[A 14-year-old with pulmonary hamartomatous lymphangiomyomatosis associated with bilateral pneumothoraces].

A 14-year-old girl was admitted because of cough, chest pain and hemosputum. Chest roentgenogram on admission showed a pneumothorax and a cavitary lesion with niveau formation in the right lung and cystic lesions in the bilateral lung fields. After bed rest and intravenous administration of antibiotics for two weeks, the right lung inflated well and the niveau formation disappeared, and the patient was discharged. One week later, she was readmitted with sudden-onset severe dyspnea, caused by bilateral pneumothoraces. Emergency tube thoracostomy and wedge resection of the bullous lesion was performed. Macroscopically, multiple small cystic changes were seen on the surface of the right lung. Histological examination revealed nodular proliferations of smooth muscle cells in the interstitium and vessel walls in the lung, which contained slit-like lymphatic channels. The diagnosis of pulmonary lymphangiomyomatosis was made. In this case, we could not measure receptors for estrogen and progesterone. Recently, hormonal therapy and oophorectomy have been reported as being useful. Tamoxifen (Norvadex) was therefore initiated, and the patient has remained well with slight dyspnea on exertion. There has been no recurrence of pneumothorax. Lymphangiomyomatosis is a rare disease of unknown etiology which occurs exclusively in women, mostly in those of reproductive age. We report a 14-year-old female patient with lymphangiomyomatosis associated with repeated pneumothorax, who had been under treatment for epilepsy. We believe this case to be of importance because of the long discussed relation between pulmonary lymphangiomyomatosis and tuberous sclerosis.

[A clinical study of tuberculous pleurisy].

Forty-eight cases of tuberculous pleurisy were examined and the following results were obtained. (1) Most of the patients were male, and there was no significant age and underlying diseases. (2) Fever and chest pain were observed mainly in younger patients, and sputum and dyspnea in older patients. (3) All of the cases examined had exudative pleural effusion, and increased ADA activity was frequently observed. (4) Mycobacterium tuberculosis was detected in the sputum of 65%, and also in the pleural effusion of 28% of the patients. The pathological diagnosis of tuberculosis was made by pleural biopsy in 83% of the patients, suggesting that pleural biopsy is very useful in the diagnosis of tuberculosis pleurisy. (5) The prognosis of the patients with tuberculosis pleurisy was good. Steroid therapy was generally ineffective.

[Asphyxial death by laryngopharyngeal tumor--two autopsy cases].

Case 1. A 44-year-old male died about 3 h after paracenteses of both palatine tonsils. A 6.5 x 2.5 x 2.0 cm tumor, caused by bleeding and edema, was revealed at medico-legal autopsy. This tumor extended from the right palatine tonsil to the right edge of epiglottis. The cause of death was determined to be acute asphyxia due to the obstruction of the airway by the tumor. Case 2. A 51-year-old female was thought to have been killed by manual strangulation. At medico-legal autopsy, three malignant tumors, each of which was not as large as the tip of the little finger, were found in the lower pharynx. There were no findings of acute asphyxia death. The cause of death was determined to be delayed asphyxia caused by the malignant tumors.

[Clinical usefulness of the quantitative assessment of exercise thallium-201 myocardial imaging in patients with ischemic heart disease].

To clarify the significance of the quantitative analysis of the area of decreased activity in exercise thallium-201 myocardial imaging, 42 patients with one vessel disease (20 patients with angina pectoris without myocardial infarction, 22 patients with old myocardial infarction: MI) were evaluated by exercise thallium scan and compared with clinical findings and wall motion assessed by left ventriculography (LVG). Patients with MI were divided into two groups by LVG (Mild MI: MI with only hypokinesis on LVG, Severe MI: MI with akinesis or dyskinesis on LVG). The degree of defected area was expressed by the ratio of the counts of defected area to the normal area (Relative Activity: RA). RA of the area of decreased activity in patients with angina was higher than that of patients with MI (77.5 +/- 4.59% vs. 65.7 +/- 7.85%, p < 0.05). RA of the area of decreased activity in patients with mild MI was higher than that of patients with severe MI (69.5 +/- 4.78% vs. 61.3 +/- 8.45%, p < 0.01). The above results suggest that the regional thallium-201 uptake ratio in the scintigram may help to differentiate viable zones from necrotic ones and detect the degree of MI.

[A case of subacute traumatic aneurysm of the thoracic aorta with rare symptom of atelectasis in the left lung].

