C-myc expression in lymphocytes of MRL/MP-lpr mice activated by A23187 and TPA.

In order to analyse the molecular mechanism of lymphocyte activation defect in MRL/MP-lpr (MRL/1) mice, c-myc proto-oncogene expression was examined in MRL/1 lymph node cells stimulated by various agents. Since a transient increase of c-myc RNA in early hours is required for lymphocyte activation, detection of c-myc messenger RNA is useful to determine whether or not an appropriate signal is transduced to the nucleus. Stimulation by concanavalin A (Con A) plus 12-O-tetradecanoyl phorbol 13-acetate (TPA) or A23187 plus TPA markedly increased c-myc mRNA expression and cell proliferation, whereas stimulation by Con A alone failed to do so. These results suggest that an abnormality exists in the early signal transduction process, and that it could be bypassed by calcium influx and direct activation of protein kinase C.

[Testicular follicle-stimulating hormone receptors and effectiveness of human menopausal gonadotropin-human chorionic gonadotropin treatment in idiopathic male infertility].

To investigate the relationship between testicular follicle stimulating hormone (FSH) receptors and the effectiveness of human menopausal gonadotropin (hMG-hCG) treatment in idiopathic male infertility, 48 infertile men were examined. Most of the 14 patients without detectable testicular high affinity FSH receptors did not show any increase in sperm count after the hMG-hCG treatment, whereas 23 of the 34 patients with FSH receptors responded to the treatment. In patients with FSH receptors, patients with a middle or high Johnsen's score count responded more than those with a low score count did. From the above results, it seems that both the presence or absence of testicular FSH receptors and the histological appearance of spermatogenesis predict responsiveness to hMG-hCG treatment in infertile men.

[Endocrinological profile of human spermatogenic impairment--evaluation of function of Leydig and Sertoli cells in vivo or in vitro studies].

In vivo and in vitro studies were performed to determine the function of Leydig and Sertoli cells of the human testis with various degrees of spermatogenic impairment. The increases in basal and peak serum levels of luteinizing hormone (LH) and follicle stimulating hormone (FSH) after LH-RH administration correlated with the degree of impairment of spermatogenesis, and the basal peripheral blood levels of testosterone and that after human chorionic gonadotropin (hCG) administration for patients with moderate or severe impairment were significantly lower than the values for those with mild impairment. The concentration of testosterone in the internal spermatic vein of varicocele patients with or without hCG treatment did not differ between in mild and moderate impairment. In studies on cultured Sertoli cells, the production rate of plasminogen activator in patients with severe impairment was significantly lower than that in patients with moderate or mild impairment. The decrease in testicular high-affinity binding site for FSH correlated with the degree of hypospermatogenesis found in idiopathic male infertility, but, on the contrary, the hCG (LH) receptors showed no correlation with the degree of impairment of spermatogenesis. In the investigation of the relationship between testicular FSH receptors and the effectiveness of human menopausal gonadotropin (hMG)-hCG treatment on idiopathic male infertility, the presence or absence of testicular FSH receptors predicted the responsiveness to the treatment.

Endoscopic diagnosis of lymphangioma of the small intestine.

In three patients with lymphangioma of the small intestine, the preoperative diagnosis of lymphangioma was made by endoscopy with biopsy and radiology. A typical endoscopic finding was an elevated polypoid tumor, yellowish-white to tan. The surface was smooth, often with white specks, and could be impressed by touching lightly with biopsy forceps. Endoscopic examination revealed satellite lesions not detected radiologically. Lymphangioma has heretofore been successfully diagnosed preoperatively by endoscopy in the duodenum. This report of accurate preoperative diagnoses of lymphangioma in the small intestine, other than the duodenum, using endoscopy, may be the first to be documented.

Comparative drug depletion in domestic animals and birds.

Decoquinate (Rhone-Poulenc Inc) and Narasin (Eli Lilly and Co) were selected as model drugs for a comparison of metabolism between major (cattle and chickens) and minor (sheep and quail) species. Decoquinate has been studied in all four species. Narasin studies are in progress in chickens and quail. More than 96% of injected 14C-decoquinate (DQ) was eliminated from blood of all species within 1 hr. Disappearance of the remaining 1 to 4% from blood was rapid for all species. Half-times for DQ appearance in excreta were all less than one day. Cumulative excretion of DQ in eggs of chickens and quail was about 1% for both species. Disappearance of DQ from tissues was essentially complete in 14 days. More than 80% of injected 14C-narasin was eliminated from blood within 1/2 hr. Disappearance of the remainder was rapid for both chickens and quail.

[Testicular follicle-stimulating hormone receptors in idiopathic male infertility].

To ascertain whether abnormalities in testicular binding of follicle-stimulating hormone (FSH) are related with spermatogenic impairment found in idiopathic male infertility, we measured FSH receptors in testicular tissues obtained by biopsy from 48 infertile men. The 48 infertile men were divided into 3 groups by histological grading using Johnsen's score count, and testicular FSH receptors among these groups were compared. Although 9 of the 18 infertile men with low Johnsen's score count and 5 of the 15 men with middle score count had no obvious FSH binding sites with high affinity, all 15 men with a high score count had high affinity FSH binding sites. In connection with high affinity binding sites for FSH, maximum binding number (Bmax) decreased significantly with the degree of spermatogenic impairment, but the association constant (Ka) was similar among the 3 groups. The present findings demonstrate that the decrease in testicular high affinity binding sites for FSH is related with the degree of spermatogenic impairment found in idiopathic male infertility.

