Computed tomographic diagnosis of enterovesical fistulae: barium evacuation method.

We reviewed and compared the computed tomographic (CT) findings for 5 patients suspected of having enterovesical fistulae with convential radiographic studies and the results of cystoscopy. Prior to scanning, patients were rectally administered 200 ml of 0.8% wt/vol barium solution, and then evacuated before they were administered 200 ml of olive oil via the anus and 100 ml of olive oil via the urethra. The CT following the barium evacuation successfully demonstrated the enterovesical fistulae in 4 patients. It was also diagnostic in a clinically suspected case in verifying the absence of an enterovesical fistula.

Lack of correlation between haemagglutination and adherence to epithelial cells in Yersinia pseudotuberculosis.

Yersinia pseudotuberculosis was examined for its haemagglutinating activity and adherence to cultured epithelial cells (HEp-2) in relation to possession of a virulence (VW) plasmid and to growth conditions. VW-lacking (VW-) bacteria were isolated from ten VW+ strains of each serovar which, after they were grown on CFA plates at 37 degrees C, agglutinated the erythrocytes from five different species. In contrast to the bacteria possessing the plasmid (VW+) half of the VW- bacteria, grown on CFA plates at 37 degrees C, did not agglutinate any of the erythrocytes used and the other half agglutinated only human erythrocytes. Furthermore, when grown on CFA plates at 25 degrees C, neither VW+ nor VW- bacteria showed a haemagglutinating activity. When the bacteria were grown in CFA broth, only two strains grown at 25 degrees C did not agglutinate any of the erythrocytes tested. The VW+ and VW- bacteria of the remaining strains, grown either at 25 degrees C or 37 degrees C, showed relatively high haemagglutinating activity. Adherence to HEp-2 cells did not correlate with haemagglutinating activity in Y. pseudotuberculosis; the VW+ bacteria grown at 37 degrees C adhered to HEp-2 cells more efficiently than either the VW- derivatives or the VW+ bacteria grown at 25 degrees C, regardless of the growth medium. These results indicate that some of the haemagglutinins detected on Y. pseudotuberculosis are not involved in the adherence to HEp-2 cells.

[Experience of HLA-matched platelet transfusion in patients who are refractory to random donor platelets].

Platelets collected from random and multiple donors were as effective on transfusion as those obtained from single donor by platelet pheresis if patients were not alloimmunized. In alloimmunized patients, effects of platelet transfusion were correlated with the HLA-matching grade; 100% effective in A-match, 92% in B-match and 65% in C-match. This correlation was not observed if the patient had infection. The transfusion of A- or B-matched platelets resulted in a lowering or disappearance of HLA antibody activities in most patients. However, the transfusion of C-matched platelets caused increasing or appearance of HLA antibody activities in some patients. The ABO-incompatibility did not show a significant effect on platelet transfusion when anti-A or anti-B antibody activities in donors and antiglobulin tests on patients' red cells were negligible. Non HLA, platelet specific antibodies were produced in some patients and presumed to be participated in the destruction of transfused HLA-matched platelets.

Sequence of an intestinal cDNA encoding human motilin precursor.

A cDNA clone encoding the human motilin precursor was isolated from an intestinal library using synthetic oligonucleotide probes. The predicted amino acid sequence indicates that the motilin precursor consists of 115 amino acids and includes a 25-residue N-terminal signal peptide followed by the 22-amino-acid motilin sequence and a long, 68-residue C-terminal peptide. The amino acid sequence of human motilin predicted from the cDNA sequence is identical to its porcine counterpart, which has been determined by protein sequencing. Proteolytic processing of promotilin to motilin occurs at the sequence, Lys-Lys, this being the first reported instance of processing occurring at a pair of Lys residues. In other precursors it occurs at Lys-Arg, Arg-Arg, Arg, or very rarely Lys.

Sequence of an intestinal cDNA encoding human gastric inhibitory polypeptide precursor.

Gastric inhibitory polypeptide (GIP) is a 42-amino acid hormone that stimulates insulin secretion in the presence of glucose. Complementary DNA clones encoding human GIP were isolated from a library prepared with RNA from duodenum. The predicted amino acid sequence indicates that GIP is derived by proteolytic processing of a 153-residue precursor, preproGIP. The GIP moiety is flanked by polypeptide segments of 51 and 60 amino acids at its NH2 and COOH termini, respectively. The former includes a signal peptide of about 21 residues and an NH2-terminal propeptide of 30 amino acids. GIP is released from the precursor by processing at single arginine residues. There is a region of nine amino acids in the COOH-terminal propeptide of the GIP precursor that has partial homology with a portion of chromogranin A as well as pancreastatin.

CT-pathologic correlation in small peripheral lung cancers.

To evaluate the morphology of small peripheral lung cancers, we studied thin-section CT images of 18 small peripheral lung cancers (14 adenocarcinomas, two squamous-cell carcinomas, one large-cell carcinoma, and one carcinoid) in 17 patients. After surgical resection, specimens were sliced transversely, and the gross morphology and histology were correlated with the appearance of the lesion on preoperative thin-section CT images. CT images showed fine speculations in 78%, a notch in 83%, pleural retraction in 78%, and convergence of peripheral vessels in 83% of the 18 lung cancers. Furthermore, in cases of papillary adenocarcinoma, the lesions had heterogeneous densities (78%) and small cavitations (67%). CT showed a peripheral fluffy zone in most of the well- or moderately differentiated papillary adenocarcinomas. This correlated well with tumor cells lining the alveolar walls, observed in pathologic studies. Comparison of thin-section CT images with pathologic data suggests that the demonstration of the lung-tumor interface and the tumor's internal texture, while not specific for malignant lesions, can make the CT diagnosis of small peripheral lung cancers more accurate.

[A case of holoprosencephaly with parietal encephalocele].

Holoprosencephaly, as described by DeMyer, is a cerebral malformation in which the embryonic prosencephalon fails to divide into cerebral hemispheres. According to the degree of division in the prosencephalon, he classified holoprosencephaly into three types; alobar, semilobar and lobar type. We have experienced a case of holoprosencephaly with parietal encephalocele. The patient was admitted to our hospital 4 hours after birth because of parietal encephalocele. A radical operation was carried out and encephalocele was diagnosed histologically. After discharge from our hospital, his psychosomatic development was remarkably delayed. On Oct. 20, 1985, he was re-admitted for further examination. CT-scan and cerebral angiography showed characteristic findings. The classification of holoprosencephaly and dorsal cyst malformation is discussed. On encephalocele with holoprosencephaly, it is said that there may be an extra-calvarial extension of the dorsal sac or ventricular system. But, Hutchinson stated in his experience that first, a closure disorder of the neural tube caused encephalocele. And later a disorder of diverticularization produced the holoprosencephalic abnormality. He suggested that there were two independent disorders of organogenesis which might rarely occur in the same patient. We also reported a similar case of holoprosencephaly with parietal encephalocele which has some controversial problems.