A case of subacute traumatic aneurysm of the thoracic aorta with atelectasis in the left lung is described. A 17-year-old male was admitted to our hospital because of traumatic shock. Radiographs revealed multiple fractures of upper and lower extremities, but on the chest roentgenogram there was no abnormal finding without slightly widened mediastinum. Next chest roentgenogram, four days later, showed left pleural effusion, atelectasis in the left lung and tracheal shift to the left. By the following examination of CT and DSA, the patient was diagnosed as a traumatic aneurysm at the aortic isthmus. The graft interposition of the descending aorta was performed with help of temporary bypass. He was making satisfactory progress after the operation. Reports about the operation of traumatic aneurysm of the thoracic aorta in subacute phase are relatively rare compared with those in chronic phase. No case, except this case, with tracheal shift to the left because of atelectasis in the left lung has been reported previously in Japan. It must be considered that the trachea may be shifted to the left by atelectasis of the left lung like this case.

[Progress of leukocyte research].

Over 170 years ago Metchnikoff described the bacteriocidal activity of leukocytes. It has been clarified that leukocytes play an important role in first defense mechanism against infection due to microorganism. Leukocytes immigrate to the lesion, which various microorganisms invade across mucosal barrier, and phagocyte and kill ingested microorganisms. The inability of the chronic granulomatous disease (CGD) granulocytes to generate superoxide has prompted the analysis of NADPH oxidase responsible for superoxide generation. The study on CGD has provided profound informations into NADPH oxidase system of leukocytes. Recruitment leukocytes to inflammatory lesion is essential for the inflammatory process, and it is regulated by various chemotactic factors and the adhesion molecules interaction between leukocytes and endothelium of vessels. Identification of interleukin-8 (IL-8) to be one of chemotactic cytokines and its gene cloning offered the progressive understanding for the immigration mechanism of leukocytes from vessels to inflammatory lesion. On the other hand, LFA-1 deficiency revealed that emigration, adhesion and phagocytosis of leukocytes are associated closely with the adhesion molecules on their surfaces. More recently, the cytokine networks of myelopoiesis or inflammatory process have been clarified. Among various cytokines, the cloning of G-CSF has provided the clinical application of rhG-CSF for neutropenic patients who may be induced by the treatment with anticancer agents. This symposium focused on above-mentioned items for an important aspect of neutrophil research.

Synergistic effects of interleukin-1 beta and interleukin-3 on the expansion of human hematopoietic progenitor cells in liquid cultures.

In the present study, we show that recombinant human interleukin-1 beta (rhIL-1 beta), which has no effect on the proliferation of human progenitor cells, has synergistic effects on the expansion of human progenitor cells induced by rhIL-3 in liquid cultures. The synergistic effects of rhIL-1 beta with rhIL-3 were observed in liquid cultures using not only fresh bone marrow mononuclear cells, but also selected populations of nonadherent cells, non-T nonadherent cells, and CD34-positive cells. Anti-granulocyte-macrophage colony-stimulating factor (anti-GM-CSF) antibody partially blocked the synergistic effects of rhIL-1 beta on the proliferation of colony-forming unit (CFU)-GM burst-forming unit-erythroid (BFU-E), and CFU-Mix in liquid cultures in the presence of rhIL-1 beta plus rhIL-3, suggesting that the synergistic effects of rhIL-1 beta plus rhIL-3 are explained in part by the secondary production of GM-CSF. Limiting dilution assays and liquid culture assays using CD34-positive cells indicate that rhIL-1 beta directly increases the numbers of colony-forming cells in liquid cultures. These results suggest that rhIL-1 beta has unique direct and indirect effects on the expansion of hematopoietic progenitor cells in liquid cultures.

Inhibitory effect of interleukin-4 on the in vitro growth of Ph1-positive acute lymphoblastic leukemia cells.

We investigated the effect of recombinant human interleukin-4 (rhIL-4) on the in vitro growth of human leukemia cells in liquid culture and 3H-thymidine incorporation and found inhibitory effects on the growth of leukemic cells from patients with Ph1-positive acute lymphoblastic leukemia (Ph1 ALL) and three Ph1 ALL cell lines. However, no inhibitory effects were seen in Ph1-positive leukemic cell lines derived from patients with chronic myelogenous leukemia in blast crisis and various types of Ph1-negative leukemia cells, including B-lineage leukemia cells. In a flow cytometry assay of IL-4 receptor (IL-4R), all three Ph1-positive ALL cell lines showed the presence of IL-4R on their cell surfaces, and the IL-4-dependent inhibition on the growth of Ph1-positive ALL cells was abrogated by the addition of either monoclonal or polyclonal antibodies against rhIL-4. Other cytokines, including IL-2, IL-3, granulocyte-macrophage colony-stimulating factor (CSF), granulocyte-CSF, and IL-6, showed no inhibitory effects on the growth of Ph1-ALL cells, but tumor necrosis factor-alpha (TNF-alpha) and interferon (IFN)-alpha, -beta, and -gamma displayed slight inhibitory effects in a high concentration. The growth inhibition induced by rhIL-4 in the Ph1-positive ALL cells was not abrogated by the addition of antibodies against either IFN-gamma or TNF-alpha. Furthermore, these cells showed no significant production of IFN-alpha, -beta, or -gamma or TNF-alpha after exposure to rhIL-4, thus indicating that the growth inhibition of Ph1-positive ALL cells by rhIL-4 is not associated with IL-4-stimulating production of these factors. rhIL-4 caused significant inhibition of the tyrosine kinase activity in these Ph1-positive ALL cells, similar to Herbimycin A, an inhibitor of tyrosine kinase that inhibited the tyrosine kinase activity in these cells. Our finding suggests that the clinical evaluation of rhIL-4 may offer promising therapeutic possibilities for patients with Ph1-positive ALL.

