[Antitumor activity of a new platinum complex (R)-(-)-2-aminomethylpyrrolidine (1,1-cyclobutane dicarboxylato) platinum (II) (DWA2114R) by its serial administration].

Antitumor activity of a newly synthesized platinum complex, DWA2114R, by the serial administration was examined and compared with that of cis-diammine-1,1-cyclobutane dicarboxylato platinum (II) (CBDCA). In mice transplanted s.c. with tumor, the serial i.p. administration resulted in the increases of both maximal tolerated dose (MTD) and growth inhibitory ratio (GIR) of DWA2114R than single administration. Such increases in MTD and GIR were also shown by CBDCA, but the degree of these increases, such as the ratio of MTD or GIR by the serial administration compared to that at the single administration, was higher in DWA2114R than CBDCA. GIR of DWA2114R by the serial administration was higher than that of CBDCA at the doses to induce the same toxicity which was estimated by body weight loss. In addition, in the experiment using ascites tumor-bearing mice, better antitumor activity of DWA2114R was shown by the elongation of survival time. These results indicate that the cumulative toxicity of DWA2114R is lower than that of CBDCA, which causes the therapeutic advantages of DWA2114R in the serial administration.

The alteration in the pattern of pulmonary metastasis with adjuvant chemotherapy in osteosarcoma.

Alterations in the pattern of pulmonary metastasis of patients with osteosarcoma treated with adjuvant chemotherapy were studied. Thirty two patients who were treated with both radical surgery and adjuvant chemotherapy were observed (chemotherapy group). As a control, sixty two patients treated with radical surgery alone were also assessed (non-chemotherapy group). The chemotherapy group demonstrated a reduction in the number and a delay in the appearance of metastases. The tumour doubling time did not differ between the chemotherapy and non-chemotherapy groups. The initial site of metastasis among the chemotherapy group was most commonly in the lower lung field, whereas among the non-chemotherapy group it was usually to the middle lung field. The chemotherapy group survived longer than the non-chemotherapy group after developing pulmonary metastases. Histological examination showed that the metastatic foci stimulated the primary sites.

Surgical management of ureteroceles in children: strategy based on the classification of ureteral hiatus and the eversion of ureteroceles.

Experience in the surgical management of ureteroceles in children is reported. Six patients with intravesical and 16 with ectopic ureteroceles are included. In the case of intravesical ureterocele of noneverting type. Transurethral distal incision of the ureterocele was performed without reflux. In the case of everting ureterocele, excision with reimplantation of the ureter was performed, thus avoiding reflux inevitably induced by transurethral incision. In the patient with an ectopic ureterocele, a one-stage operation with complete excision of the ureterocele and ureteral stump was attempted. Our choice of operative procedure was decided after the assessment of the following features: cystography and cystoscopy to assess eversion, type of ureteral hiatus (common or separate), whether there was reflux or not to the ipsilateral ureter, and renal function by scintigraphy. Accordingly, hemi-nephroureterectomy or twin ureteroneocystostomy were most commonly performed. When based on these thorough preoperative evaluations, our results were quite favorable.

Lipocortin-like 33 kDa protein of guinea pig neutrophil. Its distribution and stimulation-dependent translocation detected by monoclonal anti-33 kDa protein antibody.

33 kDa protein of neutrophil is a lipocortin-like protein, as proposed from its biochemical properties, amino acid composition, and the homology of its amino acid sequence to human lipocortin I. The localization and translocation of 33 kDa protein (p33) in blood cells of guinea pig were studied by immunoblotting (Western blotting) and immunocytochemical fluorescence methods using polyclonal and monoclonal mouse anti-p33 antibodies. The protein was determined to be present only in the cytoplasm of neutrophils, but not in cells such as the monocyte, lymphocyte, platelet, and other bone marrow cells. The translocation of the protein from cytoplasm to cell membrane was coupled with the increase in intracellular calcium ion and in superoxide generation induced by a chemotactic factor. These findings suggest that p33 may have an important role not only in the regulatory mechanism of phospholipase A2 (PLA2) activity but also in other transmembrane signaling.

Calcium pyrophosphate dihydrate crystal deposition disease in the cervical ligamentum flavum.

The authors describe three cases of cervical radiculomyelopathy caused by calcium pyrophosphate dihydrate crystal deposition disease (CPPDcdd). Radiological investigations revealed nodular calcifications, 5 to 7 mm in diameter, in the cervical ligamentum flavum compressing the spinal cord. Light microscopic, scanning electron microscopic, and x-ray diffraction studies were performed on all three surgical specimens obtained by laminectomy. In two of the cases x-ray microanalysis and transmission electron microscope studies were also performed. This study defined the presence of two patterns of crystal deposition in the ligamentum flavum. One is a nodular deposit, in which hydroxyapatite crystals are seen in the central part of the nodules, with calcium pyrophosphate dihydrate (CPPD) being distributed thinly around them. The other pattern is a linear deposit seen in multiple ligaments and composed of pure CPPD, which causes minimal thickening of the ligaments. A transitional pattern between the two types was also observed. This study revealed details of the nodular deposition of crystals in the ligamentum flavum and demonstrates that CPPDcdd and so-called "calcification of the ligamentum flavum" are the same disease: namely, CPPDcdd. Hydroxyapatite is assumed to have been transformed from CPPD.

Results of transtrochanteric rotational osteotomy for nontraumatic osteonecrosis of the femoral head.

