Ultrasonic evaluation of scrotal swellings.

The diagnostic value of ultrasonography for 101 scrotal swellings of 94 patients was assessed. The 101 swellings showed sonographically 87 extratesticular, 11 testicular and 3 combined lesions. This sonography was concluded to be a satisfactory diagnostic aid for scrotal cystic lesions, testicular tumors and the localization of scrotal lesions.

Immunohistological phenotyping of thyroid infiltrating lymphocytes in Graves' disease and Hashimoto's thyroiditis.

Subsets of lymphocytes in the thyroid were immunophenotyped by their surface antigens in frozen tissues of Hashimoto's thyroiditis and Graves' disease. Using triple layer immunoperoxidase staining (IP), monoclonal antibodies (T3, Leu 3, T8, anti-Tac and Leu 7) were employed to detect markers of T cell subsets, activated T cells, and a natural killer associated antigen. B cells were identified by 2 step IP with anti-IgD antisera. Excluding those cells forming lymphoid follicles, the density of lymphocytes infiltrating between thyroid epithelial cells was much higher in Hashimoto's thyroiditis than in Graves' disease. However, relative proportions of subsets were similar in both diseases. Most of the infiltrating cells were T3 positive T cells (T3+), with more T8+ (suppressor/cytotoxic T) than Leu 3+ (inducer/helper T). Some Leu 7+ were occasionally seen, but surface IgD positive mature B cells (IgD+) were almost absent. In contrast, IgD+ cells were densely aggregated in primary lymphoid follicles and mantle zones of secondary follicles. In these regions, Leu 3+ cells were about twice as frequent as T8+ cells. Some Leu 7+ and scarce Tac+ cells were also found. The present study indicates a major involvement of immunoregulatory T cells in autoimmune thyroid disease, and also suggested intrathyroidal maturation of B cells.

[Clinical effect of UFT on prostatic cancer].

A phase II study of UFT, a mixture of futraful and uracil in a rate of 1:4, was performed for prostatic cancer in 5 cooperative institutions. UFT was orally administered at a daily dose of 600 mg (t.i.d.) for 4 weeks. Twenty-two patients treated with UFT were evaluated according to Koyama-Saito's criteria. Tumor staging determined by transrectal ultrasonography and other methods revealed stage B in 4 patients, stage C in 7, and stage D in 11. The overall response rate was 18.2%. Complete response was obtained in 1 patient with stage B disease, partial response in 3, minor response in 1, no change in 15, and progressive disease in 2. With respect to toxicity, anorexia was observed in 9 patients, nausea and vomiting in 8, stomatitis in 4, and diarrhea in 3. White blood cell count was less than 3,000 cells per mm3 in 1 patient, and in another case, hepatic function disorder was observed. From the results obtained, the use of UFT is TS considered desirable for prostatic cancer.

[A randomized controlled study to compare 1-hexylcarbamoyl-5-fluorouracil (HCFU) and 1-(2-tetrahydrofuryl)-5-fluorouracil (Tegafur) for the prevention of post-operative recurrence of bladder cancer].

To evaluate the effect of 1-hexylcarbamoyl-5-fluorouracil (HCFU), a derivative of 5-fluorouracil (5-FU), in preventing postoperative recurrence of bladder cancer, a randomized controlled study with 1-(2-tetrahydrofuryl)-5-fluorouracil (Tegafur) as the reference standard was performed. HCFU was given orally 600 mg a day and Tegafur was given orally, 800 mg a day. The following results were obtained: Of 103 patients, 51 were given HCFU and 52 Tegafur, the non-recurrence rate in the group treated with HCFU was 70.8% after 1 year and 54.9% after 2 years of follow up, and that of the group treated with Tegafur was 56.5% and 46.2% respectively. The rate of non-recurrence in the HCFU group was significantly higher (p less than 0.01) than that of the Tegafur group during the period of follow up between 450 and 539 days. Of the 87 patients, who took the drugs for more than 90 days, the rate of non-recurrence in 43 patients receiving HCFU was 79.2% after 1 year and 60.4% after 2 years, compared to 62.5% and 50.0% respectively for 44 patients receiving Tegafur. The non-recurrence rate of the HCFU group was also significantly higher than that of the Tegafur group in the period between 450 and 539 days after operation (p less than 0.05). The incidence of side effects was 35.6% and 51.7% in HCFU and Tegafur patients, respectively. No significant difference of side effects was found between HCFU and Tegafur.

[Photodynamic therapy of superficial tumors and carcinoma in situ of the bladder].

Photodynamic therapy (PDT), in which hematoporphyrin derivative is activated by an argon-dye laser, was performed on 36 superficial tumors using a 400 micron quartz fiber and cystoscope (spot PDT), and also carried out in 7 patients with primary or secondary CIS, 9 patients with secondary CIS associated with TaTl tumors and 2 patients with multiple small tumors, using a motor-driven laser light scattering optic automatically controlled by a computer (total PDT). In spot PDT, for tumors up to 2 cm in size the light intensity should be 300 mW/cm2 for 5 to 10 minutes or more. For the CIS therapy, the light dose used was 10 to 30 Joules/cm2. There has been no recurrence at present in 8 of the 18 patients at a mean follow-up period of 6.6 months. The techniques and problems associated with total PDT are reported.

