[Successful combination chemotherapy including deoxycoformycin in a case of cutaneous T cell lymphoma].

A 44-year-old woman was admitted to our department because of fever and skin eruptions on August, 1991. Physical examination revealed superficial lymph node swelling, hepatosplenomegaly and generalized erythroderma. Laboratory findings were as follows; WBC 21,490/microliters with 67% lymphocytes including flower cells. The surface phenotype of lymphocytes was positive for CD2, CD4, CD25, CD29 suggesting helper-inducer T cell. Skin and lymph node biopsies revealed the infiltration of T cells with indented nuclei. Anti-HTLV-1 antibodies in the serum and HTLV-1 proviral DNA analysis by PCR method were negative. She was diagnosed as CTCL, and she was treated with prednisolone. However, her erythroderma deteriorated gradually, in spite of well-controlled lymphocyte counts. Combination chemotherapy, utilizing vincristine, etoposide and cyclophosphamide, was effective against organomegaly but not against generalized erythroderma. After DCF was initiated at a weekly dose of 7.5 mg, her erythroderma improved rapidly and markedly with the disappearance of severe itching, and she achieved complete remission. Our results suggest that DCF is beneficial for chemotherapy-resistant generalized erythroderma in CTCL.

[A case of selective IgM deficiency associated with systemic lupus erythematosus].

We report a case of selective IgM deficiency associated with systemic lupus erythematosus (SLE). A 34-year-old female suffering from SLE was admitted with proteinuria and general fatigue. Laboratory findings revealed a very low serum IgM level, almost lower than 12 mg/dl. Renal biopsy findings showed diffuse proliferative lupus nephritis (DPLN). In immunofluorescent microscopy, IgG was the most strongly stained followed by IgA, but IgM staining was only faint. As for the immunophenotype of the T cells, the OKT4/OKT8 ratio was normal. Response to both phytohemagglutinin (PHA) and concanavalin A (ConA) was normal. However, responses of B cells to both pokeweed mitogen (PWM) and Staphylococcus aureus Cowan strain I (SAC) were significantly reduced. Surface IgM-positive B cells were decreased. These results indicate that the patient had B cell dysfunction, involving impairment of B cell differentiation. In this report, we discuss selective IgM deficiency and SLE documented in the literature.

Gene transfer with liposomes to the intraocular tissues by different routes of administration.

PURPOSE: To determine whether a reporter gene carried by liposomes can be introduced into the ocular tissues in vivo by different routes of administration. METHODS: Three different kinds of liposomes carrying plasmid DNA with beta-galactosidase gene were applied topically to the eye or were injected into the anterior chamber, subretinal space, and vitreous of adult Wistar rats. Gene expression was detected by enzymatic color reaction using X-gal as a substrate in enucleated eyes 1 day, 1 week, and 1 month after topical application or injection. RESULTS: Topical application could transfer the gene to retinal ganglion cells. Injection into the anterior chamber delivered the gene to the basal layer of the corneal epithelium, ciliary epithelium, stroma of the ciliary body and iris, and retinal ganglion cells. Injection into the vitreous or subretinal space resulted in the expression of the gene in the ciliary epithelium, stroma of the ciliary body and iris, retinal ganglion cells, and retinal pigment epithelial cells. CONCLUSIONS: Efficient and stable transfer of the functional gene could be achieved by liposomes in the cornea, iris, ciliary body, and retina of rats. Liposomes appear to be a promising vehicle for delivering therapeutic genes in vivo to mammalian intraocular tissues.

[Combined treatment with resection, intraoperative hypotonic and post-operative systemic chemotherapy using cis-platinum for a mediastinal yolk sac tumor: a case report].

Generally yolk sac tumor in the mediastinum is a rare and highly malignant neoplasm. We report a case of yolk sac tumor, which was treated with resection, and then intraoperative hypotonic and postoperative systemic chemotherapy. A 33-year-old man was diagnosed as a mediastinal tumor with chest X-ray and chest CT findings. At operation he had mediastinal tumor which invaded to the part of the right upper lobe and pericardium, disseminated on the right diaphragm. A large tumor was removed with partial pericardiectomy and wedge resection of the right upper lobe. After resection hypotonic chemotherapy with cis-platinum was performed. This tumor had endodermal sinus structure, Schiller-Duval body, and eosinophilic hyaline globolus which were histological characteristics of yolk sac tumor, and the value of serum alpha-fetoprotein (AFP) was high. After operation with this diagnosis systemic chemotherapy consisted of cis-platinum, etoposide were performed. Patient was discharged uneventfully 8 months later.

[Thrombotic thrombocytopenic purpura after administration of ticlopidine].

On June 29, 1995, a 49-year-old man was admitted with acute onset of fever, petechiae on his legs, and mental confusion He had suffered hypertension since 6 months previously and was on nicardipine (60 mg/day), ifenprodil (60 mg/day) and ticlopidine (300 mg/day). He had been on ticlopidine for 4 weeks and on the other drugs for 6 months. Soon after admission he had frequent grand mal seizures and needed mechanical ventilation. A diagnosis of TTP was made. He was treated with plasmapheresis (50 ml/kg per day), aspirin 81 mg/day and dipyridamole 300 mg/day. On the sixth day his mental status returned to normal. He recovered gradually from microangiopathic hemolytic anemia, thrombocytopenia and elevated serum creatinine. We reviewed the literature and discussed the possible mechanism of TTP related to the use of ticlopidine.

Bovine and porcine trabecular cells produce prostaglandin F2 alpha in response to cyclic mechanical stretching.