From 1976 to 1985, transtrochanteric rotational osteotomy was applied to 58 hips of nontraumatic necrosis of the femoral head. Of these, 52 hips were available for the present study. The follow-up periods ranged from one to ten years (average, 5.1 years). Satisfactory results were obtained in 69%, as there was no evidence of progressive collapse of the head. Hips that had had no collapse or slight collapse before osteotomy had a significantly better prognosis, compared with those that had marked collapse of the head. The extent of the lesion as depicted on the preoperative lateral roentgenogram also influenced the eventual function. Patient satisfaction was significantly greater when these lesions were less than 70% (31/37), compared with greater than 70% (5/15).

The use of flow cytometry as a diagnostic aid in the management of soft-tissue tumors.

The diagnosis and staging of soft-tissue tumors is a complex problem, and even the experienced pathologist sometimes finds it difficult to determine whether a particular lesion is benign or malignant and whether a sarcoma is high or low grade. However, this information is essential in planning treatment. Flow cytometric analysis of nuclear DNA is a method to determine the number of cells that are in the process of replicating or dividing (S or G2, or M phase), since these cells have abnormal concentrations of DNA (DNA aneuploidy). Previous studies from our laboratory have established the relative value of this technique as an adjunct in the staging of primary bone tumors. In the past four years, 146 soft-tissue lesions have been evaluated by flow cytometry, using propidium iodide staining of isolated cells that were obtained in the fresh state. The tumors were forty-two benign neoplasms, ten lesions of synovial origin, thirty desmoids (aggressive but not malignant), and sixty-four sarcomas ranging in grade from 1 to 3 on a 3-point scale. The over-all values for flow cytometry showed that a number of factors correlated well with the grade of the tumor, but the best correlations were with the mean concentration of DNA (a calculated average for concentrations of DNA for the various types of cells in the lesion), the total percentage of cells in S-phase plus the G2 and M-phases (called percentage of replicating and dividing cells), and the presence or absence of DNA aneuploidy.(ABSTRACT TRUNCATED AT 250 WORDS)

Inhibition of metabolic response of polymorphonuclear leukocyte by biscoclaurine alkaloids.

Effects of biscoclaurine alkaloids on the various stimulus-responses of PMN, especially on the O2-. generation of PMN, were investigated. Results obtained were: cepharanthine inhibited various metabolic responses of PMN, its biological action probably being due to its membrane modifying action. Inhibition of O2-. generation by cepharanthine was stronger than any other inhibition of metabolic responses of PMN. The inhibitory effect of various biscoclaurine alkaloids on the O2-. generation of PMN was the descending order of tri-, di- and mono-ether type; the coclaurine type showed only a weak effect.

Neutrophil specific 33 kDa protein: its Ca2+- and phospholipid-dependent intracellular translocation.

A 33 kDa protein from neutrophils has been shown to associate reversibly with phosphatidylserine containing liposomes in a Ca2+-dependent manner. The protein was purified from guinea pig neutrophils. Immunoblotting and cytochemical studies with polyclonal and monoclonal antibodies to the protein revealed that the protein is commonly distributed in neutrophil cytoplasm of different animal species. The protein was translocated to the plasma membrane by treatment with stimuli. Thus the 33 kDa protein is neutrophil specific and may be involved in transmembrane signaling.

Production of tumor growth-inhibiting protein by the neonatal mouse brain: dependence on intracellular glutamine metabolism.

We previously reported that the mouse brain at the neonatal stage but not at the adult stage secreted a carcinostatic factor of 62,000 Da, termed NBCF, which inhibited clonal growth and DNA synthesis of malignant cells preferentially over those of normal cells. In the present study, NBCF production by the neonatal mouse brain in culture was investigated. Addition of L-glutamine to the culture medium markedly promoted NBCF production in a concentration-dependent manner. The production seemed to be specific to glutamine, since no promotive effect was exerted by L-glutamic acid, its analogue DL-alpha-aminoadipic acid, L-aspargine, or L-aspartic acid or by other amino acids or vitamins. NBCF production was markedly reduced in culture medium either devoid of L-glutamine or containing 6-diazo-5-oxo-L-norleucine, a glutamine antagonist, or L-methionine sulfoximine, an inhibitor of glutamine synthetase. Thus NBCF production was promoted by extracellular supply, intracellular synthesis, and utilization of L-glutamine but was not affected by its deamidated form or homologue amino acids. On the other hand, NBCF production was completely repressed by addition of cycloheximide to the culture medium. The repressive effect was also exerted by actinomycin D although not completely, whereas cytosine beta-D-arabinofuranoside did not repress NBCF production. These results indicated that NBCF production by cultivation was independent of DNA replication but dependent mostly on a transcription stage and its following stages and partly on a translation stage from the preexisting mRNA to the protein.

Chondroblastoma arising in the triradiate cartilage. Report of two cases with review of the literature.

Chondroblastoma is a relatively rare benign bone tumor of cartilage origin. Roentgenologically it presents usually as a region of lytic destruction of bone with a thin sclerotic rim in the epiphysis of long tubular bone. Less than 9% occur in the pelvic bones but show a tendency to arise from the triradiate cartilage. We present two cases of chondroblastoma originating in the triradiate cartilage, each showing extensive lytic bony destruction and an intrapelvic soft tissue mass. A review of the literature suggests that chondroblastoma of the triradiate cartilage shows an aggressive radiological appearance.