[Pre- and post-operative changes of serum proteins in renal cancer patients].

Using a micro-single radial immunodiffusion method, quantitative studies have been made on the serum proteins of 30 renal cancer patients; 15 patients without metastasis and 15 patients with metastasis, and expressed by percentage of those obtained on 98 normal volunteers regarded as 100%. Of 45 proteins studied, 4 (Pre, Alb, alpha 2HS and Tf) significantly decreased and 10 (alpha 1AT, alpha 1AG, alpha 1X, Hp, Cp, C9, C5, beta 2III, C4 and C3PA) significantly increased before nephrectomy in both patient groups. In postoperative follow-up studies of the non-metastasis patient group, Pre, Alb and alpha 2HS increased, while alpha 1AT, alpha 1AG, Hp, C9, C5, beta 2III, C4 and C3PA decreased, and normal levels were attained within 6 months after nephrectomy. In the metastasis patient group, no significant changes of these serum proteins were observed. Serum proteins showing significant difference between the 2 patient groups were more prominent 6 months after nephrectomy, namely, the majority of the dysproteinemia caused by the presence of renal cancer did not disappear even 1 month after extirpation of the tumor. alpha 1AT was normalized 1 month after nephrectomy at a highly significant difference, P less than 0.001, and considered one of the promising proteins in terms of clinical course evaluation. As a new finding, renal cancer group with metastasis showed a decrease of FN postoperatively suggesting increase of a fibrin deposition and fibrinogen metabolism in cancer tissue.

Whole bladder wall photoradiation therapy for carcinoma in situ of the bladder: a preliminary report.

Whole bladder wall photoradiation therapy, using a hematoporphyrin derivative as a sensitizer, the red light (wavelength 630 plus or minus 1.6 nm.) of an argon dye laser as the source of excitation energy and a motor driven laser light scattering optic, was performed in 2 patients with multicentric carcinoma in situ of the bladder. The total light dose was 10 J. per cm.2. Acute vesical inflammatory changes developed in the bladder wall lining, associated with a reduction of bladder capacity, which, however, disappeared within 3 months after photoradiation therapy. The anticancer effect of photoradiation therapy was followed by urinary cytologic and cystoscopic examinations for 4 months. Occasional discharges of clusters of vacuolated cancer cells or sheets of vesical detached cells in the urine were observed for 2 weeks after photoradiation therapy but, thereafter, repeat urinary cytologic and cystoscopic examinations revealed no malignancy.

Altered balance of immunoregulatory T lymphocyte subsets in autoimmune thyroid diseases.

Three monoclonal antibodies recognizing cell surface antigens of total peripheral (OKT3), helper/inducer (OKT4) and suppressor/cytotoxic (OKT8) T lymphocytes were used by an indirect immunofluorescence technique to enumerate peripheral T lymphocytes in 25 patients with Graves' disease (including 4 euthyroid Graves' patients), 16 patients with Hashimoto's thyroiditis and 22 normal controls. Total lymphocyte count and percentages of overall T and helper/inducer T cells among peripheral lymphocytes in these conditions showed no significant difference from those of the controls. Percentage of suppressor/cytotoxic T cells, however, was decreased in Graves' disease patients with or without hyperthyroidism. The ratio of helper/inducer T cells to suppressor/cytotoxic T cells was increased in Graves' disease population and slightly increased in hypothyroid Hashimoto's thyroiditis patients. The ratio correlated with the mitogenic response of peripheral mononuclear cells to phytohaemagglutinin, but not with the serum levels of thyroid hormones nor with the titres of thyroid autoantibodies. These findings are in accordance with the results of previous functional studies and indicate possible defects in suppressor T lymphocytes in autoimmune thyroid disease.

[A hyperthermic perfusion therapy using peplomycin and ethanol for bladder cancer].

Distilled water containing 40 micrograms/ml peplomycin and 2% ethanol was used as a perfusate in 8 patients with superficial bladder tumors and 2 with deep bladder tumors for 2 hours at 43 degrees C. In addition, immediately before the perfusion treatment, 5 mg of peplomycin was injected intramuscularly. Prior to treatment, the nature and extent of the tumors were determined by ultrasonography, cystoscopy and cystography. The therapeutic effect of the hyperthermic perfusion was evaluated by the same manner as used previously. Partial tumor regression was obtained in 6 of the 8 patients with superficial bladder tumors. The 2 patients with deep bladder tumors showed no tumor regression. Most of the patients had bladder discomfort such as irritation, pollakisuria and so on, during and/or after perfusion. No patient developed acute pyelonephritis.

Combined scintiscans in the diagnosis of thyroid carcinomas.

We correlated scintigraphy and histological findings in 40 cases of nodular goiter in which two or more kinds of scintigraphy were performed preoperatively. 201Tl scintigraphy for imaging thyroid carcinoma showed a diagnostic agreement of 80% for thyroid lesions and 82% for metastatic lesions in the cervical lymph nodes. These agreements were higher than with 99mTc-bleomycin scintigraphy. Thus, 201Tl scan can be used to evaluate 'cold' thyroid nodules and to visualize metastatic lesions. 201Tl scan was also found valuable as a diagnostic measure for local recurrence of thyroid carcinoma. However, there were a few false-positive and false-negative results.
