[Clinical experience of enoxacin in the treatment of nongonococcal urethritis].

Enoxacin (ENX) is a new derivative of pyridonecarboxylic acid and has a broad spectrum antibacterial activity. In this study, the effects of ENX on nongonococcal urethritis (NGU) were examined in 20 patients. ENX was administered orally 3 times a day at a daily dose of 600 mg for 2-23 days. Before treatment, Chlamydia trachomatis was isolated from urethral epithelium in 5 of the 20 patients. Of the 18 patients evaluated in this clinical trial, 15 (83%) responded well, and ENX was effective in 3 of the 5 Chlamydia positive cases and in 12 of the 13 Chlamydia-negative patients. As adverse reactions, headache was observed in one case and numbness of limbs in another case. No abnormality induced by ENX was recognized in laboratory findings. It is concluded that ENX is a useful drug in the treatment of patients with NGU, particularly in whom Chlamydia trachomatis is not isolated.

[Lymphocyte cytotoxicity against autologous tumor cells and the effect of interferon-beta on their activities. (1) Evaluation by modification of target tumor cells].

IFN is known to enhance NK activity against cultured cell lines such as K562, but not against frozen autologous tumor cells. In order to obtain increased NK cytotoxicity using IFN-beta, various modifications were performed on autologous tumor cells. IFN-beta induced more enhanced NK cytotoxicity of normal lymphocytes when frozen tumor target cells were cultured for 4-5 days in the medium, or when these cells were treated with Vibrio cholerae neuraminidase (VCN). However, in an autologous setting, IFN-beta did not enhance NK cytotoxicity against either cultured autologous tumor cells or VCN-treated tumor cells. Also, IFN-beta did not enhance cytotoxic T cell activity against autologous tumor cells induced by mixed lymphocyte-tumor cell culture, although IFN-beta was able to induce enhancement of allospecific cytotoxic T cells mediated by mixed lymphocyte culture.

Transmission and genetic shift of human immunodeficiency virus (HIV) in vivo.

HIVYU-6 and HIVYU-7 were isolated from an acquired immune deficiency syndrome patient (MK) and his asymptomatic sexual partner (MM), respectively. YU-6 readily infected not only peripheral lymphocytes from normal individuals but also human T-cell lines such as H9, HUT-78, MOLT-4 and MT-4; YU-7, on the other hand, could not infect H9 and MT-4 cells. Furthermore, although autologous serum failed to neutralize YU-6, it was neutralized by the heterologous serum from the partner. Restriction endonuclease analysis of YU-6 demonstrated that it was a mixture of viruses. We have isolated two clones from YU-6 (YU-6-a and YU-6-b) by a plaque assay method and showed that YU-6-a had one more KpnI site than YU-6-b. It was also evident that YU-7 derived from YU-6-a, but had already shifted genetically from YU-6-a. Transmission of human immunodeficiency virus through heterosexual contact and a possible genetic shift of YU-6-a, b and YU-7 from a common progenitor virus in vivo is discussed.

[Prognosis of gastric cancer patients with special reference to the area of serosal cancerous invasion].

The area of serosal cancerous invasion and intraperitoneal free cancer cells were examined in 360 gastric cancer cases. The positivity rate of intraperitoneal free cancer cells was 20% in the group with under 20 cm2 of serosal invasion, 59% with 20 cm2 to 30 cm2, and 76% over 30 cm. The postoperative survival rate was significantly lower in the group with free cancer cells than in the group without them. Prognosis was better in case with the area of serosal invasion was smaller. Hematogenous metastasis was frequent under 20 cm2, but over 20 cm2, peritoneal metastasis predominated.

Malignant peritoneal mesothelioma as a pelvic mass. Application of athymic mice in clinical pathology.

We reported herein an unusual presentation of malignant peritoneal mesothelioma which appeared as a solid pelvic tumor in a 73-year-old man. The solid tumor was confined in the pelvic cavity, except for one tiny disseminated nodule on the mesocolon. The pelvic tumor was resected with whole pelvic organs en bloc. Histologically, the tumor consisted of tubular and sarcomatous structures, suggesting the biphasic malignant mesothelioma. One of the transplanted tumors in athymic mice was cystic and its histology had striking similarities to cystic mesothelioma. Transplantation of human neoplasms to athymic mice may be able to reproduce histological diversity of the tumor, even if it is not fully expressed in the original tumor.

Metabolism of 25-hydroxycholecalciferol in human promyelocytic leukemia cells (HL-60). Isolation and identification of (5Z)- and (5E)-19-nor-10-oxo-25-hydroxycholecalciferol.

Human promyelocytic leukemia cells incubated with 25-hydroxy[26,27-methyl-3H] cholecalciferol (1 microCi) or non-radioactive 25-hydroxycholecalciferol (550 micrograms) produced significant quantities of two vitamin D3 metabolites. The two metabolites were isolated and purified by methanol chloroform extraction and a series of chromatographic procedures. The metabolite purification and elution positions on these columns were followed by radioactivity and their ultraviolet absorption at 310 nm. The two metabolites have been unequivocally identified as (5Z)- and (5E)-19-nor-10-oxo-25-hydroxycholecalciferol by ultraviolet absorption spectrophotometry, mass spectrometry, Fourier-transform infrared spectrophotometry and co-chromatography with synthetic compounds on a high-performance liquid chromatograph. (5E)- but not (5Z)-19-nor-10-oxo-25-hydroxycholecalciferol was able to induce HL-60 cell phenotypic and functional differentiation. However, these two metabolites of 25-hydroxycholecalciferol did not bind specifically to the chick intestinal 3.7 S. receptor protein for 1 alpha,25-dihydroxycholecalciferol. The precise biological role of these metabolites is as yet unclear.