[Influence of temperature on the function of the human testes].

The influence of temperature on human Leydig and Sertoli cell functions was investigated in vitro using the organ culture technique. Specific 125I-FSH and 125I-hCG binding sites in testes from patients with prostate cancer cultured for 1, 3, 5 or 7 days at 33 degrees C or 37 degrees C were measured and compared. There were no significant differences in 125I-FSH or 125I-hCG binding sites between the two temperature groups at 1 to 7 days of culture. Testosterone concentration in the medium obtained from testicular organ culture at 33 degrees C did not differ significantly from that at 37 degrees C at 1 to 7 days of culture. In conclusion, a high temperature may not disturb the Leydig and Sertoli cell functions in a short term.

[Down-regulation of rat and human testicular hCG receptors by hCG].

Changes in rat and human testicular human chorionic gonadotropin (hCG) binding sites induced by hCG were estimated in vivo and in vitro. After a single administration of hCG, the specific hCG binding sites were significantly reduced for 7 and 5 days in rat and human testes, respectively. However, hCG binding sites had recovered to pretreatment values by the 14th day after the administration. Occupied hCG binding sites measured in both rat and human testes accounted for about half of the reduced binding sites on the day after administration of hCG, but thereafter the number of sites occupied was not correlated with the reduction of the binding induced a dose-related significant loss of the specific hCG binding sites for 7 and 5 days in rat and human testes, respectively. Thereafter, the number of binding sites gradually increased. These patterns of change in hCG binding sites in vitro were similar to those in vivo. These findings suggest that the reduction in hCG binding sites in rat and human testes by hCG is due not only to occupancy but also down-regulation of the binding sites. In conclusion, the present testicular organ culture method is useful to study hormonal regulation of testicular function, especially in human testes.

[Local administration of recombinant interferon-beta to patients with cancer-associated body cavity fluids].

The effects of local administration of interferon-beta produced by recombinant DNA technology (GKT-beta, Kyowa Hakko) on malignant effusion were studied in cancer patients and evaluated on the basis of the Criteria for the Evaluation of the Clinical Effects of Solid Cancer Chemotherapy (effects on malignant effusion). Out of 13 cases of peritoneal effusion, the effects were evaluated in 9 cases. Four of these cases remained free from ascites for 4 weeks and were evaluated as complete response (CR). Administration of GKT-beta alone was performed in 4 of 7 cases of pleural effusion, and CR was obtained in 1 case each of breast cancer, gastric cancer and malignant mesothelioma. In cases of gastric cancer, CR was obtained in 5 of 10 cases. The above results suggest that GKT-beta is a very effective drug against malignant effusion.

[Pharmacokinetics and urinary excretion of aspoxicillin after consecutive administration at a dose of 4 grams].

Aspoxicillin (ASPC), a new injectable penicillin, was administered to 5 healthy male adult volunteers once a day at a daily dose of 4 g for 5 consecutive days to study its absorption and excretion. The results of this study are summarized as follows: 1. In consecutive administration of ASPC for 5 successive days, no remarkable changes were observed in serum concentrations and urinary excretion after each administration of ASPC. Therefore, no tendency of accumulation of the drug was recognized. 2. The serum ASPC concentration showed its peak values ranged from 212.3 to 224.8 micrograms/ml at completion of the intravenous drip infusion of ASPC. 3. Urinary recovery rates of ASPC ranged from 70 to 80%. 4. There were neither abnormal findings in subjective and objective symptoms nor abnormal values in physical and clinical laboratory test due to the administration of ASPC.

[A case of renal angiomyolipoma treated by partial nephrectomy--the management of renal angiomyolipoma from the conservative point of view].

A 25-year-old female was admitted for further investigation of the right renal mass, which was noted during evaluation for hepatitis. She had no personal or family history of stigmas of tuberous sclerosis. On physical examination, a movable hard smooth mass of fist size was palpable in the right abdomen. Drip infusion pyelography revealed an elevation of right kidney. Ultrasonography revealed that the abdominal mass had a high amplitude echo area. On CT the mass was heterogeneous with irregular margin and had regions of low attenuation value, suspicious of a tumor with high fat content. An angiogram of the right renal artery revealed a hypervascular tumor showing multiple saccular aneurysms, and absence of arteriovenous shunting. Based on the aforementioned findings, the mass was diagnosed as renal angiomyolipoma. Partial right nephrectomy was performed through an extraperitoneal approach. Pathological diagnosis was a renal angiomyolipoma. Convalescence was uneventful, and the remaining parenchyma of right kidney was working well on drip infusion pyelography three months after operation. The management of renal angiomyolipoma from the conservative point of view is also discussed.