Clinical efficacy of sucralfate in the treatment of gastric ulcer.

We performed a randomized, single-blind study in Japan to investigate the efficacy and safety of two dosage regimens of sucralfate granules, 2 g, twice daily (b.i.d. group, n = 30) and 1 g, 4 times daily (q.i.d. group, n = 27), on ulcer healing in 57 patients with endoscopically proven gastric ulcer. Endoscopy was performed after 4 weeks and, if complete healing was not achieved, again after an additional 4 weeks. Of 57 patients, 11 were excluded from the statistical analysis because of protocol violation (six in b.i.d. group, five in q.i.d. group). Of 46 patients eligible for the analysis of healing rates, four patients in the b.i.d. group (all at 8 weeks) and five patients in the q.i.d. group (two at 4 weeks and three at 8 weeks) were withdrawn due to patients' inconvenience. As the possibility that the withdrawals were due to the treatment failures could not be denied, we used the Kaplan-Meier method and generalized Wilcoxon test/logrank test for the calculation and evaluation of healing rates in this study, respectively. Healing rates at 4 and 8 weeks were 50% and 94% in the b.i.d. group and 35% and 68% in the q.i.d. group. There was no significant difference in healing rates between the groups. No serious adverse effect was observed in either group. These results suggest that the 2 g b.i.d. dose of sucralfate in granule form is at least as effective as the conventional dose of 1 g q.i.d. in the treatment of active gastric ulcers and could lead to better patient compliance.

Elevated serum level of glycolipid sulfotransferase in patients with hepatocellular carcinoma.

Activity of glycolipid sulfotransferase (cerebroside sulfotransferase) in serum was elevated in 21 (33%) of 63 patients with hepatocellular carcinoma (HCC, mean +/- S.E., 349 +/- 32 pmol/ml per h, n = 63, P less than 0.001) compared to healthy subjects (172 +/- 12, n = 85). Ho significant elevation of the sulfotransferase level was observed in liver cirrhosis (219 +/- 28, n = 10) in which many of biochemical HCC markers increase concomitantly. The elevation of sulfotransferase was independent of the production of alpha-fetoprotein and of aminotransferase levels in HCC, providing complementary value for alpha-fetoprotein-negative HCC cases. However, the sulfotransferase levels (234 +/- 21, n = 32, P less than 0.01) in sera from patients with renal cell carcinoma, in whose involved tissues the enzyme was demonstrated to increase markedly, were less than in HCC.

[The clinical application of multi-voxel 1H-CSI (chemical shift imaging) in brain tumors].

The 1H-Chemical shift imaging (CSI) method was developed and applied to reveal the metabolic changes in brain tumors. In the tumor, the decrease of NAA and the increase of lactate were observed by the CSI. These changes were more remarkable in the malignant tumors than in the benign tumors. The lactate was also observed in the tissue surrounding the tumors, which was supposed to be produced in the brain with hypoperfusion condition or to be leaked out from the tumor. The 1H-CSI has an advantage to demonstrate the pathophysiological changes in a wide area of the brain.

Phosphorylation of coagulation factor II by phospholipid/Ca(2+)-dependent protein kinase (protein kinase C).

Prothrombin is a major constituent of the blood coagulation cascade and requires phospholipid and Ca2+ for its activation. We have found that phospholipid/Ca(2+)-dependent protein kinase (Protein kinase C) phosphorylates prothrombin and the associated apparent Km value for prothrombin (0.86 microM) is comparable to the Km value reported for most known substrates of protein kinase C. A 2-dimension separation analysis revealed that serine residue was apparently phosphorylated by PKC. The phosphorylation was inhibited by such phosphatidylserine- and/or Ca2+ competitive protein kinase C inhibitors as trifluoperazine, palmitoylcarnitine and gossypol. These results suggest that protein kinase C phosphorylation was involved in the regulation of blood coagulation.

