectively. PD should be considered for application in other diseases besides renal failure.

[An amino acid substitution determining G1m(x) allotypic marker].

We confirmed the amino acid substitution responsible for G1m(x) allotypic marker, which was estimated by Cook & Steinberg (Mol. Immunol. 16:555-558 (1979), by primary structure analysis of the C-terminal BrCN peptide from normal human IgG1 carrying G1m(x+) or G1m(x-). The results indicate that IgG1 carrying G1m(x) have glycine instead of alanine at position 431 (Eu numbering). And we discussed the epitope on tertiary structure of IgG1-Fc fragment.

[Surgical treatment of infant atrial septal defect: report of two cases].

The operation for the isolated secundum type of atrial septal defect (ASD) is usually conducted electively in the older age. Small number of cases, however, inevitably require surgical treatment for the reason of intractable heart failure. We have experienced such two infants with the isolated secundum type of ASD, whose conservative treatment was ineffective.

Cloning and nucleotide sequence of the 2,3-dihydroxybiphenyl dioxygenase gene from the PCB-degrading strain of Pseudomonas paucimobilis Q1.

The bphC gene encoding 2,3-dihydroxybiphenyl dioxygenase was cloned from biphenyl-degrading and chlorinated biphenyl-degrading Pseudomonas paucimobilis Q1, and its complete nucleotide sequence was determined. The DNA-derived protein sequence provides the primary structure of 298 amino acids. Polyclonal antibodies raised against this protein from P. paucimobilis Q1 failed to cross-react with the previously isolated 2,3-dihydroxybiphenyl dioxygenase from Pseudomonas pseudoalcaligenes KF707 [Furukawa, K., & Arimura, N. (1987) J. Bacteriol. 169, 924-927. Furukawa, K., Arimura, N., & Miyazaki, T. (1987) J. Bacteriol. 169, 427-429], despite the close similarities of these proteins in terms of their native as well as subunit molecular weights, cofactor, and enzymatic activities. The sequence homology of the 2,3-dihydroxybiphenyl dioxygenase from the two different sources is examined.

Establishment of three bleomycin-resistant human carcinoma cell lines and their cross-resistance to other antitumor agents.

Three bleomycin-resistant (BLMr) human carcinoma cell lines (HeLa-BLMr, KB-BLMr, and Hepd-uvBLMr) were established in culture by progressively increasing the concentration of bleomycin (BLM). HeLa-BLMr and KB-BLMr were obtained after 5 and 2 months of incubation with BLM, whereas the establishment of Hepd-uvBLMr required 3 months of incubation with BLM after ultraviolet treatment. These cells have been successfully subcultured for more than 150 passages during more than 2 years in the presence of 1 microgram/ml of BLM. The degrees of BLM resistance were 20-fold, 11.6-fold, and 186-fold for HeLa-BLMr, KB-BLMr, and Hepd-uvBLMr, respectively, and the resistant phenotype of both HeLa-BLMr and Hepd-uvBLMr was stable when they were cultured for 30 passages in BLM-free medium, but unstable in KB-BLMr. Although each cell line exhibited cross-resistance to 3 to 5 other antitumor agents including peplomycin, combined use of BLM with a polyene antibiotic, a calcium channel blocker, or a poly(ADP-ribose) polymerase inhibitor overcame the BLM resistance in vitro to various degrees.

A supersensitive dot-hybridization method: rapid and quantitative detection of host-derived DNA in recombinant products at the one picogram level.

We have developed a highly sensitive method of DNA dot-blotting hybridization to detect host-derived DNA (nuclear DNA and plasmid DNA) in a recombinant product. This method has two distinctive features compared to the conventional hybridization method: firstly, a highly specific radioactive probe is prepared by using ultrasonicated DNA, instead of untreated DNA, as a template for the oligo-labeling reaction; secondly, the signal to noise ratio is increased by the use of lambda phage DNA as non-homologous DNA. This method enabled us to detect host-derived DNA at the one picogram level without using a radioisotope of high specific activity and long exposure times.

Platelet activating factor-induced early pregnancy factor activity from the perfused rabbit ovary and oviduct.

Early pregnancy factor, a glycoprotein, and platelet activating factor, a phospholipid, are early signals of pregnancy appearing during the preimplantation period. The aim of this study was to determine if the administration of platelet activating factor to rabbits would stimulate the production of early pregnancy factor and, if so, to ascertain the organs responsible. Early pregnancy factor was assayed after the administration of synthetic platelet activating factor to intact rabbits, rabbits that had undergone bilateral salpingo-oophorectomy, and isolated in vitro perfused rabbit ovary and/or oviduct. Although serum early pregnancy factor activity was detectable 30 minutes after platelet activating factor injection in intact rabbits, no activity was found in rabbits whose ovaries and oviducts had been removed. Perfusate early pregnancy factor activity was detectable only in the presence of both ovary and oviduct, and was first evident 120 minutes after platelet activating factor exposure. However, combined samples of perfusates from the ovary alone and oviduct alone expressed positive early pregnancy factor activity. Furthermore, the addition of platelet activating factor to either single or combined perfusates did not increase early pregnancy factor activity. Our data imply that platelet activating factor induces the production of ovary and oviduct early pregnancy factor components independently.
