[The prognosis of Graves' disease after antithyroid drug treatment: clinical significance of the T3 suppression test and abnormal thyroid stimulators].

The prognostic significance of both the triiodothyronine (T3) suppression test and the detectability of thyroid stimulating immunoglobulins in patients with Graves' disease who had been treated with antithyroid drugs was evaluated. Eighty-three patients underwent a T3 suppression test after having been euthyroid for at least 6 months. In 33 patients, the human thyroid stimulator (HTS) was assayed by measuring cyclic AMP increase in cultured thyroid adenoma cells, and TSH-binding inhibitor immunoglobulins (TBII) were measured by using the radioreceptor assay of TSH. Among 43 patients who had discontinued the drug treatment, 37 patients were under observation for 6-42 months. When a fall in 30-minute thyroid 99mTcO4- uptake of 50% or more after T3 administration was defined as positive suppression, the relapse rate was 30% in non-suppressive cases and 26% in suppressive cases. The relapse rate was lower in cases whose pre-suppression uptake was less than 3.0% (3 out of 17 patients) or in cases whose uptake after T3 administration was less than 0.8% (1 out of 9 patients). Of 15 patients with negative suppression, 5 (33.3%) were positive in HTS and 4 (26.7%) were positive in TBII. On the other hand, two (11.1%) each of 18 patients with positive suppression were positive in HTS and TBII respectively. Neither HTS nor TBII had been detected at the cessation of therapy in any of the 12 patients who remained euthyroid during the follow-up period. On the other hand, four (44.4%) out of 9 patients who relapsed had been positive in either HTS of TBII. Thus the Graves' disease specific immunoglobulins were found to be significantly associated with the relapse of the disease (p less than 0.05). The above data indicates that regardless of the suppressibility of 99mTcO4- uptake after T3 administration, the rate of recurrence is high when the uptake after T3 is more than 0.9% and/or when either HTS or TBII are positive.

Anticancer drug sensitivity in vitro in the bladder cancer cell line, KK-47 and prophylactic use of carbazilquinone and urokinase in bladder cancer.

Using colony formation technique and KK-47 cell line established from a human bladder transitional cell carcinoma, the effect of 6 anticancer drugs, thio-TEPA, Bleomycin, mitomycin C, carbazilquinone, Adriamycin and cis-Platinum, were compared. On the results of tests performed to establish the drug concentration required to achive a 50% inhibition of cell survival, carbazilquinone was chosen for the prevention of recurrences of bladder cancer. The two groups studied consisted of 56 patients (previously untreated groups) who were rendered free of tumours by surgical intervention and of 19 patients (thio-TEPA failures group) who had experienced a persistent recurrence of tumours after prophylactic thio-TEPA instillations and were presumed free of the recurrence of tumours after the next surgical intervention. The 2 groups were subjected to prophylactic combined intravesical instillation therapy with carbazilquinone and urokinase. In the previously untreated group, 6 of the 56 patients (10.7%) had a recurrence of tumours, and the recurrence rate after 21 months was 16.7%, using the actuarial method. In the thio-TEPA failures group, 12 of the 19 patients (63.2%) had a recurrence of tumours, a rate at 21 months of 76.1%. A considerable drop in the recurrence rate was obtained by the combined instillation therapy in the previously untreated group. The results in the thio-TEPA failures group suggested the presence of a cross-resistance between both alkylating agents, and of a persistent susceptibility to multifocal lesions. No bone marrow depression was observed but an episode of anaphylactic shock attributable to the use of carbazilquinone occurred in 1 out of a total 75 patients.

Surgical treatment of the Wolff-Parkinson-White syndrome in infants and children.

Eleven pediatric Wolff-Parkinson-White (WPW) syndrome patients underwent surgery. Four had left, 5 right cardiac type and 2 had right septal type WPW syndrome. Two patients had 2 accessory conduction pathways (ACP). Ebstein's anomaly and secundum type atrial septal defect were the association congenital cardiac diseases in one patient each. Indications for surgery included repeated and/or long-lasting paroxysmal supraventricular tachycardia (PSVT), ineffective drug therapy, cardiac failure due to frequent tachycardia, short effective refractory period of the ACP, and simultaneous surgery for associated congenital cardiac diseases. Pre- and intra-operative examinations, including ECG, VCG, UCG, body surface mapping, intracavitary recording by catheter electrodes, computerized epicardial mapping, and endocardial mapping, were performed for the precise localization of the ACP. The surgical method was basically the same as is used for adults. Anterior median stermotomy was used primarily in right cardiac and right septal type and left anterior thoracotomy was used in 3 of 4 cases of the left cardiac type. Eight of 11 cases, two of which had 3 ACPs, were completely cured and in 3 there was evidence of postoperative pre-excitation. However, the PSVT attacks disappeared almost completely and drug therapy is not required at present.