Bovine and porcine trabecular cells in Petri dish cultures were exposed to cyclic mechanical stretching of the dish bottom for 30 second cycles (2 Hertz) at a strain magnitude of 4.5 mm/m for up to 3 days. Growth rate of the cells was not affected by the stretching but production of prostaglandin F2 alpha (PGF2 alpha) increased. We believe one stimulus for production of PGF2 alpha in trabecular cells may be the stretching of the trabecular meshwork caused by changes in aqueous flow and intraocular pressure.

Apoptosis regulated by growth factors in multicellular spheroids of trabecular cells.

Bovine and porcine trabecular cells were incubated up to one month in a polystyrene dish nonadherent environment for suspension culture, and their morphological changes were examined by light and electron microscopy. The trabecular cells formed multicellular spheroids during the first day. Apoptosis occurred selectively and continuously in the spheroids, which had initially been filled with cells. After one month, the final structure of the spheroids had become a meshwork of cells separated by large extracellular spaces. Epidermal and basic fibroblast growth factors prevented apoptosis of the trabecular cells in the spheroids which remained filled with cells for the entire observation period. The trabecular multicellular spheroids may represent the in vivo process of trabecular meshwork formation and can be used as a system for identifying regulatory factors involved in this process.

[Immediate and late results of the surgical treatment of the aneurysms and dissections of the ascending aorta].

PURPOSE: To report our experience and results with the surgical treatment of aortic aneurysms and dissections of the ascending aorta as well as the techniques that have been used. METHODS: Between May 1982-May 1995, 54 patients were operated on and divided in two groups: group A was composed of 25 patients with aneurysms or chronic dissections of the ascending aorta. The Bentall and DeBonno procedure was performed in 18, five were submitted to prosthetic aortic graft associated with aortic valve replacement and two prosthetic aortic graft without aortic valve replacement. Group B had 29 patients with acute dissections of the ascending aorta (type A) who were submitted to 10 prosthetic graft alone, nine Bentall procedures, five prosthetic graft with aortic valve replacement, four aortic repair and one direct suture of the dissection. The survival curve was obtained by the Kaplan-Meier method. RESULTS: Total hospital mortality rate was 13% and the late was 18.5%. Group A-immediate mortality rate was 8% (two patients): Low cardiac output and stroke; late mortality rate was five (20%): sudden death in three, pulmonary embolism one and infectious endocardite one. Group B-hospital mortality rate was five (17.2%) patients: low cardiac output three, multiple organs failure one and stroke with pneumonia one; late mortality rate was five (17.2%), distal redissection in three, sudden death in two. The average survival time was 86 +/- 12 for group A and 75 +/- 13 months for group B. CONCLUSION: The surgery of the aneurysm and dissections of the ascending aorta has shown immediate favorable results and a thorough follow-up to get better late results is needed.

Effective anticoagulation by argatroban during immunoadsorption therapy for malignant rheumatoid arthritis with a high polymorphonuclear leukocyte elastase level.

Polymorphonuclear leukocyte elastase (PMNE) is a glycoprotein with a molecular weight of about 30000 and exists in 3 isozymes (1). It has a biological role in host defense mechanisms, but it sometimes induces abnormal blood coagulation and injury of the vascular intima (1,2). We were not able to obtain a satisfactory anticoagulant effect when using heparin during immunoadsorption therapy in a patient with malignant rheumatoid arthritis and high PMNE level, but found that argatroban, a selective antithrombin agent, produced satisfactory anticoagulation. This finding is interesting as far as the various pathologic roles of PMNE are attracting increasing clinical attention (1,2,3).

Inhibition of epidermal growth factor binding system by ionizing radiation in A431 human squamous carcinoma cells.

To elucidate the effect of ionizing radiation on the membrane anchored signal transduction, the binding of 125I epidermal growth factor (EGF) to its receptor (EGF-R) and the EGF-dependent EGF-R tyrosine phosphorylation were examined in a human squamous cell carcinoma cell line, A431. The significant suppression of 125I EGF binding to A431 cells was observed from 3-5 h after 10 Gy irradiation, whereas this inhibition was not observed both in non-irradiated and in 5 Gy-irradiated cells. This phenomenon was mediated by the protein kinase C pathway, because the inhibition was not observed in cells which had been pretreated with phorbol ester and treated with an inhibitor of the enzyme, H7. Scatchard analysis showed that the receptor affinity was decreased. In contrast, the level of EGF-dependent EGF-R-tyrosine phosphorylation was not decreased, compared with non-irradiated cells. These results suggest that ionizing radiation may modulate the function of EGF/EGF-R interaction through the direct activation of protein kinase C.

Effect of granulocyte colony-stimulating factor (G-CSF) on chemotherapy-induced oral mucositis.

In this study, the ability of granulocyte colony-stimulating factor (G-CSF) to treat or prevent chemotherapy-induced oral mucositis in patients with advanced breast cancer was evaluated. A total of 14 patients who received intraarterial (i.a.) adriamycin (ADM) preoperatively were divided into two groups according to whether or not G-CSF was given. Thus, group A (n = 7) was given G-CSF and group B (n = 7) was not. G-CSF therapy reduced both the incidence and duration of ADM-induced oral mucositis, and a positive correlation was also seen between the incidence of mucositis and ADM-induced leukopenia (< 2,000/mm3). Group A was further divided into two subgroups according to whether G-CSF was given after or before the leukopenia had dropped below 2,000/mm3: group A-1 (n = 3) and group A-2 (n = 4), respectively. ADM-induced mucositis was observed in two of the three patients in group A-1, but in none of the four patients in group A-2. These results strongly support the idea that G-CSF can effectively treat and prevent ADM-induced oral mucositis.