Effects of dynorphin1-13 on opiate binding and dopamine and GABA uptake in stroked cat brain.

We previously reported that the opioid peptide dynorphin1-13 improves survival chances in stroked cats. Some evidence also suggests that changes in dopamine and gamma-aminobutyric acid (GABA) uptake may be associated with stroke. In the present study, therefore, we determined binding of the opiate [3H]ethylketocyclazocine (EKC), as well as dopamine and GABA uptake in various brain regions of control, stroked and dynorphin1-13-treated stroked cats. Cats were stroked by middle cerebral artery occlusion. In the EKC binding study, the Kd of the high-affinity site of the occluded cortex was significantly increased, relative to that of both the unoccluded side and control cortex. Dynorphin1-13 treatment reversed this effect, lowering the Kd to control level. In the dopamine uptake study, the Km was decreased and Vmax was increased significantly in unoccluded cortex, compared with that in the occluded cortex or in control cortex. Again, dynorphin1-13 reversed these effects, raising the Km and lowering the Vmax. However, the Km of occluded cortex was also increased so that it became significantly higher than that of control cortex. The Km of unoccluded subcortex in stroked cats treated with dynorphin1-13 was significantly reduced compared with control. In the GABA uptake study, there was no significant change in any parameter. The change in opioid binding observed here and its reversal by dynorphin1-13 are consistent with the notion that the peptide's beneficial effect on stroke is mediated through opiate receptors. Since opioid systems in the brain are known to have association with dopaminergic ones, the change in dopamine uptake could also be the result of an opioid effect.

Inhibitors of 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced multinucleated cell formation and HTLV-I p19 antigen expression in HTLV-I-infected T-cell line KH-2Lo.

To elucidate the biological mechanisms of 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced phenotypic changes in HTLV-I virus-infected human T-cell line KH-2Lo cells, inhibitors of TPA-induced ornithine decarboxylase (ODC), protein kinases and calmodulin were examined for their effects on TPA-induced multinucleated cell formation and HTLV-I p19 antigen expression. Among the inhibitors of TPA-induced ODC activity, alpha-difluoromethyl ornithine (DFMO), 1,25(OH)2D3 and its analogues, and retinoic acid were tested. As inhibitors of protein kinases, 1-(5-isoquinolinyl-sulfonyl)-2-methylpiperazine (H-7), N-[2-(methylamino)ethyl]-5-isoquinoline-sulfonamide (H-8) and 1,(5-isoquinolynylsulfonyl)-2,3-dimethylpiperazine (H-5) were used. In addition, a calmodulin inhibitor, N-(4-aminobutyl)-5-chlor-2-naphthalenesulfonamide (W13) and its inactive analogue, N-(4-aminobutyl)-2-naphthalenesulfonamide (W12) were also tested. 1,25(OH)2D3 and its active analogues inhibited both TPA-induced HTLV-I p19 antigen expression and multinucleated cell formation after 4 days of culture with TPA. On the other hand, an inhibitor of ODC, DFMO, the protein kinase inhibitors, the calmodulin inhibitor and retinoic acid suppressed TPA-induced HTLV-I p19 expression but did not suppress multinucleated cell formation.

Application of hepatitis B core particles produced by human primary hepatocellular carcinoma (PLC/342) propagated in nude mice to the determination of anti-HBc by passive hemagglutination.

Human primary hepatocellular carcinoma (PLC/342), carried by nude mice, produces hepatitis B core particles as well as hepatitis B surface antigen particles. Core particles purified form PLC/342 tumors displayed epitopes of hepatitis B core antigen (HBcAg) but not epitopes of hepatitis B e antigen (HBeAg) on their surface, unlike core particles prepared from Dane particles, derived from plasma of asymptomatic carriers, that expressed epitopes of both HBcAg and HBeAg. Core particles obtained from PLC/342 tumors were applied to the determination of antibody to HBcAg (anti-HBc) by passive hemagglutination. The assay detected anti-HBc not only in individuals with persistent infection with hepatitis B virus and in those who had recovered from transient infection, but also in patients with acute type B hepatitis, indicating that it can detect anti-HBc of either IgG or IgM class. A liberal availability of core particles from tumors carried by nude mice, taken together with an easy applicability of the method, would make the passive hemagglutination for anti-HBc a valuable tool in clinical and epidemiological studies, especially in places where sophisticated methods are not feasible.

Possible involvement of protein kinase C in interleukin-1 production by mouse peritoneal macrophages.

Both lipopolysaccharide (LPS) and phorbol-12,13-dibutyrate (PDBu), a protein kinase C-activating phorbol ester, induced interleukin-1 (IL-1) production in mouse peritoneal macrophages. Prolonged treatment of the cells with PDBu led to the down-regulation and complete disappearance of protein kinase C. In these cells, PDBu did not increase IL-1 production, but LPS still stimulated IL-1 production although the maximum level was slightly reduced. These results suggest that protein kinase C and another unknown signal pathway are involved in LPS-induced IL-1 production.