Diastolic pressure-volume relationship during handgrip exercise in patients with coronary artery disease.

There is some controversy regarding the mechanisms of an upward shift in the left ventricular diastolic pressure-volume curve during ischemia. The effects of handgrip exercise on the pressure-volume curve were examined in 21 patients with coronary artery disease and in 6 control subjects. Pressure-volume curves were constructed from digitized left ventricular pressure and volume derived from biplane left ventriculogram. Diastolic pressure-volume curve shifted upward in 12 patients with coronary artery disease during handgrip exercise (Group 1), but not in the other 9 patients who were similarly afflicted (Group 2). The upward shift did not occur in any control subject. No difference was observed in rate-pressure product gain during exercise. In Group 1, left ventricular end-diastolic pressure increased (p less than 0.01) and ejection fraction was reduced (p less than 0.01), although it did not change in Group 2. In Group 1, the time constant was prolonged (p less than 0.01) with no change in the coefficient for elastic modulus. In Group 2, these parameters remained unchanged. Group 1 was accompanied by more extensive asynergy than Group 2. Thus, isometric handgrip exercise resulted in an upward shift in the diastolic pressure-volume curve in patients with coronary artery disease. Incomplete relaxation and/or the viscoelastic properties of the left ventricle associated with ischemia could be responsible for this phenomenon.

The integrity of the conserved 'WS motif' common to IL-2 and other cytokine receptors is essential for ligand binding and signal transduction.

Recent studies have identified a new family of cytokine receptors, which is primarily characterized by the conservation of periodically interspersed four cysteine residues and the W-S-X-W-S sequence ('WS motif') within the extracellular domain. However, the role of such conserved structures still remains elusive, in particular that of the WS motif. Interleukin-2 (IL-2) is known to play a critical role in the clonal expansion of antigen-stimulated T lymphocytes, and the IL-2 signal is delivered by one of the receptor components, the IL-2 receptor beta (IL-2R beta) chain. The IL-2R beta chain, unlike the IL-2R alpha chain, belongs to this receptor family. In the present study, we analyzed the function of the WS motif of IL-2R beta (Trp194-Ser195-Pro196-Trp197-Ser198) with the use of site-directed mutagenesis. Our results indicate the critical role of the two Trp residues in the proper folding of the IL-2R beta extracellular domain and point to the general functional importance of the WS motif in the new cytokine receptor family.

A key amino acid determining G3m(b) allotypic markers.

A key amino acid substitution specific for allotypic Gm b markers, b0, b3, b5, were determined through sequence analyses of the pFc' fragments of IgG1 (Su) and IgG3 (Ba[Gm(g)], Bu[Gm(b1b3)], and Kam[Gm(b3st)]) myeloma proteins. The results indicate that serine at position 384 is responsible for the specificities. It is considered from crystallographic data of IgG-Fc [Deisenhofer et al. (1981) Biochemistry 20:2361] that two residues, the serine and isoleucine specific for IgG3 subclass at position 422, cause the structural change responsible for b markers. The two residues are close to each other in the CH3 domain. The allocations of the epitopes are estimated to be on two bends (residue no. 382-392, 411-424) between the beta-strands, whose amino acid residues are present in wide contact area [Novotny et al. (1987) Immunol. Today 8:26).

Treatment of chronic non-A, non-B hepatitis with interferon.

The efficacy of interferon therapy (IFN) was investigated in 46 patients with chronic non-A, non-B (NANB) hepatitis, of would 40 (87.0%) were positive for anti-HCV antibody (Ab) (C-100-3). Three kinds of IFN were used; human lymphoblastoid interferon (HLBI), interferon alpha-2b and interferon beta. Total doses of IFN ranged from 1 million units (MU) to 10 MU and treatment duration ranged from 2 weeks to 144 weeks. Of 46 patients 34 (73.9%) responded to IFN. Nine patients have maintained normal ALT levels and 5 patients have maintained near-normal ALT levels for more than 6 months after cessation of IFN treatment. In these cases the titers of anti-HCV Ab had decreased significantly at the end of IFN therapy and 6 months after IFN therapy respectively. The mean age was young and the mean disease duration was short in effective cases. As for doses and treatment duration of IFN, low doses of IFN requires long treatment duration to acquire continuous efficacy and high doses of IFN requires rather short treatment durations. Therefore, early IFN treatment, higher doses and longer periods of IFN treatment may improve the response rate of patients with chronic NANB hepatitis.