Experimental and clinical study of the pre-excitation syndrome.

Ventricular pre-excitation was experimentally produced in mongrel dogs. Their ECG and epicardial maps were analyzed. Forty-two cases of W-P-W syndrome were operated on between 1969 and May, 1980. The relationship between ECG, particularly the delta wave and localization and meaning of pre-operative examinations such as vectorcardiography, echocardiography, body surface mapping, intracavitary potential study, and cardiac pacing, were presented and discussed. As an intraoperative study, epicardial mapping was indispensable and was the most accurate method in determining the ACP. Endocardial potential study was another meaningful method in certain cases. Detachment of the atrium from the ventricle with an incision along the annulus and at the opposite of the pre-excitation area resulted in complete correction in 32 out of 34 cases between 1973 and May, 1980. Six cases of multiple ACPs were also corrected, although 4 needed a second operation. Indications for surgery of the W-P-W syndrome should be extended further in view of the high success and safety rate of the surgery.

[The computerized display of epicardial mapping (author's transl)].

UNLABELLED: The epicardial mapping is necessary for the surgical treatment of drug-resistant cardiac arrhythmia. The procedure for the epicardial mapping consists of three stages: 1) recording of heart potentials, 2) extraction of excitation transmission time, and 3) display of epicardial mapping. In this paper, our new microcomputer-based system was discussed. It was achieved by this system to simplify the method for the extraction and to make the colorgraphic display of epicardial mapping. Two methods for extraction were used in this study to compare with manual operation: 1) D method; extracting the time, when the differentiated wave takes maximal potential, and 2) Wi method; extracting the time of (Formula: see text). RESULTS: 1) similar epicardial excitation maps were obtained by these computerized methods, and 2) the most approximate values of the transmission time was obtained by Wi (i=2) method.

Interruption of multiple accessory conduction pathways in the Wolff-Parkinson-White syndrome.

Of 35 patients with Wolff-Parkinson-White syndrome operated on, 5 were seen with two accesory conduction pathways each; all of these were successfully interrupted. In one patient, one accessory conduction pathway each was located in the right and left side of the heart; in the other 4, both pathways were confined to the right side. In 2 patients with unilateral (right side) multiple accessory conduction pathways, Ebstein's anomaly was also present. In 1 patient with Ebstein's anomaly, the second unilateral accessory conduction pathway was discovered intraoperatively and was successfully interrupted. The remaining 4 patients required a second operation to interrupt the other pathway. A delta wave completely different from the preoperative one appeared 4 to 10 days after interruption of the first major pathway, and the second operation was performed 14 days, 42 days, four months, or five years after the first operation. All 5 patients survived, and long-term follow-up revealed no signs of morbidity.

Simultaneous repair of anomalous origin of the left coronary artery from the pulmonary artery and mitral regurgitation.

A 20-month-old boy with anomalous origin of the left coronary artery from the pulmonary artery was successfully treated by combining aortic implantation of the left coronary artery and simultaneous mitral valve replacement. We describe our operative technique and emphasize that in the definitive treatment of this anomaly, it is advisable to not only correct the malformation but also repair the mitral valve damage caused by secondary changes due to infarction.

Simultaneous surgery for Wolff-Parkinson-White syndrome combined with Ebstein's anomaly. Interruption of multiple accessory conduction pathways.

We detail the simultaneous surgical treatment of Ebstein's anomaly combined with Wolff-Parkinson-White syndrome in a patient who presented with multiple accessory conduction pathways. The extensively deformed tricuspid valve was replaced by a Hancock xenograft and the atrialized ventricle was plicated. Surgery was performed under the beating heart condition and this made possible electrical mapping which facilitated the localization of both accessory pathways. By the method presented here, all existing accessory pathways can be pinpointed during open heart surgery and the time of their interruption can be established. Two and a half years after surgery, chest X-ray revealed marked reduction of the cardiothoracic ratio and the patient experienced no postoperative tachycardia episodes.

Double pacemaker implantation.

A demand-type pacemaker and an inductive radiofrequency atrial pacemaker were implanted in four patients presenting with sick sinus syndrome, bradycardia-tachycardia syndrome, Wolff-Parkinson-White syndrome, atrial flutter, and paroxysmal atrial tachycardia, and in one patient having ventricular tachycardia with ectopic ventricular beats. Our follow-up of these patients showed that double implantation is feasible and effective in controlling different types of arrhythmia occurring in the same patient.
